A multi-center clinical trial was performed to evaluate the clinical usefulness of 123I-orthoiodohippurate (123I-OIH) in patients with renal and urinary disorders. 123I-OIH was evaluated to be "safe" in all 259 cases it was injected. 123I-OIH was useful in all 248 cases analyzed for the overall clinical usefulness evaluated by the investigators. The clinical significance of the drug was also evaluated in terms of renal blood flow function, renal parenchymal function, urinary function and the ability to differentiate between renal and urinary disorders. In the comparison with 123I-OIH (90 cases), 123I-OIH was superior as evaluated by the investigators and the Committee both. In the comparison with 99mTc-DTPA (113 cases), 123I-OIH was evaluated as superior by the investigators, but no significant difference was found by the Committee. In image qualities, 123I-OIH was evaluated as superior to 131I-OIH and equal to 99mTc-DTPA by the Committee. In overall efficiency, 123I-OIH was evaluated as being more valuable than 131I-OIH in 92% of the 90 cases and more valuable than 99mTc-DTPA in 50% of the 113 cases. 123I-OIH was evaluated as being less valuable than 131I-OIH in no cases and less valuable than 99mTc-DTPA in 7% of the 113 cases. These results suggest that 123I-OIH is superior to 131I-OIH and equal or superior to 99mTc-DTPA as a radiopharmaceutical for gamma camera-renography.
Cerebral blood flow was measured in 56 healthy subjects who ranged in age from 1.3 to 80 years. The Xe-133 inhalation method with ring type single photon emission computed tomography (SPECT); HEADTOME was used. The cerebral blood flow were calculated by the method of Kanno & Lassen. Then the value were corrected for end-tidal CO2 concentration that had an excellent correlation with PaCO2 and was one of the most sensitive factors to change cerebral blood flow. The brain-size index was obtained by measuring the lateral and antero-posterior diameters from the SPECT images. The mean cerebral blood flow in children less than five years of age was 90-100 ml/100 g/min that was approximately twice that found in adults. The mean cerebral blood flow decreased rapidly with age until 24.0 years of age in male and 21.7 in female. Thereafter, there was a slow decrease and a negative correlation with age was found. In adults, the mean value of the cerebral blood flow of females was slightly higher than that of males (p less than 0.01). In contrast, the brain-size index of females was significantly smaller than that in males (p less than 0.01). There was a significant negative correlation to an inverse proportionality of cerebral blood flow and brain-size index (p less than 0.002). But, there was no significant correlation between total flow index that obtained to multiplied mean flow by brain-size index and brain-size index or gender.

Thallium-201 myocardial perfusion imaging with dipyridamole was performed on 78 patients (pts) with suspected coronary artery disease (CAD). Twenty eight pts had normal coronary arteries (control group), and 50 pts had 50% or greater stenosis in one or more major coronary arteries (CAD group). In control group, aminophylline (Am) was infused intravenously on 12 pts during dipyridamole testing. Mean washout rate (WR) of these 12 pts was greater than that of 16 pts without Am (42% vs 35%, p less than 0.01). Then normal limit of WR was defined separately from the pts of control group with and without Am. The sensitivity for detecting the pts with CAD by WR analysis which normal limit was calculated separately with and without Am was 92% (46/50). When normal limit of WR was determined from all pts in control group, the sensitivity decreased to 84% (42/50). Am significantly affected thallium-201 washout during dipyridamole testing. WR of CAD pts should be estimated by specific set of normal limit with and without Am.
