Employing peripheral blood leucocytes as the model of the study, we proved how ketotifen blocks the inhibiting effect that the antigen induces over the adenyl cyclase activity in asthmatic individuals with sensitization to Dermatophagoides pteronyssinus. The strength of the Ketotifen effect is comparable to that produced on beta-receptor stimulation by isoproterenol. On the other hand, we found that within the same age range, the activity of adenyl cyclase in asthmatic individuals is comparable to healthy individuals, although the response is not the same to isoporoterenol which is greater in the control group. This is probably due to the diminished number of beta-receptors in asthmatic individuals as compared to healthy ones.
For hundreds of years urticaria has been an intriguing problem for researchers. Together with angioedema it constitutes a common condition that affects 20% of the general population. The etiologic diagnosis is obtained in a variable percentage of cases, according to the different studies published. The clinical course and the association with angioedema are also variables in the different works. It was the diversity of results that led us to undertake the present study. We selected 161 histories of children who came to our department of allergology; these children whose ages ranged from 1-12 years were diagnosed of urticaria and/or angio edema. The number of males was slightly higher than females and the most affected age group was that between 7-12 years. The acute and acute intermittent forms predominated especially in atopic children, highly associated with angioedema; chronic urticaria was less frequent. Within the etiologic factors, food allergy played an important role, followed by drug allergy. It was not possible to reach an etiologic diagnosis in 39.13% of cases.
The Melkersson-Rosenthal Syndrome consists of recurrent edema of the lips, intermittent facial edema and a furrowed tongue. This is the classic triad which defines the syndrome, although it is accepted that the presence of two manifestations is sufficient to make the diagnosis. Distribution is universal although the majority of the cases are described in the European literature. The case of a 37 year-old female is presented. She came to consult for persistent edema of the upper lip, of a three-year duration, which started abruptly without any clear etiologic correlation with acute episodes that disappear spontaneously or with corticoid treatment. Associated symptoms included migraine headaches which started years earlier. The complimentary examinations were normal except for a mild elevation of the sedimentary rate, and the biopsy was compatible with granulomatous cheilitis. In conclusion, MRS has to be considered as a diagnostic possibility in a patient who consulted for recurrent edema associated with other dermatologic and neurologic manifestations, although not necessarily having the complete triad.
The aim of this paper is to study the optical density (O.D.) values, correlated with an ELISA system to determine specific IgG4 levels against D. pteronyssinus in the serum of 20 control subjects, 35 asthmatic patients without previous immunotherapy (I.T.) and 77 patients who had variable periods of I.T. The group of non-treated patients showed specific IgG4 levels which were significantly higher than those of the control group (p less than 0.05). In this group of patients, 31.43% of the cases showed levels superior to the control group. All treated patient groups showed specific IgG4 levels which were significantly higher than those of the control group and than those of the non-treated patients, with various levels of significance (see Table IV). Of the asthmatic patients with I.T. as a whole group, 59.74% showed O.D. levels superior to the control group. The highest level of specific IgG4 was found in the patient group with the longest time of I.T. There was no significant correlation between the specific IgG4 levels with the different immunologic parameters (cutaneous reactivity, HRT, total and specific IgE). As for the clinical picture, there were no significant differences in comparing active with inactive patients during the period of study. There were no statistically significant differences in the levels observed between patients with a good clinical course and those who did not respond favourably, although all the groups of patients with a good response had higher specific IgG4 levels. This way, these antibodies may play the role of a blocking agent.
There is no doubt that ketotifen has continued to be an effective prophylactic anti-allergic drug throughout the years since its discovery. Therefore, we decided to perform a retrospective study of this pharmacologic agent to evaluate its efficacy, duration of treatment, as well as its tolerance in different allergic processes. A total of 1,000 histories of allergic patients were reviewed and 450 were selected by chance from 1981 through 1984. The patients were divided as to their diagnostic etiology, whether affected by pollen or house dust. Ketotifen efficacy was graded from 0-3 (bad, regular, good and excellent), according to its clinical evaluation. In general, children responded better to ketotifen treatment than adults (reduction in other symptomatic drug intake), although statistical significance was found in both populations. However, drug efficacy was better in pollen than house dust. Induced asthma and rhinitis in the children population. On the other hand, no statistical difference was found between sensitization to house dust or pollens in the adult population. To be truly effective, ketotifen should be taken for at least 3 months or more. We only found 12 (2.66%) patients who presented somnolence: the rest tolerated the drug well, with no secondary side effects.
