In a cross-sectional survey of 5420 children in northern Bangladesh, 124 were reported to have night blindness by their parents. Of these, 105 cases along with controls matched for age, sex, and neighbourhood had their scotopic vision examined under standard condition using a luxometer, underwent an ophthalmological examination, and had their serum vitamin A level determined. The mean serum vitamin A level was lowest among children identified as night blind by both their parents and the investigators (16.3 micrograms/dl; 95% confidence interval (CI), 13.9-18.7) and highest among those identified as not night blind by both their parents and the investigators (23.6 micrograms/dl; 95% CI, 21.3-25.9). The results show that parents' report of their children's night blindness had low sensitivity compared with diagnosis using standard observations of scotopic vision with a luxometer.

Two mammalian expression plasmids, each carrying a cDNA encoding a different allele of dihydrofolate reductase (DHFR) present in the Chinese hamster lung cell line DC-3F, were constructed. These simian virus 40 promoter-enhancer-based plasmids, designated pSVA75 and pSVMQ19, are identical except for a G-->A transition at nucleotide 286 of the DHFR coding sequence. Due to this change, the enzyme expressed by pSVA75 contains Asp95, while the enzyme expressed by pSVMQ19 has Asn95 [Melera et al., J. Biol. Chem. (1988) 1978-1990]. Both forms of the enzyme are catalytically equivalent and are produced to similar levels in DC-3F cells [Yu and Melera, Cancer Res. (1993) 6031-6035; H.Y., A.H. and P.W.M., in preparation]. Clonal cell lines expressing one or the other DHFR allele were obtained via transfection of DHFR- Chinese hamster ovary cells, and 74 clones of each type isolated. These were pooled and divided into 40 aliquots, each of which was then subjected to selection by growth in sequentially increasing concentrations of methotrexate (MTX). Analysis of the resulting drug-resistant populations revealed that cells producing Asp95 DHFR dominated with an overall frequency of 3:1, and therefore, under these growth conditions, display a selective advantage over those producing Asn95 DHFR. These data extend previous observations showing that independent selections of the heterozygous parent cell line DC-3F in MTX result in threefold more MTX-resistant lines overexpressing the Asp95-encoding DHFR allele than the Asn95-encoding DHFR allele.(ABSTRACT TRUNCATED AT 250 WORDS)

Resistance to the intracellular Ca2+ pump inhibitor thapsigargin (TG) is associated with overexpression of both Ca2+ transport ATPase and the multidrug resistance (mdr) transporter P-glycoprotein (pgp). This is supported by increased resistance to TG following transfection of a functional pgp1 cDNA, and reversal of TG resistance with known inhibitors of pgp function. However, pgp is unlikely to represent the only mechanism of resistance to TG. Cell lines selected for high levels of resistance to TG (250-fold) show only a 3.7-fold increase in pgp expression and a 2-fold increase in cross-resistance to other drugs of the mdr class. Overexpression of endogenous Ca2+ transport ATPase may represent a second mechanism of resistance to TG. Increased Ca2+ ATPase expression (3-fold) is seen in cells made resistant to TG, and TG resistance increases with the transfection of a specific Ca2+ ATPase cDNA into DC-3F cells. If these transfectants are then made resistant to TG, both the endogenous Ca2+ ATPase and the exogenously transfected Ca2+ ATPase become overexpressed. These studies suggest that while TG may be a substrate for pgp, acquired resistance to TG can involve alterations in both pgp and Ca2+ ATPase expression. Additional, as yet unidentified, mechanisms of resistance may be involved in resistance to TG.
A number of studies suggest that an inverse correlation exists between the epidermal growth factor-receptor and the estrogen receptor expression in primary human breast carcinoma as well as in established human breast carcinoma cell lines. Recent studies suggest that the epidermal growth factor-receptor does not regulate the estrogen receptor gene expression. Whether the estrogen receptor regulates the epidermal growth factor-receptor gene expression is not known. We addressed this question by stably transfecting the estrogen receptor cDNA into the estrogen receptor-negative human breast carcinoma cell line MDA-MB-231. Constitutive expression of functional estrogen receptors in the transfectants resulted in increased mRNA levels of both epidermal growth factor-receptor and transforming growth factor alpha. Estradiol treatment of transfected cells, although enhancing transforming growth factor alpha mRNA levels, did not modulate epidermal growth factor-receptor mRNA levels. The estrogen receptor-transfected cells grown in estrogenic regular medium, however, exhibited lower constitutive levels of epidermal growth factor-receptor mRNA than in steroid-stripped medium, suggesting that estrogens coupled with some factors normally present in the regular medium may indeed downmodulate epidermal growth factor-receptor mRNA. Sodium butyrate treatment enhanced epidermal growth factor-receptor mRNA levels in nontransfected cells grown in regular estrogenic as well as in steroid stripped medium. Sodium butyrate enhancement of epidermal growth factor-receptor mRNA levels was completely abolished in estrogen receptor-transfected cells grown in regular estrogenic medium and blunted in steroid stripped medium. Using various epidermal growth factor-receptor gene promoter-CAT constructs in transient transfection assays, we further demonstrate that sodium butyrate enhanced transcription of the epidermal growth factor-receptor gene. The putative sodium butyrate responsive element(s) appears to localize within the proximal 384 bp of the epidermal growth factor-receptor gene promoter region. Although the interactions between estrogen receptor and epidermal growth factor-receptor are rather complex, taken together, our data suggest that estrogen receptor can indeed modulate the epidermal growth factor-receptor mRNA expression.
The insulin-like growth factors (IGFs) are potent mitogens for malignant cell proliferation. The majority of secreted IGFs are bound to specific IGF-binding proteins (IGFBPs) that are secreted by a large number of cells. These proteins may either inhibit or enhance IGF actions. Breast carcinoma cells secrete a variety of IGFBPs. We have previously demonstrated that retinoic acid (RA) inhibition of IGF-I-stimulated MCF-7 cell proliferation is associated with increased IGFBP-3 levels in the conditioned media. We therefore investigated the effect of recombinant IGFBP-3 as well as IGFBP-2, -4 and -5 on IGF-I stimulation of DNA synthesis and IGF-I binding in the MCF-7 human breast carcinoma cell line. IGFBP-2 and -3 enhanced IGF-I stimulation of DNA synthesis in MCF-7 cells while IGFBP-4 and -5 had no effect. Transfection of MCF-7 cells with an IGFBP-3 expression vector resulted in the enhanced secretion of IGFBP-3 with an accompanying increase in IGF-I binding as well as increased cell proliferation upon treatment of the cells with IGF-I. IGF-I preincubation of MCF-7 cells transfected with control pSVneo plasmids results in cells refractory to further IGF-I stimulation of thymidine incorporation while IGF-I continues to stimulate [3H]-thymidine incorporation in IGFBP-3-transfected MCF-7 cells, suggesting that IGFBP-3 protects the cells from IGF-I-mediated down regulation of its receptor. Therefore, IGFBP-3 secreted by MCF-7 cells can enhance IGF-I stimulation of DNA synthesis, increase IGF-I binding to these cells, and prevent IGF-I-induced desensitization of its own receptor, suggesting that IGFBP-3 plays a significant role in IGF-I-mediated breast carcinoma proliferation.

