Primary neuritic leprosy: a reappraisal at a tertiary care hospital.

AIM: To study the clinico-epidemiological profile of primary neuritic leprosy. METHODS: Retrospective analysis of the leprosy records of clinically diagnosed patients of PNL who attended the Leprosy Clinic from 2000 to 2004 was carried out for details of presenting complaints, age and sex distribution, duration, number of nerves involved and pattern of nerve enlargement, BI (skin), nerve abscess, and deformities. RESULTS: There were 32 (4.6%) patients of PNL out of a total of 686. Majority (56.2%) had complaints for less than a year. There were 29 (90.6%) males, and 3 (9.4%) females, with 15-30 years as the commonest age-group (65.6%) involved. Paraesthesia and numbness were the presenting complaints in 20 (62.5%), and motor deficit (paresis) in 11 (34.4%); deformities (claw hand, foot drop, trophic changes) were seen in 16 (50%) cases. Polyneuritic pattern was noted in 21 (63.56%) patients and mononeuritic in 11 (16.5%) with ulnar nerve as the most commonly enlarged nerve (63.6%). Nerve abscess was noted in 4 (12.5%) cases. Slit-skin smear was positive in 2 (6.2%) cases only. DISCUSSION: PNL continues to be common in India. Sensory complaints are early and more common. The disease is more common in males. Polyneuritic pattern was predominant, and the ulnar nerve was the most commonly involved nerve. Majority of the cases belong to the tuberculoid spectrum. Early diagnosis depends on complete neurological examination in order to reduce the sequelae of the disease.

Hypertension in hypoglycemic diabetics treated with beta-adrenergic antagonists.

To evaluate the response during insulin-induced hypoglycemia, diabetics treated with relatively selective or nonselective beta-adrenergic blocking agents were studied. Placebo, atenolol (100 mg/day), and propranolol (80 mg b.i.d.) were administered to 12 insulin-treated diabetics for 1 week in a double-blind, randomized, crossover fashion with a 2-week washout between treatments. Sufficient intravenous insulin (0.2-0.6 units/kg) was administered to decrease plasma glucose 68% from the basal level or to less than 60 mg/dl within 90 minutes. Blood pressure changes at the nadir of plasma glucose were +15.2/-9.9 mm Hg for placebo, +27.9/0 mm Hg for atenolol, and +38.8/+14.3 mm Hg for propranolol. Diastolic blood pressure changes induced by propranolol were significantly different from those induced by atenolol (p less than 0.01) and placebo (p less than 0.01), and systolic pressure changes were significantly different (p less than 0.02) between propranolol and atenolol. Mild seizures developed in two patients treated with propranolol. Their blood pressure changes at the plasma glucose nadir were +56/+22 and +86/+31 mm Hg. Other symptoms of hypoglycemia were more frequent during beta-blocker than during placebo treatment. Differences in response may be related to the relatively selective adrenergic blocking effect of these drugs.

Titrimetric determination of ascorbic acid using chloranil.

Ascorbic acid is oxidized and quantitatively titrated with chloranil (2,3,5,6-tetrachloro-1,4-benzoquinone) in the presence of hexamethylenetetramine in acetone-water; the end point is determined visually by the appearance of a golden yellow color. Colored solutions are assayed by setting the initial absorbance at 451 nm to zero or the minimum, titrating with chloranil solution, and measuring absorbance after each increment of titrant. A plot of the volume of chloranil added against the absorbance gives a straight line with the volume intercept as the end point. Interference by the thiol group of cysteine, glutathione, etc., is avoided by masking with acrylamide; interference by iron(II) is masked with ammonium thiocyanate and sodium potassium tartrate. Hydrogen sulfite and thiourea (which do not interfere) are added as antioxidants during extraction of ascorbic acid from drugs and fruits.

Estimates of contraceptive prevalence based on service statistics and surveys in Gujarat State, India.

This report demonstrates that the estimates of contraceptive prevalence based on surveys are substantially lower than those based on the service statistics generated by the Indian family planning program. The reasons for this discrepancy were examined by contacting a subsample of acceptors recorded by female health workers as users in their registers. This inquiry indicated that the health workers themselves knew that 15-39 percent of the women who had been recorded as users of reversible methods were not really using them. About 19-27 percent of the recorded users of IUDs and only 3-4 percent of the recorded condom users confirmed use of the devices. Overall, the nonusers formed 59 and 64 percent of the recorded users contacted in Bharuch and Panchmahals districts, respectively. The estimates of nonuse of contraceptives in the follow-up survey are high enough to reconcile most of the observed discrepancy between the two sets of estimates of contraceptive prevalence.

Microfilariae of Wuchereria bancrofti in a lymph node aspirate. A case report.

Microfilariae of Wuchereria bancrofti were detected by fine needle aspiration biopsy (FNAB) in an axillary lymph node from a 27-year-old female. The patient also had intestinal tuberculosis. A routine hemogram did not show any eosinophilia, and three consecutive night blood samples also were negative for microfilariae. This was an early case of filariasis without microfilaremia and was detected by FNAB of the lymph node. The association of filariasis and tuberculosis was purely coincidental.

The impact of the quality of family planning services on contraceptive use in Peru.

Through linkage of a Demographic and Health Survey to a situation analysis, this article explores whether current contraceptive use in Peru is affected by the service environment in which a woman resides. The investigation focuses explicitly on the impact of the quality of family planning services and finds that, net of personal and household characteristics, a significant, albeit small, effect exists for one specification of quality in the total sample and for the other specification a nearly significant (p = .053) effect exists. The analysis reveals that contraceptive prevalence would be 16 to 23 percent greater if all women lived in a cluster with the highest quality of care compared with the lowest. Methodological problems that arise in measuring quality of care at the cluster level and in linking quality to individual contraceptive use are also addressed.

Percutaneous balloon pericardiotomy: non-surgical treatment for patients with cardiac tamponade.

We present two patients with recurrent large pericardial effusions who were treated with balloon pericardiotomy. This is a novel treatment in which a non-surgical pericardial window is created via the percutaneous subxiphoid route using a balloon dilating catheter. Our two patients initially had standard pericardiocentesis performed. However, large pericardial effusions soon returned in both patients so further and more definitive treatment was required. Following balloon pericardiotomy, neither patient developed a significant pericardial effusion after months of follow up. Given our experience with percutaneous balloon pericardiotomy, as well as the experience of others, we believe that this approach will become the preferred treatment for large pericardial effusions or tamponade, especially in patients with cancer.

Blue rubber bleb nevus syndrome: a cause of gastrointestinal hemorrhage.

Blue rubber bleb nevus syndrome is an uncommon disorder manifested by cutaneous and gastrointestinal hemangiomas and gastrointestinal hemorrhage causing anemia. We describe a young man who presented with hematemesis and melena and had multiple bluish rubber bleb-like hemangiomas over the body and in the stomach, jejunum and colon. The patient was treated with iron supplements for anemia; he is doing well 6 months later.
