[Partial cystectomy for bladder carcinoma: lyophilized human dura as a bladder wall substitute].

Ten cases were treated by partial cystectomy for bladder carcinoma (anterior wall 5 cases, dome 1 case, dome to anterior wall 2 cases, lateral wall 2 cases) and these defects were covered with lyophilized human dura (LHD). A few minutes before use, LHD was put into saline for rehydration and, for anastomosis, we used 2 zero plain catgut. In eight cases cardiovascular complications, DM and liver cirrhosis were found before operation. The mean operation time +/- SD was 109 +/- 26 minutes, and the indwelling catheter was left in place for 15 to 42 days. In three cases, prolonged urine leak from the wound continued after operation, and in one case temporal VUR was found without clinical significance. No severe urinary tract infection was found in all cases. The inner surface of dura was almost epithelialized after 8 weeks, 8 cases were in good clinical condition but one case was treated by TUR for recurrence of bladder tumor 1 year and 3 months after operation. Our results with LHD as a substitute for the bladder wall have been satisfactory. The main advantage of this technique is that large vesical segmental resection is possible to such an extent as to satisfy Prout's criteria (at least 3 cm of apparently normal bladder are available to be excised around the tumor).

[A case of bilateral germ cell tumor of the testis].

A 28-year-old man visited our hospital with a complaint of left testicular pain and swelling. Left orchiectomy was performed, and its pathological finding was embryonal carcinoma (seminoma in a small portion). Though no sign of metastasis was detected and the serum tumor markers, alpha-fetoprotein, and beta-human chorionic gonadotropin (HCG) decreased in value as expected, he has undergone two courses of chemotherapy (PVB regimen). Twenty months after operation the patient found contralateral testicular induration, and serum beta-HCG was abnormally high, 36.0 ng/ml. We performed right orchiectomy. The pathology revealed pure seminoma. He has been followed up without adjuvant therapy.

[Female urethral diverticulum calculi: a case report].

A case of urethral diverticulum with calculus in a 53-year-old woman is reported. She was admitted to Isahaya General Hospital complaining of a localized swelling in the external genitalia. We found a urethral diverticular calculi by urethrography. Urethral diverticulectomy was carried out without any complications. The removed stones were in 2 pieces and they were composed of calcium phosphate and ammonium urate.

[Case report: an unstable bladder with bilateral epididymitis, urge incontinence and unilateral reflux].

We report a 10-year-old boy with and unstable bladder, bilateral epididymitis, urge incontinence, bed wetting, and unilateral reflux. Previously, he complained of urinary incontinence and was operated on elsewhere with Y-V plasty in the bladder neck and pulling out a balloon catheter through the urethra. Following the surgery the urinary force was weakened, incontinence got worse and bilateral epididymitis broke out frequently. Since the patient was refractory to anti-cholinergic medicine, sacral nerve block, antireflux operation and narrowing the bladder neck were attempted. The surgery was successful in eradicating reflux and urinary tract infection, but failed to normalize the extremely widened bladder neck. Urge incontinence and bed wetting recurred 11 months after the sacral block, which was effective in a selected patient. We believe that Y-V plasty at the bladder neck should not be indicated for a patient with bladder dysfunction closely related to an unstable bladder.

[Ectopic ureteral opening: clinical assessment of 13 cases].

Thirteen cases of ectopic ureteral opening, surgically intervened during the past 10 years, were reviewed regarding the patients profiles, diagnostic procedure, treatment and concomitant anomalies. The diagnostic procedures are consisted of the conventional methods such as an intravenous urography, a vaginography, and a renoscintigraphy, which are important to identify and locate the immature kidney or the kidney with duplication anomaly. An angiographic examination is to be abandoned because it is invasive and rather insensitive. The surgical treatment should be determined depending on the renal function remained, ureteral morphology and the site of the ureteral opening.

[Two cases of a foreign body in the renal pelvis].

