Neuropsychiatric manifestations of typhoid fever.

Various neuropsychiatric syndromes in typhoid fever were seen in nine patients of multiple drug resistant (both in vitro and in vivo) Salmonella typhi infection of a total of 270 suspected cases in the last two years. All but one patient received oral norfloxacin (400 mg 12 hourly) for two weeks with complete recovery, while the other patient died. Bacteremia and toxemia seem to be the only common factor responsible for neuropsychiatric complications.

Role of ciprofloxacin in typhoid fever.

A study was conducted to investigate the effects of ciprofloxacin in typhoid fever and to compare its efficacy with chloramphenicol. Eighty patients between 20-45 years with positive blood culture were included in the study. Seventy five percent patients treated with ciprofloxacin became afebrile within 72 hours while with chloramphenicol it took upto 120-144 hours for the same percentage of patients to become afebrile. Four patients resistant to chloramphenicol, cotrimoxazole and ampicillin/amoxycillin, also responded to ciprofloxacin. There were no significant adverse effects indicating that ciprofloxacin is safe and effective drug for resistant and nonresistant cases of typhoid fever.

Full length and alternatively spliced pgp1 transcripts in multidrug-resistant Chinese hamster lung cells.

In an effort to better understand the preferential resistance to actinomycin D displayed by the multidrug-resistant Chinese hamster lung cell line DC-3F/ADX, we have cloned from those cells a number of cDNAs representing p-glycoprotein gene transcripts. Of the 12 clones isolated, all represent pgp1 transcripts and one, pADX165, contains a 4304-base pair insert with an open reading frame encoding a 1276-amino acid protein that is the homolog of the mouse mdr3/mdr1a gene product. A domain by domain comparison of this protein with p-glycoproteins capable of supporting multidrug resistance, i.e. human mdr1, mouse mdr1/mdr1b, and mouse mdr3/mdr1a, shows that, in addition to the ATP binding sites, the second, fourth, and eleventh transmembrane domains and the four small intracellular loops, IC-1, IC-2, IC-4, and IC-5, are highly conserved and are therefore likely to be important for the maintenance of p-glycoprotein function. Of the remaining 11 cDNA clones, 9 were found to be truncated versions of pADX165. Two others, however, pADX185 and pADX124, contained internal deletions resulting in open reading frames capable of encoding lnovel forms of p-glycoprotein. S1 nuclease and RNase protection analysis demonstrated that these cDNAs represent transcripts present in a number of different multidrug-resistant Chinese hamster lung cell lines. Hence, both are considered to be splicing variants of the hamster pgp1 gene primary transcript.

Systemic lupus erythematosus in a black South African child. First documented case report.

Systemic lupus erythematosus (SLE) is poorly described among black children in Africa despite being more frequent among some black adult populations than their white counterparts. The first black South African child with SLE is documented. The patient was a 10-year-old girl who had fever, facial rash (with complement (C4) deposited at the dermo-epidermal junction of normal skin), weight loss, central nervous system involvement (depression, withdrawal, retinal exudates), renal involvement (glomerular filtration rate 54 ml/min/1.73 m2; membranous nephropathy with mild mesangial proliferation; World Health Organisation classification Vb), alopecia, lymphadenopathy, hepatomegaly, positive Coombs test, hypeocomplementaemia, anti-DNA antibodies and positive anti-nuclear factor.

The Rett syndrome: the first case report from Pakistan.

We report an 11-yr-old girl who visited the outpatient department of Islamabad Children's Hospital with uncontrolled seizures, psychomotor retardation and hand washing movements since early childhood. She had an uneventful peri- and neonatal history with a normal head size at birth and fulfilled the criteria of classic Rett syndrome. This is the first report of the Rett syndrome in Pakistan.

Neurology in Islamabad Children's Hospital--present status and problems.

Pediatric neurologic disease is still progressive health problem in developing countries, Disorders of mental and motor development (34.8% of total 5234 neurological patients in 1988-89), seizure disorders (25.8%) and infections of the nervous system (18.9%) are the most common neurological problems in the Islamabad Children's Hospital (ICH), Pakistan. These figures differ from those in children's Hospitals in developed countries such as Japan. ICH needs to establish a good neurology department with better physiotherapy and rehabilitation centre, a workshop and a facilitated physiological laboratory. The global situation can be controlled by improving the basic life and fundamental factors to improve the hygiene and general health status of mothers and their children. The provision of rural rehabilitation services is also needed. Also international technical cooperation and support would be helpful.

Care of the elderly in north and west Belfast.

A survey of all elderly people aged 65 years and over from North and West Belfast receiving long-term care in residential and nursing homes, and in psycho-geriatric and geriatric care was undertaken. A total of 967 subjects was studied and physical dependency and mental impairment documented. The high dependency of residents in geriatric and psycho-geriatric care was highlighted, with greater levels of dependency than among those receiving nursing home care. Professional assessment prior to admission should be common to all long-term care facilities and is essential if services for the projected demographic increase in numbers of very elderly people are to be provided, and inappropriate admission and expenditure avoided.

The effect of dobutamine on middle cerebral artery blood velocity in volunteers: a preliminary study.

