[A study on luteal function during consecutive cycles (author's transl)].

UNLABELLED: It is conceivable that luteal function is varied with different menstrual cycles. A study was therefore undertaken to investigate for changes in luteal function during consecutive menstrual cycles. METHODS: From those women receiving ambulatory treatment at the hospital 30 displaying a biphasic basal body temperature (BBT) curve were chosen for the study. BBT records were obtained from each of these women for at least 3 consecutive cycles and a total of 194 cycles thus rendered available were evaluated for the pattern of BBT, the duration of high phase (DHP), high phase score (HPS) and planimetric luteal index (PLI). Twenty of these 30 women were subjected further to endometrial dating at the 7th-8th day of high phase with simultaneous determinations of serum estradiol (E2), progesterone (P) and prolactin (PRL) during 2 consecutive cycles. RESULTS: The incidences of abnormal values of the parameters for at least one of the consecutive cycles studied were: DHP; 13.3% pattern; 50-100%, HPS; 40.0%, PLI; 13.3%, E2; 25%, P; 25%, PRL; 30%, endometrial dating; 65%. These results indicate a considerable variability of luteal function.

[A study on the significance of bromocriptine (CB-154) administration in normoprolactinemic anovulatory women (author's transl)].

UNLABELLED: The ovulation-inducing effect of bromocriptine (CB-154) and the underlying mechanism in the management of anovulatory infertility with normoprolactinemia were investigated. Thirty-seven normoprolactinemic women (anovulatory cycle, 11 cases; 1st grade amenorrhea, 18; and 2nd grade amenorrhea 8) with anovulia showing plasma prolactin levels of less than 25 ng/ml were give CB-154 daily in doses of 2.5 mh from the second day of menstruation or withdrawal bleeding (1) to assess the ovulatory induction rate, (2) to determine the serum PRL, LH, FSH and E2 levels before and during the drug administration, (3) to measure the serum PRL concentration by TRH test prior to CB-154 medication and (4) to assess size heterogeneity of serum PRL with the use of lyophilization. RESULTS: (1) Ovulation occurred in 21 of the 37 patients, of a total of 42 menstrual cycles studied, 33 were ovulatory, indicating a high rate of repeated success. (2) There was a significant decrease of serum PRL following administration of CB-154, and the finding was the same in the group of women with successful ovulatory induction. Serum LH, FSH and E2 levels did not show any significant change following CB-154 therapy. (3) Prl concentrations were suppressed more in cases of unsuccessful induction than those in successful induction. (4) PRL responses to TRH tended to be lower in cases of successful induction than those in unsuccessful cases. (5) The Little PRL content ratio decreased after administration of CB-154, and this data have suggested that not only the problem of immunologically measured serum PRL levels but the qualitative problems also bear relations to the effect of this hormone on the endocrinological environment.

Changes after major resection of experimental cirrhotic liver.

Liver cirrhosis was induced experimentally in dogs--hemodynamically, by constriction of the hepatic vein and pharmacologically, by administration of dimethylinitrosamine (DMNA)--after which approximately 45 per cent of the liver was resected. Results indicate that death after major resection of a cirrhotic liver was caused not only by too extensive a resection but also by the rapid progression of cirrhotic changes in the remnant liver during the postoperative course.

Head and neck reconstruction with pectoralis major myocutaneous flap: CT evaluation.

Sixty-one CT scans in 20 patients who had undergone head and neck reconstructive surgery using a pectoralis major myocutaneous (PM-MC) flap were retrospectively evaluated to assess the usefulness of Ct in the follow-up of these patients. The normal CT findings in patients with PM-MC flaps are described. Of 13 cases with tumor recurrence, CT examination correctly detected recurrent masses in all cases, including 3 in which there was no clinical sign of recurrence. Postoperative masses mimicking tumor recurrence included deformed breast tissue, hematomas, lymphoceles, and abscesses. Computed tomography was of great value in the detection of tumor recurrence, but careful interpretation is required because a postoperative complication or anatomic alteration may lead to the erroneous diagnosis of tumor recurrence.

Anomalous left brachiocephalic vein: CT findings.

Eight cases of anomalous position of the left brachiocephalic vein were demonstrated by CT. Six coursed downward lateral to and below the aortic arch to enter the superior vena cava (SVC). Two cases were double left brachiocephalic veins. The superior branch was in the normal position, but the inferior vessel coursed below the aortic arch. Most of these subaortic left brachiocephalic veins enter the SVC at the same level or caudal to the azygous arch. Only two cases were associated with congenital heart disease. This anomalously positioned vessel is asymptomatic but must be distinguished from the pulmonary artery and persistent left SVC, especially when open heart surgery for cardiac malformations is being considered.

Unilateral renal cystic disease.

Unilateral renal cystic disease (URCD) is a distinct entity that is one of the renal cystic diseases. URCD consists of a cluster of multiple cysts in part or most of one kidney with no association of cystic disease in the contralateral kidney. URCD is a nonfamilial, nonprogressive disorder and is not related with autosomal dominant polycystic kidney disease (ADPKD). We report a case of URCD with six-year CT follow-up. Confinement of the cystic disease to one kidney with an absence of cysts in other organs such as liver or pancreas distinguish URCD from ADPKD. Absence of an encapsulated mass and intervening normal renal parenchyma between the cysts can differentiate URCD from cystic renal tumors.

