[Immunologic characteristics women with recurrent spontaneous abortion of unknown etiology. I. Cellular immune response].

The purpose of this study was to analyze the cellular immune response of women with recurrent spontaneous abortion (RSA) of unknown etiology. The study group was consisted of 117 nonpregnant women with RSA and 44 healthy, nonpregnant multigravidas (as a control). The following immunological parameters were analyzed in peripheral blood of two groups of women: percentage of CD3+, CD4+, CD8+, CD3+/HLA-DR+ and CD16+/CD56+ cells, lymphocyte proliferative response to mitogen and allogenic, and chemiluminescence of neutrophils. The results show that nonpregnant women with RSA of unknown etiology differ in some parameters of immune response form controls. The changes of peripherial blood T lymphocytes subpopulation were observed, consisted on lower CD8+ percentage and higher CD4+ to CD8+ T cells ratio, but lymphocytes and neutrophils activity seems to be unchanged. It seems, that among couples experiencing RSA of unknown etiology higher evidence of the same HLA antigents is not observed, when compared with couples of normal fertility.

[Immunologic characteristics of women recurrent spontaneous abortion of unknown etiology. II. Characterization of humoral immunology].

The purpose of this study was to analyze the humoral immune response of non-pregnant women with unexplained recurrent spontaneous abortion (RSA). The study group was consisted of 117 nonpregnant women with RSA and 44 healthy, non-pregnant multigravidas (as a control). The following immunological parameters were analyzed in peripheral blood of two groups of women: the effect of autologous sera on proliferation of lymphocytes stimulated with phytohemagglutinin (PHA) or paternal lymphocytes (in mixed lymphocyte reaction--MLR), incidence of MLR-blocking antibodies (MLR-BAbs) incidence of antinuclear and anticardiolipin antibodies. The results show that women with RSA of unknown etiology differ in some parameters of immune response--higher incidence of antinuclear and anticardiolipin antibodies, absence of serum factors suppressing lymphocyte proliferation in response to PHA and paternal alloantigens--in comparison with nonpregnant normal multigravidas.

CD3-/HLA-DR+, CD3+/HLA-DR+ cell levels and PHA stimulation of lymphocytes obtained from women with recurrent spontaneous abortions and subjected to partner's lymphocyte immunization.

It is assumed that 80% of recurrent spontaneous abortions (RSA) of unknown etiology are of immunological origin. The proposed experimental treatment of those patients could be immunization with paternal lymphocytes, though the exact mechanism through which this therapy exerts its effect is still unknown. Some authors suggest that lymphocyte alloimmunization alters the number and activity of particular subpopulations of peripheral blood lymphocytes. The aim of this study was to evaluate the proliferative O activity of lymphocytes stimulated with phytohemagglutinin (PHA) and the percentage of peripheral lymphocytes carrying CD3+/HLA-DR+ and CD3-/HLA-DR+ markers, present in 32 selected for this study RSA women subjected to paternal lymphocytes' alloimmunization performed prior to conception and in early pregnancy. We conclude that, the post-immunization changes seem to have different quantitative and qualitative character comparing to those seen in normal pregnancy. They seem to avert unfavourable immunological phenomena observed in RSA women, allowing for successful continuation of pregnancy.

[Cytometric analysis of lymphocyte phenotype in cord and newborn blood].

At birth and in early childhood, phenotypic and functional investigation of the immune system is often necessary to identify primary immunological disorders and may provide additional diagnostic and prognostic information in certain intrauterine infections. We compared, using of flow cytometric immunophenotyping, a broad panel of lymphocyte subsets in paired samples obtained from healthy infants at birth (cord blood) and at 3 days of age. Results were compared to those of normal children (7 to 17 years) evaluated using the same methods. Significant age-related differences were demonstrated in lymphocyte subpopulations. Our data can be used as age-matched reference values for blood lymphocyte immunophenotyping.

[Infection with parvovirus B19 in pregnant women].

