Continuous monitoring of panic.

Data on naturally occurring panic attacks were gathered through continuous self-monitoring for 94 patients suffering from panic disorder with agoraphobia. A total of 1276 panic attacks were collected. In this article various aspects of panic attacks, including severity, duration and time of onset and situations in which panic occurs are addressed. In addition, the symptoms of panic were investigated, examining the (in)variability of attacks within each patient and the patterning of symptoms in the entire group of patients. The most important findings were as follows: attacks occurred predominantly in nonphobic situations; nocturnal panic attacks were generally more severe than attacks during the day; symptom patterns across various attacks, stemming from the same patient, were rather variable; and finally, a substantial number of the attacks (40%) did not meet the DSM-III-R criteria for number of symptoms.

[DNA metabolism in lymphocytes of patients with lupus nephropathy].

DNA metabolism in lymphocytes was evaluated in patients suffering from the nephrotic syndrome decompensation, basing on measurement of endonucleases (DNases) activity in these cells. The examination involved 17 patients, aged 27.7 +/- 7.11 with clinical and biochemical active disease, all with the nephrotic syndrome decompensation and erythrocytes in urine. A significant rise in the enzyme activity was observed in T and B lymphocytes (p < 0.001) in all the patients, with a distinct 3-fold increase in enzyme activity in B lymphocytes, when compared with the other cell populations. To elucidate the nature of the observed changes in DNases activity in systemic lupus erythematosus, nucleus proteins of the cells were separated electrophoretically in acrylamide gradient with immobilized DNA. Degradation processes in nucleic acids were considerably more efficient than the DNA synthesis in B lymphocytes of these cells. A smaller increase in the enzyme activity in T cells than in B lymphocytes results exclusively from more intense catabolism of nucleic acids not parallel to the intensified DNA synthesis in these cells.

Preliminary results from the third flight of the Millimeter Anisotropy Experiment (MAX).

Preliminary results from the June 1991 flight of MAX are presented. Simultaneous observations were made in bands centered at 6, 9, and 12 cm-1 with a bolometric receiver operating at 300 mK. The experimental sensitivities are the highest reported at angular scales of 0.3 degrees to 1.0 degrees. Interstellar dust is observed to have an emissivity [symbol, see text] nu 1.4+/-0.3 and to correlate with the Infrared Astronomical Satellite (IRAS) 100- map. After removal of emission from interstellar dust, 1.3 hr of integration on a 6 degrees scan yields an upper limit of temperature difference Delta T/T < 2.6 x 10(-5) at a Gaussian autocorrelation function centered at 0.5 degrees. The experiment and data analysis are described.

[Q fever antibody titer--follow-up study in cattle with special reference to pregnancy].

In 290 Q fever positive cattle from three 2000 head dairy farms in the former district of Erfurt (Thüringen) the course of titers was examined serologically over several months by means of the complement fixation test (CFT). In 47.2% of the cows serologically observed for 2 up to 28 months complement fixing antibodies against Coxiella burnetii could be demonstrated til the end of the investigation period. Repeated tests during pregnancy showed increase of antibody titers in the first 4 months and after a short decrease again from the 5. til the 7. month. By observing the antibody titers during several pregnancies each time a new increase comparable to a booster immunisation could be found. This may explain the persistence of Q fever antibodies in cows for several years. The results of this investigation suggest that from a high antibody titer it can not be concluded an abortion in a positive cow being caused by a Coxiella burnetii infection.

[Late hemorrhagic disease in newborn infants. Is the current preventive treatment with oral vitamin K adequate?].

During recent years, we have observed two cases of haemorrhage due to vitamin K deficiency which developed late in the neonatal period. One patient was a female infant aged six weeks with severe intracranial bleeding and the other was a female infant aged three weeks with marked haemorrhage from the umbilicus. Both of these infants were entirely breast-fed and had received vitamin K (1 mg fytomenadion) orally at birth. Both infants had unrecognized alfa-1-antitrypsin deficiency with liver involvement. In other European countries, many cases of late haemorrhagic disease of the newborn due to vitamin K deficiency have been registered in infants who had received oral vitamin K prophylaxis. On the basis of these observations and investigations which suggest that oral vitamin K prophylaxis is not so effective as intramuscular administration, it is suggested that the present oral vitamin K prophylaxis should be altered.