Evaluation of antibacterial activity of three dentin primers using an in vitro tooth model.

This study compared the antibacterial activities of three dentin primers and investigated a newly designed experimental system using a bovine tooth model method for evaluating antibacterial activity by comparing this method with a conventional disk diffusion method. The antibacterial activities of SA primer in Clearfil Liner Bond, LB primer in Clearfil Liner Bond II, and ED primer in Panavia 21 were evaluated using the conventional disk diffusion method. The 50 microliters aliquot of each primer was applied to three sterilized paper disks, then placed onto Tryptic Soy agar plates already inoculated with Streptococcus mutans. After anaerobic incubation for 48 hours, the diffusion of antibacterial components was determined using the inhibition zone produced around the paper disk. The diameter of the inhibition zones was measured and the average calculated. Standardized cavities (diameter 5.0 mm, depth 3.0 mm) were prepared on the labial surfaces of bovine teeth and inoculated with S mutans (10(6) CFU/microliter) following sterilization by 60Co gamma rays (50 KGy). The teeth were divided into four groups: SA primer, LB primer, ED primer, and a control group. Except for the control teeth, the cavity preparations were treated with the respective dentin primers, and then firmly sealed with a temporary sealing material. The teeth were placed in bottles containing melted Tryptic Soy agar. Five ml of Tryptic Soy broth was then added to the surface of the hardened Tryptic Soy agar. After 1 week's incubation of the teeth in the bottles at 37 degrees C, the number of bacteria remaining in each cavity was counted, except for eight specimens, which were used for SEM observation. The ED primer showed the widest inhibition zone in the disk diffusion test, which was significantly different from the other primers. Using the bovine tooth model, all dentin primers showed antibacterial activity, with significant differences found among the four groups. The results indicated that ED primer had the strongest antibacterial effect among the three primers.

Retroperitoneal squamous cell carcinoma: metastasis from uterine endometrial carcinoma?

A case of a retroperitoneal squamous cell carcinoma (SCC) in a 61-year-old woman is reported. Imaging studies demonstrated a well-defined solid and cystic mass with suggested inferior vena cava (IVC) invasion. She had had a history of uterine endometrioid adenocarcinoma (EAC) with squamous differentiation (Grade 1) five years previously. Based on the pathological findings, this retroperitoneal neoplasm was thought to be a metastasis from the uterine EAC.

Remineralization across the resin-dentin interface: in vivo evaluation with nanoindentation measurements, EDS, and SEM.

OBJECTIVE: The purpose of this study was to evaluate the in vivo remineralization of the possible non-resin infiltrated hybridoid layer between the hybrid layer and the subjacent dentin substrate using nanoindentation, energy dispersive x-ray spectroscopy microanalyses (EDS), and scanning electron microscopy (SEM) technologies. METHOD AND MATERIALS: Twenty Class V cavities were placed in healthy adult monkey teeth. Each cavity was total etched with 37% phosphoric acid for 60 seconds, rinsed, and air dispersed, and SA-Primer was applied to the collagen layer. Cavities were divided into two groups: In group 1, Protect Liner (low-viscosity resin) and Clearfil AP-X (resin composite) were placed per manufacturer's directions, and no bonding agent was placed on the acid-etched interface. In group 2, Clearfil Photobond (bonding agent) was applied, and Protect Liner and Clearfil AP-X were placed as in group 1. Teeth were observed at 7 days (control) and 6 months by nanoindentation, EDS, and SEM. RESULTS: Six-month data showed an increased nanohardness in areas 5 pm adjacent to the demineralized or partially demineralized dentin interface. Following treatment with a conventional adhesive system on the acid-etched interface (group 2), there were increased nanohardness and calcium EDS measurements in the substrate just below the resin-dentin impregnated layer. CONCLUSION: Our 6-month in vivo nanoindentation and EDS data demonstrate that the non-resin infiltrated zone becomes remineralized following adhesive resin treatment.

A suggested method for mixing direct filling restorative gallium alloy.

Good wettability is one of the desirable physical properties of mercury-free gallium-based alloys (Gallium Alloy GF). However, wettability, while providing good adhesion to the cavity wall, has the adverse effect of causing stickiness to the inside of the capsule during mixing, and also to the metal hand instruments used for packing. To control this stickiness the alloy mixture was treated with a small amount of alcohol using two different methods. In both groups (alcohol-added and alcohol-treated groups), 5, 10, or 15 microliters of alcohol was added. However, for the alcohol-treated group, the alcohol was shaken from the mixture. In both groups, remarkable improvement was seen in their handling properties, for the alloy mixture did not adhere to the inside of the capsule and was easily taken out as one mass. Compressive strength, tensile strength, and creep were tested in the alcohol-added/-treated groups, and compared with those of a control (conventionally mixed gallium alloy) and a high-copper amalgam (Spherical-D). All tests were done according to ISO 1559 (International Organization for Standardization, 1986), and the results were analyzed using one-way ANOVA (Duncan, P < 0.05). Surface microhardness (KHN) and dimensional change during hardening were evaluated according to ISO 1559 and compared to the control results. In mixing Gallium Alloy GF, an addition of less than 5 microliters of alcohol had the effect of preventing the alloy mixture from sticking to the inside of the capsule and remarkably enhanced the handling properties. This suggested mixing technique did not alter either the mechanical properties tested for this material or the desirable dimensional expansion that occurs during hardening.
