Cloning of the HLA class II region in yeast artificial chromosomes.

Yeast artificial chromosomes (YACs) have been applied to clone the entire class II region of the human major histocompatibility complex (MHC), including its flanking regions, in a contig over 1.5 million base pairs (bp) long. The human DNA inserts in the YACs have a size between 60 and 1300 kbp and were isolated from two EcoRI partial digest libraries. The gaps between DRA and DRB, DRB and DQA, and DOB and DPA, which had not been cloned by other means, have been bridged with YAC clones. The contig extends through the 400 kpb of DNA between the DRA and C4 genes, thus linking the class II region with the complement gene cluster in the class III region. The cloning in YACs has been supported by a conventional cosmid walk of 290 kbp in the C4-DRA region. Restriction enzyme sites in the YAC clones were compared to the sites in the cosmid walk, to published cosmid clones, and to the already existing physical maps, leading to a detailed characterization of a region of the human genome over 1500 kbp. The YAC clones will be valuable for functional analysis of the MHC.

[Choleperitoneum following Kehr's T-tube removal: conservative treatment with culdocentesis associated with sphincterotomy and endoscopic drainage of the bile ducts].

The Authors present a case of bile peritonitis due to a biliary fistula developed after T tube removal from the common bile duct. Highlighting poor clinical manifestations they report an alternative conservative treatment, by drainage of the bile leakage with culdocenthesis and successive endoscopic spincterotomy and transpapillary drainage.

Cell microfilaments disorganization is an early cytopathic effect in mengo virus replication.

The effect of mengo virus replication on cell morphology and cytoskeletal organization has been studied in LW murine fibroblasts. Cell modifications were followed by scanning electron microscopy. The cytoskeleton was stained by immunofluorescence using antitubulin antibodies for microtubules, antivimentin antibodies for intermediate filaments and rhodamine-conjugated phalloidin for microfilaments. Cytopathic effects began in the early phase of virus infection amd were correlated with broad modifications of the microfilament network. Immunofluorescence experiments showed a rapid decrease in the number of bundles and clumps of filaments in the early stages of infection. Antibodies to tubulin and vimentin revealed no change in the pattern of microtubules and intermediate filaments. Viral protein synthesis was required for the morphological changes observed after viral infection.

[Palliative gastroenteroanastomosis in pancreatic carcinoma].

The surgical treatment of pancreatic carcinoma is palliative in 90% of cases. The authors report their experience with 25 patients with unresectable pancreatic exocrine cancer, 23 of whom underwent palliative surgery. Sixteen patients had a bilioenteric by-pass and in 7 cases a gastroenterostomy was associated. The postoperative mortality rate was 8.7% and morbidity 28.6%. Eight patients with bilioenteric bypass only survived and only two of these required gastroenterostomy later on because of neoplastic duodenal obstruction. The shorter hospital stay and the longer survival of patients treated by bilioenteric by-pass only suggest that prophylactic gastroenterostomy is unnecessary and should be associated in selected patients only.

[Adhesiveness of microorganisms to some soft resins].

Considering the fact that one of the main causes of the soft resins modification in prosthetic dentistry is the effect caused by a variety of microbe, experiments were carried out "in vitro" of microbe, experiments were carried out "in vitro" on three of the most commonly used resins with a modified method, based on the use of lymphoprep, it has been possible to study the capacity for adherence of bacteria and mycetes commonly found in the oral flora, over both the short and the long term. Results of this research underlined how ethylene and acetate based vinyl resins, probably for their physical and chemical characteristics, provide the best resistance to colonization by microbes, with special reference to mycetes. This is presumably an advantage for their use "in vivo" for a greater and improved conservation over the time, owing to this reduced interaction with microbe action.

[Multiple coronary-pulmonary fistulas: description of a case].

An uncommon case of multiple coronary artery-pulmonary artery fistulas, associated with a mitral valve disease, is reported. The coronary fistulas presence was suspected on the ground of Doppler-echocardiography and the diagnosis was subsequently defined by coronary angiography. The utility of Doppler-echocardiography in the differential diagnosis from patent ductus arteriosus is discussed.

Incidence of some infectious diseases in odontostomatology--section I.

In the medical studies there are many reports on the risk of being infected that the dental staff run, especially as far as it concerns viral diseases. The first worth is about the research into the problem of infections, in particular of viral hepatitis B, made by the Authors in the area of Modena. The results establish the potential presence of such risk; so, the Authors suggest to the people working in edental clinics to follow carefully the procedures of general and specific prevention.

