Kurt Schneider's 'Zwischen-Fälle', 'mid-cases' or 'cases in between'.

Kurt Schneider's concept recognizes an intermediate area between the two groups of the major psychoses: the 'Zwischen-Fälle', in translation 'mid-cases' or 'cases in between'. We investigated 108 such cases, and we found that some features external to the symptomatology distinguished the 'mid-cases' or the 'cases in between' from schizophrenia and suggested particular similarities to the affective psychoses. Such external factors were: age, heredity, educational and occupational level, broken home situation, life events, and necessity for long hospitalization.

[Brain organic melancholia].

We describe the syndrome of 'organic melancholia', i.e. the form of melancholia which in addition to the endogenous symptoms also shows organic psychopathological symptoms and is connected with brain damage. We found considerable differences between organic and cryptogenic melancholia: the development of organic melancholia has a regular course which can be distinguished into four stages: (1) the asthenic-hypochondriac, (2) the purely depressive, (3) the organically depressive and (4) the purely organic stage. The outcome of organic melancholia - a severe organic psychodeficiency, often combined with neurological symptoms, and the irreversible destruction of the professional life of the patient - is one of the more important differences distinguishing the organic and the cryptogenous type. Our findings are based on long-term observations of 30 patients.

Early auditory evoked potentials (EAEP) in vertebral basilar insufficiency.

Stimulation with a short tone pip elicits an acoustic nerve compound action potential (I) and different waves (II--VII) in the inital 10 ms. Seven waves have been studied in 40 control subjects and five waves in 12 patients with vertebral-basilar insufficiency. Abnormalities of the different waves were observed at levels such as cochlea and/or acoustic nerve, medulla, caudal pons, rostral pons, and midbrain. The recording of early auditory evoked potentials (EAEP) is a noninvasive method of confirming impairment of the auditory pathway caused by a reduced vascular supply of vertebral and basilar arteries.

A double-blind trial with amitriptyline and lofepramine in the treatment of endogenous depression.

A double-blind trial was undertaken to compare the antidepressant efficacy and the side effects of Lofepramine with those of Amitriptyline in the treatment of endogenous depression. The study involves 22 acutely ill endogenously depressive patients. 11 patients were treated with Lofepramine and the remaining 11 with Amitriptyline. The results demonstrate that both substances are effective in the treatment of depression. However, the therapeutical efficacy of Lofepramine is significantly greater than that of Amitriptyline. Furthermore, the tolerance of Lofepramine is better than that of Amitriptyline, and the side effects of Amitriptyline, especially in blood pressure dysregulation, are greater than those of Lofepramine.

[The initial symptoms of cerebral atrophy (author's transl)].

The paper reports about the initial symptoms of cerebral atrophy. A general view of the special literature about the aetiology, diagnosis and clinical forms of cerebral atrophy will be given. The initial symptoms of cerebral atrophy will be given. The initial symptoms of cerebral atrophy, their structure, the connections between them and their social consequences will be discussed. The difficulties of the use of a standard psychopathometrical trial including all the symptoms will be described. We think that the description of symptoms in each individual case is more precise than a collective description under a vague syndrom-labelling.

[Word field disturbances and sentence field disturbances as differential diagnosis criteria between endogenous and exogenous paranoid-hallucination syndrome].

Through the 'classical' psychopathological symptoms only, one sometimes cannot clearly differentiate between an endogenous and an exogenous psychosis. But through the identification of word field disturbances and sentence field disturbances, which are characteristic schizophrenic symptoms, one can put the diagnosis of schizophrenia in such cases. This diagnosis can be stated through the course of the illness and its therapy.

[An interpretation deduced from word field disturbances and sentence disturbances in schizophrenia: a psychological study].

We investigated the occurrence of word field disturbances and sentence field disturbances in the text of schizophrenic patients. Through the connection of these disturbances with relevant biographical data of the patient and through a psychological interpretation we found that the occurrence of these disturbances is not accidental, but has a relevant psychological meaning for the patient.

Chronobiology and its implications for pharmacotherapy of endogenous depression.