Self-monitoring of panic attacks and retrospective estimates of panic: discordant findings.

An event sampling method was used to study the frequency of panic attacks during treatment of agoraphobics. Results revealed a much lower incidence of panic attacks in agoraphobics according to self-monitoring than was expected on account of their retrospective estimation. When more stringent criteria for panic attacks are applied, retrospective overestimation becomes even more apparent. The implication of this finding for the classification of panic disorder patients is discussed.

The function of agoraphobia in the marital relationship.

A secondary analysis was carried out of data from a previous experiment on the treatment of agoraphobia. This analysis shows that, contrary to some reports in the literature, agoraphobics do have more problems in the relationship with their partner than the average members of the normal population. The analysis also shows that individual-oriented behavioural treatment of the agoraphobia significantly improves the marital relationship. The findings contradict the theory that agoraphobia serves a function in the relationship with the partner.

["Fifth day fits": a syndrome of neonatal convulsions. Can this condition be defined as an independent disease entity in Denmark?].

A 12-year retrospective study to elucidate the incidence of "fifth day fits" including 127 infants with neonatal convulsions was carried out in the Randers area of Denmark. The incidence of neonatal convulsions was 8.3/1,000 live-born infants. The etiology in 74 cases of latent and 53 cases of convulsive seizures was asphyxia (38%), intracranial hemorrhage (17%), hypoglycaemia (9%), hypocalcaemia (4%), infections (6%) and developmental defects (4%). In 28 cases (22%) the etiology was unknown. In only two of these cases, the seizures started on day 4-6, and no peak incidence was found.

[Removal of a displaced nasopharyngeal tube from the esophagus using an inflated catheter].

A safe and easily employed method of removal of a blunt foreign body from the oesophagus is described. The present case was a newly born infant with Pierre Robin's syndrome and congenital heart disease. A nasal tube had become displaced and the clinical condition did not permit endoscopic removal. By means of an inflatable balloon catheter introduced through the tube to the stomach, the foreign body could be removed without difficulty.

Augmentation of interleukin 2-activated cytotoxicity after treatment of cells with inhibitors of interferon production.

Low-density human lymphocytes cultured with recombinant interleukin 2 (rIL-2) generated a high level of interferon(s) (IFN). Consistently more IFN including IFN-tau was produced during the first 3 days of culture with rIL-2 than during the subsequent 4 days. That ability was mainly associated with mepacrine+ cells and was decreased by low concentrations of leucine methyl ester (Leu-O-Me) or ammonium chloride. Leu-O-Me was employed either for the pretreatment of cells before the culture or as the additive to culture medium. The decrease in IFN production after pretreatment was associated with enhanced rIL-2-activated cytotoxicity. Similarly, 1 mM of ammonium chloride or 1 mM of Leu-O-Me added to rIL-2 supplemented cultures for 3 days showed an association between inhibition of IFN-tau generation and increased activation of cytotoxic activity. Thus NK cells appear to regulate their own response to rIL-2 activation and the control mechanism seems to be associated with the ability of the cells to produce IFN(s) and possibly other cytokines.

Recovery of natural cytotoxicity after bone marrow transplantation.

Reconstitution of non-MHC restricted cytotoxicity was followed in 7 and in 4 cases after auto- and allotransplantation, respectively. In autografted cases a high level of natural cytotoxicity, which exceeded the pre-transplant values, was seen at the beginning of hematological reconstitution. At that time, the high percentage of DR+ peripheral blood mononuclear cells (PBMC) was found, while the percentage of CD16+ cells was in the range of the pre-transplant values. A high level of cytotoxicity was a transient phenomenon. In all cases, 40 days after grafting the natural cytotoxicity was again in the range of the pre-transplant values. In allotransplantation the peak of cytotoxicity was seen soon after the beginning of hematological recovery and the activity decreased after 24 to 48 days after grafting.

[Pathomorphology of chronization and outcomes of experimental mycoplasma-viral pneumonia].

