[Pharmacologic characteristics of human neurogenic bladder: response to KCl, carbachol, ATP and CaCl2].

Pharmacologic characteristics of the human neurogenic bladder to KCl, carbachol, ATP and CaCl2 was investigated in vitro in comparison with the control bladder. Patients with neurogenic bladder underwent augmentation enterocystoplasty because of low bladder compliance or uninhibited contraction which resulted in urinary incontinence and/or vesicoureteral reflux. 1. There was no difference in the contractile strength to KCl between the neurogenic bladder and the control. 2. The contractility (both contractile strength and the value of ED50) to carbachol was than in the control. 3. The contractile strength of the neurogenic bladder to ATP showed greater efficacy than that of the control bladder. 4. The contractility of the neurogenic bladder (contractile strength and the value of ED50) to CaCl2 was significantly greater than that of the control. 5. In the neurogenic bladder there was no correlation between pharmacological responsiveness and the clinical parameters including the data of cystometry. In conclusion, the human neurogenic bladder demonstrated supersensitive responses to carbachol, ATP and CaCl2.

[Anterior decompression surgery of aged patients with cervical myelopathy].

One hundred and thirty-nine patients with cervical myelopathy who underwent anterior decompression surgery were reviewed to determine the characteristics and surgical outcome in 26 elderly patients over the age of 65. The mean age of the aged group was 69.7 and that of the remaining 113 patients was 50.0. Extensive spur removal and interbody fusion with iliac bone graft were performed in all patients. Evaluation of clinical grades was based on the Japan Orthopedic Association Score (JOA score) on admission, at discharge and at the time of follow-up (mean 26.8 months). The improvement rate was calculated from Hirabayashi's formula at each point. Preoperatively, the mean duration of illness was longer and the mean JOA score was significantly lower in the aged group. Furthermore, low incidence of soft disc and multi-level lesions were characteristic for the aged. Final JOA score increased from 8.2 to 13.0 in the aged group, while that in the non-aged group increased from 10.5 to 14.8. These differences were statistically significant at each point. Excellent and good results which were designated as an improvement rate of 50% or more were obtained in 73.1% of the aged group, and in 79.6% of the non-aged group. Postoperative complications were slightly more frequent in the aged group, all of which, however, were dislocation of bone graft seen in 3 patients. As compared with the non-aged group, poorer surgical outcome in the aged group was attributed to preoperative poor clinical condition, long duration of preoperative symptoms, low incidence of soft disc herniation and multi-level lesions which were characteristic features for the aged patients.(ABSTRACT TRUNCATED AT 250 WORDS)

Novel oligomannose-type sugar chains derived from glucose oxidase of Aspergillus niger.

The primary structure of the N-linked sugar chains of glucose oxidase from Aspergillus niger was investigated. These sugar chains were released from the polypeptide backbone by hydrazinolysis, and the reducing ends of the sugar chains were pyridylaminated. HPLC of the pyridylamino sugar chains with an amide-silica column showed at least seven sugar chain peaks. Chemical and exoglycosidase digestion and 400 lMHz H-NMR studies of the sugar chains of lower molecular weight showed that these were novel oligomannose-type sugar chains, (Man)5-7 (GlcNAc)2, with the structure: +/- Man alpha 1----3Man alpha 1----3(Man alpha 1----6)Man alpha 1----6(+/- Man alpha 1----3Man alpha 1---3)Man )Man beta 1----4GlcNAc beta 1----4GlcNAc.

Transglycosylation activity of endo-beta-N-acetylglucosaminidase from Arthrobacter protophormiae.

Endo-beta-N-acetylglucosaminidase from Arthrobacter protophormiae has transglycosylation activity. Treatment of (Man)6(GlcNAc)2Asn with the enzyme in the presence of free N-acetylglucosamine gave a mixture of (Man)6GlcNAc and (Man)6GlcNAc beta 1----4GlcNAc mixture. N-Acetylglucosamine at the reducing end of the latter sugar chain was found by HPLC of the carbohydrate composition and of an exoglycosidase digest of the pyridylamino derivative of the reducing-end residue, and by 400 MHz 1H-NMR spectroscopy.

[Neurogenic bladder in HAM (HTLV-I associated myelopathy)].

Bladder dysfunction is one of the major symptomatologies characteristic to HAM (HTLV-I associated myelopathy). Four patients, 3 females and 1 male, were diagnosed by neurologists to have HAM with spastic gait disturbance and increased titer of antibody to HTLV-I. They complained of urge incontinence, bed wetting, difficulty in micturition and/or pollakisuria. Urodynamically, in 3 of them severe uninhibited detrusor contractions were observed. On the other hand, in one case detrusor contractility was lost completely at voiding. In all patients, bladder sensation was well preserved. Corticosteroids and interferon could not improve their urological symptoms. Clean intermittent catheterization (CIC) put on 3 patients who had a significant amount of residue relieved them of urinary incontinence. We believe that HAM in patients suffering from severe difficulty in micturition is a good indication for CIC.

