Arterial supply in the human pectoralis minor muscle.

In the following, we report our findings obtained as a result of injecting an acrylic pigment in the arteries supplying the pectoralis minor muscle in 50 lateral chests of 26 Japanese adults (15 males and 11 females). In the pectoralis minor muscle, the muscular bundle near the terminal is supplied by A. processus coracoideris (Pc, Sato-Takafuji, '85) or A. coracobrachialis (Cb, Sato, '80) of A. axillaris (Ax), while its middle upper and lower peripherals are supplied by A. thoracoacromialis (Ta) and A. partis abdominalis (Pab, Sato, '76), respectively. Further, the upper and lower peripherals at its origin are supplied by A. thoracica suprema (Ts) and A. thoracica lateralis (Tl), respectively. Pc, Cb, Pab and Ts may occasionally be absent. Arteries supplying this muscle are classified according to their origins and routes of distribution, as follows. Type I-a: Pc or Cb, Ta, Pab, Ts and Tl are present, 32%; Type II-a: Ts is absent from Type I-a, 14%; Type III-a: Pb and Pc are absent from Type I-a, 20%; Type IV-a: Cb, Pc and Ts are absent from Type I-a, 10%. Type b is Type a without Pab. The rates of appearance of Type I-b, II-b, III-b and IV-b were all 6%. The ratios of distribution in area a were as follows, in order of decreasing ratio: 37.6% for Pab (37 cases), 32.4% for Tl (49 cases), 30.2% for Ta (49 cases), 10.8% for Ts (32 cases), 7% for Cb (9 cases), and 6.37% for Pc. In the pectoralis minor muscle, the major supplying arteries are Pab, Tl and Ta, and where Pab was absent, this was compensated for by a branch of Ta. The total number of supplying arteries in this muscle was two to five, with the majority, or 36%, having four arteries. As for sex differences in the incidence of each type, Type I-a appeared more often in males (40%) than in females (20%). The rates according to the ribs of origin were 46%, 36%, 16%, and 2% for types 2.3.4.5, 3.4.5, 2.3.4, and 2.3.4.5.6, respectively. It was interesting that all Type II and Type IV cases without Ts corresponded to Type 3.4.5 without the muscular bundle arising from the second rib, as it suggests the process of degeneration of the pectoralis minor muscle in Homo.(ABSTRACT TRUNCATED AT 400 WORDS)

[A case of gastric malignant lymphoma markedly responding to chronic daily and oral administration of low-dose etoposide].

A 71-year-old woman was diagnosed as malignant lymphoma (non-Hodgkin's lymphoma, diffused mixed cell type) which involved gastric lesions. Combined chemotherapies were undertaken using VCR, CPA, 4-epi-ADM and PRED and then etoposide, 4-epi-ADM and PRED. After about 5 months, recurrent lesions were revealed in the right supraclavicular lymph node, so we conducted additional chemotherapy. However, the patient rejected intravenous therapy and required out-patient therapy, so daily oral administration of etoposide (50 mg/day) was carried out. Because of the toxicities of alopecia, the dose was reduced to 50 mg every other day, then to 25 mg every other day. No recurrent signs have been seen for the past 3 years. Bone marrow suppression was little. Chronic oral administration of low-dose etoposide is the potential salvage therapy of gastric malignant lymphoma.

[The muscular arch of the axilla and its nerve supply in Japanese adults].

