Medical management: reflecting on some ripples in the pond.

Using current research into the management training of doctors in the UK, looks at some of the personal rather than just the organizational issues which arise from the development of doctors as managers. The variety of interpretations of the role raises a number of questions, some of which are highlighted: for example, the status of the management activity for doctors; the option of professional retreat from, or isolation in, difficult managerial roles; part-time management; disempowerment of other professionals; re-entry needs to full-time professional clinical work; women doctors as managers and the double- or even triple-glazed ceilings which they face. Some positive trends are evident, e.g. the impact of successful female chief executives as role models, the impact of training, but no one solution has emerged and this trend itself is seen as encouraging, given the context of a complex and ever changing environment.

Outsourcing therapy services. A strategy for professional autonomy.

Explores the problems and opportunities for professions allied to medicine (PAMS) in the new NHS internal market. Uses a mechanism from IT, namely outsourcing, to provide a new organizational model for the effective delivery of care by this group of professionals who are marginalized by the new role of the doctor as manager.

The role of aluminium for adverse reactions and immunogenicity of diphtheria-tetanus booster vaccine.

In a randomized, double-blind clinical trial, 235 schoolchildren aged 10 years received either a regular, aluminium-adsorbed diphtheria-tetanus vaccine or the same vaccine in fluid form, in order to investigate if local side effects could be diminished by exclusion of aluminium. System reactions were rare and local reactions frequent in both groups but larger local reactions were even more pronounced in the non-adsorbed vaccine group. Antibody responses to both vaccines were excellent and even slightly higher to the non-adsorbed diphtheria-tetanus vaccine (p = 0.0135 and 0.0014 for anti-diphtheria and anti-tetanus, respectively). This study has shown that fluid diphtheria-tetanus vaccine does not solve the problem of extensive local side effects induced by the diphtheria-tetanus school booster. Alternative strategies, such as changes in vaccine composition, dosage and spacing, will have to be considered.

Prevalence of iron deficiency in Swedish adolescents.

The prevalence of iron deficiency was determined in Göteborg, Sweden, in a sample of 15- to 16-y-old girls (n = 220) and boys (n = 207) using serum ferritin (SF). In a recent study in women on the relationship between SF and stainable bone marrow iron, it was established that at a cutoff value for SF of < 16 micrograms/L in 75% of women with no iron stores SF concentration was below this value (sensitivity 75%), whereas in 98% of iron-replete women it was above this cutoff value (specificity 98%). The present study showed that in 40% of the girls and 15% of the boys SF was below this cutoff value, indicating iron deficiency. Low SF concentration was associated with significant decreases in transferrin saturation, Hb concentration, mean corpuscular Hb, and mean corpuscular volume. The results from this cross-sectional study showed that, with decreasing SF, the decrease of values for these parameters occurred already before SF had reached the level 16 micrograms/L, suggesting that SF can be validly used as a single criterion of iron deficiency. Using the cutoff value SF < 16 micrograms/L, the figures for the prevalence of iron deficiency in adolescents in different countries were compared and found to be rather similar in Australia, Canada, the United States, and Sweden. High iron requirements combined with the present low-energy life-style leading to an insufficient supply of dietary iron may be a reasonable main explanation for the paradoxical, high prevalence of iron deficiency in adolescents in affluent societies.

Construction and molecular dynamics simulation of calmodulin in the extended and in a bent conformation.

Analysis of sequence similarity and comparison of the three-dimensional (3D) structures of troponin C and calmodulin have revealed a sequence in the central helix of calmodulin with a high probability for bending. The three amino acids known to form a bend in the N-terminal portion of troponin C are also found in the central helix of calmodulin. The modelling of a bent calmodulin structure, using the dihedral angles of the three residues in the bend of troponin C as a 3D template, results in a conformation of calmodulin where the N- and C-terminal domains are able to form contacts. Dynamics simulations starting from the X-ray structure of calmodulin and from the modelled bent calmodulin were carried out to compare flexibility and correlated movements of Ca2+ in the binding loops. Both conformations of calmodulin remained stable during the period of simulation. In the simulation of calmodulin in the extended form, the motions of the Ca2+ atoms in the two domains (Ca2+1 and Ca2+2 in one domain, and Ca2+3 and Ca2+4 in the other) are correlated. In the simulation of the bent form, an additional correlation between the Ca atoms in the two different domains is observed. The results are compatible with the occurrence of a bent conformation of calmodulin in the presence of targets, and with increased Ca2+ affinity and cooperativity of the Ca(2+)-binding loops in the calmodulin-peptide complexes.