Enzymatic synthesis of novel oligosaccharides by use of transglycosylation activity of endo-beta-N-acetylglucosaminidase from Arthrobacter protophormiae.

The transglycosylation activity of endo-beta-N-acetylglucosaminidase from Arthrobacter protophormiae was used for the enzymatic synthesis of novel oligosaccharides. When (Man)6(GlcNAc)2Asn was used as a substrate for the transglycosylation, (Man)6GlcNAc-Glc, (Man)6GlcNAc-Man, (Man)6GlcNAc-chitobiose, and (Man)6GlcNAc-gentiobiose were synthesized. Their structures were identified by HPLC, ion spray mass spectrometry, and digestion with glycosidases. Endo-beta-N-acetylglucosaminidases hydrolyzed the pyridylamino derivatives of these oligosaccharides.

[A case of bladder transitional cell carcinoma with sigmoidovesical fistula due to diverticulitis].

We present a case of bladder transitional cell carcinoma with sigmoidovesical fistula due to diverticulitis. The patient was a 65-year-old male who had been suffering from recurrent abdominal pain with high fever for the past 6 months. He noticed pneumaturia and cloudy urine several times. He visited a doctor complaining of macroscopic hematuria and high fever, and was treated under the diagnosis of acute prostatitis. In our University Hospital cystoscopy showed a papillary bladder tumor. Pathological study of the tumor revealed transitional cell carcinoma, grade II, noninvasive type. Multiple diverticulosis of sigmoid colon was identified with barium enema examination. We performed transurethral resection of the bladder tumor. Three weeks later, the sigmoidovesical fistula was treated by primary resection of the sigmoid colon and wedge resection of the bladder including fistula.

[A case of juvenile vesical endometriosis with unilateral renal agenesis and bicornate uterus].

A case of vesical endometriosis with unilateral renal agenesis is reported. A 13-year-old girl complained of difficulty in urination and lower abdominal pain during the menstruation. Detail urological examinations revealed left renal agenesis and intravesical cystic mass. The mass was located in the left vesical lateral wall, obstructing the vesical outlet and containing dark-brown-coloured fluid in it. She finally suffered from urinary retention following the menstruation and underwent a resection of the mass together with a part of the vesical wall. During the operation, the uterus was found to be a bicornate one. The resected mass was diagnosed as an endometriosis based on the histological findings. Eighteen months after the operation she is free from any symptoms during the menstruation and recurrence of endometriosis.

[A case of hysterical conversion manifested by pain in face and head].

We report a case of hysterical conversion, which was initially diagnosed as trigeminal neuralgia. The pain in the face and head which the patient complained seemed to be consistent with symptoms of trigeminal neuralgia. But it could not be relieved by repeated peripheral nerve blocks and even by Gasserian blocks. Failure to relieve pain by such blocks and concomitant hemifacial spasm with closure of the eye appeared to afford indication of microvascular decompression surgery. Injection of edrophonium before operation, for the purpose of differential diagnosis of myasthenic ptosis, made the patient free from all symptoms, i.e. facial pain, spasm, and ptosis. The diagnosis of hysterical conversion was made on this irrational episode. When we looked back the total clinical course, it was revealed that the patient's complaints and symptoms had been rather atypical, too various, and not anatomical. We also confirmed that various other episodes of this patient which had not received our attention during treatment were psychological features of hysteria, but we could not make his psychological stress clear, which had been in the background of hysterical conversion.

Immunological agglutination kinetics of latex particles with physically adsorbed antigens.

The influence of the properties of antigens and particles on the immunological agglutination kinetics of the antigen-coated latex particles was studied. Horse cytochrome c, hen egg-white lysozyme (HEL), bovine serum albumin (BSA), and Aspergillus sp. glucose oxidase were physically adsorbed onto the surfactant free latices of styrene-methacrylic acid (MAA) copolymer (P (S/MAA)) and polystyrene (PS). The initial rates of the immunological agglutination of these protein-coated particles initiated by the addition of antibodies were quantified by the absorbance change at a wavelength of 680 nm. The initial agglutination rates of the particles covered with smaller antigens were lower. This effect of the molecular size of antigens was larger in P(S/MAA), because small antigens are probably buried in the hydrous polymethacrylic acid layer on the surface of particles. Thus, both the molecular size of antigens and the surface properties of particles affect the sensitivity of the immunological agglutination. On the other hand, the dependence of the initial rate of the immunological agglutination on the ionic strength and pH was similar irrespective of antigen-particle systems. The initial agglutination rates were largest at an ionic strength of approximately 0.05 at pH 7.0 and decreased with increasing pH. This dependence of the sensitivity on the pH and ionic strength is attributed to the electrostatic interactions of particle-particle and antibody-particle.

