Does the edge-to-face interaction between aromatic rings occur in cyclolinopeptide A analogues?

We measured 1H-NMR, fluorescence and CD spectra of cyclolinopeptide A (CLA), its tyrosine analogues with each or both phenylalanines substituted by tyrosine (c-[LeuIleIleLeuValProProTyrPhe], c-[LeuIleIleLeuValProProPheTyr] and c-[LeuIleIleLeuValProProTyrTyr]), and their linear counterparts with the starting sequence leu-Ile-Ile-Leu-Val-Pro-Pro-Phe-Phe (LA). It follows from CD spectra that the conformations of all cyclic peptides are similar to that of CLA; the conformations of linear peptides are more diversified, with the conformation of [Tyr9]LA being most similar to CLA. NMR studies suggest that aromatic rings in cyclic peptides are situated perpendicular to each other, manifesting edge-to-face pairing. Accordingly, the residue in position 9 is shielded ('edge'), and a residue in position 8 is the shielding one ('face'). This effect is not present in the case of linear peptides. Fluorescence quantum yields were much lower for cyclic peptides than for linear ones, indicating the interaction of closely located aromatic chromophores. Those quantum yields depend on the relative position of Tyr in the peptide chain. Another factor influenced by the position in the peptide chain is the optical activity of aromatic side chains (optically active in position 8, inactive in position 9). This phenomenon could be explained by the differences in the side-chain conformation of both aromatic residues.

A randomised, double-blind study on the efficacy of Tołpa Torf Preparation (TTP) in the treatment of recurrent respiratory tract infections.

TTP is a new immunomodulating drug of natural origin, registered in Poland for human use. In the randomised, double blind study to assess the therapeutic efficacy of the Tołpa Torf Preparation (TTP) in the recurrent respiratory tract infectious participated 39 young patients (age 16-22). TTP was administered orally, 5 mg daily for three weeks. During 3 months follow-up period favourable results of treatment were obtained in 14 of 20 TTP treated patients and in 8 of 19 of the placebo patients. The therapeutic effects were seen even after the 6 months follow-up period. The phagocytic activity of granulocytes was significantly stimulated in the TTP--treated patients but not in the placebo-treated patients. The results suggest that TTP is effective drug in the treatment of recurrent respiratory tract infections with undefined infectious etiology. In the period of the treatment as well as during 6 months observation no side effects were noticed.

[Pharmacokinetic examination of theophylline in pregnancy in cases of fetal anoxia].

The purpose of the investigation was to determine the effects of pregnancy on theophylline pharmacokinetics and to clarify the clinical effect of theophylline treatment in cases of threatened anoxemia of foetus. The study was carried out in 15 pregnant women between 28 and 32 weeks of pregnancy with chronic anoxemia of the foetus. The women obtained Aminophyllinum "Polfa" 250 mg (201.3 mg of theophylline) every 12 hours for 4 days to achieve the steady-state of theophylline. After the first dose the drug concentration reached the level of theophylline in range of 12.05 +/- 4.57 micrograms/ml in 15 minutes to 3.62 +/- 0.74 micrograms/ml in 12 hours. The subsequent doses increased the concentration and in steady-state the concentration fluctuates between 21.48 +/- 10.00 micrograms/ml and 6.20 +/- 1.56 micrograms/ml, what coincides with established therapeutic range of theophylline (10-20 micrograms/ml). Elimination of the drug was characterised by calculation of the elimination rate constant and drug-clearance. Both parameters are higher after a single administration what suggest the accumulation of theophylline after multiple drug administration by the saturation of enzymes taking part in metabolism of the drug. Attention should be paid higher values of half-life in pregnancy in comparison with those obtained previously in healthy volunteers. Comparative investigations of the level of theophylline in amniotic fluid in relation to maternal blood in steady-state showed a high correlation between these two values (r = 0.7867, p < 0.006). The proportion of the concentration of theophylline in relation to that in maternal blood is about 60% (0.5978 +/- 0.1599).(ABSTRACT TRUNCATED AT 250 WORDS)

[Pharmacokinetic investigations of verapamil used as a concomitant drug in treatment of premature labor].

