[Sequential combination chemotherapy consisting of vincristine, peplomycin, methotrexate, cis-diamminedichloroplatinum (II) and adriamycin in urothelial cancer].

The VPM-CisA (vincristine (VCR), peplomycin (PLM), methotrexate (MTX), cisplatin (CDDP) and doxorubicin (ADM), regimen was used to treat 33 patients with urothelial tract tumors. Twenty-two patients had bi-dimensionally measurable disease parameters and 11 patients with locally advanced tumors were given postoperative adjuvant chemotherapy. The protocol consisted of 0.6 mg/m2 VCR on days 1 and 3, 3 mg/m2 PLM on days 1 to 4, 3 mg/m2 MTX on days 2 and 4, 35 mg/m2 CDDP on day 4, and 20 mg/m2 ADM on day 5. These doses were adjusted for each case: the above mentioned dose x [(80/(40+Age]2 +[(Karnofsky's performance status/100)2]. Of these patients, 28 (86 percent) were treated adequately, including 8 (36 per cent) who achieved a complete (2) or partial (6) remission. The mean duration of survival was 65.2 weeks for complete and partial responders, and 48.8 weeks for non-responders, which was not a statistically significant difference. Of 11, who were given post-operative adjuvant chemotherapy (mean observation period: 83.5 weeks) 9 were alive without evidence of disease, 1 had a recurrence 8 months after first chemotherapy, 1 died due to pulmonary and liver metastasis 2 years after the chemotherapy. Toxicity included mild myelosupression, moderate anorexia, vomiting, and severe gastric ulcer, pulmonary fibrosis.

An electronic device for hearing-handicapped drivers: evaluation of performance.

There are several countries in the world in which people with severe hearing loss are not eligible for a car driver's license. As a technical approach to solve this problem, an electronic device is developed which detects traffic-alarm-sounds, i.e. horns of cars, sirens of emergency vehicles and alarm signals of railroad crossings, and then displays them as vibration to the driver. The basic operating principle of the device is that those traffic-alarm-sounds have marked regularity in some particular frequency region whereas the traffic noise is a wide-band random noise. The real time detection of the regularity is realized by use of a phase-locked-loop and a simplified lock-in amplifier. The results of simulation experiments and road tests demonstrate that the performance of the device is satisfactory except in the case of the detection of the alarm signals of railroad crossings.

[Pneumocystis carinii pneumonia in renal transplant recipients treated with cyclosporin].

Four cases of pneumocystis carinii pneumonia occurred among 38 renal transplant recipients. Diagnosis was confirmed by cyst concentration technique in 2 cases. The other 2 cases were clinically, from the rapid improvement of fever, pulmonary infiltrates and hypoxia following a therapeutic trial of high dose sulfamethoxazole-trimethoprim. All patients responded to treatment with high dose sulfamethoxazole-trimethoprim. Three patients survived pneumocystis carinii pneumonia, but i died due to aspergillosis. One patients showed serological evidence of concomitant cytomegalovirus infection.

[The foreign body granuloma after pyeloplasty: report of a case].

We report a rare case of foreign body granuloma on plain catgut which developed after Anderson-Hynes pyeloplasty for obstruction of the ureteropelvic junction in a 3-year-old boy. We use plain catgut as the suture material on pyeloplasty and seldom experience such a complication. Regarding suture material on pyeloplasty, excessive sutures intended to make anastomosis water-tight to prevent urinary leakage should be avoided in children because they are apt to form foreign body granuloma.

[Amygdaloid kindling in weanling rat and rekindling upon maturization].

To understand the epileptic phenomenon in the immature brain and to investigate whether or not the epileptogenesis acquired in young age is retained after maturation, we carried out the amygdaloid kindling preparation (AM-K) on rat. Ten 3-week-old Wistar rats (3-week-old group) and ten 10-week-old rats (10-week-old group) were stimulated every 2 hours during 09:00-17:00, from five days after operation (Experiment 1). Nine kindled weanling rats (K-group) and ten weanling rats with no stimulation only electrodes implantation (non-K-group) were rekindled upon maturization (Experiment II). Stages of AM-K were classified according to the criteria of Racine. The results obtained were as follows: 1) The 3-week-old group needed significantly fewer stimulations and a shorter AD-durations during early stages (C0-2) of amygdaloid kindling, in comparison with the 10-week-old group. However, there was no significant difference between the two groups as to stimulations and AD-duration during later stages (C3-4) of the kindling. 2) The K-group were rekindled significantly fewer stimulations and a shorter AD-durations than non-K group during early stages. On the other hand, there was not significant difference between the two groups as to stimulation and AD-duration during later stages of the kindling. From the results, the following conclusions were made; The seizure susceptibility of weanling rat's limbic system is high and the epileptogenesis in the limbic system can be maintained even up to adulthood.

