A 62-year-old woman with a precancerous duodenal polyp in the suprapapillary region of the second portion of the duodenum was surgically treated by pancreato-duodenectomy. The postoperative course and follow-up was uneventful. A statistical analysis of primary duodenal tumor cases (excluding tumors of the duodenal papilla) reported in Japan during the period from 1962 to 1977 was also reported.

An 88-year-old woman who developed a nodule in a lesion of seborrheic keratosis on the frontal scalp was observed. Histopathologically, the nodule was characterized by lobular acanthosis consisting of glycogen rich clear cells and squamoid cells with squamous eddies and melanin blockade melanocytes. Each lobule was covered with an orthokeratotic horny layer which formed wedge-shaped masses in its central area without formation of an intervening granular layer. The clinical course and histopathological findings suggest that the tumor cells in seborrheic keratosis may tend to a new development of trichilemmal differentiation.

The clinical course of a patient with bioprosthetic failure of aortic and mitral Ionescu-Shiley valves (which contain bovine pericardial leaflets) is presented. Failure occurred in less than 4 months and was due to infection that resulted in calcification and severe stenosis. On examination of tissue specimens under light and electron microscopes, the pathological process was seen to be similar o that occurring with infected porcine prostheses.

Since November 1973, intraoperative radiotherapy has been performed on five patients with unresectable, advanced carcinoma of the bile duct at the hepatic hilus and six patients with unresectable carcinoma of the gallbladder. A treatment cone with a diameter ranging from 4--10 cm was directly applied at the lesion. A single dose of 2500--3000 rad with 11 to 20 meV electrons was delivered. In all patients, recanalization of the obstructed bile duct was observed by the postoperative cholangiography. The local efficacy was confirmed histopathologically in eight autopsied materials and a specimen resected 105 days after radiotherapy. Mean survival time of patients with unresectable tumors was 10.9 +/- 5.6 (SD) months after this radiotherapy. Intraoperative radiotherapy increased the effectiveness and length of palliation for the unresectable lesion.
Forceps biopsy of the distal part of the common bile duct was performed in 27 patients with cholelithiasis, under choledochoscopic guidance during surgery. Significant choledochitis was diagnosed in 72 per cent of the patients, including two with carcinoma of the head of the pancreas. Two patients with carcinoma of the head of the pancreas underwent biopsy in an attempt to obtain evidence of neoplasm. Furthermore, one patient suspected of carcinoma of the left hepatic duct tributaries received the same treatment and the malignancy was confirmed. No complications occurred. The prime indication for biopsy is to prove the presence of malignancy at this region and to provide an index for the management of cholangitis combined with choledocholitiasis.
Proliferation and differentiation of the epidermis in organ culture of adult human skin by the sponge matrix method were studied histologically and autoradiographically, and the following results were obtained: 1) On the first day of culture, mitotic figures were already observable in the epidermis. The outgrowth of epidermal cells at the margins of the explants started. On the second day, there was transformation to a zone that will be referred to as the newly formed stratum corneum in the upper epidermis. 2) On the third and fourth days, the increased growth of epidermal cells caused thickening of the epidermis. Simultaneously, Malpighian cells progressively differentiated into a cornified layer. 3) On and after the fifth day, the basospinous cell layer was reduced in thickness in most of explants. On the ninth and tenth days, DNA synthesis in the basal layer was still obvious, although the epidermis showed a thickness of only one or two cells overlaid with a large number of horny layers. 4) In the culture medium supplemented with corticosteroid, the epidermal growth was slightly depressed with lessened formation of stratum corneum in the early stages of culture as compared with the explants cultured in the basic medium. The reduction of the basospinous layer was scarcely notable after the fifth day. Even after 10-11 days, epidermal cells were well preserved and their stratified squamous architecture was less disorganized. It seemed that corticosteroid could prolong the survival of adult human skin in vitro. These findings indicate that this culture technique could be used as a model for organ culture of adult human skin.
A ventricular inhibited demand cardiac pacemaker was inhibited in the presence of intimate contact between the active temporary bipolar electrode ring and a retained inactive permanent bipolar electrode tip. Electromagnetic interference effects, lack of insulation, lead breaks, and loose connections were all ruled out as the cause of pacemaker suppression.
Twenty-three patients with carcinoma of the hepatic hilus, including the upper one-third of the extrahepatic bile duct, were treated by surgical resections. Gross appearances of this lesion are classified into polypoid, nodular, schirrhous constricting and diffusely infiltrating types. Each type has a different resectability and prognosis from the macroscopic point of view. The cell type in all patients was adenocarcinoma. Operation was curative in 100 per cent of four patients with the polypoid type. The chance for survival was definitively better for patients with this type than with the other three. Preoperative percutaneous transhepatic cholangiography as well as endoscopic retrograde cholangiography and celiac angiography can provide an excellent index to the resectability of the tumor and prognosis. Our experience suggests that gross appearance of the tumor may be valid as a prognostic factor in the treatment of the lesion.
