Quantitation of a whole blood assay for human natural killer cell activity.

A method is described for the measurement of human natural killer cell activity using heparinized whole blood in a 51chromium release assay. Fractionation-reconstitution experiments showed that the cytotoxic activity was abolished by removal of the Fc receptor bearing lymphocytes, but not by the elimination of monocytes and granulocytes. Autologous or pooled plasma was not found to possess inherent cytolytic activity. By analogy to an enzyme kinetic reaction, the results were expressed as kinetic lytic units (KLU) which were defined as the maximum number of target cells that could be lysed per unit time per milliliter of whole blood. The buffy coat cytotoxicity (BCC) assay is quick, easy to perform, and suitable for screening and monitoring of natural cytotoxicity. Since this methodology preserves the actual concentration of natural killer cells, it may represent a truer reflection of in vivo events.

Unreliability of nocturnal penile tumescence recording and MMPI profiles in assessment of impotence.

The authors describe a multidisciplinary approach to the assessment of impotence that includes the recording of nocturnal penile tumescence and analysis of the patient's MMPI profile. Ten patients were studied who, on the basis of physical and psychiatric assessment, were identified as being organically or psychogenically impotent. In each case, the application of decision rules based on either nocturnal penile tumescence or the MMPI would have led to misdiagnosis.

Priapism after aortic valve replacement.

An unusual case of priapism is described. The cause was unknown but the onset of the condition coincided with the start of Coumadin therapy which was given orally to a 65-year-old man who had undergone aortic valve replacement. Management of this patient was difficult. Conservative measures failed, but a corpus spongiosum--cavernosum shunt successfully resolved the problem. The authors emphasize that priapism must be treated urgently if potency is to be maintained. They also consider the various possible causes and treatment of priapism.

Treatment of superficial bladder cancer.

Most patients with bladder cancer initially present with localized, potentially curable tumours. Endoscopic surgery offers the best opportunity to eliminate these early lesions, but the rate of tumour recurrence after adequate resection is high (around 70%). Conventional methods of treatment have a place in the management of early bladder neoplasms, but their success rate is still unsatisfactory and they frequently fail to decrease the risk of recurrence. New drugs and more effective forms of administration have enhanced the use of chemotherapeutic agents. Fundamentally different approaches, such as specific immunotherapy, the use of laser energy and photodynamic therapy, are emerging as valuable approaches in the treatment of superficial bladder cancer and the prevention of recurrence. Randomized trials to assess their value and a concerted multidisciplinary effort with combined treatment give hope for effective control of early bladder cancer.

Renal oncocytomas: clinical, radiological and histological features.

Proximal renal tubular adenomas with oncocytic features, also known as renal oncocytomas, are rare, probably benign, tumors of the kidney. They are easily misdiagnosed as carcinomas. However, radiological, histological and ultrastructural characteristics allow their differentiation from adenomas and adenocarcinomas. We report 4 cases of renal oncocytomas and review the literature.

Minocycline and generalized cutaneous pigmentation.

The recent literature contains sporadic mention of localized pigmentary accumulations in skin, bones, teeth, and thyroid tissue, presumably related to the use of systemic minocycline hydrochloride. It is our purpose to report the occurrence of generalized, cutaneous, dark blue-gray hyperpigmentation that developed in a woman while she was being treated with minocycline (Minocin) for acne vulgaris. We believe this pigmentary change to be minocycline-induced.

Pilomatricoma: treatment by incision and curettement.

The usual method of treatment of pilomatricoma (calcifying epitheliona of Malherbe) is excision. We present a review of eight patients with ten tumors treated by incision and curettement. Follow-up ranged from 1 to 10 years, and there have been no recurrences. An alternative method of therapy with excellent results is presented.

The occurrence of nail pitting in Reiter's syndrome.

Some authors have stated that nail pitting never occurs in Reiter's syndrome. The case of a patient with incomplete Reiter's syndrome with nail pitting and HLA-A2, 28; B18, 27 is presented. Patients who have both psoriasis and Reiter's syndrome have demonstrated increased frequency of HLA-A2 and B27 and markedly decreased frequency of HLA loci associated with psoriasis. The particular finding of nail pitting is presented to illustrate that this change may occur in Reiter's syndrome and may reflect a predisposition to the development of psoriasis or psoriasiform lesions conferred by HLA-A2 with B27, as suggested by previously reported HLA typing studies.

Immunochemical demonstration of human chorionic gonadotrophin (hCG) in tissue of breast carcinoma.

Immunoreactive human chorionic gonadotrophin (hCG) was found in 9 of 65 surgically removed malignant breast tumours. The concentrations found ranged from 5 to greater than 500 mIU hCG/g tumour. Of the nine, seven extracts were assayed for total protein content; when expressed as mIU/mg extract protein, the hCG concentrations ranged from 0.28 - > 18.2 mIU/mg protein. Comparison of oestrogen receptor and hCG presence in the tumours showed no strong correlation. All tumours in which hCG was detected were shown to be of ductal origin. The hCG was measured by a beta-chain specific radioimmunoassay which measures only hCG even in the presence of high levels of other gonadotrophin hormones. In further study of these specimens an immunoperoxidase staining technique was used to stain for hCG in formalin-fixed sections of the same tumours assayed by RIA. This technique confirmed hCG in the positive specimens examined (tissue was available for 7 of the 8 positives) and no significant staining in any negatives. The hCG was shown to be localized within the cytoplasm and on the surface of the malignant cells but not in surrounding normal cells. This intracellular localization demonstrates that the malignant cells may produce hCG, rather than stimulating the pituitary to do so.

Tissue immunoreactive prolactin hormone in breast cancer.

The prolactin contents of the cytosol extracts of 159 human mammary carcinomas was measured by specific radioimmunoassay. Of these extracts 157 contained measurable prolactin (detection limit, 7.8 ng/g tumor). The levels measured were in most cases greater than would be expected to be due to only serum contamination. The levels of prolactin did not correlate with either estrogen or progesterone receptor content of the tumors, or menopausal status of the patient.

In vitro augmentation of cytotoxicity against tumor cell lines by mumps virus.

Cultures of leukocytes from normal subjects and bladder and kidney cancer patients contain cells cytotoxic against various tissue culture human cell lines. Postculture cytotoxicity was increased, in some experiments, after cocultivation with an inactivated strain of mumps virus. Effector cells seem to have an Fc receptor inasmuch as the activity was removed by absorption on immoblized antigen-antibody complexes.

Immunochemical demonstration of immunoreactive insulin in human breast cancer.

The immunoreactive insulin contents of the cytosol extracts of 57 human mammary carcinomas were measured by specific radioimmunoassay. All of these extracts contained measurable insulin (detection limit 1.3 ng/g tumor). In some cases (23) the levels were quite greater than the normal human serum levels. Immunoperoxidase technique revealed the presence of immunoreactive insulin within the cytoplasm of tumor cells in 6 cases.
