Functional characterization of peripheral blood neutrophils in patients with hyperthyroidism.

The thyroid hormones may exert an important effect on neutrophil functions which play a key role in the defence against microbial pathogens. Therefore investigations of the biological functions of neutrophils in peripheral blood of 80 women with hyperthyroidism were performed. The following functions of neutrophils were examined: nitroblue tetrazolium (NBT) reduction ability, phagocytosis, adherence to fibers as well as spontaneous migration of leukocytes and activity of lysozyme in serum. The total number of leukocytes and neutrophils was also determined. The results were compared with those in the control group consisting of 40 healthy women. The data show that in patients with hyperthyroidism, the number of leukocytes and neutrophils was decreased (respectively 4.90 x 10(9) +/- 1.20, 2.70 x 10(9) +/- 0.54 in patients, and 5.40 x 10(9) +/- 1.30, 3.00 x 10(9) +/- 0.74 in the control subjects), index of spontaneous reduction NBT was increased (17.1% +/- 4.5 in patients and 11.1% +/- 4.5 in the control group), index of phagocytosing cells was decreased (63.0% +/- 16.0, 80.3% +/- 14.0 in patients and 72.2% +/- 12.1, 87.4% +/- 12.0 in the control group), an increase in adherence (71% +/- 17 in patients and 58% +/- 10 in the control subjects), and a decrease in the lysozyme activity in undiluted and diluted serum was found (185 IU/ml +/- 85,209 IU/ml +/- 78 in patients and 284 IU/ml +/- 84,352 IU/ml +/- 140 in the control group).

[Clinical features of lecithin-cholesterol acyltransferase deficiency].

Lecithin-cholesterol acyltransferase (LCAT) is involved in esterify of free cholesterol and in the cholesterol esters transport from peripheral tissues to the liver. Genetically dependent lack of enzyme activity leads to Fish Eye Disease and to familial LCAT deficiency. There are specific abnormalities of plasma lipids and lipid deposits in multiple tissues (familial LCAT deficiency) or in corneal only (Fish Eye Disease). Clinical features of familial LCAT deficiency include corneal opacities, anemia, and proteinuria. Renal failure is the most frequent complication, occurring in the fourth decade. Treatment of familial LCAT deficiency is based on infusions of plasma or whole blood and on kidney transplantation.

[Activity of lysozyme in serum and selected granulocytic cytoenzymatic markers evaluated in patients with duodenal ulcer treated with ranitidine].

In 20 patients with active, endoscopically confirmed duodenal ulcer, serum lysozyme activity, metabolic and phagocytic activity of neutrophils using the NBT spontaneous and stimulated test and stimulated test, and cytoenzymatic reaction for alkaline phosphatase were evaluated before treatment and after two weeks of treatment with ranitidine 150 mg every 12 hours. In some of the studies carried out the differences were obtained between the mean results which turned out to be statistically significant. In the group of patients with duodenal ulcer after two weeks of treatment with ranitidine, a statistically significant increase was found of diluted serum lysozyme activity as compared to the activity of this enzyme before the treatment. After the treatment with ranitidine, a statistically highly significant increase was noted of alkaline phosphatase activity in the neutrophils as compared to the activity of this enzyme assessed in patients with untreated duodenal ulcer. On the basis of the obtained results of studies the conclusions were formulated: 1) The increase of diluted serum lysozyme activity in the patients with duodenal ulcer after two weeks of treatment with ranitidine may evidence the presence of inhibitors of the enzyme in the serum of the studied group. 2) In patients with duodenal ulcer after two weeks of treatment with ranitidine the activity increases of cytoenzymatically determined alkaline phosphatase in the neutrophils, while the cytoenzymatically determined values of the markers in the test with NBT remain unchanged.

[Phagocytosis and NBT test and phosphatase activity of peripheral blood neutrophils in patients with untreated duodenal ulcer].

In 21 patients with active, endoscopically confirmed untreated duodenal ulcer and in a control group of 20 persons, the studies were carried out of peripheral blood neutrophils which included: 1) test with the assessment of neutrophil phagocytic activity against live bacterial cells of the Staphylococcus aureus Oxford 209P standard strain and against latex particles, 2) test of nitroblue tetrazolium reduction (NBT test) by neutrophils, 3) evaluation of alkaline phosphatase activity in the neutrophils by a cytoenzymatic technique. In the group of patients as compared with the control group a decrease was observed of neutrophil phagocytic activity against live bacterial cells, and absence of similar differences was noted when latex particles were the stimulating factor. In the studied group an increase of the value of the index of spontaneous NBT reduction by neutrophils was observed as well as a decrease of the value of the index of latex-stimulated reduction of this dye. Besides that, higher alkaline phosphatase activity was observed in the neutrophils in patients as compared to the activity of this enzyme in the neutrophils of healthy persons. It may be supposed that in patients with duodenal ulcer, peripheral blood neutrophils reveal changes in the function of membranous structures of these cells, and disturbances of redox processes evaluated in the test with NBT.

