[A study of pancreatic insulinomas by electron microscopy. 9 cases (author's transl)].

Nine hypoglycaemia-inducing pancreatic tumours were studied by electron microscopy. In 8 of these tumours, it was possible to identify, within the cell cytoplasm, secretory granules with a "paracrystalline" content, identical in appearance to the granules of the B cell of the normal human pancreas. Thus electron microscopy would appear to be a reliable and sensitive method for the morphological identification of these endocrine tumours of the pancreas.

Adenoma, Islet Cell↗

[Necrosis in acute pancreatitis. Surgical and anatomopathological comparisons. 7 cases].

The surgeon should take pains to section and study himself the operative specimen after excision for acute pancreatitis, in order to understand the true nature of the lesions, which the most attentive and competent pathological examination cannot describe as vividly as direct examination by the operator. Often he will be surprised to find that the lesions, predominant in the capsule, are less profound and less severe than he had thought at first sight. Findings of this sort, if they were to be confirmed, would put into doubt the principle itself of routine pancreatectomy for acute pancreatitis.

Acute Disease↗

[Intranuclear inclusions in giant cell bone tumor. Demonstration by electron microscopy].

In a case of giant cell tumour of bone, electron microscopy revealed filamentous inclusions within the nuclei of all the giant cells. Although not invariably present in such tumours, their similarity with the nucleocytoplasmic inclusions seen in osteoclasts in Paget's disease is underlined. This raises interesting questions as to their nature and the histogenesis of these disorders.

[Continuous and interrupted intestinal sutures. Experimental and clinical study (author's transl)].

The authors report a clinical study of 101 cases of continuous digestive suture and an experiment comparing intestinal healing after continuous and interrupted sutures. A certain discrepancy exists between the excellent clinical results and the pathological and microscopic findings with continuous sutures, and in particular the presence of pseudo-diverticular pouches in 20% of cases developing at the expense of a mucosal evagination within the suture. If carefully performed, continuous suture seems excellent, preventing by its watertightness, the onset of early fistula.

Animals↗

[Jejuno-ileal short-circuit operations in the treatment of obesity. 16 personal cases (author's transl)].

The authors report a personal experience of 16 cases and in the light of data in the world literature, the authors discuss the controversial method of treatment of very severe obesity when medical treatment has failed. The onset of various complications linked mainly to intestinal malabsorption, should make one careful in the indications for surgery which may, however, be applicable in certain cases.

Adult↗

[Zollinger-Ellison syndrome and parathyroid adenoma. Limits of Wermer's syndrome].

The authors report the case of a patient with pancreatic and parathyroid adenomatosis and discuss whether it belongs to Wermer's syndrome. After a study of the various possible endocrine involvements, the complexity of hormonal inter-relations is studied, the prognosis and classification of multiple endocrine tumours are then discussed.

Adenoma↗

[Sarcomas of the gallbladder].

The authors report two recent personal cases including one explored by retrograde cholangiography and discuss the main clinical, pathological characteristics of this rare disease. They emphasize the severity of the course which does not usually last more than a few weeks after the onset of the first signs.

Aged↗

[Chronic painless pancreatitis. Apropos of 9 cases].

The authors report 9 cases of painless chronic pancreatitis and review the literature on this apparently rare condition. They emphasise the contrast between absent or minimal pain and the severe pathological lesions. In the light of these cases and pathological specimens, they attempt to interpret the pain during chronic pancreatitis. Pain seems to occur when there is a downhill course.

Adult↗

[Churg-Strauss syndrome. Its nosological place. Apropos of a new case diagnosed before death].

Allergic granulomatous angeitis or Churg and Strauss disease, seems to form part of the disease of necrotising angeitis. Clinically, it may be considered when periarteritis nodosa is associated with asthma and marked eosinophilia. The diagnosis depends on the pathological findings including the venous and pulmonary lesions, giant cells and the presence of extra-vascular granulomas. The recognition of this disease does not, however, permit one to give a precise prognosis. In the absence of progress concerning the pathogenesis, treatment will be the same as that given in other forms of necrotising angeitis.

Adult↗

[Retroperitoneal enteroid cyst].

The author report a case of enteroid retroperitoneal cyst treated by enucleation. Such a lesion raises mainly the problem of histological recognition and embryological interpretation for which there are two possible theories: (1) Uptake, before sealing off of the embryo, of a cell group of endoblastic origin which develops some distance from the digestive tract. (2) Retroperitoneal course of a simple teratoma of enteroid type. Such a possibility of teratoma leaves open the possibility of malignant transformation.