[Mass flow anesthesia delivery system and its application].

Microcomputer-based anesthesia delivery system using mass flow control devices has been developed. System is superior in computer control to conventional anesthesia machine because there is no mechanical setting. System consists of three thermal mass flow controllers (TMFC), one vapor source controller (VSC), personal computer system, and air circuits. Personal computer system has facilities of digital output, analogue input and analogue output interfaces. TMFCs and VSC are devices which operational principle is based on the thermal conductance and they are controlled by electrical signals from computer in the system. TMFCs control the mass flow of nitrous oxide, oxygen and carbon oxide according to each input voltage signal. VSC regulates mass vapor of halothane in order to keep set value up by means of alteration of oxygen carrier gas flow. Computer always monitors the gas flow of the TMFCs and VSC so that computer compensates the change of the carrier gas flow of VSC by regulating the TMFC for oxygen. Mixed gas from TMFCs and VSC is supplied through the air circuit to subject. Characteristics of the system was measured by mass spectrometer and flowmeter. Stability was 0.05% change after 30 minutes at the initial set of 3%. Experiment on animals using mongrel dogs has been performed to verify the system functions.

Acceleration of cleavage of the carbon-cobalt bond of sterically hindered alkylcobalamins by binding to apoprotein of diol dehydrase.

Cleavage of the C-Co bond of sterically hindered alkylcobalamins bearing neither an adenine moiety nor functional groups, such as isobutylcobalamin, neopentylcobalamin, and cyclohexylcobalamin, was markedly accelerated by their interaction with apoprotein of diol dehydrase, although these cobalamins do not function as coenzyme. Acceleration of the conversion of alkylcobalamins to enzyme-bound hydroxocobalamin was stoichiometric and obeyed first-order reaction kinetics. These results, together with strong competitive inhibition by these alkylcobalamins with respect to adenosylcobalamin, indicate that acceleration of the C-Co bond cleavage by the apoenzyme is due to labilization of their C-Co bond by binding to the active site of the enzyme. This labilization is considered to be caused by a steric distortion of the corrin ring which is induced by specific tight interaction of the cobalamin moiety with apoprotein. The importance of such a labilizing effect for activation of the C-Co bond of adenosylcobalamin in enzymatic reactions is discussed.

An analysis of frequency response of motor units during voluntary isometric contractions at various speeds.

To ascertain the differences in discharge properties of a hand muscle between ramp and ballistic contractions, firing frequencies of 48 motor units of adductor pollicis muscle were analyzed during 3-kg isometric voluntary contractions at various force speeds. The force exertions were classified into 3 degrees: slow ramp, fast ramp, and ballistic contractions. The instantaneous frequencies (IFs) of motor units increased gradually when the force exertions were progressed to the target in slow and fast ramp contractions. The slopes of the linear regression equations between IFs and force levels were almost all constant during slow ramp contractions and were scattered slightly during fast ramp contractions. However, the slopes during ballistic contractions scattered widely. The y-intercepts of the same regression lines increased slightly with respect to the increment of force speed when slow and fast ramp contractions were performed. However, a remarkably wide variation of the y-intercepts was observed during ballistic contractions. Motor units could be fired at high frequencies at the beginning of discharge when ballistic contractions were performed as compared with slow and fast ramp contractions.

Increased 1,2-diacylglycerol content in myopathic hamster hearts at a prenecrotic stage.

1,2-Diacylglycerol (DAG) has been suggested to be a secondary messenger. In this study, we determined the amount of 1,2-DAG in heart tissue from Syrian hamsters with hereditary cardiomyopathy at 30 days (prenecrotic stage) and 90 days of age by thin-layer chromatography with flame ionization detection (TLC-FID). Myocardial triglyceride contents were higher at 30 days of age and lower at 90 days of age compared to the levels in age-matched normal hamsters. Decreases in major species of phospholipids in hearts were observed only at 90 days of age. However, elevated 1,2-DAG content in myopathic hearts was found at 30 days of age, whereas there was no difference between the two groups at 90 days of age. It is suggested that the increase in 1,2-DAG at the prenecrotic stage is involved in the pathogenesis of the cardiomyopathy.

A calmodulin antagonist (W-7) and a protein kinase C inhibitor (H-7) have no effect on atrial natriuretic peptide release induced by atrial stretch.