A case of methimazole-induced hypothyroidism in a patient with endemic goiter: effects of endogenous TSH hyperstimulation after discontinuation of the drug.

Serum thyroid hormone and TSH concentrations were monitored in a patient with multinodular endemic goiter and severe methimazole (MMI) induced hypothyrodism up to 190 days after drug withdrawal. Serum concentrations of TT3, TT4 and TSH returned to normal values at the 6th., the 140th, and the 120th. day respectively. Within the first 20 days after MMI withdrawal the increase of serum T3 levels was correlated with the observed decrease of serum TSH concentrations. Successively T3 values decreased and T4 levels progressively increased. Six months after MMI withdrawal basal serum TSH concentration was normal while an exaggerated response to TRH was observed. We think that this peculiar hormone pattern is due to iodine depletion. In this case TSH hyperstimulation increases predominantly T3 secretion demonstrating the reduced thyroidal ability to produce T4 when hyperstimulated.

Biliary bypass for an "accessory" biliary duct. A case report.

A case of accessory biliary duct draining segments V and VI of the liver, accidentally injured during cholecystectomy and repaired by biliary-enteric anastomosis, is reported. An accessory bile duct may occur in 15-20% of these patients. Ligation or repair is always recommended depending on the size and volume of bile flow. Roux-en-Y loop reconstruction is to be preferred as it provides the best results with the lowest risk of leakage, stenosis and cholangitis.

HIDA scan in the follow-up of biliary-enteric anastomoses.

In order to assess the patency and function of biliary-enteric anastomoses performed in our Department of Surgery, 21 patients entered the following study, provided an informed consent was obtained. All the patients were affected by benign biliary tract diseases and underwent either Roux-en-Y hepaticojejunostomy (11 cases), or side-to-side choledochoduodenostomy (10 cases). The 21 patients were evaluated with Tc-99m-HIDA scanning at intervals of 20 days-36 months after the surgical procedure (mean 14 months). The images were obtained after intravenous injection of the radioactive medium (5 mCi) and the scans were taken at 1 min (1 frame/s), 3 min (1 frame/10 s), and 56 min (1 frame/2 min). THe data were analyzed by a Digital PDP 11/34 Computer System. This method allowed us to assess each individual patient for the patency of the anastomosis and, by computer analysis, to build up a profile of the timing of the passage of the radioactive medium through the anastomosis, a delayed passage across the anastomosis was always pathological. In conclusion, the 99m-Tc-HIDA scanning used in our study for long-term follow-up of biliary-enteric anastomoses is reliable and allows an assessment of prognosis.

Initial treatment of thyrotoxic Graves' disease with methimazole: a randomized trial comparing different dosages.

We evaluated the efficacy of different doses of methimazole (MMI) as the initial therapy for Graves' disease. Fourteen patients were treated with 15 mg/die of the drug (group A) and 14 with 30 mg/die (group B). Blood samples for T3, T4, FT3 and FT4 were obtained before beginning therapy, every 48 h during the first 12 days and on the 45th day of treatment. All these hormonal parameters fell significantly from the 2nd day of therapy in both groups. All the patients, except for one in group B, had normal or subnormal levels of thyroidal hormones on the 45th day of treatment. The comparison between the two groups of regression coefficients over the first 12 days showed no significant differences. The absolute decrease of each examined parameter on day 12 was positively correlated with the relevant pretreatment value. These results demonstrate that doses of MMI (15 mg/die) much lower than those commonly recommended are able to rapidly control thyroidal overproduction as effectively as 30 mg/die.

Repeated DNA sequences in the distal long arm of the human X chromosome.

Two DNA probes from within a single large insert from a recombinant phage-DNA library that was constructed from flow-sorted chromosomes enriched for the human X chromosome were shown to hybridize with repeated X-specific and autosomal DNA sequences. The X-chromosomal repeated sequences were assigned to the distal long arm of the X chromosome by both hybrid mapping and in situ hybridization. Fine mapping places these repeats in a region of Xq28 between DX13 (DXS15, in distal Xq28) and factor VIII (F8C, in proximal Xq28). The location of the X-specific repeats makes them potentially useful for future investigations of diseases mapping to the distal long arm of the X chromosome, such as the fragile X syndrome.