Five volunteers given dobutamine up to 4 micrograms/kg/min had significant increases in mean arterial blood pressure, systolic blood pressure, and stroke volume (P less than 0.05). Heart rate did not increase significantly. Pulsatility index, an index of middle cerebral artery maximum blood velocity waveform shape increased significantly in the dobutamine group (P less than 0.05). Time-averaged mean velocity did increase during the dobutamine infusion but the change was not statistically significant. Dobutamine significantly alters the blood velocity profile of the middle cerebral artery in volunteers even in low doses of 4 micrograms/kg/min. Transcranial Doppler may be a useful technique to monitor changes in cerebral artery blood velocity dynamics induced by dobutamine. These measurements can be performed easily and repeatedly at the bedside.

Observational study of erythrocyte protoporphyrin screening test for detecting low lead exposure in children: impact of lowering the blood lead action threshold.

We examined a retrospective sample of 1800 children on whom both erythrocyte protoporphyrin (EP) and blood lead (BPb) measurements were taken. The primary objective was to ascertain whether EP is a cost-effective screening test for low but increased BPb concentrations and to establish the optimal thresholds. The data did not provide evidence of an EP threshold at low BPb concentrations; however, the data did show a significant age effect. A subset of 500 children for whom both EP and hematocrit data were available showed no correlation between those variables. Age-specific operating characteristic curves, total error, and cost analyses are presented. The latter sets bounds on the relative cost of EP testing, above which only BPb determination should be performed. The implications of these findings are discussed in light of impending changes in U.S. federal guidelines for preventing lead poisoning in young children.

Kinetics and mechanism of decomposition of N-chloroamino acids. II: conformationally restricted models.

The kinetics of decomposition of the following conformationally restricted N-chloro-alpha-amino acids were studied: 1-amino-1-cyclopentanecarboxylic acid (2), 1-amino-1-cyclohexanecarboxylic acid (4), 2-amino-2-norbornanecarboxylic acid (6), and 2-amino-2-benzonorbornanecarboxylic acid (8). The first-order rate constants obtained were 0.520, 5.197, 0.198, and 0.078, respectively, which correlated with the ring strain in the structurally related cyclic ketones cyclopentanone, cyclohexanone, norborane-2-one, and benzonorbornane-2-one. The data are supportive of a concerted mechanism for the decomposition reaction involving an imine-like transition state.

The use of 125I-HIPDM for studying tissue response due to toxic effects of cyclosporin-A in rats.

125I-HIPDM was used to study the response of various tissues in cyclosporin-A, CyA, treated and control rats. The rats were given 50 mg/kg of CyA for 7 consecutive days. The liver, kidney and heart showed significant increase while the spleen had a pronounced decrease in the uptake of 125I-HIPDM in CyA treated compared to control rats. This difference in the uptake of 125I-HIPDM between CyA treated and control rats is assumed to be the tissue response to toxic effects of CyA. The results indicate that CyA is toxic to liver, kidney, spleen and probably heart. There was no difference in the uptake of 125I-HIPDM in the lung and brain of CyA treated and control rats. This lack of difference is assumed to indicate that CyA does not adversely affect the lung and brain.

An adherent subline of a unique small-cell lung cancer cell line downregulates antigens of the neural cell adhesion molecule.

Small-cell lung cancer (SCLC) lines are distinguished from non-small-cell lung cancer (NSCLC) lines by their growth in floating aggregates, in contrast to the adherent monolayers formed by NSCLC cells in culture. Of 50 well-characterized SCLC lines recently described by the National Cancer Institute (NCI)-Navy Medical Oncology Branch, only four variant cell lines (SCLC-v) grew as adherent monolayers. One line, NCI-H446, was unique in growing long-term with coexisting floating and surface adherent subpopulations. We have physically segregated these two populations over many passages in vitro to enrich for relatively pure cultures of floating and adherent cells. No differences in c-myc expression, keratin pattern, or cytogenetic appearance were found between the adherent and floating sublines. However, expression of the neuroendocrine marker neuron-specific enolase in the floating cells was three times that found in the adherent cells. The floating subline also had much greater surface expression of neuroendocrine tumor antigens detected by monoclonal antibodies UJ13A and HNK-1, which have been recently shown to detect the neural cell adhesion molecule (NCAM) on SCLC cells. Two other adherent SCLC-v lines were also found to be unreactive with UJ13A and HNK-1, generalizing the association between NCAM expression and the growth of most SCLC cultures as floating aggregates. In conclusion, we have an interesting model to study expression of NCAM as related to the adhesive properties of SCLC cells.

Long term effects of adult-thymectomy on natural killer activity and spontaneous tumor incidence.

Long term effects of adult-thymectomy on both natural killer (NK) activity and spontaneous tumor incidence were investigated in C57BL/6 mice maintained in specific pathogen free atmosphere. As compared with the age matched sham-thymectomized mice, a substantially low incidence of spontaneous tumour (6%) and a high level of splenic NK activity were noted one and a half year later in C57BL/6 mice thymectomized at 6 weeks of age. In contrast, sham-thymectomized mice showed a high incidence of the spontaneous tumor (57%) associated with a low level of splenic NK activity. These results suggest that NK cells play a critical role to prevent the occurrence of spontaneous tumor in vivo.