We examined 94 axillary regions of 47 Japanese adults and found the muscular arch of the axilla (Maa) in five sides of three cadavers as well as the tendinous arch of the axilla (Taa) in two sides of two cadavers. The results are summarized as follows: 1) The frequency of Maa was 6.4% of the total bodies and 5.3% of the sides in this series. 2) In the left side of a 57-year-old male (No. 427), Maa was attached to the surface of the coracobrachialis muscle after fusing with the dorsal surface of the inserting tendon of the pectoralis quartus muscle. Both muscles were supplied by the caudal pectoral nerve (Npc) from the medial pectoral nerve. Moreover, in this same specimen, the sternalis muscle was recognized on the ventral surface of the pectoralis major muscle. In the left side of a 93-year-old female (No. 386), the cranial part of the muscular arch of the axilla (Cpa) was extended to the coracoid process by a tendon and attached to the abdominal part of the pectoralis major by two muscle bundles supplied by independent branches from Npc. One muscle bundle was attached to the lower margin of the abdominal part of the pectoralis major on the same plane, and the other bundle was located on the dorsal surface of the abdominal part. In a 74-year-old female (No. 411), the well-developed lateral part of the muscular arch of the axilla (Lpa) was attached to the inferior side of the tendinous arch. According to Ruge (1914) and Kasai et al. (1977), this arch was in the transition of the muscle bundle of Cpa to the arch. In the right side of the same specimen, only the thoracodorsal nerve (Ntd) was distributed into Lpa, whereas in the left side, only Npc supplied branches to Lpa. 3) The axillary arch was classified into 8 types based on the form and the supplying nerve of Cpa and Lpa. Cpa consisting of the muscle bundle is Type I, and Cpa consisting of the tendinous arch is Type II. We proposed that only Type II-A, with Cpa as tendinous arch and no Lpa, be designated as Taa (found in two cases), and the others as Maa. The following types were found in this study: Type I-A, consisting of only Cpa supplied by Npc (two cases); Type I-D, consisting of Cpa supplied by Npc and Lpa supplied by Ntd (one case); Type II-B, consisting of the tendinous arch and Lpa supplied by Npc (one case); Type II-D, consisting of the tendinous arch and Lpa supplied by Ntd (one case). 4) From the above findings, it can be suggested that Maa of varying shapes have been formed by a portion of the latissimus dorsi muscle supplied by Ntd, together with the pectoralis subcutaneous muscle, consisting of the pectoralis abdominalis, humeroabdominalis, humerodorsalis and ventrolateralis muscles supplied by Npc. The latter three muscles were proposed by Ura (1937) as the panniculus carnosus muscle, which was well developed in some lower mammalian orders. However, early investigators suggested that Maa was derived from the panniculus. Maa might have occurred as a rudimentary phylogenetic remainder in an early human embryonic stage.

[Diagnostic evaluation of plasma free gamma-carboxyglutamic acid in maintenance hemodialytic patients with renal osteodystrophy].

The levels of serum Bone Gla Protein (BGP) and plasma free gamma-carboxyglutamic acid (free gamma-Gla) were measured in 36 patients on maintenance hemodialysis and compared with conventional parameters of renal osteodystrophy (ROD), such as C-PTH, intact PTH, Al-PIII and hydroxyproline levels in serum, and sigma GS/D. Even though the values of BGP were significantly elevated in patients group, 64.5 +/- 1.74 ng/ml, but the low values within normal range could be found in 3 of 36 patients, also. On the other hand, plasma levels of free gamma-Gla were markedly elevated in all cases as high as 10.40 +/- 3.10 nmol/ml in average compared to 1.29 +/- 0.37 nmol/ml (n = 15) in healthy controls. Comparative analysis of BGP with other parameters showed exclusively significant correlation as follows, C-PTH: r = 0.833 (p less than 0.01), intact PTH: r = 0.702 (p less than 0.01), Al-PIII: r = 0.771 (p less than 0.01), hydroxyproline: r = 0.462 (p less than 0.01), sigma GS/D: r = -0.407 (p less than 0.05). Respective for its higher levels, comparative analysis of plasma levels of free gamma-Gla with other parameters failed to show a significant correlation except for correlation with C-PTH (r = 0.459, p less than 0.01). Based upon these results, we could confirm a marked elevation in plasma levels of free gamma-Gla in all patients, but couldn't point out its clinical value in ROD in this study.

[The diagnostic critical value in DFO low dose loading test on patient with aluminum accumulation compared to each parameter of ROD].

One hundred two hemodialyzed patients were examined to determine the standard level of delta Al value which is the difference of serum Al concentrations between pre and post DFO loading test. We applied low dose of DFO 15 mg/kg in this loading test. Some significant negative correlations were found between delta Al and MCI, sigma GS/D, osteocalcin, free-Hydroxyproline (dialysate) and free-gamma-Carboxyglutamic acid (dialysate). Each correlation rate was -0.58, -0.44, -0.76, -0.57 and -0.51 respectively. In addition tendencies of correlation were found between delta Al and ALP and between delta Al and %QCT (QCT/mean QCT in patient's age x 100). And statistical significant differences were found between (0 less than delta Al less than or equal to 150 micrograms/l) group and (150 micrograms/l less than delta Al) group in each osteobiochemical parameter. These results indicate that 150 micrograms/l is the lower diagnostic standard level of delta Al in 15 mg/kg DFO loading test.

Studies of serum bone Al-P isoenzyme and serum osteocalcin in patients on maintenance hemodialysis.

