[A case of carcinoma of the esophagogastric junction with marked eosinophil infiltration].

A case of carcinoma of the esophagogastric junction with marked eosinophil infiltration is reported. The patient was a 56-year-old man. He had complained of abdominal discomfort for three months. After examination, a total gastrectomy was performed. Carcinoma of Borrmann III type was found at the esophagogastric junction. Pathohistological findings showed that mucinous adenocarcinoma and marked eosinophil infiltration were present in the primary lesion and metastatic lymph nodes, but there was no evidence of eosinophilia. These results suggested that the tumor might produce eosinophil chemotactic factor. However, the factor could not be detected.

[A case of resected esophageal cancer with systemic tumor microembolism].

A 54-year-old man developed severe respiratory failure and died four months after surgery of esophageal cancer. The patient showed clinical symptoms of subacute cor pulmonale as reported by Brill and Robertson. Autopsy revealed the presence of tumor microembolism in the small arteries of the lung, heart, liver, etc. Generalized tumor embolism in the arteries, unlike those in the veins, has rarely been reported. The mechanism of systemic tumor embolism in this case was assumed to have been transpulmonary passage of tumor cells into various organs without causing pulmonary metastasis.

Primary gastrointestinal lymphoma. A clinicopathologic study of 102 patients.

Clinicopathologic findings in 102 patients with primary gastrointestinal lymphomas were reviewed. Abdominal pain was the common presenting symptom. The primary sites of the tumors were: 67 in the stomach, 24 in the small intestine, 7 in the ileocecal region, 3 in the large intestine, and 1 in the esophagus. The disease more frequently affected males than females and showed peak incidence in the 5th decade of life. Gastric lymphomas usually presented with a single lesion, but multiple lesions were frequent in the small intestine. The body and/or antrum of the stomach were the commonest sites of the lymphomas. Gastric lymphomas were diagnosed at an earlier stage than intestinal lymphomas. There was 1 case with Hodgkin's disease. The remaining 101 patients with non-Hodgkin's lymphomas were classified according to the Rappaport and the Kiel classifications. The proportion of nodular lymphomas in the present series was 7%. The frequencies of diffuse histiocytic type ard germinal center cell tumors were 62 and 74%, respectively. Cox's multivariate analysis for prognostic factors revealed that the stage of the tumor, sex, and age were prognostically significant.

[Successful treatment of mediastinal seminoma with combination chemotherapy of cis-diamminedichloroplatinum (CDDP), vinblastine (VBL), and bleomycin (BLM)].

Complete response (CR) has been achieved in a case of advanced mediastinal seminoma using PVB therapy (CDDP, VBL, BLM). A 21-year-old man visited our department with the complaint of superior vena cava syndrome. Chest X-ray film, ultrasonography and CT scan revealed a large anterior mediastinal mass. Histological proof of mediastinal seminoma was obtained by needle biopsy. The patient was treated with a combination chemotherapy, consisting of CDDP, VBL and BLM according to Einhorn's PVB regimen. After three courses of this regimen, complete disappearance of the tumor was obtained. The patient underwent further radiation treatment (Linac: 4,000 rad), as a salvage therapy, at the anterior mediastinum, and has since been doing well without any sign of recurrence for a follow-up period of nine months.

Production of a monoclonal antibody to a membrane antigen of human T-cell leukaemia virus (HTLV1/ATLV)-infected cell lines from a systemic lupus erythematosus (SLE) patient: serological analyses for HTLV1 infections in SLE patients.

Human T-cell leukaemia virus (HTLV1/ATLV), which causes adult T cell leukaemia (ATL), is an infectious, lymphotrophic retrovirus unique for humans. The present study was undertaken to determine whether HTLV1 had any pathogenetic role for systemic lupus erythematosus (SLE). The incidence of antibodies to ATL cell-associated antigens (ATLA) in sera from patients with SLE and other collagen diseases was investigated by an indirect immunofluorescent cytoplasmic staining of an HTLV1-infected cell line (MT-1). A radioimmunoassay was also performed to detect antibodies to HTLV1 protein and crude membrane fraction derived from an HTLV1-producing cell line MT-2. Furthermore, an Epstein-Barr virus (EBV)-transformed B cell line (ES-1) was constructed from an SLE patient, which produced a monoclonal antibody (IgG, lambda) reactive to an HTLV1-related cell-membrane antigen expressed on MT-1 and MT-2 cells. The specific reactivity of the monoclonal antibody was analysed by an indirect immunofluorescent cell-membrane staining and a microcytotoxicity test. The incidence of anti-ATLA antibodies was not different among SLE and other collagen diseases. The monoclonal antibody produced by ES-1 stained and killed HTLV1-infected cell lines specifically, but did not react with other human lymphoid cell lines. This monoclonal antibody failed to react with peripheral blood mononuclear cells (PBMC), mitogen-induced T cell blasts, and iododeoxyuridine-treated T cells from SLE patients. Thus, a possible role of HTLV1 in the aetiology of SLE was not established.