The present study evaluates the protective action of ketotifen on exercise-induced bronchospasm in patients diagnosed with bronchial asthma of different aetiologies. The patients were classified in 2 groups. The first group with seasonal asthma was made up of patients with pollen hypersensitivity, while the second group with perennial asthma was made up of patients with bacterial aetiology or sensitivity to the Dermatophagoides mite. Spirometry and airway resistance measurements (in basal conditions, 5 to 10 minutes after a resistance test which consisted of 6 minutes' free running on a treadmill) was performed on each patient. Those patients in which significant spirometry or Raw alterations were detected received a 15-day treatment of ketotifen (a dose of 1 mg every 12 hours); after this time, the tests mentioned before were repeated. We found that in the seasonal asthma group, ketotifen was effective in protecting against exercise. However, we did not observe the same effect in the perennial asthma group. In the light of these results, we propose the possible existence of different mechanisms in triggering off exercise-induced bronchospasm, according to their aetiologies.
We selected 38 symptomatic pollen-sensitive rhinitic patients without bronchial pathology and with normal pulmonary functional study (volume flow curve). A control group consisting of 10 individuals was studied adjacently. The study was performed within the pollen season. The determination of Raw value was carried out through increasing doses of histamine challenge employing the Apta-Jaeger apparatus. The results were expressed in PC20 wherein 26% of the pollen-affected patients presented bronchial hyperreactivity (positive histamine bronchial provocation test). In this group the values of basal Raw were found to be increased with respect to the control group (p less than 0.025) and pollen-sensitive patients with negative histamine bronchial provocation test (p less than 0.01). On the other hand, the pollen-sensitive group with negative response to histamine bronchial challenge presented Raw values which were similar to the control group.

Enteroparasites rarely reach limits of clinical severity although their different manifestations oblige one to consult pediatricians and other specialists. In our cases, patients consulted the Department of Allergology for the presentation of recurrent urticarial reactions, recurrent cough and other manifestations that can be interpreted as allergic in origin. It is the objective of this present study to carry out a review and a statistical study of the incidence of intestinal parasitism in children who consulted for some type of allergic manifestation. We intend to evaluate the unfavorable effects that enteroparasites produce in these patients which can perpetuate the clinical symptomatology. Six hundred and eleven children with ages ranging from 2 to 12 years, and who were attended in our allergology consultancy during a 2 year period, were studied. An intracutaneous test to the different group of food and inhalant antigens, stool examination and investigation of enterobius was carried out on all of them. On 37 children an "in vitro" test to evaluate cellular and humoral immunity was carried out aside from the other explorations mentioned.

Various mechanisms intervene in the etiopathogenesis of otitis media with effusion (OME), but to date it is not clear which mechanism is the most important. We studied twenty children affected with persistent otitis media with effusion (OME) inspite of the indicated treatments, and the possible incidence of atopic features, total serum IgE and in effusion, obtained by myringotomy and aspiration were evaluated. In order to evaluate the presence of atopy, an allergic history and skin tests against the different suspected allergens (inhalants and foods) were realized.
In view of the lack of agreement among the various authors regarding the pathogenesis of atopic dermatitis (AD) we decided to conduct an immunological study on patients affected by atopic dermatitis, whether associated with allergic respiratory disease or not. Twenty five patients were studied using T lymphocytes and their subpopulations, the lymphoblastic transformation test and serum immunoglobulin parameters. We found a decrease in the T mu/T gamma ratio in patients with pure AD relative to those with associated respiratory disease as well as a direct correlation between the T mu/T gamma ratio and IgE, which support the role of the T regulatory system in IgE production.