The study carried out on 35 pregnant women, who were threatened with fenoterol and verapamil by premature delivery. The pharmacokinetics of verapamil have been examined after single dose and in steady state using drug in a dose of 2 x 80 mg/day (15 women) and 3 x 40 mg/day (8 women). Verapamil in a dose of 2 x 80 mg/day induced average steady state concentration in range of 40.6 +/- 7.8 ng/ml to 134.2 +/- 56.9 ng/ml and in a dose of 3 x 40 mg/day 28.9 +/- 5.1 ng/ml to 69.8 +/- 11.2 ng/ml. Since most authors take as optimal therapeutic level of verapamil in blood the range of 100-200 ng/ml, it seems that dosage regimen of 2 x 80 mg/day can be recommended. Additionally, verapamil concentration in maternal blood and in amniotic fluid were determined simultaneously in a group of 12 pregnant women. Verapamil readily penetrates placenta reaching in amniotic fluid about 70% of the concentration observed in maternal blood.

[Clinical evaluation of Ocuflur].

Presented is the initial clinical evaluation of Ocuflur, a compound of Allergan. The influence of the drug on the size of the pupil and the reaction of the pupil to a light flash was tested in volunteer subjects. The efficacy of the drug was tested intraoperatively.

Complications and pseudocomplications after Lich-Grégoir antireflux plasty.

With exact diagnosis and correct surgical technique, the Lich-Grégoir anti-reflux plasty is safe and complications are few--reflux recurrences are rare. The only serious complication is neurogenic bladder voiding disturbance, which has been observed in some cases after one-stage bilateral procedure and excessive mobilization of the bladder neck. Postoperative ureteral obstructions of the urine flow, generally due to oedemas, are thus temporary. They can be regarded as pseudocomplications.

Peptides related to the active fragment of "proline rich polypeptide", an immunoregulatory protein of the ovine colostrum. Spectroscopic and computer modeling studies.

The preferred solution conformation of the PRP-hexapeptide (Tyr-Val-Pro-Leu-Phe-Pro) and of some of its structural analogues was investigated by NMR-spectroscopy, spectrofluorimetry and computer simulation technic. It was found that the preferred conformation is characterized by cis'-conformation of Pro3 and the gamma-turn on the Leu4-residue: for Val2 and Phe5 a beta-structure seems to be privileged. In such a conformation Val2 and Leu4 residues occupy exactly the same positions in space as residues i and i + 3 in an alpha-helix. It suggests that the PRP-hexapeptide can interact with receptor protein inducing or stabilizing its helical conformation by "knobs into holes" packing.

[Female hypospadias: a rare cloacal abnormality].

The term "hypospadia feminis" means a congenital false urethral opening in the anterior vaginal wall proximal to the hymenal ring. In comparison to the male hypospadia it is a rare event. Obligatory an obstructive uropathy is found, but the pathophysiology remain obscure.

[False results of measuring digoxin levels in patients with renal failure. Comparison of the RIA--FPIA methods].