[The usefulness of prolonged high dose delayed contrast computed tomography for the diagnosis of intracranial angiographically occult vascular malformations].

It is difficult to diagnose angiographically occult vascular malformations. When conventional CT (plain & contrast enhancement) can not demonstrate the lesions, it was impossible to diagnose in the past. We developed a new technique which enabled us to diagnose them. Materials and methods; Seven cases of spontaneous intracerebral hemorrhage, in which no abnormality was detected by repeated magnified serial angiography with subtraction technique and prolonged injection technique, were examined. In each case, conventional CT (plain & enhancement using 100 ml of 60% meglumine iothalamate) was performed first, which was immediately followed by the administration of 220 ml of 30% meglumine iothalamate for one hour. CT is taken at the end of the infusion. It, we call, is prolonged high dose delayed contrast CT: PHDD-CT. Total dose of iodine used in this technique was 59.22 gI (1.0 gI/kg body weight: BW). Results; Contrast enhancement effect of PHDD-CT was much better than that of conventional CT in all the cases. In three cases, the lesions were more clearly delineated in PHDD-CT. In four cases, only PHDD-CT could demonstrate the lesions. No side effect was observed. Comments; Several techniques for better enhancement have been reported, however they used large volume of contrast medium such as 1.5 or 2.0 gI/kg BW. Our technique can be performed with more security. Based on our good results, we recommend to use this PHDD-CT technique for the diagnosis of angiographically occult vascular malformations.

Monoclonal antihuman thyroglobulin antibodies.

Six murine hybridomas secreting monoclonal antihuman thyroglobulin (Tg) antibodies (TAK 1-6) were established by cell fusion techniques. Solid phase RIA was employed to detect the anti-Tg antibody in culture supernatants of hybridomas. The characteristics of these monoclonal antibodies were analyzed by radioimmune blocking assay using rat Tg, human glycoproteins, thyroid hormones, and various preparations of Tg obtained from patients with thyroid disease as inhibitors. In the same system, competitive inhibition studies between 125I-labeled and unlabeled monoclonal antibodies were carried out to determine whether these antibodies recognized the same antigenic determinant. TAK 2 and 3 reacted with human Tg specifically and had equal binding activity using various preparations of human Tg. The other four monoclonal antibodies (TAK 1, 4, 5, and 6) cross-reacted with xenogeneic Tg (rat Tg) and their affinity for human Tg increased as the iodine content of Tg increased. Tg binding to TAK 1 and 4 was inhibited by T4, whereas Tg binding to TAK 5 and 6 was not inhibited by any thyroid hormone or their precursors. In conclusion, we prepared six monoclonal antihuman Tg antibodies. One group is specific for human Tg and recognizes the framework structure unmodified by iodination; the second group reacts with iodination-related epitopes other than iodoamino acids, and the third group recognizes determinants consisting of T4. These monoclonal antibodies provide important probes to detect the polymorphism of human Tg.

Is there multiplicity in I-J subregion products?

