[Acquired bullous epidermolysis and multiple myeloma].

A case of epidermolysis bullosa acquisita (EBA) with negative direct immunofluorescence and multiple myeloma is reported. Epidermolysis bullosa acquisita has been associated with several systemic diseases, but only three cases diagnosed of EBA have been associated with multiple myeloma in the literature.

[Neutrophilic dermatosis in inflammatory intestinal disease].

The neutrophilic dermatosis are a group of cutaneous diseases that share similar clinical and histopathologic features. They frequently occur in association with other diseases, particularly idiopathic inflammatory bowel disease. We present herein three cases of neutrophilic dermatosis and inflammatory bowel disease. We emphasize the atypical cutaneous findings and we discuss their nosologic situation.

[Antiphospholipid syndrome in a patient with T-cell skin lymphoma].

This case report describes the simultaneous occurrence of antiphospholipid antibody syndrome in a patient with cutaneous T-cell lymphoma. An abnormal T cell-B cell cooperation is postulated to explain this unusual association. To the best of our knowledge, no similar cases have been reported previously.

[Reiter's syndrome exacerbated by indomethacin].

A 26-year-old man, with a personal history of drug abuse and positive serology for HIV, had Reiter's syndrome for six years. He experienced progressive worsening of his cutaneous lesions after initiation of indomethacin therapy. The skin lesions were almost completely resolved after the discontinuance of the drug and its reintroduction resulted in a similar deterioration. To our knowledge, indomethacin has not been reported to aggravate Reiter's syndrome. This case study documents anti-inflammatory drugs as possible causal factors for triggering Reiter's syndrome. Possible implicated mechanisms are also discussed.

[Gangrenous pyoderma. A review].

Pyoderma gangrenosum is a distinctive, uncommon, ulcerative condition with an associated systemic disease in most cases. The causative mechanism of this disease is still unknown. In this article, we review the pathogenesis of pyoderma gangrenosum, its clinicopathologic picture and the currently available therapeutic alternatives. Associations with systemic disease are also reviewed.

[Metastatic Crohn disease of the scrotum].

We report herein a rare case of metastatic Crohn's disease involving the penile and scrotal skin in a 56-year old man. We review the literature about this rare entity and describe its clinicopathological characteristics.

Acitretin (Ro 10-1670) in the treatment of severe psoriasis. A randomized double-blind parallel study comparing acitretin and etretinate.

A randomized double-blind parallel trial comparing acitretin and etretinate was performed in 20 patients with severe psoriasis during a treatment period of 12 weeks. The initial dose was 30 mg/day for 4 weeks, and the mean dose at the end of the study was slightly lower in the acitretin group when compared to the etretinate group (30.7 mg/day and 33.4 mg/day, respectively). Follow-up examinations were carried out every 2 weeks, and the efficacy of treatment was evaluated by using the PASI score. Percentage improvement in the PASI score was 50% at week 10 in both groups, and the difference between them at week 12 was insignificant. Both quantitatively and qualitatively, acitretin and etretinate do not significantly differ.

Epidermotropic primary neuroendocrine (Merkel cell) carcinoma of the skin with Pautrier-like microabscesses. Report of three cases and review of the literature.

Three cases of primary cutaneous neuroendocrine carcinoma involving the face are reported. In the first patient the tumor developed in the scar of a previously excised basal cell carcinoma of the nose, recurred locally, and metastasized to a regional lymph node 2 1/2 years after initial treatment. In the second and third patients the tumors were located on the forehead and were removed with a wide excision. In the second patient the tumor metastasized to regional lymph nodes 1 month after treatment. Light microscopic examination in all patients showed an undifferentiated, small cell, dermal tumor with prominent epidermotropism, forming Pautrier-like microabscesses. Immunohistochemical staining for neuron-specific enolase was positive, and ultrastructural examination revealed numerous cytoplasmic neurosecretory granules and paranuclear aggregates of intermediate-sized filaments. The literature concerning epidermal involvement by primary neuroendocrine carcinoma of the skin is reviewed; the relation with basal cell carcinoma and treatment are discussed.
