BACKGROUND: Patient education has been accepted widely over the past years as a valid component of disease management in patients with chronic diseases. The aim of our study was to evaluate the effects of patient education in patients with inflammatory bowel disease. Participation in a patient education program should increase the patient's disease-related knowledge and positively influence the patient's quality of life, depression, beliefs of internal control and social activity. METHODS: We studied 36 patients with Crohn's disease and ulcerative colitis: 18 were educated in four sessions, 18 were not educated (control-group). The following topics were presented: diagnosis, causes and course of disease; medication and surgery; nutrition and social benefits and stress management and coping with the disease. RESULTS: The effects of the program were evaluated before, immediately and three months after the intervention using questionnaires. Participants in the program showed a significant increase in knowledge after three months compared with the non-participants. There were positive, although not significant effects on psychosocial variables. CONCLUSION: Patient education is a valuable means to increase knowledge of disease and treatment in patient's with Crohn's disease and ulcerative colitis. Effects on psychosocial variables need to be examined on a long term basis in larger patient groups.

The present activity of BM/PBPC transplantations is based on government funded transplantation centers. About 50 alloBMT/PBSC or 1.3 transplant per million per year were performed in the last two years with a tendency to further increase. An increase in the number of autoBMT/PBSC is also seen with about 75 autotransplantations or 2 transplantations per million of people per year. Indications and standard laboratory and clinical practices in Poland are similar to those in other European countries. However, chemotherapy prevails over total body irradiation (TBI) in conditioning regiments. An average long term overall survival of patients with hematological malignancies after alloBMT is higher in paediatric transplantations than in adults (0.56 vs 0.34, p < 0.03) and in patients with a standard risk than in cases with advanced disease (0.48 vs 0.36).

Data are presented on the factorial structure, internal consistency, and validity of the Dutch adaptation of the Buss-Durkee Hostility Inventory (the BDHI-D). Factor analyses of the responses of 463 subjects revealed two scales measuring Overt Aggression and Covert Aggression. The reliability of both subscales is good. Concordance with other self-report measures reveals satisfactory convergent and divergent validity.
The validity of the Dutch adaptation of the Buss-Durkee Hostility Inventory was investigated. On the basis of the ratings of the staff of a residential treatment center for adolescents with conduct disorders, two contrast groups (high versus low aggression) were formed. In all, 67 male adolescents participated in the study. The subscales of the inventory clearly discriminated between the two groups. A discriminant function analysis revealed that the inventory could correctly classify the subjects into aggressive and non-aggressive with 96% accuracy. The pattern of correlations between the inventory and an anger scale and a social desirability scale provide further support for the construct validity of the instrument.
Peripheral blood mononuclear cells (PBMC) and bronchoalveolar lavage (BAL) cells obtained from sarcoidosis patients were investigated for the presence of mRNA for IFN-gamma. RNA was extracted from freshly obtained cells. In 12 cases PBMC were studied and in seven cases BAL cells were also available for concomitant investigation. In eight patients mRNA for IFN-gamma was present in blood and was significantly associated with stage I disease (P = 0.030). By comparison, BAL cells were less frequently positive for IFN-gamma transcripts (P = 0.039). Only one patient had simultaneous expression of IFN-gamma gene in blood and BAL cells. All other patients including four positive in blood were negative in BAL. The presence of IFN-gamma transcripts in PBMC was significantly associated with the capacity of these cells to generate IFN-gamma in unstimulated cultures. PBMC from patients showing IFN-gamma mRNA in blood were frequently high IFN-gamma producers in anti-CD3 MoAb-stimulated cultures.
By means of a recently constructed anamnestic instrument, the questionnaire Childhood Unwanted Sexual Events (the CHUSE), the incidence of childhood experiences with sexual threat and/or abuse is investigated among 152 female psychiatric patients. The construction and applicability of the questionnaire are described. A Guttman scale-analysis shows a unidimensional construct (severity of the sexual abuse) for the CHUSE. Within this psychiatric population, sexually abused women report significantly more psychopathological symptoms than non-abused women. The correlation between severity of the abuse and severity of the psychopathological symptoms is investigated. The use of questionnaires concerning sexual abuse is compared with the more common interview techniques, and suggestions for future research are given.