Impact of pertussis on the afflicted child and family.

The effects of pertussis in an unimmunized population were investigated in 61 children consecutively identified by positive culture for Bordetella pertussis. Parents were interviewed twice, at a mean of 3 and 11 weeks after onset of symptoms. All children had a disease duration of more than 3 weeks. Behavioral changes were reported for 84% of the study children. Parents reported negative effects for themselves in 95% and for siblings in 63% of the families. The main problems were disturbance of night sleep, staying home from work and isolation of the family. Parental knowledge of the disease was generally good; a majority had few contacts with the medical services and coped with the disease themselves. Serious concerns for the child were reported by 51% of parents and fears for permanent sequelae by 10%. The attitude to pertussis vaccination was positive in 89% of cases at both interviews. The study has thus shown that pertussis in an unimmunized child population represents a heavy burden.

Specific immunoglobulin for treatment of whooping cough.

Immunoglobulin treatment for whooping cough (pertussis) is widely believed to be ineffective although there are sound reasons for regarding the condition as a toxin-induced disease. We wondered whether the lack of success with pertussis immunoglobulins might be attributable to inadequate dose, so we designed a randomised, double-blind, placebo-controlled trial of two immunoglobulin preparations. The study was conducted at three Swedish hospitals. We enrolled 73 children aged less than 36 months who were admitted with a clinical diagnosis of whooping cough. On admission they were assigned to one of three groups: (a) monocomponent pertussis toxoid vaccine; (b) two-component acellular vaccine also containing filamentous haemagglutinin; or (c) 20% albumin solution (placebo). The immunoglobulins had a high antitoxin content and had been raised with acellular pertussis vaccines. Diagnosis of pertussis was confirmed by laboratory tests and the follow-up was completed in 67 children. The main study group consisted of 47 children with less than or equal to 14 days of disease before therapy. Duration of whoops post-treatment was 8.7 days (95% Cl 4.8, 12.6) in the 33 children receiving immunoglobulin vs 20.6 (95% Cl 11.9, 29.3) in the 14 receiving placebo (p = 0.0041). Mean number of whoops during the first week of follow-up was also significantly reduced (p = 0.0196). We found that early treatment was important, since the effect on duration of whoops was most pronounced when disease duration before treatment was less than or equal to 7 days. There were no significant differences between the two immunoglobulin preparations. High-dose specific pertussis immunoglobulin with a high antitoxin concentration has a beneficial effect in the treatment of whooping cough.

Gd-DTPA-enhancement magnetic resonance imaging of the cavernous sinus.

Based upon our experiences with 7 cases and a literature review a survey is provided on the possibilities and limitations of preoperative diagnosis of space-occupying lesions of the cavernous sinus using MRI with Gd-DTPA enhancement. Advantages are, that in many cases arteriography is no longer necessary, and that the diagnostic sensitivity is higher as compared with CT. The pattern of enhancement does not allow for histological distinction among the various lesions.

Cumulative incidence of pertussis in an unvaccinated preschool cohort based on notifications, interview and serology.

The cumulative incidence of pertussis at six years of age in the first unvaccinated cohort after cessation of immunisation in Sweden was investigated by serological methods and by a validation of the national epidemiological surveillance system by reports from the Child Health Centres (CHCs). In 312/385 (81%) eligible children, both an interview with the parents and a blood sample from the child could be obtained. The CHC reports yielded a cumulative incidence of 31%, whereas the serological assays found 54%. Of the cases reported by the parents to the CHCs, 89% were seropositives. In the additional cases of pertussis reported at the study interview, only 61% could be confirmed (p less than 0.001). Among children with reported severe cough not suspected to be pertussis, 46% were seropositive, distributed as 33% seropositives in cases with cough duration of less than 4 weeks and 69% for longer coughs (p less than 0.01). In the CHC reports, the parental diagnosis was found to have been confirmed in three-quarters of cases by medical personnel. The CHC reporting system was thus found to be reliable with an observed specificity of 93%, but sensitivity of only 52%. Thus, even this surveillance system, which yields the highest incidence rates, underestimates the incidence of the disease.

Cutaneous squamous cell carcinomas and papillomaviruses in renal transplant recipients: a clinical and molecular biological study.

