Clinical and audiological characteristics of tonal and noise tinnitus.

A study was made of the clinical and audiological characteristics of tonal and noise tinnitus through a correlation of detailed patient interviews, comprehensive neurootological examination results, and clinical data on tinnitus type, frequency, loudness and bandwidth obtained with our newly developed tinnitus-measuring device. Of the 205 cases studied, 61% had noise tinnitus and 39% had tonal tinnitus. Higher incidences of noise tinnitus were discovered in patients aged over 50 years as well as in those with longer tinnitus histories and higher-level hearing losses; noise bandwidths in these patients also tended to be larger. Our data indicated that cases with noise tinnitus, especially those with wider bandwidths, tended to be associated with wide-range cochlear damage.

[Effects of L-ascorbic acid and bone metabolism factors on alkaline phosphatase activity of and 45Ca2+ incorporation by ROS 17/2.8 cells].

Among several bioactive substances known as coupling factors, transforming growth factor-beta (TGF-beta), interleukin-1 (IL-1), and prostaglandin (PG) E1 and E2 increased not only the activity of alkaline phosphatase but also the rate of incorporation of 45Ca2+ into ROS 17/2.8 during a 3-day culture: the former two factors are known to be formed at the site where bone is resorbed, while PG's are known as one of the factors involved in bone resorption. Parathyroid hormone, another hormone that affects bone metabolism, elevated the incorporation of 45Ca2+ by and decreased the alkaline phosphatase activity of the cells. The facts indicate the possibility that the osteoblastic cells are involved in the transport of calcium ions when bones are being resorbed. On the other hand, when these osteosarcoma cells were cultured in DMEM containing ascorbate and beta-glycerophosphate, followed by staining with silver nitrate by the procedure of von Kossa, there appeared many groups of cells that were positively stained as dark brown spots. Cells were then cultured under the same conditions in the presence of radioactive calcium, and the radioactivity accumulated was measured. The result showed that the presence of both ascorbate and beta-glycerophosphate in the culture medium dramatically increased the accumulation of 45Ca2+. It appears from these facts that ROS 17/2.8 cells are capable of incorporating and/or accumulating calcium ion if they are cultured under appropriate conditions. These cells will probably be able to produce a calcified matrix in vitro.

[A case of extraskeletal Ewing's sarcoma of the chest wall].

A case of extraskeletal Ewing's sarcoma arising in the chest wall of a 15-year-old male is reported in this paper. His complaint was chest and back pain. Physical examination and laboratory data disclosed no abnormality. Radiographs and computed tomography revealed chest wall tumor, and resection of the tumor with a portion of the eighth rib was performed. Histologically, there was no involvement of the rib at all. The tumor tissue was composed of solidly packed tumor cells which were uniform in size and shape. Individual tumor cells were small, round-shaped, and had scanty, ill-defined cytoplasm. The diastase-digested, PAS positive material was found in the cytoplasm. NSE (neuron specific enolase) was positive, but EMA (epithelial membrane antigen) LCA (leucocyte common antigen), and keratin were negative. With these results, we got a diagnosis of extraskeletal Ewing's sarcoma. Combination chemotherapy with vincristine, cyclophosphamide, adriamycin, actinomycin D was administered after operation, and there is no evidence of recurrence after 12 months of follow-up.

Clinical significance of IgG anti-Sm antibodies in patients with systemic lupus erythematosus.

Free Sm was purified by gel filtration and anti-Sm affinity chromatography. Using this purified antigen, an ELISA for anti-Sm was performed. Three hundred and fifty patients with various rheumatic diseases were studied with respect to immunoglobulin classes of anti-Sm by ELISA. A high frequency of IgG anti-Sm was specifically detected in patients with systemic lupus erythematosus (SLE), but IgA and IgM anti-Sm showed a low frequency and also was detected in other diseases. In patients with SLE, anti-Sm significantly correlated with lung fibrosis and pericarditis. In our longitudinal study, there were increases in titer of IgG anti-Sm preceding central nervous system exacerbation and serositis. IgG anti-Sm was found to be not only a diagnostic marker but also a reliable measure of disease activity in SLE.

[Influence of hepatic arterial embolization on diabetic patients with hepatocellular carcinoma].

