Hb Grady and alpha thalassemia: a contribution to the problem of the number of Hb alpha structural loci in man.

Hematological evaluation and data from chain synthesis analyses in six members of the family with two members having Hb Grady (i.e., and alpha chain variant with elongated chains due to an insertion of three amino acid residues [1]) indicate the presence of multiple nonallelic Hb alpha structural loci in the single Hb Grady heterozygote. The data support the earlier stated hypothesis that the Hb alpha Grady locus resulted from a crossing over between chromosomes of two tandemly repeated Hb alpha loci. The presence of an alpha thalassemia condition in one of the two Hb Grady heterozygotes increases the relative production of the alpha Grady chain by a factor of two.

The in vitro effect of primaquine on red cell phospholipid metabolism.

Primaquine converts erythrocytes into osmotically fragile spherocytes. Since primaquine increased the rate of acylation of erythrocyte lysophosphatidylcholine (LPC), it has been suggested that a deficiency of LPC is responsible for its hemolytic action. We have examined the validity of this hypothesis. In the absence of added lysophosphatides, primaquine slightly decreased the incorporation of albumin-bound palmitic acid-14C into phosphatidylcholine (PC) whereas that into phosphatidylethanolamine (PE) was more depressed. When unbound dispersions of fatty acid were used, red cells exposed to primaquine manifested a slight increase in palmitic acid-14C incorporation into PC whereas that into PE remained decreased. In the presence of exogenous LPC, control cells increased their incorporation of bound palmitic acid-14C into PC while decreasing it into PE. In these same experiments, red cells incubated with primaquine demonstrated a much greater enhancement of LPC acylation than control cells. Lysophosphatidylethanolamine acylation, while depressed, was still slightly greater than that of cells incubated with drug alone. The concentration of LPC in control and primaquine-treated erythrocytes incubated with exogenous LPC was comparable and was approximately five times that of cells not incubated with LPC. The osmotic fragility of erythrocytes correlated poorly with their concentration of LPC. The effect of primaquine on the incorporation of bound palmitic acid into erythrocyte PC was influenced by exogenous LPC with the reaction markedly increased in its presence but slightly depressed in its absence. The proposal that the hemolytic action of primaquine is due to a decrease in red cell LPC is not supported by the data.

The place of surgery in nodular thyroid disease.

The case records of 419 patients undergoing thyroidectomy for nodular goitre are examined in the light of the final pathology and the incidence of carcinoma. In patients presenting with a clinically single nodule, the incidence of carcinoma was 10%; in multinodular goitres this incidence was 4%. A policy for the surgical treatment of nodular goitre is proposed.

Changes in pulmonary function in workers exposed to vinyl chloride and polyvinyl chloride.

To determine whether occupational exposure to vinyl chloride gas and polyvinyl chloride dust is associated with changes in pulmonary function, spirometry and maximum expiratory flow-volume curves were obtained in 348 workers in a VC polymerization plant. The major finding was diminution in air flow in 200 workers (57.5 percent). This abnormality correlated with age and duration of exposure. A relationship with smoking was noted only in younger workers with exposures of less than 10 years. When age exceeded 40 years or exposure 20 years, prevalence of this impairment was similar in smokers and nonsmokers, suggesting that occupational or other environmental factors were operative.

Antegrade spermatic vein phlebography in testicular tumors. A preliminary report.

Phlebography of the spermatic veins may yield valuable information about metastases from testicular tumors. Retrograde venous injection of contrast medium is not always possible but the antegrade approach performed in connection with orchidectomy was successful in all of the 5 cases attempted. If funicular lymphography is performed in connection with the removal of the testis, spermatic vein phlebography may be added with a minimum of additional effort. Whenever orchidectomy for malignant testicular tumor is planned, the possible value of additional diagnostic information from spermatic vein phlebography should be considered.

Kaluresis and diuresis after administration of antidiuretic hormone to hyperkalemic dogs.

Dogs infused with 2 meq KCl/kg per h exhibit electrocardiographic evidence of prelethal cardiotoxicity in about 3 h when serum potassium reaches a level between 10.2-10.5 meq/liter. During this time, their urine output of 30 ml/h is equal to the volume of KCl infused. Studies of the potassium distribution in these animals indicate that 20 percent of the infused ion is added to the extracellular fluid and red blood cell mass, 20 percent is excreted in the urine, while the remaining 60 percent is unaccounted for and presumably transferred to intracellular fluid. Dogs treated with moderately large doses of antidiuretic hormone intramuscularly before and during KCl infusion delay development of prelethal cardiotoxicity for about 5 h, with serum potassium levels comparable to those of untreated dogs. In addition, treated animals display a considerable diuresis and kaluresis with urine volumes nearly 4 times that of the volume infused. The potassium ion distribution in animals given antidiuretic hormone is much different from that of untreated dogs, with 55 percent of the infused ion found in the urine, about 15 percent in extracellular fluid and red blood cell mass, and only 30 percent presumably transferred to intracellular fluid. Transfer of potassium to intracellular fluid was calculated to be 3.1 plus or minus 0.7 meq/kg in antidiuretic-hormone-treated animals and 3.8 plus or minus 0.7 meq/kg in untreated (control) animals. Since these values are, within experimental error, quite comparable, it is possible that antidiuretic-hormone-induced kaluresis and diuresis are involved in protecting some animals from the effects of hyperkalemia by delaying the attainment of cardiotoxic blood levels.

Y-autosome translocation in the howler monkey, Alouatta palliata.

Two modal diploid chromosome numbers were encountered in a population sample of howler monkeys, Alouatta palliata. The modal diploid number of males was 53, females: 54. Karyotypes prepared by G-band and C-band techniques identified a Y-auto-some translocation in the males.