The viral infection caused by Parvovirus B19 which occurs at pregnant women may be reason of many different kinds of complications during pregnancy. Until this time it is not known the frequency of the Parvovirus infections at pregnant women in Poland. We have based our studies on a group of 78 pregnant women with symptoms of a abortion, a premature imminent labour, premature labor and intrauterine death of foetus. In 10 cases (12.8%) we have confirmed a presence of antibodies IgM class antiparvovirus B19 at patients serum. It seems that the Parvovirus infection is one of most often reasons of unsuccessful pregnant.

Morphological characteristic of the skull with gnathoschisis and palatoschisis.

The authors described human skull with bilateral gnathoschisis and palatoschisis belonging to adult female from the Tchuktchi tribe (18-19 century). An attempt to determine a degree of morphological deviations caused by this defect has been made in comparison to the normal skulls of the control series. The described skull was smaller, particularly in the middle facial region. The nasal aperture was widened with subsequent high palate.

Secular changes of body height and mass in the children and adolescents of Lódź.

The report illustrates secular changes in the development of body height and mass in Lódź school children and youths in a period of time covering 65 years between 1930-1994. The lowest values of both traits were found in the early post-war period (1948-1949) increasing gradually until the end of the seventies. From that time the secular trend diminished successively, restraining completely in the last years (1992-1994). This seems to be connected with the economic crisis of the eighties in Poland, and the deep transformation of socio-economic relations in the last several years. According to the secular changeability values, the rate of that process in relation to younger groups of children increases considerably from the 12th year of life. The biggest changes (in centimeters and kilograms per decade) can be observed in the age of 15 and 16 years. Usually more distinctive variableness is apparent in boys than in girls.

[Occurrence of antinuclear antibodies in women with endometriosis and unexplained infertility].

The relationship between female infertility, endometriosis and antinuclear antibodies (ANA) in the blood of women without clinical symptoms of autoimmune disease is described. ANA in the titre of 1:40 and more, was detected in 63% women with endometriosis, 70.4% women with idiopathic infertility, 3.3% women with hypothalamic-pituitary dysfunction and 5.6% healthy nonpregnant women. The occurrance of ANA in endometriosis and idiopathic infertility was more frequently connected with negative postkoital test and the number of spontaneous abortions. We suggest, that (auto)immunological mechanisms play an important role in endometriosis and unexplained infertility.

T lymphocyte subpopulations and lymphocyte proliferative activity in normal and pre-eclamptic pregnancy.

The purpose of this study was to assess whether any changes occur in the cellular immunity in normal and pre-eclamptic pregnancy. T lymphocyte subpopulations and the lymphocyte proliferative responses to mitogens (PHA, Con A, PWM) in the fetal calf serum were examined in normal pregnant and pre-eclamptic primiparas in the third trimester of pregnancy. In normal pregnancy the absolute and percentage numbers of CD3+ and CD4+ T cells were significantly lower and the CD4+/CD8+ ratio almost halved, in comparison with non-pregnant subjects. In the pre-eclamptic women a decreased absolute and percentage content of CD8+ T cells and increased percentage of CD3+ and CD4+ lymphocytes were found--in comparison with the normal pregnant women--which led to an almost 2.5-fold increase of the CD4+/CD8+ ratio. No disorders were found in the lymphocyte proliferative responses to mitogens in either normal or pre-eclamptic pregnancy. We conclude that despite the shifts among T lymphocyte subsets, mitogen-induced lymphocyte proliferation presents its functional stability and unchanged reactivity in normal and pre-eclamptic pregnancy.

Antinuclear antibodies in women with recurrent pregnancy wastage and their prognostic value for immunotherapy.