We report two cases in which the renal stone originated from a foreign body which was used to treat a urinary tract fistula by a gynecologist. The first patient developed a vesicovaginal fistula following an emergency Caesarean operation. At the time of correcting surgery, a 5 Fr ureteral catheter was used as a splint catheter elsewhere, 108 mm of which somehow migrated up to the renal pelvis and a stone was formed 4.5 years later. The second patient underwent a radical hysterectomy for uterine cervical cancer. Postoperatively, a ureterovaginal fistula developed, for which a double-J ureteral catheter was indwelt. When she visited our clinic 6 months later, we found the double-J catheter and renal stone in her left renal pelvis. In both cases the catheters and renal stones were successfully removed by means of the percutaneous nephrolithotomy (PNL).

[A case of renal cell carcinoma on chronic hemodialysis].

We report a case of renal cell carcinoma on maintenance dialysis for chronic renal failure. A 61-year-old male, treated for diabetes mellitus for 20 years, developed diabetic nephropathy and has been put on maintained hemodialysis during the past 2 years. He complained of asymptomatic hematuria. Computed tomographic scan and renal angiography suggested the presence of a malignant neoplasm in his right kidney. He underwent right nephrectomy, and hemodialysis was resumed on the 4th day post operation. Pathologic examination revealed renal cell carcinoma, clear cell type, together with hyalinized glomeruli, where an acquired cystic change was not detected.

Pathology of the peripheral nerve in the twitcher mouse following bone marrow transplantation.

The peripheral nerve of the homozygous twitcher mouse (twi/twi), a murine model of globoid cell leukodystrophy (GLD), was examined following bone marrow transplantation (BMT). The light and electron microscopic studies revealed markedly increased numbers of remyelinated fibers and almost complete disappearance of the typical inclusion-laden macrophages in the trigeminal and sciatic nerves of the twi/twi which survived beyond 100 days of age. The pattern of remyelination appeared to be normal. GLD inclusions were still observed in the cytoplasm of some of the remyelinating Schwann cells and demyelinated fibers were still present in 108-day-old twitcher although no features of active demyelinating processes were observed. Thus, basic metabolic abnormality is still present despite clinical improvement in the twi/twi mouse following BMT.

Calcification of a ventriculoperitoneal shunt tube. Case report.

A 16-year-old boy who had undergone a ventriculoperitoneal (VP) shunt because of hydrocephalus at 8 years of age complained of pain around the right neck and chest. He concomitantly had a slight fever of unknown etiology, which had been lasting for several years. Skull and chest roentgenograms revealed an unusual calcified shadow around the shunt tube. After removal of the shunt apparatus, his pain and fever disappeared. Silicone tubes used in a VP shunt apparatus may induce fibrous connective tissue proliferation around the tubes in both children and adults, but no reports of radiologically verified calcification of a VP shunt tube are found in the literature, to the best of our knowledge. The possible mechanism of calcification of the VP shunt tube is discussed.

Ureteral crossover method: animal experiment for correction of reflux.

The ureteral crossover method for the correction of bilateral vesicoureteral reflux, characterized by ureteral overlapping in the submucosal space and directing the submucosal tunnels toward a point other than the bladder neck, has been tested in 10 dogs. The operation was successful in 18 of 20 ureters (90%). This technique has two merits: (1) availability of a sufficient length of submucosal ureter without altering the original ureteral entrance bilaterally; and (2) easy ligation of the new orifices in the middle of the operative field. It is concluded that ureteral overlapping does not harm the ureteral physiology and that the surgical validity of this method has been verified.

Expression of the E2 open reading frame of papillomaviruses BPV1 and HPV6b in silkworm by a baculovirus vector.

To produce and characterize the nature of the E2 protein of papillomaviruses, we have developed a system to express the DNA sequence containing an open reading frame (ORF) as a fusion protein in cultured insect cells and silkworms. The DNA fragments of the E2 ORF predicted from the DNA sequence of bovine papillomavirus type 1 and human papillomavirus type 6b were linked to the N-terminal part of polyhedrin gene of the Baculovirus Bombyx mori nuclear polyhedrosis virus (BmNPV) vector. Hybrid proteins composed of polyhedrin protein (52 amino acids) and the E2 proteins of BPV1 (410 amino acids) or HPV6b (346 amino acids) were efficiently produced in B. mori cells and silkworm larvae infected with recombinant viruses. The amount of E2 fusion proteins produced by recombinant viruses was comparable to that of polyhedrin produced by wild type BmNPV. The hybrid proteins were immunologically reactive to antiserum against polyhedrin.

Screening of edible plants against possible anti-tumor promoting activity.

