[The role of retraining in urinary incontinence in women].

Sphincter rehabilitation is currently an excellent therapeutic approach to urinary incontinence in women, on the same level as surgery and medical treatment. The indication for rehabilitation must be based upon clinical and paraclinical (urodynamic and radiological studies) assessment. Sphincter electro-stimulation results in awareness of perineal contraction. Biofeedback improves the strength and persistence of contraction. Long term results of such rehabilitation can be maintained only if the patient continues exercises alone. Priority indications for rehabilitation are: postpartum incontinence, incontinence in women desiring further pregnancies, sphincter insufficiency. Results of the present study involve 74 patients with stress urinary incontinence, giving 58% good results, 22.5% improvement and 19.5% failures.

Fistulas of the upper urinary tract: percutaneous management.

We treated 40 patients with urinary fistulas by interventional radiology. The antegrade percutaneous route, catheterization of the ureter and bypassing of the fistula enabled ureteral stenting in 36 patients (90 per cent). Criteria for successful treatment were healing of the fistula, normal renal function (evaluated by excretory urography and radionuclide studies) and absence of secondary stenosis at 6 months. Of the patients 28 (70 per cent) were treated successfully. The number of nephrectomies after failure of percutaneous techniques (5 of 40, or 12.5 per cent) seems lower than in the case of surgery. The results were excellent for fistulas occurring after endourology (all 9 successful) or after ureterointestinal anastomoses (7 of 8). On the other hand, the results appear disappointing in patients with fistulas in transplanted kidneys (3 of 4 failures).

[Preperitoneal trans-ileal cutaneous ureterostomy. Apropos of 46 cases].

A technic of preperitoneal trans-ileal cutaneous ureterostomy, first described by Archimbaud in 1979, was used to treat 46 patients. Results are compared with those of a series of 12 cases reported by Bricker. Postoperative mortality was relatively high (8/46) particularly as the diagnosis in most cases (39/46) was a cancer and not a neurologic bladder. The advantages of this technic are considered to be mainly the high situation of the loop allowing irradiation of the pelvis, the stability of the length of loop, and above all, the total isolation of the uretero-ileal anastomoses external to the greater peritoneal cavity allowing percutaneous treatment of urinary fistulae without precipitation and without the need for repeat surgery.

Transcriptional control of thyroglobulin gene expression by cyclic AMP.

Transcription of thyroglobulin (Tg) gene is under the positive control of thyrotropin (TSH). The mechanism of this control has been further investigated. Rats were treated with triiodothyronine (T3) to decrease their endogenous TSH production. Following the intravenous injection of bovine TSH, a 3-fold stimulation of Tg gene transcription could be detected in isolated nuclei as early as 1 h after treatment. The TSH effect was also observed in tissue fragments incubated in vitro under conditions where a concomitant stimulation of cAMP accumulation was detected. Forskolin, a universal activator of adenylate cyclase, was able to mimic TSH action on Tg gene transcription. We conclude that TSH controls transcription of Tg gene directly via its known interaction with receptors on thyrocytes and that cAMP is a physiological mediator of this effect.

[Analysis of 278 sphincterotomies of the urethral stria in 232 male patients].

The authors report 278 external sphincterotomies performed on 232 male neurological patients, presenting with specific etiologies requiring surgery: vesico-ureteral reflux (33.6%), ureterohydronephrosis (18.9%), and spontaneous hyperreflexia (7.4%). The level of the spinal cord injury was superior to T8 in 32.3% of cases (double the corresponding figure for paraplegic patients) and inferior to L1 in 31.2% (25% more than for paraplegics). Short-term results are good in 70% of cases, in terms of dysuria and infection. Iterative sphincterotomy is required in 19.8% of cases, and surgery of the neck of the bladder is required in 34.8%. In the medium term, 83.7% of the good results show an improvement. Continence is achieved in 14.4%, and 12.2% of the patients are incontinent. The mortality rate was 1%, and 10% of the patients suffered from postoperative hemorrhage. Long-term results show 3.2% of secondary impotence, and 10% of patients requiring urinary diversion.

Molecular cloning of complementary DNA: preparation of a plasmid vector with low transformation background.

A simple method that allows the rapid preparation of oligo dG-tailed plasmid vectors is presented. The procedure involves purification of the tailed molecules by hybridization to oligo dC-cellulose followed by a stepwise thermal elution. The resulting plasmid is virtually devoid of transformation activity in the absence of oligo dC-tailed DNA fragments. It allows construction of cDNA libraries with as low as 1% of colonies harboring wild-type plasmids.

[Value of antibody coated bacteria in the localization and follow-up of chronic urinary tract infections (author's transl)].

Antibody coating of urinary bacteria was performed in order to localize the site of recurrent urinary tract infections in 39 para or tetraplegics. The antibody-coated bacteria test was positive with anti human globulin in 19 out of 39 patients: upper urinary tract infections 7/10, prostatitis 3/3, lower urinary tract infections 9/26. Of the 19 patients with positive antibody-coated bacteria test 14 were tested with monospecific antisera: 13 were IgG positive and 11 were IgA positive; IgM was never present. Among the 11 IgA positive tests, we search for the presence of IgA secretory piece in 8 and all were positive. The immunoglobulins on the bacterial wall result, in part, from local production of urinary antibody. Although positive results with anti human globulins occurred frequently in patients with lower urinary tract infections, this test appears to be a useful screening test for localization of infection in patients with recurrent urinary tract infection.

Semiological value of painful sensitivity of the testis in spinal cord injuries [proceedings].

About 85 cases of spinal cord male injured patients; the authors bring their experience of the painful sensitivity of the testis by squeezing the gland. This sign has a semiological value about the topography and the level of complete and incomplete cord syndromes. It is of no value for the testicular fertility. The rehabilitation of neurological bladders must take this sign into account so as to appreciate the type of micturition.

Differential diagnosis and limitations of external sphincterotomy.

A study of 1000 neurologic bladders has shown us that a well-balanced working of the bladder must be keenly searched for, and that sphincterotomy must be early indicated, and even performed several times if necessary. So 100 sphincterotomies performed on male patients, out of 730 cases, have shown us that their one-and-only risk, incontinence, depended chiefly of the condition of the detrusor, therefore on the short or long evolution of the sclerosis of the external sphincter, which is only one out of all the possible impediments.