[Comparison of selected indices of nonspecific immunity estimated in peripheral blood of patients with untreated duodenal ulcer and after treatment with ranitidine].

In 20 patients (8 women and 12 men) aged of 20-56 years (38 years on the average) with the yet untreated duodenal bulb ulcer selected indices of nonspecific immunity in peripheral blood (total leukocytes number and absolute neutrophils (N) number, N adherence to fibre, spontaneous leukocytes migration in a 3 hour test, N phagocytic activity and bactericidal activity of plasma and leukocytes) were estimated. Those indices were again estimated after two week of treatment with ranitidine, 150 mg every 12 hours. After ranitidine treatment reduction of the total leukocytes number and absolute N number was found, as compared with the pretreatment results. In addition to that treatment an increase of N adherence and their phagocytic activity was observed with unchanged leukocyte capacity for spontaneous migration. No statistically significant difference was observed in bactericidal activity of plasma and leukocytes. The following conclusions were reached. 1. Two-week treatment of duodenal ulcer patients with 150 mg ranitidine every 12 hours was followed by reduction of the total leukocytes and N numbers in peripheral blood, and by augmentation of their adherence and phagocytic activity. 2. Bactericidal activity of plasma and leukocytes and leukocytes capacity for spontaneous migration did not show any changes after two-week ranitidine treatment of duodenal patients.

Functional characterization of peripheral blood neutrophils in patients with primary hypothyroidism.

The clinical observations of increased risk of bacterial infection in patients with severe hypothyroidism motivated investigations of the biological function of neutrophils in peripheral blood of 30 women with primary hypothyroidism. The following functions of neutrophils were examined: phagocytosis, adherence to fibers, as well as spontaneous migration of leukocytes. The total number of leukocytes and neutrophils was also determined. The results were compared with those in a control group formed by 20 healthy women. The data show that in patients with primary hypothyroidism, the adherence of neutrophils was increased (63 +/- 8% in patients and 53 +/- 11% in control subjects) and spontaneous migration decreased (45 +/- 20 cm2 in patients and 65 +/- 30 cm2 in the control group). However, there were no significant deviations of phagocytic activity of neutrophils in hypothyroid individuals as compared to euthyroid controls.

[Cytochemical examination of peripheral blood neutrophils of patients with hypothyroidism].

In patients with primary hypothyroidism a decrease of activity of acid phosphatase and beta-glucuronidase and an increase of arylsulphatase activity were found in cytochemical investigations of peripheral blood neutrophils. In the group of patients a decrease was also observed of the value of indices of spontaneous and stimulated nitroblue tetrazolium reduction. In patients with primary hypothyroidism a number of abnormalities may develop manifesting themselves as changed outfit of the neutrophils with lysosomal enzymes, and weakening may occur in these cells of redox processes evaluated by the test with nitroblue tetrazolium. The presence of the above mentioned features in the neutrophils may be interpreted as a part of metabolic and functional disturbances in the tissues of various organs in patients with hypothyroidism.

[Evaluation of phagocytic activity of peripheral blood neutrophils in patients after kidney transplantation treated with cyclosporin and prednisone or with azathioprine with prednisone].

In 32 renal transplant patients neutrophil phagocytic activity was evaluated using living bacterial cells of Staphylococcus aureus Oxford 209 P strain. The total group of patients was divided into two subgroups according to the kind of the applied immunosuppressive treatment 1) cyclosporin--12 patients treated with cyclosporin A along with prednisone and 2) azathioprine--20 patients treated with azathioprine along with prednisone. The index of phagocytizing cells i.e. the percentage of neutrophils phagocytizing the bacterial cells was assessed in the investigation of phagocytosis. In the renal transplant patients as compared with the control group, statistically significant increase in the phagocytic of peripheral blood neutrophils in relation to the used bacterial cells of Staphylococcus aureus Oxford 209 P strain was found. Statistically significant differences in the investigated range were also present between the compared subgroups. In the cyclosporin subgroup statistically significant increase was revealed in the percentage of phagocytizing neutrophils.

[Lysozyme activity in undiluted and diluted blood of patients treated with cyclosporin A with prednisone or azathioprine with prednisone after kidney transplantation].