This report describes an investigation of 128 patients on maintenance hemodialysis. Their serum bone Al-P isoenzyme (Al-P III) values were measured by the Rosalki method, and simultaneous estimations were made of their serum osteocalcin levels as a diagnostic indicator of renal osteodystrophy (ROD). ROD was evaluated clinically from Jensen's criteria, based on a four-stage classification of the radiographic evidence for subperiosteal resorption: no change (stage 0), minor change (stage I), and definite change (stages IIa and IIb). The serum Al-P III levels in the stage IIa and IIb patients showed significantly elevated values (p less than 0.01), i.e., 4.98 +/- 6.23 and 20.51 +/- 13.46 KAU, respectively. In contrast, their serum osteocalcin levels were not appreciably different. Patients with elevated N-PTH values of 0.30 ng/ml or more were found to have serum Al-P III levels of 20 KAU or more, and all of these patients were categorized under the stage IIb classification. It is concluded therefore that in chronic renal failure patients on hemodialysis, measurements of the serum Al-P III are highly useful for diagnosing ROD, and may also be considered as a critical parameter for assessing the indication for parathyroidectomy (PTX).

[Changes in the management of urolithiasis: endourologic treatment of calculi in the upper urinary tract].

Three hundred seventy cases with calculi in upper urinary tract were treated by endourology, 210 cases were subjects of percutaneous nephrolithotripsy (PNL) and 160 cases of transurethral ureterolithotripsy (TUL), in Nara Medical University Hospital and 13 affiliated hospitals. Of the PNL cases, PNL was completed in 184 cases (87.6%) and not completed in 16 cases (12.4%). 168 cases (80.0%) had successful results by PNL alone, stone-free or stone fragments less than 5 mm, in spite of the stone location and size. No serious adverse effect was experienced throughout the study. However, changes like a scar formation or fibrosis around the nephrostomy tract were revealed as a late complication by excretory urography, computed tomography and renal scanning. Of the 160 TUL cases, TUL was successful in 132 cases (82.5%) and unsuccessful in 28 cases (17.5%). The results of TUL for the calculi above the upper iliac cresta were not favorable, success rate for stone 6-10 mm in diameter was 70.3% and that for stones larger than 11 mm 37.5%. However, the results of TUL for the calculi below the upper ileac cresta were favorable, the success rate for the 6-10 mm calculi was 91.5% and for the calculi larger than 11 mm 88.4%. Of 28 TUL failures, 7 cases underwent PNL and 21 cases received open surgery. Throughout our endourologic treatments, the most problematic stone was calculi impact in the ureter. The indication of endourology for the calculi in upper urinary tract is discussed with consideration of ESWL which is becoming the main stream of stone treatment.

[Removal of intra-atrial tumor thrombus of renal cell carcinoma: report of a case].

A case of left renal cell carcinoma with a tumor thrombus extending into the vena cava and the right atrium is reported. A 49-year-old female presented with a one month history of palpitation, dyspnea, and leg edema. CT-scanning and angiography revealed a left renal tumor with a tumor thrombus extending into the right atrium. Left nephrectomy and the removal of an intra-atrial tumor thrombus were performed under cardiopulmonary bypass. The postoperative course was unfavorable and the patient died on the 42nd day after the operation because of multiple organ failure in spite of repeated hemoperfusion. Operative procedure and prognosis of renal cell carcinoma with tumor thrombus extending into the right atrium are discussed.

[Clinical analysis of primary vesicoureteral reflux in children].

The retrospective analysis of 64 patients between 1 month and 14 years old with 87 primary vesicoureteral units with reflux was reviewed. They were followed-up for a period of 3 to 60 months. Thirty one patients were managed conservatively by chemotherapy (group A) and 28 patients were treated surgically. As the anti-reflux operation the Politano-Leadbetter procedure was used in 27 patients and the modified Politano-Leadbetter procedure in 1 patient (group B). Five patients were not treated (group C). In 13 of the 30 renal units of group A, the vesico-ureteral reflux disappeared spontaneously and in group B the post-operative cystourethrography revealed cure of reflux in all the renal units. In 2 of the 21 renal units of group A excretory urography (IVP) revealed improvement and in 3 deterioration. Post-operative IVP revealed improvement in 13 of 39 renal units of group B and in 2 deterioration. In 3 of the 22 renal units of group A the renogram revealed improvement and in 3 deterioration. The post-operative renogram revealed improvement in 8 of the 28 renal units of group B and deterioration in renal units. Analysis was also made according to the vesicoureteral reflux grade.

[Mixed gonadal dysgenesis with seminoma: a case report].

A case of mixed gonadal dysgenesis with anaplastic seminoma is reported. A 43-year-old, female, who was brought up as a female, was admitted to our hospital for swelling of left inguinal region. The physical examination revealed a short stature, ambiguous external genitalia but no Turner's stigmata. A child's head-sized tumor was noticed in the left inguinal region by CT scan. The histopathological diagnosis of this tumor was anaplastic seminoma. The right gonad was removed at laparotomy with lymph node dissection in the pelvis. Histological examination of right gonad revealed streak gonad and immature uterine with fallopian tube. In the Japanese literature, 7 cases including our case of mixed gonadal dysgenesis with gonadal tumor have been reported.