OBJECTIVE: The purpose of this comparative outcome study was to investigate whether the effects of exposure in vivo treatment for panic disorder with agoraphobia could be enhanced by adding interventions specifically for panic attacks before the start of exposure treatment. The additional effect of two types of treatment for panic attacks--pharmacological (fluvoxamine) and psychological (repeated hyperventilation provocations and respiratory training)--was examined. Thus, the combined treatment of panic interventions with exposure in vivo could be compared to exposure in vivo alone. METHOD: Ninety-six patients were randomly assigned to four treatment conditions: double-blind, placebo-controlled fluvoxamine followed by exposure in vivo, psychological panic management followed by exposure, and exposure in vivo alone. Outcome was assessed by self-report measures, a standardized multitask behavioral avoidance test, and continuous monitoring of panic attacks. Seventy-six patients completed the study. RESULTS: All four treatments were effective and resulted in a significant decrease of agoraphobic avoidance. Moreover, the combination of fluvoxamine and exposure in vivo demonstrated efficacy superior to that of the other treatments and had twice as large an effect size (difference between pre- and posttreatment scores) on self-reported agoraphobic avoidance. The other treatments did not differ among each other in effectiveness. CONCLUSIONS: Results of the study indicate that the short-term outcome of exposure in vivo treatment can be enhanced by adding fluvoxamine treatment. Psychological panic management combined with exposure was not superior to exposure alone of equal duration.
Thirty-two patients suffering from panic disorder with agoraphobia were treated with repeated hyperventilation provocations and respiratory training, followed by exposure in vivo. The treatment was evaluated with a comprehensive set of outcome measures, including self-report questionnaires, a multitask behavioural avoidance test and continuous monitoring of panic. The treatment was found effective for the majority of patients in diminishing the frequency of panic attacks and agoraphobic avoidance. The clinical relevance of the treatment effect was evidenced by the considerable number of patients that recovered. The effect of the treatment was sustained over a three and six month follow-up period. The prognostic value of a number of variables for treatment outcome was also investigated. Three variables accounted for the majority of the variance in treatment outcome: a higher pretreatment level of agoraphobic complaints, use of psychotropic medication and a longer duration of the disorder were associated with poorer outcome. Other variables, such as the therapeutic relationship and the quality of the marital bond, had no prognostic value.
49 cases were reviewed with respect to the risk factors determining the outcome of alloBMT. The presence of HLA/MLC matched sibling or alternatively genetically haploidentical but phenotypically matched family or unrelated donor makes the transplantation possible. In the latter situation matching at the level of genomic DNA is advisable. Among prognostic factors stage of the disease and age of recipients play a significant role. More aggressive conditioning regime is associated with a risk of severe GvHD. A role of microbial status in promoting GvHD was discussed. An overall long term cumulative was 45% being higher in children (65%) and in patients in early stage of the disease (51%). Acute GvHD causes majority of losses (24%). Mild or moderate GvHD was associated with the highest survival due to a low risk of both transplant-related mortality and relapse. Unfortunately, a high proportion of patients were infected with hepatitis B or C virus.
The results of allogenic bone marrow transplantation (allo-BMT) in 26 children with ALL treated with the same initial- and relapse-BFM-protocols, but transplanted in different centers (Poznań, Wroclaw, Hannover) after conditioning with two different regimens have been compared. Ten children (6 in Poznań, 4 in Wroclaw) were conditioned for BMT with busulfan and cyclophosphamide when fractionated TBI (FTBI) was not available there. Sixteen children obtained FTBI and etoposide (11 in Hannover, where in children with ALL exclusively chemoradioconditioning regimen has been employed, and 5 in Poznań). It has been found, that the chemoconditioning procedure was probably less effective (5-year EFS 18%) than the regimen with FTBI (7-year EFS 60%). Therefore, TBI seems to be at this point still mandatory in pediatric ALL patients, unless combinations of chemotherapeutic drugs might be able to substitute the radiation in the future.
Analysis of skin biopsy specimens for the presence of adhesion molecules, composition of cellular infiltrates, Ki-67 antigen expression, and examination of serum for interleukin 6 (IL-6) and tumor necrosis factor alpha (TNF-alpha) levels, wes performed in patients after allogeneic bone marrow transplantation (alloBMT), to study the pathomechanism of acute graft-versus-host disease (aGvHD). It was found that: 1) early hematological recovery constitutes a risk factor for grade IV GvHD, 2) vascular cell adhesion molecule-1 (VCAM-1) is present in the matrix organizing the cells in the bone marrow and in aGvHD infiltrates, 3) HLA DR antigens aberrant expression in epithelial cells, as well as 4) strong expression of Ki-67 is seen in early stages of aGvHD. These immunopathomorfological lesions are cytokine-dependent. High levels of IL-6 and TNF-alpha were found in sera of patients affected with the aGvHD process and infectious complications. An increase of IL-6 in the course of aGvHD is a sign of poor prognosis. These data support the notion that cytokines facilitate the cell accumulation at the site of aGvHD at the beginning of this process and again, at the final stage of the disease, cytokines high levels are associated with the organ damage.