Cystourethrograms characteristic of bladder instability in children.

Cystourethrograms characteristic of bladder instability are illustrated. A repeated increase of intravesical pressure causes functional and morphologic deformities of vesicourethral configuration: ballooning of the proximal urethra, ureteric reflux, and trabeculated bladder during the storage phase; and dilatation of bladder neck and proximal urethra, and a pearl string-like configuration of urethra during the voiding phase. The clinical significance of these observations is presented.

Calcified metastatic brain tumor.

The case of a 57-year-old woman with a calcified metastatic brain tumor, histologically confirmed to be a squamous cell carcinoma, is reported. This patient is unusual because this metastatic squamous cell carcinoma contained an extraordinary huge conglomerated calcification, that was well-defined radiographically. This case is documented with a discussion of the pathogenesis of the calcification.

Characteristically distinct isolates of the nuclear polyhedrosis virus from Spodoptera litura.

More than 100 isolates were plaque-purified to examine the genetic variations in four wild stocks of Spodoptera litura nuclear polyhedrosis virus (NPV) collected in Japan. These isolates were characterized by their in vitro host range in three established insect cell lines, growth characteristics, polyhedral protein, DNA restriction endonuclease pattern and DNA hybridization. The isolates were separated into four distinct groups: (I) isolates corresponding to Autographa californica NPV, (II and IV) two different groups of isolates of S. littoralis NPV which had been previously characterized and (III) isolates with no correspondence to any reported virus group. Of the S. litura NPV wild stocks, two were mixtures of more than two different groups of NPVs. We have discussed the advantage of having a mixture of different NPV groups in the same wild virus stocks.

The efficacy of prazosin HCl in the treatment of urinary flow obstruction due to prostatic hypertrophy.

Prazosin HCl was administered to 20 patients with urinary obstructions associated with prostatic hypertrophy. Significant reductions were recorded in the nighttime and the mean 24-hour urination frequencies. Patient-assessed symptoms diminished significantly, and uroflowmetry revealed significant increases in average and maximum urine flow rates. Residual urine volume decreased, the residual urine ratio declined, and bladder compliance tended to increase. The prazosin HCl therapy produced no significant effects on blood pressure or pulse rate, and no side effects appeared. The therapy was judged to be 'extremely beneficial' in 10%, 'beneficial' or better in 50%, and 'marginally beneficial' or better in 85% of all the patients.

[Solitary cerebral varix. Case report].

A 69-year-old male presented with a 4-month episode of tonic seizures of the right arm and leg. Neurological examination revealed no abnormal findings. In the electroencephalogram, sharp wave trains were seen dominantly in the left frontal region. Computed tomography scans showed a spotty hyperdense lesion in the left insular area with minimal enhancement. Left carotid angiogram demonstrated no abnormalities in the arterial and capillary phases. In the venous phase, however, a varicose dilatation of the posterior insular vein was found, which was also noticed in the delayed phase. Magnetic resonance T1-weighted images failed to detect the lesion, but T2-weighted images showed a slight signal focus in the same area. After administration of 300 mg of phenytoin daily, the frequency of attacks markedly decreased and the patient was kept under close outpatient observation.

Subarachnoid hemorrhage possibly caused by a saccular carotid artery aneurysm within the cavernous sinus. Case report.

Aneurysms arising from the intracavernous portion of the internal carotid artery very rarely rupture. A patient is presented in whom rupture of an aneurysm wholly within the cavernous sinus caused a subarachnoid hemorrhage. The aneurysm was successfully clipped via a direct surgical approach. The possible mechanism by which subarachnoid hemorrhage occurred is briefly discussed.

[Partial sacral agenesis. Characteristics of urodynamic findings].

We investigated fifteen patients with partial sacral agenesis who complained of urological disorders. Those with a myelomeningocele were not included. The mean age at the first visit was 13 years old. Bilateral and unilateral sacral agenesis were observed in nine and six patients, respectively. Chief complaints comprised incontinence and/or nocturnal enuresis in 10 patients, pollakisuria 5, recurrent fever attack due to urinary tract infection 5 and difficulty in micturition 2. In 4 cases vesicoureteral reflux was observed. At an initial urodynamic study bladder compliance varied from 1.4 to 37.0 (mean 10.4) ml/cmH2O and uninhibited contractions were present in 10 of them. Three patients were judged to have normal or almost normal bladder function. After treatment with clean intermittent catheterization and/or anticholinergics, compliance improved to a mean of 15.1 ml/cmH2O and the uninhibited contraction decreased in its amplitude or disappeared completely in 8 of 10 cases. It was suggested that patients with partial sacral agenesis originally suffered from a lower motor neuron lesion. The severity of the sacral deformities and other neurological symptoms (gait or sensory disturbance) were not related with the urodynamic findings. However, it was found that the more the cystogram was deformed the lower was the bladder compliance.