Papillomaviruses are strongly implicated in squamous cell carcinomas arising on mucosal surfaces of normal individuals, and in the skin carcinomas of epidermodysplasia verruciformis suffers. Renal transplant recipients often have numerous skin warts and, in Australia particularly, a very high risk of developing cutaneous squamous cell carcinoma. To determine the magnitude of this risk, and to test whether papillomaviruses are specifically associated with these cancers, we examined 188 renal transplant recipients for skin cancers and tested 235 biopsy specimens of (histologically proven) squamous cell carcinomas for the presence of viral DNA. The risk of developing squamous cell carcinoma increased with duration of transplant: the probability being 25% after 9.5 years (standard error = 1.3 years) rising to 50% at 20.6 years (standard error 6.8 years). Factors which did not appear to affect the risk of tumour development included the patients sex and their skin type. However the age at transplant significantly altered the risk with patients transplanted at greater than 35 years developing tumours about four times more rapidly than patients less than or equal to 35 years. Extensive hybridisation tests for the presence of papillomavirus DNA in squamous cell carcinomas were negative, as was the polymerase chain reaction amplification method using general L1 gene oligonucleotide primers. Our data do not support a role for papillomavirus in the maintenance of cutaneous squamous cell carcinoma.

Changing cultures--determining domains in the NHS.

This article explores changes in the National Health Service (NHS) as an organisation, in the context of the emerging managerial culture. This new culture is also seeking to influence the other forms of organisational culture which have co-existed until now in the NHS; its success in doing so is limited by the lack of a shared value system within the new management culture. The issue is explored with reference to domain theory as suggested by Kouze and Mico (1979), and subsequently amended by others, to fit the developments occurring in the NHS. Consideration is given to the following issues with reference to the three domains of politics, management and professions. Can we establish: the continued existence of each domain; the changing location of groups between each domain; the interrelationship between the domains: the location of issues within or between the domains. The theory is then located within the broader discipline of organizational behaviour to provide a revised model for thinking about and acting upon the cultural change in the new NHS; the organizational learning now required is shown to require the following activities by each domain; identification, interpretation and communication.

Immunity and immunization of children against diphtheria in Sweden.

The level of immunity to diphtheria and the effect of vaccination with different doses of diphtheria toxoid was investigated. The 457 study children, 6, 10 and 16 years of age, had as infants received routine primary vaccination with three doses of diphtheria-tetanus-toxoid or diphtheria-tetanus-pertussis vaccine, and the 16 year-olds also had received a booster dose of tetanus with a small dose of diphtheria at the age of ten. Prior to the study booster, 15% of the 6-year-olds had antitoxin levels against diphtheria less than 0.01 IU/ml, the given minimum level for protection. Of the 10-year-olds, 48% had titres less than 0.01 IU/ml, while the corresponding figure for the 16-year-olds was 24%. After a booster injection of 0.1, 0.25 or 0.5 ml of diphtheria-tetanus vaccine, more than 97% of the children showed titre levels greater than or equal to 0.1 IU/ml, while levels of greater than or equal to 1 IU/ml, indicating titres sufficient for long-term protection, were attained by 23-96%. Systemic reactions were few and moderate. Local reactions were of little clinical significance. In a group of 5-years-olds given diphtheria-tetanus primary vaccinations over wider intervals, only 1.4% had antitoxin titres less than 0.01 IU/ml. The results show a need for serologic monitoring of vaccination programmes.

Intradural spinal cord lesions: Gd-DTPA-enhanced MR imaging.

Twenty-five patients with suspected spinal cord neoplasms were studied with high-field-strength magnetic resonance (MR) imaging (1.5 T) before and after administration of gadolinium diethylenetriamine-pentaacetic acid (DTPA) (gadopentetate dimeglumine). Five patients had enhancing, nonneoplastic lesions, including spinal dural arteriovenous fistulas (AVFs), cord infarction, and chronic arachnoiditis. Fifteen patients had proved spinal cord neoplasms, 13 intramedullary and two extramedullary. Four of the intramedullary tumors were detected only after Gd-DTPA administration; in five others, contrast material enhancement improved observer confidence. Gd-DTPA also demonstrated one dural AVF not detected on precontrast images. Regions of cord ischemia or infarction related to spinal dural AVF also enhanced in three patients. Advantages of Gd-DTPA include the demonstration of small isointense intramedullary tumors and the ability to permit differentiation of tumor from adjacent cord syrinx and solid tumor from postoperative gliosis and arachnoid scarring. Gd-DTPA enhancement is a useful adjunct to high-resolution MR imaging of the spinal cord.