A case of metastatic carcinoid syndrome with hypoxaemia is described. The hypoxaemia appeared to be due to intrapulmonary shunting. There was improvement in resting and exercise arterial oxygen saturations following chemotherapy with streptozotocin and 5-fluorouracil. This was followed by a decrease in the tumour bulk which may have led to a reduction in the secretion of the vasodilator products responsible for shunting. Some features of this case suggest that the hypoxaemia might have been due to dilatation of precapillary and capillary vessels of the pulmonary micro-circulation. These features are part of the so-called 'hepatopulmonary syndrome', of which this case appears to represent a less severe form.
Eosinophilic endomyocardial disease is characterised by persisting blood eosinophilia and acute endocardial lesions which progress to endomyocardial fibrosis. In most cases the cause is unknown but it has been described in association with malignant tumours. A fatal case is presented of a 64 year old woman with this disease due to a high grade sarcoma of the chest wall, an association not previously reported.

The human p53-binding protein murine double minute 2 (MDM2) is believed to function as a negative regulator of p53. The MDM2 gene was cloned and sequenced only recently and was found to be amplified in a variety of sarcomas. Although mutations in the p53 gene have been shown to occur in human breast carcinoma (HBC), no information is available on MDM2 gene expression in HBC. In this study we report for the first time that the MDM2 gene is differentially expressed in HBC. Our results demonstrate a correlation between the estrogen receptor (ER) status and the MDM2 mRNA levels. In contrast to the ER-negative cell lines, all the ER-positive cell lines were found to express higher levels of MDM2 mRNA. ER-positive ZR-75 cells express 30-fold higher levels of MDM2 mRNA than does the ER-negative cell line Hs578T. Estrogen enhanced albeit modestly the MDM2 mRNA levels in ER-positive MCF-7 cells. Estrogen enhancement of MDM2 mRNA levels was also observed in ER-negative MDA-MB-231 cells transfected with functional ERs. Our data thus suggest that estrogen may play an important role in HBC growth stimulation by modulating the expression of MDM2, which in turn may inactivate the p53 function.
Retinoic acid (RA) and estrogen modulation of insulin like-growth factor binding protein-4 (IGFBP-4) gene expression was investigated in a number of human breast carcinoma (HBC) cell lines. Our results indicate that RA and/or estrogen modulation of IGFBP-4 mRNA levels appear to correlate with the ER-status of HBC cell lines. RA and estrogen enhanced IGFBP-4 mRNA levels in ER-positive MCF-7, RRO-I and ZR-75 cell lines and the effect of both of these agents in combination was additive. Although both individually also enhanced IGFBP-4 mRNA levels in ER-positive T47D cells, their effect in combination was antagonistic in this cell line. Transfection of human ER into ER-negative MDA-MB-231 cells conferred RA and estrogen the ability to enhance IGFBP-4 mRNA but both agents failed to concomitantly modulate IGFBP-4 levels in the CM suggesting a dual regulation by these agents at transcription/post-transcription and translation/secretion levels.
