Complications in hyperthermia treatment of benign prostatic hyperplasia.

Of 435 patients treated with local hyperthermia to the prostate 27 had 29 complications, for an over-all complication rate of 6.6%. Urinary tract infections (7 cases), hematuria (6), and epididymitis (2) accounted for roughly half of the complications and could be attributed to the insertion of a catheter rather than the treatment itself but this remains to be proved. Most of the complications occurred within the first 3 treatment sessions. Despite the complication, 9 patients continued and completed the treatment course. Mean age of the study group was 73 +/- 8.3 years. Given the age of these patients, high incidence of accompanying diseases and low rate of complications, local hyperthermia should be considered a viable low risk treatment option in patients with benign prostatic hyperplasia.

Local hyperthermia of the prostate gland for the treatment of benign prostatic hypertrophy and urinary retention.

Local hyperthermia of the prostate was used to treat 72 patients who had an indwelling catheter because of urinary retention caused by benign prostatic hypertrophy. One month after completion of treatment 50% of patients were able to dispense with the catheter and 1 year later 40% remained catheter-free. The best results were achieved in patients who underwent 6 to 10 treatment sessions in conjunction with cyproterone acetate 50 mg tid administered during the treatment period only.

Controlled study of cimetidine in the treatment of benign prostatic hypertrophy.

A series of 60 patients with benign prostatic hypertrophy (BPH) was randomly divided into 2 groups; 30 received cimetidine 1 g/day for 4 weeks and 30 received vitamin C 1 g/day for the same period. The study was double-blind. No significant difference between the 2 groups was observed in mean urinary flow, subjective obstructive or irritative symptoms, or prostate size. There was a statistically significant difference in the peak urinary flow rate but this was a result of net deterioration in the control group. Cimetidine was found to be no more active than vitamin C in treating benign prostatic hypertrophy.

Protease and plasminogen activator activity in human bladder carcinoma.

The ability of human bladder tissue extracts to cleave 14C-labelled globin in the absence and in the presence of plasminogen was assayed to quantify non-specific protease and plasminogen activator (PA) activity, respectively. In normal human bladder tissue the non-specific protease activity was approximately 2-fold higher than in tissue samples obtained from transitional cell carcinoma of the bladder (TCC). In contrast, PA activity was almost 4-fold higher in TCC than in normal transition cell epithelium. Acid-treated urine from 19 patients with TCC of the bladder exhibited significantly higher levels of plasminogen activator activity than similarly treated urine from controls. These results indicate that malignant transformation of the bladder epithelial tissue results in elevated levels of PA in the tissue and in urine. Further studies are needed to assess the potential of PA determination in the management of bladder cancer patients.

Morphometric evaluation of benign prostatic hyperplasia.

Resected prostate tissue from 9 patients with benign prostatic hyperplasia was subjected to a morphometric analysis. The relative volume density of 4 prostatic tissue components was determined: fibrous (47%), muscular (8%), vascular (3%), and glandular (18%). The resolution of the stromal portion of the hyperplastic prostate into 3 subcomponents represents a finer level of resolution than that previously reported. This enhanced precision in the determination of tissue composition lends new insight into the etiology of benign prostatic hyperplasia, and provides a basis for the evaluation of novel approaches to its treatment.

Local thermotherapy of the benign prostate: a 1-year follow-up.

Local hyperthermia to the prostate has initially been found to be of considerable beneficial value in cancer of the gland and later also in benign prostatic hypertrophy (BPH) and in chronic prostatitis. The Prostathermer has been especially developed and used for this purpose. 124 patients with BPH, who have been treated by this method, were reevaluated after 1 year. This thermotherapy was well tolerated, and no noticeable complications were encountered. A sustained definite improvement in objective and subjective obstructive symptoms and signs was seen in 51% of the cases. Such definite and persistent improvement was best obtained when treatments were performed twice weekly for 60 min over a period of 3 weeks. Those patients with the more severe obstructive symptoms fared best. A full explanation for the effectiveness of this treatment of BPH has yet to be found. Although local hyperthermia cannot, at this stage, be considered as a radical form of treatment of BPH, it may offer definite relief to about 50% of the cases, particularly to those unwilling or unsuitable for surgery.

[Determination of cortisol, T4, T3 and T-uptake in serum and plasma of horses using fluorescence polarization immunoassays (FIPAs)].

The influence of duration and temperature of storage on hormone levels of whole blood, plasma and serum of horses was investigated. Using FPIAs cortisol, T4 and T-uptake could be measured while the T3-FPIA did not work appropriately. Serum and Plasma stored under the same conditions did not show any difference in cortisol, T4 and T-uptake values. In frozen heparinized plasma samples analysed on different days the cortisol and T4 concentrations fluctuated markedly. The T-uptake values were rather stable. The smallest day by day changes of cortisol and T4 in plasma were found when storing the samples at 20-22 degrees C. At 4 degrees C cortisol decreased markedly while for T4 a clear trend was not found. The cortisol concentration in whole blood stored at 4 degrees C decreased more markedly than in samples stored at 20-22 degrees C. T4 remained stable in whole blood samples stored at 20-22 degrees C over 4 days while showing fluctuations in samples stored at 4 degrees C.