Pathomorphology of pneumonia induced by successive contamination of the Syrian hamsters with a pathogenic strain of Mycoplasma pneumoniae and an influenza virus A/PR8/34 as well as after contamination with an influenza virus against the background of a previous administration of the mycoplasma membrane fractions was studied. Mixed Mycoplasma-viral infection results in a tumor-like bronchiolar epithelium proliferation having a morphological similarity with bronchiolo-alveolar or acinar carcinoma, influenza virus infection against the background of a previous administration of protein fractions of mycoplasma membranes resulted in a chronization of pneumonia with development of several types of morphological changes: reversible tumour-like proliferation of the bronchiolo-alveolar epithelium, "inflammatory" pseudotumours and sarcoid-like granulomas.

Aggressive chemotherapy with autologous bone marrow transplantation in small cell lung carcinoma.

16 patients with an advanced stage SCLC were treated with the use of aggressive chemotherapy with bone marrow transplantation. With the gaining of experience the doses have been increased from standard-dose induction and 7 g/m2 of CTX in intensification to more aggressive induction (160 mg/kg of bw of CTX and 1.6 g/m2 VP-16 in two courses in 28 days interval) and intensification (CTX 7 g/m2 and VP-16 from 1.5 to 2.0 g/m2. Most recently, we used the following intensification which, in addition to the high dose CTX (6 g/m2), consisted of VP-16 0.9 g/m2 and BCNU 0.5 g/m2. The procedure proved to be safe. Hematological recovery emerged in all patients at a very similar time after autografting, irrespective to the late intensification regime. All cases, except one, received, after the hematological recovery, prophylactic cranial and at the primary tumor site irradiation as well as 2 to 4 courses of standard dose maintenance chemotherapy. The response rate was higher in the group receiving more aggressive induction and intensification. Long-term survival was seen only in patient which received more aggressive induction and intensification. Median survival of all cases was 13 months including 3 cases which are disease-free 24, 21 and 14 months after the beginning of the treatment.

Variants of SLE--a statistical approach for discrimination of a group of SLE cases into different subgroups sharing symptomatology. A pilot study.

Symptoms of two groups of Systemic lupus erythematosus (SLE) (Group 1 constituted 65 patients examined from 1975 to 1981; Group 2 constituted 104 patients examined from 1980 to 1988), which were diagnosed according to the American Rheumatism Association (ARA) preliminary criteria, were statistically analyzed with the use of a package of statistical programs which included computation of a matrix of correlation and cluster analysis. In both groups similar frequency and associations of SLE symptoms were seen. Hence, there was a positive correlation between kidney involvement and hematological abnormalities with nDNA Abs what was a hallmark of a severe SLE. In contrast, the symptoms of severe SLE disease was rarely seen in cases with Raynaud's phenomenon and discoid lesions. Our cluster analysis further distinguished groups on the basis of renal involvement, skin symptomatology, and polyserositis. These results also were similar in both groups of patients. That gave further credence to our results and continued to support the concept of SLE variants being distinguished on the basis of the clinical picture.

[Social and economic consequences for families with preschool children related to emergency hospitalization of children under 7 years of age. A questionnaire study].

Parents of 164 children who had been hospitalized as emergencies were asked to complete a questionnaire containing questions about the social and economical sequelae of hospitalization. The parents had, on an average, 0.36 days off work per day of hospitalization and mothers had the majority of days off work. Families with two occupationally active or a single occupationally active parent had 0.63 days off work per day of hospitalization. The most prolonged periods of hospitalization were associated with fewest days off work per day of hospitalization. The most important forms of absence from work were holidays and absence without pay, while the right which Danish parents have of absence on the first day of the child's illness formed the basis for 15% of the days off work. The majority of days off work did not result in decrease in pay but parents used their holidays or free days to which they were entitled to a great extent. Three mothers lost their jobs in connection with days off work on account of illness in the child. In addition to employment problems, there were also problems in connection with transport and in caring for the siblings. In many families negative reactions were observed in the siblings.

[Poisoning with nightshade, Atropa belladonna].

Serious poisoning with atropine-like substances may produce symptoms which can be confused with acute psychosis. The diagnosis in a case of acute poisoning may be difficult and diagnostic treatment with physostigmine may be considered. A case of serious atropine poisoning caused by consumption of the fruits of deadly nightshade (Atropa belladonna) which commenced with psychosis in a boy of nine years is described. It was known that the child had eaten 20-25 of the berries. He survived after treatment with intravenous administration of physostigmine.