[Effect of UFT on urinary bladder tumors in rats induced by N-butyl-N-(4-hydroxybutyl)nitrosamine].

The effect of combined tegafur and uracil (UFT) on the development of rat urinary bladder tumors induced by N-butyl-N-(4-hydroxybutyl)nitrosamine (BBN) was studied. Two hundred F344 male rats were divided into 10 groups. Groups 1 to 5 were given 0.05% BBN in drinking water for the initial 8 weeks of the experiment, and Groups 6 to 10 were the controls of the prior 5 groups treated with BBN. UFT in commercial diet was administered at daily doses of 100mg (30.9 mg as FT-207) per kg of body weight in Groups 2 and 4 and 200 mg (61.7 mg as FT-207) per kg of body weight in Groups 3 and 5. Groups 2 and 3 received UFT throughout the period of the experiment, and Groups 4 and 5 for 12 weeks after 8 week treatment with BBN. All animals were sacrificed at 20 weeks, and studied histopathologically. In Groups 1 to 5, urinary bladder tumors developed in 20 of 20, 13 of 20, 6 of 20, 14 of 20 and 6 of 20, respectively. Incidences of tumors in the 4 groups treated with UFT were significantly lower than that in Group 1 treated with BBN alone. This result shows that UFT inhibits the development of urinary bladder tumors in rats induced by BBN.

[Fundamental studies on intravesical instillation of cis-diamminedichloroplatinum for treatment of urinary bladder tumors. I: On the effects of intravesical instillation of cis-diamminedichloroplatinum in normal beagle dogs].

Cis-Diamminedichloroplatinum (II) (DDP) is one of the most effective agents in the treatments of advanced bladder cancer, but little is known of its local and systemic toxicity when it is instilled intravesically. The objectives of this experiment were to study the local and systemic toxicity of intravesical DDP in order to know whether intravesical DDP is applicable for prophylaxis and/or definitive treatment of superficial bladder cancers. Six female Beagle dogs were used for the 48-hour retention study and 2 mongrel dogs were used for the 3-hour study. Plasma and bladder platinum concentration were measured following intravesical DDP, and also histopathological examination, urinalysis, complete blood count and blood chemistry were performed in order to know the toxicity of intravesical DDP. Plasma platinum concentration was not elevated significantly and no remarkable changes were observed in blood examinations after intravesical DDP. Bladder tissue platinum concentration was significantly elevated up to 16.96 micrograms/g . wet after 3 hours DDP retention and 4.02 +/- 2.8 micrograms/g . wet after 48 hours retention in dogs. At present, intravesical instillation of DDP seems to have minimum local toxicity and no systemic toxicity, but further study is necessary for long term topical instillation of DDP.

[Fundamental studies on intravesical instillation of cis-diamminedichloroplatinum for treatment of urinary bladder tumors. II. On the effects of intravesical instillation of cis-diamminedichloroplatinum in bladder cancer patients].

This study was performed to determine whether intravesical Cis-Diamminedichloroplatinum (II) (DDP), one of the most effective agents in the treatments of advanced bladder cancer, is applicable for prophylaxis and/or definitive treatment of superficial bladder cancers. Six patients were instilled 10 mg/20 ml of DDP immediately after TUR-Bt, but no remarkable elevation of plasma platinum concentration was observed. In one patient planned to have total cystectomy, 20 mg/40 ml of DDP was instilled daily for 10 days. No remarkable changes in symptom and blood examination was found during the instillation periods. Transitional cell carcinoma of grade 2 was necrosed extensively, but minimum cellular injury was observed in normal mucosa. Tissue platinum concentration was 3.19 micrograms/g. wet in carcinoma and 0.28 microgram/g. wet in normal mucosa. Longterm prophylactic instillation of 20 mg/40 ml of DDP was performed in 16 patients starting immediately after TUR-Bt. As the side effects of DDP instillation, contracted bladder was observed in one patient and allergic reactions, such as flush of the skin and dyspnea, were observed in 4 cases. These allergic reactions were happened suddenly after 15 approximately 19 times of DDP instillation without any prelude. At present, intravesical instillation of DDP seems to cause minimal irritation of bladder and absorption through the bladder wall, but clinical trials of intravesical DDP should be taken under careful observation until the allergic reaction by intravesical DDP are clarified.
