[Secondary hyperlipoproteinemia induced by diuretic therapy (author's transl)].

In order to study the degree and pathogenic aspects of the secondary hyperlipoproteinemia in patients under diuretic therapy we measured serum lipids, lipoproteins and the apoproteins A1, A2 and B in 12 adults after a 4 weeks placebo period and 6 weeks of treatment with chlorthalidon. There was a significant increase in atherogenic low density lipoproteins (LDL), (18%, P less than 0.05) whereas the high density lipoprotein-cholesterol Apo A1 and A2 levels were not significantly altered. The same was true for the total serum triglyceride- and the very low density lipoprotein- and LDL-triglyceride levels. The activity of lipoprotein lipase and hepatic triglyceride lipase was slightly but not significantly increased. A delayed LDL-catabolism seems to be the most probable pathogenic mechanism underlying the Chlorthalidon-induced hyperlipoproteinemia.

[Indication for selective surgical excision of hepatic metastases].

451 patients undergoing surgery for colorectal cancer showed synchronous hepatic metastases in 24.4% (110 cases). Survival of patients with hepatic metastases appears to depend on both the age of the patient and the site of metastatic deposit. A relatively good prognosis group with median survival of 14 months can be separated from the other patients with only 5 months survival. Survival of patients with hepatic metastases can thus be predicted and long-term survival (5 and 10 years) obtained by surgical excision of these metastases. Postoperative CEA determinations will also increase the rate of early surgery for hepatic metastases.

Obesity and food choices in public places.

More than 5,000 food choices were observed at nine eating sites--four where meals were served and five where snacks were served. Caloric content of food choice was strongly affected by eating site and there was great variability in the amount chosen at each site. Men chose somewhat more food than women. Body weight had no overall effect on food choice, although obese people chose somewhat more food than nonobese people at one site--that serving food with the highest caloric content. These findings are consistent with the six earlier studies of food choice, which concluded that the major influence on how much people choose to eat is where they eat, and that there is great variability in the amount they choose at any one site. The presence of obesity is not a major determinant of food choice in public places.

Transient global amnesia after clioquinol: five personal observations from outside Japan.

Five personal observations of an acute amnestic episode in younger individuals after intake of clioquinol are described together with three observations from the medical literature. In five of these cases the episode began after an unusually large dose, in three after a therapeutic one with a latency of about 24 hours. The clinical aspect closely resembled classical transient global amnesia but the episode after clioquinol lasted longer (24 hours to three days) and a more or less extensive retrograde amnesia persisted permanently. In one patient after three tablets of Mexase a clioquinol concentration of 12 microgram/ml in plasma was found 24 hours after the specified dose, which is an unexpectedly high concentration compared to those reported as late as 24 hours after a single equal dose of Mexase or any other clioquinol-containing preparation. Another patient had a brief relapse two years after the first episode, after a single therapeutic dose of another clioquinol preparation.

The concept of a sensorimotor cortex: its early history, with especial emphasis on two early experimental contributions by W. Bechterew.

A brief account has been given of the events during the last quarter of the nineteenth century which led to the concept of the sensorimotor cortex. An early review by Frederick Mott has served as a useful guide. Particular attention has been paid to two early startlingly prescient papers by W. Bechterew. Since, however, Bechterew changed his views in subsequent publications, he forfeited a major claim for priority, and his early papers were forgotten.

[The mucocutaneous and lymph node syndrome of Kawasaki. 1 case complicated by multiple aneurysms].

We report in a five month old baby a case of the cutaneomucosal and lymph node syndrome described by Kawasaki, remarkable for the onset of multiple aneurysms involving the coronary and the limb arteries. This syndrome has rarely been observed except in Japan where it seems frequent. Possibly is benign forms, without heart complications, producing a picture of febrile erythema of viral type, have not received sufficient attention. The relationship between Kawasaki's disease and periarteritis nodosa in infants is then discussed. The clinical and pathological existence of the fatal forms seems undoubted, but the generally benign course of Kawasaki's disease contrasts with the severe course of polyarteritis nodosa in infants. It is possible that the usually benign forms of Kawasaki's disease represent only the initial stage of periarteritis nodosa in infants, from which they may recover, and we have only identified until now the severe forms which are rapidly fatal. The etiology of Kawasaki's syndrome, like periateritis nodosa remains unknown.

[Fertility of semi-castrated and irradiated patients with testicular tumors].

The fertility of 62 patients has been followed up by spermatographic analysis. The patients had had malignant testicular tumors treated by semicastration and postoperative X-irradiation between 1957 and 1975. Only 52 patients produced utilizable ejaculates, and these were divided into 8 groups according to the X-ray field arrangement employed. In this way a direct correlation could be established between the field arrangement (viz. field magnitude and gonad load) and the ejaculate findings. From the results it is concluded that azoo- and oligospermia are not a hormonal change induced by the tumor but rather the consequence of X-ray damage of the spermatophores. Azoo- and oligospermia are dependent on the X-ray load on the remaining testis and on the time which has elapsed since the terminaination of treatment. About 15-20% intact fertility can still be found in these patients.