Tl-201 imaging and first-pass radionuclide ventriculography (RNV) by 4 min infusion of 0.56 mg/kg dipyridamole were performed on 22 patients with coronary artery disease in order to know the relationship of myocardial perfusion and left ventricular function after dipyridamole-infusion, and thereby to define whether Tl-201 defect with dipyridamole could imply myocardial ischemia. Initial and delayed Tl-201 images were divided into anterior, apical, and infero-posterior segments, and segmental perfusion defects were categorized as reversible, fixed and no defect. RNV on the 30-degree right anterior oblique view was also divided into anterior, apical, and infero-posterior wall to be evaluated for regional wall motion by a 5 point score before and after dipyridamole. Changes in left ventricular ejection fraction (LVEF) with dipyridamole were also calculated. Normal responses of regional wall motion and LVEF to dipyridamole were established from the data of 14 normal subjects. Reversible defects were closely associated (69%) with an abnormal response of regional wall motion (score decrease of 1 or more after dipyridamole). Both fixed defects and no defects showed little association with abnormal response of regional wall motion. Moreover, 77% of the patients having reversible-defect segment showed an abnormal response of LVEF (reduction of 3% or more following dipyridamole). However, patients without reversible defect did not show an abnormal response to dipyridamole. These results suggest that dipyridamole-induced Tl-201 defects represent a myocardial ischemia which causes a reduction of ventricular function.
During mouthpiece respiration of Xe-133 for a measurement of regional cerebral blood flow (rCBF), the breathing pattern of patients fluctuated and it caused a change of end-tidal CO2 concentration that had an excellent correlation with PaCO2 in patient without respiratory disease. The end-tidal CO2 concentration of demented patients varied within lower ranges than senile control group. The range of fluctuation on the end-tidal CO2 concentration was dependent on the type and the degree of dementia, and it fluctuated most widely at the middle stage of Alzheimer disease. Mean cerebral blood flow increased by 13.9% for each 1% increase in end-tidal CO2 concentration (3.6%/mmHg PaCO2) in the case of demented patients without cerebrovascular disease. To improve the reliability of rCBF in demented patients, especially in Alzheimer disease, the correction of rCBF data for end-tidal CO2 concentration should be performed.
Exercise first-pass radionuclide ventriculography was performed on 65 patients with coronary artery disease (CAD) and 18 control subjects with normal coronary artery, Left ventricle (LV) was divided into 5 sectors in radial, and volume curves were generated for whole LV and each sector for estimating LVEF and time to end systole (TES). The differences of TES between whole LV and each sector were calculated and these 5 differences were averaged (Variation of TES). We defined this Variation of TES as an index of LV asynchrony, and used for detection of CAD. The overall sensitivity and specificity for identifying CAD were 63% and 83% (LVEF during exercise), 51% and 83% (delta LVEF). Variation of TES during exercise had a sensitivity and a specificity of 88% and 83%, and was more sensitive than both LVEF during exercise and delta LVEF (p less than 0.01). In conclusion, Variation of TES was a good marker of LV asynchrony and useful for the detection of CAD.
Asynchronous communication was made between host (FACOM M-340) and personal computer (OLIBETTIE S-2250) to get patient's information required for RIA test registration. The retrieval system consists of a keyboard input of six numeric codes, patient's ID, and a real time reply containing six parameters for the patient. Their identified parameters are patient's name, sex, date of birth (include area), department, and out- or inpatient. Linking this program to RIA registration program for individual patient, then, operator can input name of RIA test requested. Our simple retrieval program made a useful data network between different types of host and stand-alone personal computers, and enabled us accurate and labor-saving registration for RIA test.
