[Pancreatic secretory trypsin inhibitor in urothelial cancer].

Pancreatic secretory trypsin inhibitor (PSTI) levels are known to increase in some kinds of cancer. We determined the PSTI levels in patients with urothelial cancer to evaluate its usefulness as a tumor marker. The subjects were 101 patients who had bladder cancer (n = 81) or renal pelvis and ureter cancer (n = 20). Twenty-six healthy volunteers were used as controls. PSTI was measured by an RI kit using the double antibody method, and levels above 20 ng/ml were taken as positive. Serum tissue polypeptide antigen (TPA) levels were also measured in 32 cases. None of the controls, 65% of the patients with renal pelvis and ureter cancer, and 30.9% of bladder cancer had positive levels of PSTI. The positive rate for each stage was 22.3% for Tis, Taand T1, 38.5% for T2, 73.3% for T3 and 100% for T4. The rate tended to increase with the progress of tumor infiltration. All subjects with positive readings above 30 ng/ml had progressive cancer. The usefulness of PSTI as a tumor marker was similar to that of TPA. PSTI is not a specific marker of urothelial cancer, but it showed an association with the progression of cancer. Therefore, it is considered to be a useful marker to determine postoperative metastasis, recurrence, and disease progression.

[A case of carcinoma of the tongue treated with neoadjuvant chemotherapy].

A case of carcinoma of the tongue treated with neoadjuvant chemotherapy has been reported. The patient was a 61-year-old man complaining of tumor and contact pain at the left margin of the tongue. The tumor was oval and elastic soft with pedicle, measuring 26 X 16 X 8 mm in size. The tumor was histopathologically diagnosed as a highly differentiated squamous cell carcinoma. The patient was treated with CDDP (75 mg/body) and PEP (5 mg/body) for 5 days. After two trials with about a 3-week interval, treatment result showed partial response. This result demonstrated that combination chemotherapy of CDDP and PEP is an effective regimen for remission neoadjuvant chemotherapy in patients with squamous cell carcinoma.

Acquisition and functional analysis of manding with autistic students.

Three autistic students were trained to request a specific object from an adult "supplier" with the sentence, "Give me--" and to deliver that object to another adult, the "director." Subsequently, the degree to which the object offered by the supplier controlled the "Give me--" verbal response was assessed by delivering to the student an object other than the one requested. Despite knowing the names of all objects used in the experiment, students accepted and delivered to the director any object offered by the supplier regardless of its match with the requested object. After training to say "That's not it. Give me--" when nonrequested objects were offered, students responded differentially to requested and nonrequested objects, suggesting control of the "Give me--" response by the requested object, a characteristic of a mand. These results generalized across settings and objects. Results are discussed in terms of the training technique to establish manding and the functional analysis of the resulting verbal behavior.

[Treatment of nonseminomatous testicular tumor with liver metastases].

Therapeutic course of 2 cases of nonseminomatous testicular tumor with liver metastasis is reported. One case had mixed tumors including embryonal carcinoma, choriocarcinoma and yolk sac carcinoma, and was positive pulmonary metastasis already at the initial examination. In the other case having mixed tumors of embryonal carcinoma and choriocarcinoma, metastasis to the supraclavicular lymph node was detected at the initial examination. In both cases liver metastasis occurred after complete response could be obtained by treatment chiefly consisting of PVB therapy. For liver metastasis four-drug combination treatment using cisplatin, vinblastine, adriamycin and actinomycin D was performed with partial response. However, this patient eventually died. The other case received VAB-6 therapy with complete response for liver metastasis. It is advisable to consider other modalities therapy in addition to conventional chemotherapy in cases of testicular tumor with liver metastasis since the prognosis is poor in these cases.

[A case of metastasis of gastric cancer to spermatic cord].