During last few years were reports concerning a new endogenous digoxin -like immunologic factor (DLIF) in patients not receiving digitalis. DLIF was stated in pregnant women's blood umbilical and neonatal blood as well as in patients with renal failure or hepatopathy. This phenomenon could be related to hormones changes (pregnancy) as well as alterations of cholesterol, triglycerides, free fatty acids, albumins or total protein level (nephropathy, hepatopathy). DLIF overstated determination of serum digoxin concentration, which in the case of exceptional narrow digoxin therapeutic spectrum as well as its concentration-dependent toxicity became a significant clinical problem. Effect extent of DLIF on serum digoxin level could be also related to applied analytic technics. The aim of the study was to compare two routinely applying analytic methods: polarized immunofluorescence (FPIA-TDx Abbott) with radioimmunological assay (RIA). The study was performed in patients with renal failure to estimate DLIF effect on real serum digoxin concentration as well as on extent and DLIF elimination velocity during dialysis. DLIF occurrence in patients with renal failure not receiving digitalis was experimentally stated using both RIA and FPIA methods. However, RIA revealed DLIF in all cases: before and after dialysis as well as in not dialyzed patients with a concentration above 0.3 ng/ml, when FPIA values were respectively: 0.087; 0.043; 0.078 ng/ml, which was less than 10% of digoxine therapeutic range (0.9-2.0 ng/ml). DLIF lowered in a course of dialysis to FPIA advantage, which was proved by FPIA/RIA ratio decrease nearly of a half of its predialysis value. Pre- and postdialysis values of FPIA/RIA ratio were 0,204 and 0,134 respectively.(ABSTRACT TRUNCATED AT 250 WORDS)

[Isoprinosine and climatotherapy at the Czerniawa-Zdrój health resort in the treatment of children with recurrent respiratory tract infections].

The authors used in children in with recurrent respiratory infections in 28 cases isoprenosine, in 30 climatic therapy and in 15 isoprenosine followed by climatic therapy. Isoprenosine was used in a dose 50 mg per kg b.w. for two weeks, followed by 10 days of an interruption, after which isoprenosine was gain given for ten days. Climatic therapy was carried out for six weeks. Thirty healthy children of the same age made up the control. Good or excellent results were seen in 19 out of 28 children treated with isoprenosine, in 16 out of 30 that underwent climatic therapy, and 11 out of 15 in which both modes were used. The authors discuss their own experience in treating children with recurrent respiratory infections suggesting that this model for treating sick cases be used, observing good results in two thirds.

[Comparison of Cabot/Nesbit orchidopexy with Petrivalsky/Schoemaker orchidopexy].

Two groups of each 50 children with maldescensus testis treated by two different techniques of orchiopexy are compared together. Two to five years after operation the localization and size of the testes are recorded. In the group of children with orchiopexy according to Cabot/Nesbit the failure rate was 12%, in contrast to only 6% failures and larger testes in the Petrivalsky/Shoemaker group. Therefore, the technique of Petrivalsky/Shoemaker is recommended, especially in small children.

[Therapeutic effectiveness of lidocaine in patients with ventricular arrhythmias estimated relative to measurements of drug concentration in serum].

Therapeutic effectiveness of lidocaine intravenous infusions preceded by an initial intravenous dose was studied in 29 patients with ventricular arrhythmias in a course of ischemic heart disease admitted to CCU. Clinical effects were evaluated in correlation with obtained lidocaine concentrations and basic pharmacokinetic parameters. Regression of cardiac arrhythmias was stated in 23 patients (79.3%), inclusive of all acute MI cases. Partial therapeutic effect was observed in 4 patients (13.8%) and no effect in 2 (6.9%). In 12 patients (52.2%) with stated regression of ventricular arrhythmias the full therapeutic effect was observed after initial dose administration. Serum drug concentrations were above the lower limit of the therapeutic range in all cases. Side effects were observed in 3 cases with high serum lidocaine levels caused by impaired drug metabolism or elimination due to the patient's clinical state. Correlation between the infusion rate and obtained stationary condition of drug concentration within therapeutic range seemed to be highly effective in management of ventricular premature beats caused by acute myocardial ischemia but less effective in cases of ischemic and postinfarction cardiomyopathy with heart failure.

Two new fluorophore pairs, tyrosine-4'-aminophenylalanine and tyrosine-4'-dimethylaminophenylalanine, in the determination of molecular dimensions in peptides.

Peptides and polypeptides containing tyrosine and 4'-aminophenylalanine or tyrosine and 4'-dimethylaminophenylalanine were studied by electronic absorption and fluorescence spectroscopy. Positions of the band maxima, pKa values of the aromatic ammonium group and distances between the two fluorophores in seven different compounds are compared.
