Intermittent hypoxic loading: a model system to study the early stages of myocardial lesions.

Authors applied the model system of intermittent hypoxic loading to study the development of early myocardial alterations. It was found that a primary role is played by the deficiency of high-energy phosphate synthesis and by the disturbance of energetic and transport processes. The change of Ca2+-control, activation of anaerobic glycolysis and intracellular acidosis were found to be instrumental in decreasing contractility, impairing membranes and finally in a diffuse destruction of myofilaments.

Ultrastructure and cytochemistry of cardiac intramitochondrial glycogen.

Authors have observed abnormalities of glycogen localization in cardiac muscle, after normothermic cardiac arrest. The identification of these intramitrochondrial particles as glycogen was confirmed by selective staining with periodic acid-lead citrat, periodic acid-thiosemicarbazide protein methods and by their selective removal from tissue sections by alfa-amylase. The intramitochondrial glycogen particles were of beta-type. Some intramitochondrial particles were surrounded by paired membranes which resulted from protrusion of parts of mitochondrial membrane.

[Effect of hyperbaric oxygen on aerobic bacteria].

The authors investigated the growth of 8 species of pathogenic aerobic bacteria after treatment with hyperbaric oxygen. Within 24 hours the cultures were exposed three times for 2 hours to an absolute pressure of 3 atmospheres of oxygen at 37 degrees C. In the fluid cultures as well as in the controls the inhibitory effect of oxygen was assessed in percent by means of the densitometric method. From these in vivo experiments the authors conclude that hyperbaric treatment of superficial pyogenic events accompanying obliterating vascular diseases is reasonable.

[Indication for surgery in the treatment of obliterative vascular diseases of the lower limb in patients with pulmonary tuberculosis].

The guiding principles in assessing the indication for surgery in the case of an occlusive angiopathy in patients with pulmonary tuberculosis are illustrated by 69 cases. The authors take a definite stand with respect to the therapy to be chosen and recommend a scheme of indications taking into account the complaints due to the angiopathy and impaired respiratory function which might serve as guidance in the indication for surgery.

[Operability of mediastinal tumors].

The problem of the operability of mediastinal tumours is discussed on the basis of observations made in 500 cases. The importance of angiography in the assessment of operability is stressed. Benign tumours are operable, provided the operation involves no greater risk than the basic disease itself. Exploration of tumours with uncertain preoperative diagnosis is justified if there is no counter-indication. Excision of the tumour should be performed if there is no sign definitely pointing to inoperability, i.e. if the histological type of the tumour does not exclude the success of surgery.