Interest in the study and determination of the influence of a genetic factor in the etiology of bronchial asthma was initiated by Cook. This is a study of 13 pairs of twins that were seen in our department presenting some kind of allergic illness. Each pair of twins underwent a complete clinical history and physical examination, routine laboratory analysis and radiography. In addition, they were also tested for the following: skin test, serum immunoglobulins, total IgE, histamine release test, hemagglutination test, histocompatibility antigen (HLA). Statistical analysis was done using the Chi square test, with significant results found in 4 antigens, namely A2, A30 + 31, B5 and B12.
A new application of the Sepharose bead immunofluorescence test for detection of allergen-specific IgE and IgG antibodies is described. Allergen extracts of four different grass pollens were coupled to CNBr-activated Sepharose 4B. Twenty normal and allergic sera were incubated with the allergen-coupled beads, washed and incubated with fluorescence-conjugated anti-gamma E and G globulins. After washing and staining with 0.5% trypan blue, the percentage of fluorescent beads was detected by fluorescence microscopy. In the IgG/anti IgG system, the smallest amount of IgG demonstrated was 20ng/ml; in the IgE/anti-IgE system it was 40ng/ml. Independent examination of tests gave a mean difference between observations of 4.1%. Reproducibility was also very good (var. coeff = 18%). The number of beads stained with IgE correlated well with intensity of skin reactions to the same extracts (r = 0.64; p = 5 x 10-7), the percentage of IgG stained beads being independent of skin reactivity. The Sepharose-IgE test allowed clear distinction between allergic and normal sera (p = 4 x 10-7), while the Sepharose IgG test did not distinguish between them at the time of diagnosis. However, the number of beads stained with IgG significantly increased in patients undergoing immunotherapy (p = 3.8 x 10-3). The Sepharose bead immunofluorescence test requires a very small amount of materials, is highly sensitive and easy to handle. It may be valuable in the "in vitro" diagnosis of grass pollen allergy and useful in evaluating immunotherapy.

We decided in this work to evaluate the prophylactic value of carbuterol in exercise-induced asthma (EIA), comparing this drug to salbutamol and DSCG in order to describe its usefulness in exercise-induced asthma, while employing the closing-pressure method for the early detection of airway constriction. The study involved 44 bronchial asthmatics with prior histories of EIA. Only 10 patients had noticeable symptoms after exercise. All patients were asymptomatic and without any medication for at least one week prior to the commencement of the study. A continuous running treadmill was chosen for the exercise test, and a 10 minute time-period with a maximum heart rate of 180 beats/min. was the limit. Measurements of the FEV1, PEF and Raw were also done. Drug therapy was carried out by administering 10.5 mg of carbuterol, or 4 mg of salbutamol orally, 1.5 hours, before the exercise, or a 20 mg DSCG capsule, 30 minutes also prior to exercise. We concluded that: DSCG is the drug of choice in the control of EIA, while salbutamol results were somewhat less effective. Orally administered carbuterol did not provide any protection effect as expected. Raw measurements enter as a new parameter for consideration in this type of disease, being entirely compatible with FEV1 and PEF values during assessment.
One hundred and seventy eight healthy individuals, smokers and non-smokers of both sexes were studied, grouping them according to the parameters of age, height, weight and body surface, with the aim to prepare a standard table of airway resistance. The method was performed with the apparatus APTA (Jaëger) based upon the closing pressure technique. Regarding resistance, the average value obtained was 0.66790 Kpc/L/sec-1 with a standard deviation of 0.26773. With respect to the statistical study, no significant correlations were found among the parameters of sex, age, weight, body surface and smoking habits in relation with airway resistance. Nevertheless, we obtained an inverse significant correlation between variables of height and airway resistance. As a consequence, a study of linear correlation was done between height and airway resistance, bringing about the correlation coefficient such as the equation of linear regression wherein we obtained the lines of regression and the formula of the equation.