An open pilot study on 30 in-patients with endogenous depression showed a clear-cut circadian fluctuation of the therapeutical effect of single doses of lofepramine administered at three different times of the day (8 a.m., and 12 p.m.). A single-dose drug schedule with 210 mg lofepramine at 12 p.m. also proved superior (p less than .05) to the conventional divided-dose drug schedule with 70 mg lofepramine at three times of the day. The chronobiological background and the therapeutical consequences of these findings are discussed.

[Oneirodelirium in paroxysmal hypersomnia with periodic respiration and obstruction of the airway (author's transl)].

Case report on a patient aged 57 with periodic respiration and paroxysmal tendency to fall asleep in daytime. Between sleep and waking he had typical apnoic intervals with passing, but definite, hallucinations. The cause, in accord with the literature, is considered to have been edema of the upper respiratory tract which followed at times on irradiation of a squamous epithelium carcinoma of the roof of the nasopharynx. The psychologic and respiratory disturbances and the attacks of somnolence proved fully reversible after anti-edematous treatment.

The relation of "acute and transient psychotic disorder" (ICD-10 F23) to bipolar schizoaffective disorder.

The aim of this work is to investigate differences between acute and transient psychotic disorders (ATPD; F23 of ICD-10) and bipolar schizoaffective disorders (BSAD). In a controlled prospective and longitudinal study, we compared all inpatients with ATPD treated at Halle university hospital during a 5-year period with matched controls with BSAD. Sociobiographical data were collected using a semi-structured interview. Follow-up investigations were performed at a mean of 2.2-3.3 years after the index episode or 8.2-16.1 years after the first episode by means of standardized instruments. ATPD differs significantly from BSAD on various relevant levels, such as gender (more female), age at onset (older), development of the full symptomatology (more rapid), duration of the symptomatology (shorter), acuteness of onset (more acute), preceding stressful life-events (more frequent) and long-term prognosis (better). It is concluded that ATPD and BSAD are different nosological entities.

[Inpatient treated, mono-symptomatic childhood enuresis--results of follow-up in adulthood].

32 formerly monosyndromatic enuretic patients who had been treated for this condition between 1980 and 1992 were compared with a matched control group from the general population in respect of sociobiography, psychopathological and dimensional factors (depressiveness, satisfaction with life, global functioning). Most of the former patients did not fulfil diagnostic criteria for an ICD-10 diagnosis at follow-up, although there was a 37% vs. 9% difference between former patients and controls in this respect--without a clear diagnostic pattern of such disorders. Furthermore, former patients had slightly higher depression scores and slightly lower global functioning than controls at follow-up. These results confirm that childhood enuresis has a low negative predictive value concerning the development of psychiatric disorders, although it may constitute a vulnerability factor.

Concordance of acute and transient psychoses and cycloid psychoses.

OBJECTIVE: We prospectively investigated a sample of 42 patients with acute and transient psychotic disorder (ATPD) as defined by the 10th revision of the International Classification of Diseases (ICD-10; F23) to determine the clinical and demographic features of this entity and its relationship to cycloid psychoses. METHODS: During a 5-year period, all in-patients with ATPD were identified. We systematically evaluated demographic and clinical features and carried out follow-up investigations on average 2 years after the index episode, using standardised instruments. RESULTS: We found 42 cases of ATPD (4.1%) among 1,036 patients treated for psychotic disorders or a major affective episode. There was a marked female preponderance in ATPD (79%). Fifty-five percent of cases concurrently met the criteria of cycloid psychosis according to Perris and Brockington [in Perris C, Struwe G, Jansson B (eds): Biological Psychiatry. Amsterdam, Elsevier, 1981, pp 447-450]. There was no difference in gender distribution between cycloid and non-cycloid ATPD. As expected, abrupt onset and polymorphic features were significantly more common in cycloid than in non- cycloid ATPD. At follow-up, patients with cycloid ATPD showed less persistent alterations and better social functioning. CONCLUSION: ATPD as defined by ICD-10 is a heterogeneous category. A diagnosis of cycloid psychosis is made in half of the cases of ATPD, and in these cases, the prognosis is more favourable.