Uncontrollable change of diabetes mellitus (DM) has occurred in one of our patients who had received hepatic arterial embolization (HAE) for hepatocellular carcinoma (HCC). This prompted us to examine the influence of HAE to the diabetic patients with HCC. Thirty-four patients accompanying DM who had received HAE were examined fasted blood glucose (FBG) and the liver function before and after the procedure. HAE was performed using Gelatin Sponge and Lipiodol containing anticancer agents, either alone or combined. Of 34 patients 6 showed increase of FBG level of more than two times after HAE. The FBG level had a tendency to elevate as the grade of DM advanced. The tendency was also recognized on pre-HAE oral glucose tolerance test. However, FBG elevation had no relation to the changes of liver function (GPT, Choline Esterase), the difference of embolic materials and pre-HAE status of DM control. From the results, one must be aware that HAE or Lipiodol infusion to diabetic patients with HCC sometimes may cause uncontrollable change of DM, especially in case of advanced DM patients. Consequently, careful follow-up of HCC as DM is advisable for improvement of the patients' prognosis.

Purification and characterization of Ki antigen and detection of anti-Ki antibody by enzyme-linked immunosorbent assay in patients with systemic lupus erythematosus.

Ki antigen from rabbit thymus extract was purified by ammonium sulfate precipitation, anti-Ki affinity chromatography, and high pressure liquid chromatography gel filtration. The purified Ki antigen gave a single polypeptide with an MW of 32,000 by sodium dodecyl sulfate-polyacrylamide gel electrophoresis, and it specifically reacted with anti-Ki antibody by immunoblotting. The isoelectric point of the Ki antigen was found by isoelectric focusing to be 4.3. An enzyme-linked immunosorbent assay using the purified Ki antigen was established, and the clinical significance of the anti-Ki antibody in systemic lupus erythematosus (SLE) was studied. Thirty of 140 patients with SLE (21.4%) had anti-Ki antibody by enzyme-linked immunosorbent assay, whereas 11 (7.9%) were positive by double immunodiffusion. Analysis of clinical and serologic data on patients with SLE suggested a higher prevalence of central nervous system involvement in patients with anti-Ki antibody.

Postnatal changes in an alpha subunit isoform, alpha(S), of Na+,K(+)-ATPase in the submandibular gland of rats.

The alpha and alpha(+) isoforms of Na+,K(+)-ATPase were isolated from the kidney and brain of rats and purified. Their antisera were raised to analyze the alpha isoforms in rat tissues. We found that the submandibular gland (SMG) contains a new immunoreactive alpha subunit isoform, designated alpha(S) in this report, in addition to alpha identical with those found in the kidney or brain. The new alpha(S) strongly reacted with anti-alpha-antiserum but to a much lesser extent with anti-alpha(+)-antiserum. The alpha(S) had a slightly lower molecular weight (approximately 90,000) than the brain and kidney alpha isoforms. Various fractions of SMG tissues were added to the SMG microsomes and incubated in order to test whether or not the alpha(S) is formed artificially; no increase of alpha(S) was observed by these treatments, suggesting that the alpha(S) was not the product formed from alpha during the preparation of microsome sample, but was rather a protein originally present in the SMG. The alpha(S) protein was not detected in the SMG of 2- or 5-week-old rats, but it gradually increased in rats older than 8 weeks, reaching the maximum in 30-week-old animals. The Na+,K(+)-ATPase activity in the SMG increased concomitantly with the increase of alpha(S), indicating that Na+,K(+)-ATPase comprising alpha(S) also shows enzyme activity; it is speculated that alpha(S) may have some unique and unknown function(s) in older rats.

A simple and rapid purification procedure for the preparation of tetramethylrhodamine-labeled antibodies.

A conjugate preparation of antibody (Fab) and tetramethylrhodamine (TMR) generally adsorbs free TMR which is very difficult to remove because of its strong hydrophobic binding. On the basis of criteria such as sodium dodecyl sulfate (SDS) gel electrophoresis and staining of the plasma membrane of live cells, we found that simple extraction with n-butyl alcohol or iso-amyl alcohol could remove the contaminating free dye. The procedure is especially useful when one needs to prepare conjugates with low nonspecific binding for the study of lateral diffusion of cell membrane-associated antigens.

Temporal bone pathology of a patient without hearing and caloric reaction, and with counter-rolling after chronic myelocytic leukemia.

The temporal bone pathology of a 36-year-old man who suffered from chronic myelocytic leukemia and sudden hearing loss of both ears, was studied from the viewpoint of neurotology. Neurotological examination showed bilaterally profound hearing loss and no caloric reaction but good counter-rolling reaction to the right and the left head tilt and no other abnormal neurological findings. He died of intracranial hemorrhage. A study of histopathology of the temporal bones revealed extensive destruction of the organ of Corti, dilatation of Reissner's membrane, leukemic infiltration in the cochlea but good preservation of sensory hair cells in the vestibular endorgans. Leukemic cell infiltration in perilymphatic and endolymphatic spaces, and leukemic hemorrhage in the perilymphatic spaces were observed. And also in the left internal auditory canal, obvious leukemic infiltration and marked hemorrhage were observed, but in the right internal auditory canal, no abnormal change was found. We discuss the correlation between neurotological findings and histopathological findings.