One hundred twenty-one species of edible plants (133 test-parts) were screened against possible anti-tumor promoting activity by an in vitro short-term assay system of inhibition of Epstein-Barr virus (EBV) activation induced by a phorbol-ester promoter, 12-O-hexadecanoylphorbol-13-acetate (HPA). The methanol-extracts (ME) of 14 species of the edible plants strongly inhibited the activation, 7 moderately and 12 weakly inhibited it. On partition of the randomly selected inactive ME (26 species) with ethyl acetate and water, 13 and 2 species were active, more or less, in the ethyl acetate and water soluble part, respectively. Thus, this result suggested that anti-tumor promoters occur in a wide variety of edible plants. The anti-tumor promoting activity in the crude extracts may be enhanced or reduced with co-occurring factors acting additively, synergistically or antagonistically.

Treatment of persistent enuresis. Results of severing a tight filum terminale.

A tight filum terminale was severed in an attempt to treat intractable enuresis in 27 children. Urodynamic studies demonstrated pre-operative bladder instability in all cases. Although symptom scores for enuresis improved significantly 6 months after surgery, only 7 patients benefited clinically (3 were cured and enuresis was reduced in 4). An average of 26 months later, 12 patients were either dry or their enuresis was reduced, probably due to spontaneous cure rather than surgery. We found no clinical parameters able to predict a favourable outcome. It was concluded that although a tight filum terminale is a cause of intractable enuresis, transection of the filum is not recommended because of the low success rate.

Evaluation of intravesical alum irrigation for massive bladder hemorrhage.

The efficacy of intravesical alum irrigation was analyzed after application to 9 patients with continuous and severe bladder hemorrhage. Causes of bleeding were radiation cystitis in 4 patients, vesical invasion by cervical cancer in 3, bladder cancer in 1 and cyclophosphamide-induced cystitis in 1. Though alum treatment was initially effective for control of massive bladder hemorrhage in all patients, it eventually failed to suppress a subsequent hemorrhage in 2 patients (78% success rate). No significant side effects directly related to this therapy were observed. In conclusion, alum irrigation is effective for controlling massive bladder hemorrhage for a rather short time. Therefore, additional treatment modalities should also be considered for primary diseases.

Serious complications of microvascular decompression operations for trigeminal neuralgia and hemifacial spasm.

Serious complications of microvascular decompression operations for trigeminal neuralgia or hemifacial spasm are reported. Among 278 patients who underwent microvascular decompression, 9 serious complications were observed: 1 intracerebellar hematoma with acute hydrocephalus, 1 cerebellar swelling with acute hydrocephalus, 1 supratentorial acute subdural hematoma, 2 status epilepticus, 1 infarction of the brain stem, 1 subarachnoid hemorrhage due to traumatic aneurysm, and 1 infarction in the territory of the posterior cerebral artery. Of the 9 patients with such complications, 2 died. The possible causes of such serious complications are discussed.

Ganglioside GM3 as a modulator of differentiation of mouse myeloid leukemia cells (M1-T22).

The effects of exogenously added glycosphingolipids on the differentiation of mouse myeloid leukemia cells (M1-T22) have been studied. Eight gangliosides and ten neutral glycosphingolipids were tested in terms of their induction of phagocytic activities on the leukemia cells. N-Acetyl-neuraminosyllactosylceramide (NAc-GM3) was the most effective glycolipid for inducing the activity. By the addition of 25 micrograms/ml of NAc-GM3, about 70 percent of the cells acquired phagocytic activity within 20 h incubation. GM1a showed about half the activity of the GM3. In the case of the neutral glycosphingolipids, lactosylceramide (CDH) and globotriaosylceramide (CTH) showed significant effects on the induction of phagocytic activity. Preincubation of the cells with the NAc-GM3 enhanced the effect of dexamethasone as a differentiation inducer on M1-T22 cells. When a human promyelocytic leukemia cell line, HL-60, was preincubated with the NAc-GM3 ganglioside, induction of the phagocytic activity, together with inhibition of the cell growth by phorbol ester (TPA), were markedly enhanced. From these observations, the NAc-GM3 ganglioside seems to act as a modulator of differentiation of mouse myeloid leukemia cells and also of HL-60 cells.