It is a general opinion that the renal transplant patients may show the decrease of immunity which manifests by their increased susceptibility to bacterial, viral and fungal infections. One of the important non-specific defensive factors of the organism participating in the protection against infections is lysozyme. It is the purpose of the present paper to estimate the lysozyme activity in the un- and diluted serum in the renal transplant patients and to determine the possible mechanism of the observed deviations. 32 renal transplant patients (24 men and 8 women) aged of 16 up to 50 were investigated, being in the period of 1 until 4 years after transplantation. In the investigation the kind of immunosuppressive treatment used was considered which served the purpose of dividing the total group of patients into two subgroups: one treated with cyclosporine (12 patients) and the other treated with azathioprine (20 patients). The control group consisted of 20 healthy persons, matching by age, sex and living place the group investigated. In both groups investigated and control, were determined: lysozyme activity in the serum of the undiluted and diluted blood, total leukocyte number, the absolute number of peripheral blood neutrophils as well as their adherence to fibres was evaluated. In the renal transplant patients compared with the control group and in the separated subgroups, no statistically significant differences were found concerning leukocyte count, absolute neutrophil count and adherence of those cells.(ABSTRACT TRUNCATED AT 250 WORDS)

[Cytochemical properties of peripheral blood neutrophils in patients with hyperthyroidism].

In the cytoenzymatic investigations of peripheral blood neutrophils in patients with hyperthyroidism there was found the increase of acid phosphatase activity, beta-glucuronidase, leucine aminopeptidase, and catalase moreover there was found the decrease of the activity of alkaline phosphatase. After a two-week treatment with thiamazole (methimazole++) 50 mg in 24-hour dose there was observed the decrease of acid phosphatase activity in neutrophils. During incubation of plasma containing leucocytes, from healthy persons, with L-thyroxine there was observed the increase of the activity for acid phosphatase and beta-glucuronidase. In patients with hyperthyroidism there appear many changes of enzymic equipment of neutrophils which are concerned with lysosomal and connected with cell membrane enzymes. The results of cytochemical investigations after application of thiamazole and no difference, with exception of catalase, between patients with Graves-Basedow disease and with toxic goitre and the results of investigations in vitro with L-thyroxin point out, that there is the possibility of connection between the observed changes in the range of enzymic equipment of neutrophils and the hormonal state of the investigated group of patients.

[Bactericidal activity of plasma and leukocytes in patients after kidney transplantation].

In renal transplant patients the plasma and leukocytes bactericidal activity was estimated towards the vivid bacterial cells of the standard strain of Staphylococcus aureus Oxford 209P. Considering the kind of the immunosuppression used, the patients were divided into two subgroups. The first subgroup comprised the patients treated with cyclosporin A and prednisone (12 persons) and the second one--the patients treated with azathioprine and with prednisone (19 persons). The control group consisted of 20 healthy persons. In the renal transplant patients significantly more bacterial cells survived the incubation with plasma and peripheral blood leukocytes and the reduction of bactericidal activity of the leukocytes alone was also found. The reduction of bactericidal activity of the peripheral blood leukocytes observed in the patients of the group tested may have resulted from the inefficient microbicidal systems of those cells. The kind of the immunosuppressive treatment did not seem to affect the bactericidal activity of the leukocytes and plasma examined in the renal transplant patients peripheral blood. It may be supposed that the increased susceptibility of those patients to bacterial, viral and mycotic infections is due in part to the reduced bactericidal capability of the peripheral blood leukocytes.

Neutrophil function in chemical plant workers employed in the production of dust pesticides.

In two divisions of a chemical plant producing dust pesticides, employees exposed to dust, containing 28%-65% SiO2, were examined. In the first division (group I, 38 males and 35 females), the average air dust concentration was 5.0 mg/m3, and the active substances were: chlorinated hydrocarbons, captan, carbamates and dodine. In the other division (group II, 26 males and 33 females), the air dust concentration was 4.8-5.2 mg/m3, and the active substances included: carbamates, triazine compounds, cupric oxychloride, captan, lindane, carboxine. Spirographic investigations showed signs of pulmonary emphysema (RV/TLC) in 65.8% males and 60% females of group I and 38.5% males and 40.6% females of group II. In leucocyte concentrate smears, the cytochemical reactions to beta-glucuronidase, acid phosphatase and myeloperoxidase, as well as the nitroblue tetrazolinum (NBT)-dye reduction of neutrophils were performed. The random migration and chemotaxis of isolated neutrophils, washed or incubated in 10% autologous serum, their phagocytic activity and tube adherence test were also investigated. Compared to the controls, the reaction to beta-glucuronidase as well as the NBT reduction were increased, whereas the acid phosphatase and myeloperoxidase reactions were lowered. Impairment of neutrophil chemotaxis stimulated with zymozan-activated serum was observed in all groups of workers; random migration was enhanced in workers of group I and lowered in male workers of group II. Higher phagocytosis of latex particles occurred in workers of group I and in males of group II, while tube adhesion was impaired in group I and enhanced in males of group II.