[Effects of various factors involved in bone metabolism on 45Ca2+ incorporation and alkaline phosphatase activity of ROS 17/2.8 cells].

The effects of interleukin 1, transforming growth factor-beta (coupling factors), prostaglandin E1, and prostaglandin E2 on incorporation of 45Ca2+ and on alkaline phosphatase activity were studied using cultured ROS 17/2.8 cells, one of cell lines derived from rat osteosarcoma. We found that all these factors stimulate both the incorporation of 45Ca2+ and alkaline phosphatase activity of these cells. On the other hand, one of the bone resorption hormones, parathyroid hormone (PTH), suppressed the proliferation of cells and decreased the alkaline phosphatase activity at considerably low concentrations (1 X 10(-12)-1 X 10(-11) M). However, the hormone stimulated the incorporation of 45Ca2+ by these cells in a dose-dependent manner; the maximum stimulation on day 3 was observed at 1 X 10(-7) M and it was approximately 3 times the control value. The data suggest therefore, that the osteoblasts incorporated calcium ions and transported them while bone resorption was occurring. Thus the ROS 17/2.8 cell line appears to be an advantageous experimental system for the study of calcium metabolism of osteoblasts in vitro.

[Clinical significance of red cell distribution width in polycythemia vera].

We evaluated changes in red cell distribution width-standard deviation (RDW-SD) measured using a multiple parameter automated hematology analyzer E 4000 in patients with polycythemia vera (PV). Patients with iron deficiency anemia, those with chronic myelogenous leukemia, those with primary thrombocythemia, and normal subjects were examined as controls. In the patients with PV, as in those with the other 3 diseases, RDW-SD tended to be higher than in the normal controls when red blood cell counts were high. The RDW-SD in patients with PV transiently increased following administration of a myelosuppressive, which corresponded to the transition period from microcytes to normal blood cells. It was even higher during the polycythemic period than during the myelofibrotic period. This may be associated with hematopoietic abnormality due to extramedullary hematopoiesis. RDW-SD seems to well reflect the pathologic status of PV.

[Neurilemmoma of lesser omentum].

A very rare case of neurilemmoma of lesser omentum was reported. The tumor was circumscribed and solid. And it had necrotic parts within it. Echography and X ray CT revealed its inner structure. Angiography showed similarity of benign solitary neurogenic tumor in other sites.

[Clinical experiences with intraluminal irradiation using 198Au grains in the treatment of bile duct carcinoma].

Three cases of a bile duct carcinoma were treatment with radiotherapy, using intracatheterized 198Au grains. The intracatheter 198Au grains, placed in an inner tube, were inserted into a percutaneous transhepatic catheter. The number of 198Au grains used depended upon the length of the tumor. Tips of plastic were placed between the grains to improve the spatial and temporal dose allocation. The number of grains used can be changed quite easily, according to the length of the tumor, and the period of insertion in our cases was shorter than for a 192Ir wire. These three patients also received external irradiation and, since completion of treatment, two have continued to do well for the past 5 months. There have been no systemic or local complication.

[A case of triple cancer--gastric, esophageal and hepatocellular carcinoma].

A case of triple cancer involving a tubular adenocarcinoma of the stomach, a squamous cell carcinoma of the esophagus, and hepatocellular carcinoma of the live is reported. These three cancers had been diagnosed while the patient was alive and later were confirmed by autopsy. The esophageal cancer and hepatocellular carcinoma were found almost simultaneously, 6 years after surgery for the gastric cancer. Although many cases of triple cancer have been reported, a triple cancer of this combination is very rare (0.3% of all cases of triple cancer), our case being the fifth such case in Japan.

Can tinnitus be masked by band erased filtered masker? Masking tinnitus with sounds not covering the tinnitus frequency.

In this study, the effectiveness of masking sound not covering the tinnitus frequency was tested against that of conventional masking sound in 117 subjects. Successful masking was observed in 72% of the former and 70% of the latter. There was no remarkable difference between the two maskers regarding the acceptability and intensity. The duration of residual inhibition, however, was smaller in the masker not covering the tinnitus frequency. The masking sound tested could be useful as an alternative when the conventional one is not accepted.
