A comparison of flushing fluids for initial perfusion of kidneys for transplantation.

Four solutions for initial flushing of kidneys prior to transplantation were tested under conditions designed to resemble those of clinical cadaveric donor renal transplantation. The experimental model was the dog subjected to bilateral nephrectomy with renal autograft. Kidney grafts were subjected to 15 minutes' anoxia in vivo, 30 minutes' warm ischaemia at 37 degrees C ex vivo, and two hours' cold ischaemia before reimplantation. The four solutions used were Collins (C3), Perfudex (P), hyperosmolar citrate (HC), and a solution of bovine albumin containing dog red blood cells (BBA). Effects of the flushing fluids were compared by parameters relating to dog survival, renal function, and serum enzyme levels. With all parameters studied the best results occurred in HC perfused kidneys. Results with BBA perfusion were marginally worse, while C3 perfused kidneys were again inferior. P perfused kidneys clearly did least well. The results support the use of HC for clinical application.

Survival and the immune response in patients with carcinoma of the colorectum.

One hundred and seven patients with colorectal carcinoma were examined for their immune response to autologous tumour. Patient leucocyte migration inhibition by tumour antigen, migration inhibition of normal subject's leucocytes by patient's serum, and the assessment of lymphocyte invasion of the tumour specimen were used to measure the immune response. Forty-one and 51% of patients were immune by leucocyte migration inhibition and tumour invasion by lymphocytes respectively. The immune patients had a survival advantage at three years on Chi-squared analysis. Patients whose serum contained a factor that inhibited the migration of leucocytes obtained from normal individuals had a diminished survival prognosis. The donors of these sera had a 50% chance of dying or of developing recurrent disease. These same individuals represented 30% of all those tested by this method. The immune patients included the sera donors as a subgroup 30% of all those tested by this method. The immune patients included the sera donors as a subgroup who, if excluded from the life table analysis, left a group of immune subjects who had very superior survival features.

The diagnosis and treatment of lymphocoeles associated with renal transplantation. A report of 6 cases and a review of the literature.

Six patients have developed a lymphocoele after renal transplantation, an incidence of 4%. A lymphocoele should be suspected in a patient who develops a rising creatinine with a pelvic mass or pressure effects on the pelvic veins 1 or more months after operation. The diagnosis is confirmed by intravenous urography, venography and ultrasonography: the use of the latter as a diagnostic measure is recommended. Treatment is by marsupialisation into the peritoneum or external drainage with breakdown of all loculi. Aspiration is unsatisfactory.

Immunity in colonic tumor patients after operation: determination by leukocyte-migration inhibition.

Tumor immunity in 31 colonic tumor patients was studied by determining in vitro leukocyte-migration inhibition. In tissue culture 30 per cent inhibition of patient leukocyte migration by tumor antigen could be demonstrated in half of the patients, suggesting that they were immune. This immunity was not related to the clinical or pathologic stage of the disease and varied in different patients. It was transient in some and persistent in others. In addition, a positive response at a fixed time after operation was not found.

Heparinization in aortic surgery.

A prospective study was conducted on 35 patients (25 males and 10 females) undergoing elective reconstructive aortic surgery to examine the heparin activity after a bolus dose of sodium heparin (100 U/kg) given five minutes prior to aortic cross clamping. Recording of heparin activity were made 15 minutes later, on release of the aortic clamp, at abdominal wound closure and hourly thereafter until minimal activity was reached. In 10 patients, protamine was used to reverse anticoagulation. These results were related to clinical parameters of age, renal function, plasma cholesterol, blood pressure, position of the aortic clamp and blood loss. High peak levels of heparin activity were more likely in patients with impaired renal function or high plasma cholesterol concentrations. Heparin activity was prolonged at therapeutic levels in patients with renal impairment, and in this group of patients the use of protamine was significantly increased as was the requirement for blood replacement.

The early use of operative lumbar sympathectomy in peripheral vascular disease.

The results of operative lumbar sympathectomy for both intermittent claudication and rest pain in 153 patients have been reviewed. Sympathectomy was performed as an initial procedure to further below the groin reconstructive surgery, should symptoms not be alleviated. Five year post-sympathectomy, 67% of the claudicant and 54% of the rest pain patients had avoided further surgery. Patient mortality from other manifestations of atherosclerosis was notably poor at five years and was significantly greater than the sympathectomy failure rate after the first year. The results in diabetics were not significantly different from those in non-diabetics. The possible value of lumbar sympathectomy in both claudication and rest pain is reviewed and discussed.
