[Current findings on drug eruptions].

Drug eruptions are the result of a systemic application of medicaments. They depend on immunologic and non-immunologic mechanisms. With the exception of a few medicaments, skin tests with drugs are not very reliable. Patch-tests may only be recommended with regard to a macular or a maculo-papular rash, because cell-mediated immunologic mechanisms are involved. The most valuable in vitro test is still the lymphocyte transformation test. New in vitro test procedures, e.g. based on lymphokines, are under investigation. Regarding the clinical practice, drug eruptions brought about by antibiotics, cytostatics, modern anti-rheumatics and anti-inflammatory agents, some psychotropics, as well as newer cardiovascular drugs are of special interest. All these important groups of drugs are constantly expanding. Thus the dermatologist has to reckon with their cutaneous side effects.

Diclofenac sodium: blood concentration of the slow-release form and influence on the metabolism of kallikrein.

The blood concentration of the slow-release formulation of diclofenac sodium (Voltaren) has been determined in 10 patients at various intervals over a period of 22 days. In parallel, the influence of diclofenac on the kallikrein-kinin system was investigated since important interactions between the prostaglandins and kallikrein are known. The peak concentration of the slow-release formulation was significantly decreased compared with data of the standard formulation from other studies and the peak was observed later. The areas under the concentration curves were, however, similar. No signs of drug cumulation were observed during the treatment. The kallikrein concentration decreased after one week in plasma and after two weeks in urine. The plasma concentration of kallikrein reaches control level again after two weeks. Its urine excretion was increased to control levels after the third week. These changes in the kallikrein-kinin system did not reach the level of significance. It is concluded that diclofenac sodium has only a short-term influence on the kallikrein-kinin system.

[Elevation of the serum concentration of alpha- and beta-globulins and of a particular carcinoembryonic antigen by ultrasound exposure of liver tissue and CEA-active carcinomas].

The concentration of alpha- and beta-globulins in the serum can be increased by exposure of the globulin-producing tissue of the liver to ultrasound. The ultrasound intensities producing this effect are harmless according to our present knowledge. Especially the concentration of the globulin carcinoembryonic antigen in the serum can be increased significantly by exposure of CEA-producing carcinomas to ultrasound. There is to investigate the possibility of the early detection of CEA-producing tumors using a provocation test by way of increasing still normal spontaneous CEA-concentrations in the serum by insonation of body regions in question to significant pathologic serum concentrations.

What means a sporadic positive sputum culture with scanty colonies of mycobacterium tuberculosis? (From the IUAT trial on preventive chemotherapy of fibrotic lesions).

Sporadic positive cultures, with only a few colonies, in persons with fibrotic lesions, even with stable x-ray lesions may indicate "an isolated relapse of doubtful clinical significance" (7); but in agreement with (7), our findings show that the frequency of consecutive break-down, earlier or later, is so high that either careful follow-up or sufficient preventive therapy, with consideration to drug sensitivity of the bacilli, is mandatory.

[Thalidomide in dermatology].

Based on present publications we review indications of the therapy of dermatoses with thalidomide as well as possible mechanisms of action and side effects of this drug. In reactional states of leprosy the use of thalidomide is established. Further indications are chronic cutaneous lupus erythematosus, prurigo nodularis, and eventually recurrent aphthosis and certain photodermatoses not responding to usual treatment. Therapeutical trials of thalidomide in diseases in which such a treatment is only occasionally or not at all mentioned in the literature will be reported. Concerning the mechanisms of action emphasis is put on a possible immunosuppression by thalidomide. Among the side effects the thalidomide neuropathy is stressed.

[Merkel cell carcinoma (trabecular carcinoma) of the skin].

The Merkel cell carcinoma was first designated some years ago by the descriptive term trabecular carcinoma. Both names refer to a skin tumor occurring in elderly patients. This is another example where ultrastructural differentiating criteria are necessary for a definite diagnosis i.e., identification of so-called neurosecretory-like granules by electron microscopy. We report clinical, histological, ultrastructural, and histogenetic aspects of such a disease in a woman suffering from a metastasizing Merkel cell carcinoma.

[Syphilis, infection and serology].

The in- and out patient departments of Dermatology report in Switzerland every year between 300 to 400 newly diagnosed cases of syphilis--mostly primary syphilis--to the federal public health service. The serological tests decisive for such a diagnosis have been fundamentally changed in the last few years. Among the classical tests only the cardiolipin complement binding test (Card-CFT) and the VDRL test remain still valuable. They are suitable for controlling the course of disease after treatment. Nowadays two tests are essential for the diagnosis, i.e. the TPHA and the FTA-ABS. They are highly specific for syphilis and become already positive between the 2nd and the 3rd week after inoculation. On the other hand these tests are of no value for the follow-up after therapy. A positive FTA-ABS-19S-IgM test indicates the necessity of treatment. Practical examples may illustrate the interpretation of different reactions.

[Maxillo-basal injury causing a bicondylo-angular fracture and carotid-cavernous fistula].

The authors present the case of a patient who suffered a violent maxillo-facial trauma. She suffered four mandibular fractures. During the postoperative course there was the development of very marked exophthalmos of of the right eye which was due to a carotido-cavernous fistula. The case has a double interest:--stomatological, osteosynthesis of the angular fractures being achieved by miniaturised screw-plates which ensured a solid mandible and facilitated postoperative intensive care;--neuro-ophthalmological, since the carotido-cavernous fistula closed spontaneously after arteriography during which intermittent compression was applied to the internal carotid.

[The role of Mycobacterium bovis in epidemiology of tuberculosis].

After a short review of opinions concerning the role of Mycobacterium bovis in tuberculosis of men in the past and of earlier findings in German Democratic Republic, the results of species determination of mycobacteria from 5210 patients living in the capital Berlin and in the districts of Frankfurt/oder, Neubrandenberg, Potsdam and Rostock - obtained between 1975 and 1980 -, are presented. In children, tuberculosis has generally practically disappeared, and thus disease due to Mycobacterium bovis too. In adults, the proportion of diseases caused by Mycobacterium bovis did not yet decrease, in spite of eradication of tuberculosis in cattle obtained 10 years ago. In the rural districts, pulmonary tuberculosis in males is caused in between 4.6 and 9% by Mycobacterium bovis (highest value in one age group 15.8%). Obviously, there is a long latent period between termination of exposition and disappearance of disease by Mycobacterium bovis in human beings.