[Verrucous carcinoma of the penis].

Malignant tumors of the penis are rare, and constitute only 1% of all tumors in males. The disease is more common in the black population of the USA due to genetic links and the rarity of circumcision. Penile carcinoma is extremely rare in Israel, only 10 cases having been reported during a period of 20 years. Verrucous carcinoma, often called giant condyloma acuminata, constitutes a subgroup of squamous cell carcinoma of the penis. The first description was by Buschke in 1925, and was followed by that of Loewenstein in 1939. The tumor is usually limited to the penile glans and does not invade lymph nodes. We present a 44-year-old Arab with verrucous carcinoma treated by partial amputation of the penis. He had been circumcised 11 years previously. During 18 months of follow-up there was no recurrence.

[Transurethral procedures with sacral anesthesia].

Sacral anesthesia is a safe and effective procedure which we have used in 28 resections for benign prostatic hypertrophy, and in 42 resections of bladder tumors, 7 of them in women. The average age was 69 years. Average time for prostatic resection was 49 minutes and for bladder tumors, 23. Recovery from anesthesia averaged 78 minutes in both groups. In 44.3% of the patients there was no need for other medication during operation. Hemodynamic changes were minor and were controlled by fluid replacement in a third of the patients. Complications included convulsions (2 patients), severe headache (2), arrhythmias (5) and skin rash (1). There were no late sequelae. 0.5% marcaine was the most effective drug. The success rate was 77.1% in both groups; failures were due to technical difficulties.

[Renal colic and renal calculus].

A positive diagnosis of urinary stone was made in 183 of 407 patients (44.9%) with renal colic seen in the emergency room. 182 (44.7%) had normal intravenous pyelograms, 26 (6.4%) had evidence of dilatation of the urinary system, possibly due to the passage of a stone, and 16 (3.9%) had other pathological urinary conditions, such as double collecting system or cysts. Stones were located in the upper urinary tract in 28.4%, in the midureter in 21.9%, and in the lower ureter in 49.7%. There was a statistically significant correlation between recurrent episodes of left-sided renal colic, erythrocytes in the urine and the presence of a stone in the urinary tract. No ethnic differences were found, nor was there a difference in the rates of stones found in those who were hospitalized and in those who were not.

Longitudinal study on the effect of early interceptive treatment in 4-year-old children with unilateral cross-bite.

To evaluate the results of early interceptive grinding and also the possibility of self-correction of unilateral cross-bite, 76 4-yr-old children with this malocclusion were divided into one treated and one untreated group. In the treatment group 50% of the children exhibited a normal transverse occlusion evaluated in the mixed dentition, at the age of 9. In the untreated group only 17% showed a spontaneous correction of the cross-bite. Among the studied dental variables the "max./mand. arch width difference" especially in the canine region was found to be a valuable predictor indicating whether cross-bite can be successfully treated in preschoolchildren by selective grinding. The results of this study support early treatment of unilateral cross-bite and gives the criteria whereby successful treatment by selective grinding may be expected.

Relation between sucking habits and dental characteristics in preschoolchildren with unilateral cross-bite.

The relationship between sucking habits and dental characteristics of unilateral cross-bite in 4-yr-old children (n = 76) was studied. The cross-bites were classified on the variables "number of teeth involved", incisor "overjet" and "overbite", "terminal plane shift", "midline shift", "asymmetric maxillary dental arch" and "max./mand. arch width difference". Type of sucking habit (dummy, finger) and its duration and intensity were analyzed from a questionnaire answered by the parents. The "max./mand. arch width difference" in the canine region was found to be negatively related to the intensity and duration of the sucking habit, and also negatively related to sucking a dummy compared to finger sucking. The study indicates that the "duration" and "intensity" of the sucking habit have a negative influence by reducing the transverse width of the maxillary arch in children with a unilateral cross-bite. Dummy sucking was more detrimental to the transverse dimension in the canine region than finger sucking.

Incidence and pattern of bladder tumor recurrence following combined suprapubic prostatectomy and excision of a bladder tumor.

We studied retrospectively 36 patients who had undergone combined suprapubic transvesical prostatectomy for benign prostatic hypertrophy (BPH) and excision of a simultaneous bladder tumor. The follow-up period ranged from 1 to 7 years (average 3 years) and included periodic cystoscopy and voided urine cytology. Thirteen recurrences out of 60 (21.7%) occurred at the bladder neck, in 8 out of 23 patients (34.8%) with recurrent tumors. We relate the high recurrence rate located at the bladder neck to the concurrent prostatectomy. We conclude that combined surgery for BPH and bladder tumor is not recommended since it predisposes the bladder neck to tumor implantation at surgery.

[Traumatic fracture of the corpus cavernosum with urethral rupture].

Traumatic fracture of the corpus cavernosum with urethral rupture occurs rarely, mostly during intercourse. About 200 cases have been published. Some reports favor conservative treatment, but lately it has been shown that this results in longer hospitalization and to late complications in 29%. Cavernography should be used in the equivocal cases without hematuria or signs of fracture. We present a case of fracture of the corpus cavernosum and tear in the urethra. Treatment consisted of surgical repair of the tunica albuginea and stenting of the urethra. Long term results were excellent.