Recent studies in molecular genetics have revealed the striking fact that the previously known I-J subregion (or I-J gene) does not exist at the exactly prescribed position in the I-region of the murine major histocompatibility complex (MHC). How, then, can we comprehend the established serological and functional significance of I-J genes and I-J products? To answer this question we reexamined systematically the specificities and functional activities of monoclonal anti-I-J antibodies. A series of monoclonal anti-I-Jk antibodies were newly established by fusion of B10.A(3R) spleen cells immune to B10.A(5R) lymphoid cells with P3X63-Ag8-653. Their abilities to eliminate known functions of T cell subsets and to react with I-J+ T cell clones of defined functions were examined in an in vitro secondary antibody response and by fluorescence-activated cell sorter analysis. The monoclonal antibodies (mAb) were divided into three groups: (1) those reactive with suppressor inducer T cells (Tsi), Tsi hybridomas, and the antigen-specific T cell factor (TsF) derived from them; (2) those specific for suppressor effector T cells (Tse) and Tse clones; and (3) those reactive with some but not all helper T cells. The determinants detected by these three different groups of antibodies are apparently present on separate, nonoverlapping cell populations and functionally distinct clones. The above results indicate the multiplicity of I-J products expressed on different T cell subsets, and sharply contradict the notion, derived from molecular genetics, that not even a single gene can be accommodated in the I-J subregion. To resolve this dilemma, we compared the above results with those obtained with another set of mAb that also detected I-region-controlled determinants on augmenting and helper T cells. Although the specificities of these mAb clearly mapped to within the I-region, none of them reacted with conventional class II Ia antigens of B cells and macrophages. The common properties shared by these anti-Ik and anti-I-Jk antibodies are that (1) they react only with T cells and T cell clones with I-region-controlled functions; (2) they can block some of the Ia-restricted cell interactions, including those of helper and suppressor T cells; and (3) they inhibit the syngeneic and/or allogeneic mixed lymphocyte reaction (MLR) by blocking the responder but not the stimulator cells.(ABSTRACT TRUNCATED AT 400 WORDS)

Hereditary deficiency of OKT4-positive cells: studies for mode of inheritance and lymphocyte functions.

Two Graves' patients were found to have no OKT4+ cells in their peripheral blood lymphocytes (PBL). However, the reactivity of their lymphocytes with T4(T4A) and anti Leu-3a was normal and no autoantibodies to the OKT4 determinant were found in their sera. Cytofluorographic analysis of PBL from their family members showed three types of immunofluorescence profiles with OKT4. The first type was the complete OKT4+ cell deficiency, the second was the normal percentage of OKT4+ cells with half immunofluorescence intensity and the third was the normal staining pattern with OKT4. Phytohaemagglutinin (PHA) and pokeweed mitogen (PWM) induced blastogenesis; PWM induced IgG synthesis, autologous and allogeneic mixed lymphocyte reaction, and Interleukin-2 (IL-2) production of their PBL were also normal. These results suggest that (i) the expression of determinant to OKT4 is transmitted as autosomal incomplete dominant trait and (ii) OKT4+ cell deficiency is not accompanied by a lack of the inducer/helper subset of T cells.

Biological activities of antitrinitrophenyl and antidinitrophenyl mouse monoclonal antibodies.

The biological activities of mouse IgG1, IgG2a, IgG2b, and IgE monoclonal were investigated. As has been shown with conventional antibodies, monoclonal IgG1 and IgE are the two classes capable of sensitizing the mouse for anaphylactic reactions, IgE sensitizes the rat, and IgG2a sensitizes the guinea pig. What has not been seen with conventional antibodies is that large amounts of monoclonal IgG2a or IgG2b can sensitize the mouse, very large amounts of monoclonal IgE can sensitize the guinea pig, and very large amounts of IgG1 and IgG2a can sensitize the rat. One of the IgG1 monoclonal antibodies was capable of fixing complement by the classical pathway.

Annual changes of vitamin B1, biotin and vitamin B12 in water in Lake Biwa.

Annual changes in the concentrations of B1, biotin and B12 were investigated at the designated stations in the north basin (main basin) and south basin of lake Biwa. Sampling was carried out twice a week in spring and monthly in the other seasons throughout one year in the south basin, and in the same way for three years in the main basin from April of 1978 to December of 1980. The concentrations of B1, biotin and B12 in water of the main basin over three years ranged from 106.6 to 4.1 ng/liter, from 4.17 to 0.14 ng/liter and from 5.78 to 0.19 ng/liter, respectively. On the other hand, those in the south basin ranged from 268.7 to 9.7 ng/liter, from 11.50 to 0.12 ng/liter and from 6.06 to 0.20 ng/liter, respectively. The concentrations of these vitamins in the south basin of the lake were twice as high as those in the main basin. The concentrations of these three kinds of vitamins in water remarkably changed seasonally in both the north and south basins of the lake throughout the investigation period and were generally high in September and October at every station in the main basin, except for B1 and biotin in the south basin of the lake.
