[Transnasal and transsphenoidal neuroadenolyses with alcohol. Indications. Technics. Results. Remarks about action mechanism (27 cases) (author's transl)].

The technique is of interest in deep bone pain in terminal metastasized cancer, but early relapses often occur. The hypoalgesia or analgesia obtained does not seem to result from endocrine mechanisms only, nor from the release of endogeneous opioid peptides. This neuroadenolysis seems to interfere in the pain pattern either through still unknown neurotransmitters, or through the dissociation of fibers transmitting pain to the floor of the diencephalon.

Groupamatic system: overview, history of development and evaluation of use.

This paper presents an overview and history of the Groupamatic system from the original conception by C. Matte through the present Groupamatic 360-C and the Minigroupamatic. The report includes the results of a recent evaluation in which 79 centres throughout the world participated.

[Epidural morphine for obstetrical pain relief (author's transl)].

Twenty five healthy pregnant women received an epidural injection (at levels varying from T11-T12 to L3-L4) of morphine (2 or 3 mg in 10 ml of saline solution 9 p. thousand) in order to achieve pain relief for delivery. The degree of dilation never exceeded 5 cm at time of injection. Pain level decreased in 22 cases (88 p. cent) but only 16 women (64 p. cent) were fully satisfied. Hypoalgesia begun after 25 minutes and disappeared after 19 hours. No respiratory or haemodynamic changes were noted. On the other hand, the duration of the first stage of labour decreased. The best results are seen when injection is achieved at highest levels, facing spinal cord segments supplying the cervix and the perineum. No newborn showed any sign of respiratory or neurologic depression.

[Low dose morphine epidural anaesthesia. Diagnostic and prognostic interest. 492 cases (author's transl)].

Low dose morphine epidurals (0.5 to 4 mg) provide a good way of controlling either per or postoperative pain (210 cases) or chronic, somatic, intractable pain (282 cases). Selective and metameric medullar hypoalgesia is induced at a level which varies with the level of injection. There is an acquired tolerance which restricts the use of such injections to the short-term monitoring of somatic pain. In other types of psychogenic and deafferentiation pain, there is little or no sedation, but there are maximum secondary dysphoric effects. The degree of combination of these dysphoric effects with hypoalgesia is of diagnostic interest of the type of pain involved, and of prognostic interest when both somatic and deafferentation pains are present.

[Red cell autoimmunization in a "normal" population. 63 cases (author's transl)].

Anti-red auto-antibodies were found in 69 out of 892 000 blood donors aged between 20 and 60 years, 63 of whom were followed for up to five years. This would suggest an overall incidence of 1 in 13 000 members of a "normal" population. Uncontrolled methyl-dopa treatment could be incriminated in 25% of cases. Only 10% of autoimmune subjects had slight confirmed anaemia, but 72% had biological evidence of red cell destruction (i.e. hyper-reticulocytosis, elevated serum bilirubin levels and shortened red cell life span), which demonstrates the presence of subclinical autoimmune haemolytic disease in apparently normal people. The anti-red cell auto-antibodies were of the IgG type in 97% of the subjects and were associated with various anti-tissue antibodies in 41%, thus pointing to a multisystem autoimmune disorder. In most cases the abnormality persisted or spontaneously regressed during the observation period, but long-term follow-up is required to determine whether asymptomatic red cell autoimmunization is harmless or potentially dangerous.

Evaluation of the costs and benefits of motorcycle helmet laws.

Since 1976, 28 states have repealed or significantly amended their motorcycle helmet laws. The change in legislation was not based on an evaluation of the costs and benefits of such laws. This paper attempts such an assessment by comparing the cost of motorcycle helmets with the medical costs averted due to helmet use using data primarily based on motorcycle crashes in Colorado, Oklahoma, and South Dakota. Nationwide, at least $61 million could be saved annually if all motorcyclists were to use helmets. Helmet law repeals have been observed to lead to a 40 to 50 per cent point reduction in helmet use. The associated additional medical care costs substantially exceed cost savings produced by reduced helmet use. It is estimated that helmet law repeals may produce annually between $16 and 18 million of unnecessary medical care expenditures. Several alternatives to increase motorcycle helmet use are briefly discussed. It is concluded that helmet laws are effective in encouraging helmet use among motorcyclists and will prevent unnecessary medical expenditures as well as unnecessary pain and suffering among injured motorcyclists.

Red cell antibody screening with the Groupamatic system. I. Bromelin methyl-cellulose and saline techniques: evaluation of five years' routine use.

These data comprise 1,231,024 routine tests carried out over a 5-year period on voluntary blood donors. The percentage of positive results on the machines varies from 1 to 3% of the total number of samples tested. Antibodies identified either by manual or automated techniques make up 15--20% of the positive screening reactions. Rhesus, Luewis and P systems prevail, whilst Duffy, Kidd and Ltheran systems are absent. This screening has three main advantages: the supply of plasma for our production of test sera, or therapeutic immunoglobulins; protection of the recipient; partial information on the donor's immunohaematological state, especially for the risk of giving him incompatible blood sometimes in the future.

Behavior of a cell line derived from a mouse submaxillary adenocarcinoma during the initial 480 days in vitro.

A cell line was established from a transplantable adenocarcinoma, containing viral particles of the A and B type, drived from a tumor appearing spontaneously in the submaxillary region of a male mouse of the C3H/He strain. This line, after 480 days in vitro, did not change the original epithelial-like morphology, the viral expression, the membrane immunofluorescence and the degree of agglutination by various plant lectins. After 208 days of culture, the presence of up to 3 pairs of metacentric chromosomes appeared in about 55% of the cells. However, this change in the chromosomal pattern was not sufficient, at least within the limits of our observation, to modify significantly the other parameters investigated, with the possible exception of the oncogenicity, which showed a modest decrease after 296 days of culture.