Autoimmune diseases are suspected of causing some cases of recurrent pregnancy loss. We sought serological evidence of such diseases in women with recurrent pregnancy wastage. Antinuclear antibodies were studied administrating indirect immunofluorescence test using HEP-2 cells as targets. We compared the frequency of a low-level antinuclear antibody titers in the 68 women with a history of unexplained recurrent pregnancy losses (group A) to that in 35 with explained repeated pregnancy losses (group B) and 44 healthy pregnant (group C), and 36 healthy nonpregnant women (group D). The frequency of positive ANA tests at a titer of 1:40 or higher was 51.5% in group A, 34.3% in group B, 6.8% in group C and 5.6% in group D. No negative influence on pregnancy outcome was found in case of occurrence of ANA in group A where women were treated with husband's lymphocyte immunisation. The success rate was 78.6% in patients with positive ANA and 62.5% in patients with negative ANA. This study demonstrates a high prevalence of low-titer antinuclear antibody-positive serum in patients with both explained and unexplained pregnancy losses; however, the significance of these findings remains unclear.

Essential edema-proteinuria-hypertension (EPH) gestosis and gestosis superimposing pre-existing renal disease: comparison of cellular immunity parameters.

The paper is aimed at evaluation of certain indices of cellular immunity in pregnant women with gestosis superimposing pre-existing renal diseases and comparing them with the immunological condition of women with essential gestosis. T lymphocyte subpopulations and lymphocyte proliferative responses to mitogens (phytohemagglutinin--PHA, concanavalin A--Con A and pokeweed mitogen--PWM) in the fetal calf serum and autological sera were studied. The groups of examined pregnant women in the 3rd trimester did not vary with regard to gestation age, calendar age and to the severity of edema (E), proteinuria (P) and hypertension (H) gestosis symptoms. It has been found that equal changes occur in both groups of women, regarding the examined indices of cellular immunity, when referred to normal pregnant women. In the gestotic women a decreased absolute and percentage content of CD8+ T cells and increased percentage of CD3+ and CD4+ lymphocytes were found in comparison with the normal pregnant women, which led to an almost 2.5-fold increase of CD4+/CD8+ ratio. The sera of gestotic women, in comparison with the healthy pregnant group, strongly increased the proliferation of Con A and PWM-induced lymphocytes. It may then be assumed that the observed immunological changes do not coincide with the primary causes of gestosis. We suggest that immunological factors largely contribute to the development of essential gestosis and gestosis superimposing pre-existing renal disease.

[Certain factors of cellular immunity in clinical evaluation of women with EPH gestosis symptoms].

The applicability of clinical examination was studied in women with symptoms of medium-advanced or advanced primary gestosis of subpopulation of T lymphocytes (CD3+, CD4+, CD8+) in the peripheral blood. Also studied was the usefulness of the proliferative activity of lymphocytes in in vitro cultures: spontaneous and mitogenic (PHA, ConA, PWM)--in the environment of fetal calf serum (FCS) and autologous sera: heat-inactivated and heat-non-inactivated. It has observed that the percentages of particular T Lymphocyte subpopulations and the proportion of CD4+ lymphocytes to CD8+ lymphocytes in the peripheral blood and the mitogenic activity of the heat-non in activated autologous serum in the test of lymphocyte spontaneous blastic transformation reveal a correlation with an enhancement of particular clinical symptoms of EPH gestosis and with the birth condition of newborns.

[Spontaneous proliferative activity of lymphocytes in normal and EPH gestosis complicated pregnancy].

The spontaneous proliferative activity of lymphocytes and CD4+/CD8+ ratio has been estimated in normal and EPH gestosis-affected pregnancy; also the effects of autologous and allogenic sera on this lymphocyte activity was evaluated. Although in normal pregnancy there was a very low CD4+/CD8+ ratio, the spontaneous proliferative activity of lymphocytes and the effects of autologous sera exerted thereupon were the same in either healthy pregnant women or in non-pregnant controls, what indicates a normal cellular immunity in physiological pregnancy. In gestosis an increased proliferative activity of lymphocytes and very high CD4+/CD8+ ratio was observed, when referred to non-pregnant controls. The serum of normal pregnant women did not reveal any immunosuppressive features against the activity of both autologous lymphocytes and gestosis--affected pregnancy, whereas it clearly inhibited the lymphocyte proliferation in non-pregnant controls. A different model of the autologous sera effects on the lymphocyte activity in gestosis women--when compared with the other examined women--is an evidence for altered immuno-modulating parameters of the serum in question.
