We report on the therapeutic, functional and cosmetic late results of 103 patients with 117 basal-cell carcinomas treated with soft radiotherapy (total dose: 4800-6000 R). After an average of 9.1 years 95.4% of 109 controlled primary basal-cell carcinomas could be healed and of the retreated basal-cell carcinomas 62.5%. Usually slight functional disturbances were shown in 10 cases (8.5%). The cosmetic late results were judged as good or very good in 57.2%, as average in 24.8% and in 11.1% as bad, whereas big differences were made in regard of location.
An attempt to treat a patient with pemphigus vulgaris by large volume plasma exchange with intravenous application of high doses of immunoglobulin at the end of each exchange procedure is reported. The therapeutic results--temporary clinical improvement parallel to a significant reduction in the level of circulating antiepithelial autoantibodies--are discussed in comparison with published case reports on plasma exchange in pemphigus vulgaris and bullous pemphigoid.

In the investigations of Krebs and Schaltegger between 1964 and 1972 on the structure specificity of the antipsoriatic anthrones it could be shown that at least a 1-hydroxy-9-anthrone is necessary for their efficacy. This structure was then called "minimum structure of the antipsoriatic anthrones'. Since even very minor changes of this structure in most cases lead to inactive compounds, only a few antipsoriatic anthrones have been found so far. Their most important representative chrysarobin, anthralin and I-hydroxy-9-anthrone have been known for more than 60 years. The later discovered antipsoriatic anthralin derivatives, triacetoxy-anthracene and 10-acyl-anthralin, are probably hydrolysed in the skin and thus act as their parent compound anthralin. The strong structure-activity dependence of the antipsoriatic anthrones seems to include a highly specific and complex mechanism of action.
Four patients with lymphoma-like skin diseases of a protracted course and dubious dignity are reported and reviewed together with similar cases of the literature. These cases correspond largely to the entity termed reticulohistiocytoma which was described by Crosti in 1951. The ultrastructural, histochemical, and immunohistological findings indicate tha the tumoral infiltrate consists of a cell population with close similarities to histiocytes. A possible explanation for the characteristic protracted evolution is an immunologic mechanism is causing an increased cell death rate.
Initial rate kinetic studies on phosphoenolpyruvate carboxykinase of Escherichia coli are consistent with either of two sequential kinetic mechanisms: rapid equilibrium random, or partially ordered. However, the kinetics of isotope exchange at chemical equilibrium allows clear discrimination in favor of a random mechanism containing a preferred pathway of substrate addition and product release. All plots of exchange rates vs. reactant concentrations leveled off at constant rates at saturating levels of substrates; since complete inhibition of any exchange was not observed, a compulsory sequence may be eliminated. High concentrations of phosphoenolpyruvate and (or) ATP repressed the oxaloacetate in equilibrium bicarbonate exchange, however, indicating that the latter pair of substrates add most favorably to the free enzyme. Exchange rates between various substrate-product pairs were not identical, ruling out a formally rapid equilibrium random mechanism with rate-limiting interconversion of the central complex. A comparison of the relative rates of the overall reaction and the ATP-dependent oxaloacetate in equilibrium bicarbonate exchange showed the latter to be much faster than net formation of oxaloacetate. The requirement for ATP in promoting this exchange may be rationalized in terms of substrate synergism, with occupation of the ATP binding site a requisite for its catalysis.
Culturing herpes simplex virus is rapid and economic for establishing an etiological diagnosis. Serological tests are only of limited value. Differentiation of type I and type II strains is necessary in the course of therapeutic studies. Analysis of the herpes simplex virus genom with restriction enzymes offers a novel tool for epidemiological and clinical investigations.

Lichen myxedematosus and scleromyxedema are rare disorders with cutaneous deposition of mucin, without any disturbance of the thyroid function. They are regularly associated with the presence of abnormal proteins in the serum. These proteins have been now identified as paraproteins and are one of the main symptoms of these diseases. The association of a papular mucinosis with a classical plasmacytoma is exceptional. We report a patient in whom lichen myxedematosus is associated with a multiple myeloma of the type IgG/kappa. Cultured skin fibroblasts show a marked decrease of arylsulfatase A activity. An antineoplastic-steroid therapy has brought a remission of both plasmacytoma and skin lesions.

A clinico-pathologic study on 12 patients with subungual malignant melanoma (SUM) is presented and cases from the literature are reviewed. The difficult diagnosis of SUM as well as the differential diagnosis is discussed. The SUM sometimes called "melanotic whitlow", is often misdiagnosed and therefore may worsen the prognosis. Every persistent nail disorder which does not respond to treatment should rise suspicion of SUM, especially if not pigmented.
A 66-year-old male patient with non-insulin-dependent diabetes of probably 20 years' duration presented with necrolytic migratory erythema, stomatitis, anemia and weight loss. Plasma-glucagon concentration measured with Unger's antibody 30-K was 8500 pg/ml, representing a hundredfold elevation. Two thirds consisted of high molecular glucagon fractions (10 000--40 000 Dalton). This may be an important index for detection of glucagonoma with endocrine activity. After excision of the glucagonoma the clinical syndrome was reversed and the patient recovered completely. Histological and histochemical investigation confirmed that the tumor was a glucagonoma. Despite complete removal of the tumor and a normal plasma glucagon concentration, the diabetes remained unchanged. Excessive hyperglucagonemia does not appear to play a primary role in the pathogenesis of this patient's diabetes.
A morphological study on the ultrastructure of cutaneous nerves has been carried out on 5 patients with florid psoriasis. With the electron microscope we compared nerves in the dermis from psoriatic plaques, clinically normal looking skin from psoriatics, and skin from healthy people. No qualitative difference could be found in any of these three groups. Nor did the quantitative study show any difference in the ratio of myelinated to unmyelinated axons, or in the distribution of the fibre diameter of both sorts, or in the degree of myelination. As concerns possible qualitative changes in psoriatic plaques after treatment and subsequent recovery, we have not observed any change in the small myelinated dermal nerves.