A 63-year-old man underwent total gastrectomy and partial resection of the transverse colon for Borrman IV gastric cancer on August 27, 1985. On August 8, 1986, 11 months after the operation, he visited our department with chief complaint of swelling of both groins. The spermatic cord was swollen to about 3 cm in diameter from both groins to inside of the scrotum and was hard as a club. The diagnosis of bilateral spermatic cord metastasis of gastric cancer was established and a biopsy was carried out. Histopathologically the diagnosis of spermatic cord metastasis of gastric cancer was confirmed.

Overproduction of gene products by the super-high-copy-number plasmid pNR333.

The plasmid pNR333 is a kanamycin-resistant, deletion derivative of pNR113 with an extremely high copy number in Escherichia coli and in Proteus mirabilis. In order to determine the usefulness of pNR333 as a replication gene of vector, the genes encoding chloramphenicol acetyl-transferase (CAT) and beta-galactosidase (beta-gal) were cloned individually into both pNR333 and other low-copy-number plasmids. The expression of the cloned genes was compared by measuring the specific activity of each enzyme and the amounts of the proteins produced. A hybrid plasmid pNR333-cat expressed 53 times as much activity of CAT as the low-copy plasmid S-a which had a copy number of four. The lacZ gene cloned in pNR333 produced 17 times as much beta-gal as in the low-copy-number plasmid pNR1150. These results suggest that pNR333 is a useful vector plasmid for producing a large amount of polypeptides in E. coli hosts.

Antimicrobial resistance of Campylobacter jejuni and Campylobacter coli with special reference to plasmid profiles of Japanese clinical isolates.

A total of 111 clinical isolates of Campylobacter jejuni and 10 clinical isolates of Campylobacter coli were characterized by their susceptibility to nine antimicrobial agents and by their plasmid profiles on agarose gel electrophoresis. All of the C. jejuni isolates were susceptible to chloramphenicol, ciprofloxacin, erythromycin, kanamycin, and nalidixic acid, but 55% were tetracycline resistant. In the 10 C. coli isolates, a high prevalence of multiple-antibiotic resistance was noted. Plasmids were found in 82% of the tetracycline-resistant and 15% of the tetracycline-susceptible C. jejuni isolates. Tetracycline resistance in six randomly selected C. jejuni isolates, which contained 50- or 135-kilobase (kb) plasmids, was transferred by conjugation to a Campylobacter fetus subsp. fetus recipient with recovery of a 50- or a 45-kb plasmid from transconjugants. From one multiple-antibiotic-resistant C. coli isolate, resistance to tetracycline, kanamycin, and chloramphenicol was transferred concomitantly with a 58-kb plasmid, pNR9589. Nonconjugative 98-kb plasmids, pNR9131 and pNR9581, from C. coli isolates with resistance to tetracycline, kanamycin, and erythromycin were shown by cloning experiments to code for at least kanamycin resistance. Restriction digests revealed that 50-kb plasmids from tetracycline-resistant C. jejuni isolates were identical, although plasmids from multiple-antibiotic-resistant C. coli isolates shared partial DNA homology to each other. Cloning of the kanamycin and chloramphenicol resistance genes of pNR9589 into Escherichia coli showed that the two genes are closely linked or clustered. Double-digestion analysis of the fragments encoding the kanamycin resistance of pNR9131, pNR9581, and pNR9589 showed that these three plasmids contain a common fragment related to kanamycin resistance.

Identification of the DNA sequence required for transposition immunity of the gamma delta sequence.

A plasmid containing the transposon gamma delta sequence was immune to further insertion of gamma delta (transposition immunity). Plasmids carrying a fragment containing either 0.2 kilobase pairs of the gamma end or 0.4 kilobase pairs of the delta end of the gamma delta sequence were immune, and other parts of the gamma delta sequence did not confer immunity. The terminal 38-base-pair (bp) sequence of the delta end of the gamma delta was sufficient to confer immunity, the 38-bp sequence of the gamma end conferred only moderate immunity, and the terminal 35-bp sequence, which was completely identical at both the gamma and delta ends, was insufficient to confer immunity.