We studied the potential for using dynamic single photon emission computed tomography of inhaled xenon-133 (133Xe) gas in the assessment of demented patients. An advanced ring-type single photon emission computed tomography (SPECT) "HEADTOME" with improved spatial resolution [15 mm in full width at half maximum (FWHM)] was used for tomographic measurement of regional cerebral blood flow (rCBF). All 34 patients underwent a detailed psychiatric examination and x-ray computed tomography scan, and matched research criteria for Alzheimer's disease (n = 13), senile dementia of the Alzheimer type (n = 9), or multi-infarct dementia (n = 12). In comparison with a senile control group (n = 7), mean CBF of both the whole brain and the temporo-parietal region was significantly less in the Alzheimer's disease and senile dementia Alzheimer type groups, but no significant difference was seen between the senile control group and multi-infarct dementia group. The correlation was 0.72 (p less than 0.004) between the mean CBF of the whole brain and the score of Hasegawa's Dementia Scale, and 0.94 (p less than 0.0001) between rCBF of the temporo-parietal region and the scale in Alzheimer's disease. In the senile dementia Alzheimer type group, the correlations were 0.77 (p less than 0.01) and 0.83 (p less than 0.004) respectively. No significant correlations were found in the multi-infarct dementia group. A temporo-parietal reduction in the distribution of the rCBF characteristic in the Alzheimer's disease group and a patchy whole brain reduction characteristic in the multi-infarct dementia group was detected. The ability of our improved SPECT to provide both quantitative measurement of rCBF and characteristic rCBF distribution patterns, makes it a promising tool for research or routine examination of demented patients.
As a new parametric image of cerebral lesions, a map of the ratio between regional cerebral blood flow (rCBF) and the 2nd image of a series of dynamic images was proposed and tested. Three classes (poor, mild and well) were defined based on the degree of coupling between the rCBF and the new parametric image of the affected lesion. In 16 patients with cerebral infarction, those in the acute stage (within 1 week after onset) were found to belong to the poor coupling group, and the subacute group (1 to 4 weeks) showed mild coupling. In the chronic group (over 4 weeks), well coupling was observed. All patients of TIA fell into the mild coupling group. In a stimulation study with 5% CO2 mixed air, all of the well or mild-coupled lesions changed to uncoupled. These clinical findings demonstrate the characteristics of the new proposed image.
We studied the effect of intravenous isosorbide dinitrate (ISDN) on myocardial perfusion of patients with coronary artery disease, by using exercise thallium-201 (TI-201) myocardial scintigraphy. A control study was conducted initially to assess regional myocardial perfusion rate. Left ventricular myocardium was divided into six parts: anterior, lateral, apical, inferior, posterior, and septal segments. The segmental myocardial perfusion was characterized according to TI-201 initial uptake index (IUI) of relative distribution and redistribution index (RDI) of TI-201 washout. The normal limit of IUI and RDI was established from the data of 17 persons with normal coronary arteries, and then the IUI less than or equal to 84% and the RDI greater than or equal to 1.12 was defined as abnormal. Based on IUI and RDI, each segment was characterized into three types: A type = IUI less than or equal to 84%, RDI greater than or equal to 1.12; B type = IUI less than or equal to 84%, RDI less than 1.12; and C type = IUI greater than 84%, RDI less than 1.12. ISDN was given as a dose of 0.1 mg/kg/hr, and then treadmill testing was repeated for the same duration of exercise time using the same protocol as in the control period. The segments of A type showed a significant improvement in IUI and RDI after receiving ISDN infusion, while the B and C type segments showed no change. It was also shown that the improvement of IUI and RDI of the A type segments was not as marked in multivessel disease as in cases of single-vessel disease.(ABSTRACT TRUNCATED AT 250 WORDS)

To evaluate the regional ventilation using 133Xe under different conditions, the respiratory flow and the phase with image were measured simultaneously, and sequential frames were made breath by breath. The flow signal and phase signal were converted to pulse signals by a microcomputer and stored with image data in the same file. Thus, the tidal volume and duration of each breath were measured. From sequential frames compiled breath by breath, an activity breath-number curve was made in each pixel, and the half-clearance breath number (B1/2) was calculated. The changes in conventional half-clearance time (T1/2) and B1/2 were compared under different ventilatory conditions in normal subjects and in patients with chronic pulmonary emphysema. Values of B1/2 indicated the change of turnover rate more accurately than T1/2. It is considered that B1/2 is a better parameter than T1/2 for evaluating the change in regional ventilatory function under different conditions.