The role of intracellular signals in the regulation of atrial natriuretic peptide (ANP) release was investigated using isolated rat left atria. Dibutyryl cyclic AMP and dibutyryl cyclic GMP had no effect on ANP release. Arginine vasopressin and phenylephrine, both of which activate the polyphosphoinositide system and consequently both the diacylglycerol-protein kinase C system and the Ca2+-Ca2+ receptor system, stimulated ANP release dose-dependently. The ANP release stimulated by phenylephrine was inhibited by N-(6-aminohexyl)-5-chloro-1-naphthalenesulfonamide (W-7), a calmodulin antagonist, and 1-(5-isoquinolinesulfonyl)-2-methylpiperazine (H-7), a protein kinase C inhibitor. Atrial stretch stimulated ANP release, but the release was not inhibited by W-7 or H-7. These results suggest that the mechanism responsible for phenylephrine-induced ANP release differs from that for stretch-induced ANP release.

Molecular cloning of complementary DNA encoding one of the human pancreatic protease E isozymes.

We have cloned a DNA from a human pancreatic cDNA library using a cloned rat pancreatic elastase 1 cDNA as a probe, and determined its nucleotide sequence. This cDNA contains a coding region of 810 nucleotides which encodes a 270-amino-acid protein. The deduced amino acid sequence shows less than 60% homologies with rat and porcine pancreatic elastase 1, although its substrate binding region is homologous with those of the above elastases 1. When this deduced amino acid sequence was compared with known amino acid sequences of pancreatic proteases other than elastases, it was found to contain an amino acid sequence which was highly homologous with the N-terminal amino acid sequence of porcine pancreatic protease E. We also purified human pancreatic protease E isozymes from human pancreatic juice, and determined their N-terminal amino acid sequences. One of the isozymes does not hydrolyze elastin but does hydrolyze a synthetic substrate. Endoglycosidase F digests glycoside bonds of the isozyme. These results suggest that the cDNA cloned by us corresponded to one of the human protease E isozymes.

Effect of tetrahydrobiopterin and 5-hydroxytryptophan on hereditary progressive dystonia with marked diurnal fluctuation: a suggestion of the serotonergic system involvement.

A daughter and her mother developed hereditary progressive dystonia with marked diurnal fluctuation (HPD) at the age of 4 and 34, respectively. L-Dopa, tetrahydrobiopterin (BH4) or 5-hydroxytryptophan (5-HTP) was orally administered to them. L-Dopa cured completely their symptoms. 5-HTP as well as BH4 improved their symptoms, especially dystonic movements. Biopterin and 5-hydroxyindoleacetic acid concentrations in CSF increased during BH4 medication. These findings suggest that the serotonergic system of the central nervous system might play some role in the pathogenesis of dystonia in HPD.

[Hydronephrosis presenting as the first sign of malignant lymphoma of the small intestine: report of a case].

A 45-year-old man was admitted to hospital on November 26, 1985 with the chief complaint of left hypochondrial pain. Excretory and retrograde pyelography revealed left hydronephrosis due to extrinsic obstruction of ureter. Computerized tomography and angiography revealed that a tumor of the small intestine was the cause of ureteral obstruction. In addition to the presence of a tumor, a fistula in the small intestine was disclosed on the upper gastrointestinal series. During the operation, a large mass which involved several organs was identified without mobility. The sophisticated operation was composed of wide resection of small intestine including the tumor, left hemicolectomy, left nephroureterectomy, splenectomy, partial pancreatectomy, duodeno-ileostomy and transverse sigmoidostomy was done on December 19, 1985. Pathological diagnosis was malignant lymphoma, diffuse small cell type infiltrating ureter, kidney and perirenal connective tissue. Because of poor postoperative course systemic chemotherapy was not performed and he died of disseminated intravascular coagulation on April 2, 1986.

Accuracy in the noninvasive estimation of pulmonary blood flow by identification of the CO2 uptake system.

Estimation of pulmonary blood flow, controlled by a roller pump, was carried out in dogs using a system identification technique. The system parameters in a linearized CO2 uptake model were estimated by the maximum-likelihood method from the input consisting of alveolar ventilation and inspired pCO2 and the output of end-tidal pCO2. The auto- and cross-correlations of the residuals showed that the model used for estimation was appropriate. The pulmonary blood flow calculated from the system parameters contained a degree of error which showed an average coefficient of variation of 23.8%. Error analysis revealed that the main sources of error lay in the estimates and in the error-magnifying effect arising in the calculation of the flow from these estimates. Reduction of the error in the estimates requires a long set of data for estimation; on the other hand, the error-magnifying effect is determined by physiological constants, so that it cannot be small even in clinical application.
