Tissue copper content in primary and metastatic liver cancers.

Tissue copper contents in 38 primary and 45 metastatic hepatic malignancies and 15 control livers were analyzed by atomic absorption spectrophotometry. The average copper content of 15 control livers was 23.1 +/- 13.0 micrograms/g dry weight (microgram/gdw). The copper content of five cholangiocellular carcinomas (CCCs) and 45 metastatic cancers was almost equal to the control level. Thirty three hepatocellular carcinomas (HCCs) contained a larger amount of copper (61.5 +/- 76.8 micrograms/gdw) than the control livers (p less than 0.05), but the copper content of HCCs showed a considerably wide variation. The average copper content of nine minute HCCs (126 +/- 112 micrograms/gdw) was significantly (p less than 0.05) higher than that of 24 large HCCs (37.2 +/- 39.9 micrograms/gdw). Histologically, orcein and paramethylaminobenzylidene rhodamine positive granules were seen in eight and four of nine minute HCCs, respectively. These granules were also found in some large HCCs, but were never found in CCCs and metastatic cancers. It was concluded that these excessive accumulations of copper and copper-binding proteins might present a helpful finding to distinguish some cases of HCC, especially small HCC, from CCCs, metastatic cancers and hypertrophic regenerative nodules of cirrhotic livers. The significance and possible pathogenesis of these copper accumulations in HCCs require further studies.

Pancreatoblastoma in Japan, with differential diagnosis from papillary cystic tumor (ductuloacinar adenoma) of the pancreas.

Twelve cases of pancreatoblastoma, 7 males and 5 females, were 5 years of age in average and showed an organoid structure consisting of acinar differentiation with squamoid corpuscles. Seven cases died, but 5 lived after surgery. On the contrary, 17 cases of papillary cystic tumor, ductuloacinar adenoma of the pancreas, one male and 16 females, were 21 years of age in average and disclosed a monomorphous pseudopapillary pattern of ductuloacinar type of tumor cells with some degenerative and granulomatous changes. All of the cases had suffered no recurrence after resection. Immunohistochemically, strong positivity for alpha 1-antitrypsin may be associated with the autodigestive process and limited growth of this tumor. Electron microscopically, both tumors showed centroacinar and acinar types of tumor cells. Both tumor cells frequently contained well-developed RER, zymogen-like granules, and annulate lamellae. The ductular or acinar lumina were clear in pancreatoblastoma, but not in ductuloacinar adenoma due to degeneration. Though both tumors disclosed contiguous histogenesis, pancreatoblastoma should be differentiated from ductuloacinar adenoma, based upon the young age, almost equal sex ratio, unfavorable prognosis, high serum AFP level in three measured cases, an organoid structure containing frequent mitotic figures, and the invasive growing margin.

Microangiopathic changes of subepidermal capillaries in end-stage renal failure.

Skin biopsies were taken from 63 patients with end-stage renal failure (ESRF) and 5 normal volunteers, 53 of whom were on maintenance hemodialysis (HD) and the other 10 without HD. They were examined with light and electron microscopy and subepidermal capillaries were subjected to image analysis. Thickening of the basement membrane (BM) with multilamination of the basal lamina, endothelial activation and chronic inflammatory cell infiltration with participation of mast cells were evident. The extent of these BM changes was correlated with the duration of HD, but not significantly correlated with the clinical findings indicative of arteriosclerosis. There was significant correlation between the duration of HD and mast cell proliferation or eosinophilia. We suspect that the subepidermal capillary changes represent an inflammatory reaction provoked by uremic toxins or by the repeated use of an artificial organ.

Aluminum accumulation in the skin in end-stage renal failure.

Accumulation of aluminum (Al) of the skin of end-stage renal failure (ESRF) patients was analysed by means of an energy dispersive X-ray spectrometer (Kevex 7000 series). The Al detecting ratio was higher in the hemodialysis (HD) groups than in the non-dialysis ESRF group (p less than 0.025) and the normal volunteer group (p less than 0.01). There were no significant differences between the duration of HD (group I and group II), and between the non-dialysis ESRF and the normal volunteer groups. High Al accumulation detected in the skin of the HD groups may be attributed to the dosing of aluminum hydroxide gel (Al-gel), judging from the fact that all cases in the HD groups had been dosed with Al-gel (0.75-4.5 g/day). High Al detections were observed in 2 cases each of the HD and nondialysis ESRF groups responding HBs-antigen positive.

[A clinicopathological study of 108 consecutive primary hepatic malignancies in the University of Occupational and Environmental Health].

At our University, the rate of hepatic malignancies among the total number of those autopsied was shown to be higher than the mean rate of Japan. The 108 cases investigated included 92 hepatocellular carcinomas (HCCs), 10 cholangiocellular carcinomas (CCCs), 4 cases of combined HCC and CCC, one undifferentiated carcinoma, and one angiosarcoma. The positive rate of serum HBsAg among the HCCs in our series was almost equal to the mean rate of Japan. Twenty-eight HBsAg positive and 64 negative HCCs were clinicopathologically compared. The mean age at death was ten years younger for the positive cases than in the negative ones (p less than 0.05). However, there was no significant difference between both groups in any other biologic or pathologic feature, except for a few minor matters. We describe herein the summary of our cases with reference to a comparison study between HBsAg positive and negative HCC cases.

Nodular regenerative hyperplasia of the liver associated with idiopathic membranous glomerulonephritis.

Nodular regenerative hyperplasia of the liver was found in a 67-yr-old man who had been suffering from nephrotic syndrome for years. Main clinical symptoms and signs were edema, proteinuria, hypertension, and abnormal studies of electrocardiogram. His renal disease had been relatively well controlled by administration of corticosteroids, but he died suddenly on March 12, 1986. Autopsy revealed fresh and old myocardial infarct and membranous glomerulonephritis. The liver weighed 1530 g and showed a diffuse nodular configuration except for in the area of Zahn's red infarct. Fibrosis was minimal in most areas, and each nodule was circumscribed by compressed atrophic parenchyma. However, this case was unusual compared to most cases of nodular regenerative hyperplasia because there was prominent fibrosis in the perihilar portion. Paucity of interlobular arterial branches was confirmed by a crude morphometry. We describe herein this somewhat unusual nodular regenerative hyperplasia case with some discussion concerning the histogenesis.

A clinicopathological review of 12 autopsied cases of adult T-cell leukemia.

Twelve autopsied cases with adult T-cell leukemia (ATL) were reviewed clinicopathologically. The prognosis of three cases who had suffered from severe cutaneous lesions was much better than that of the other nine cases with no or negligible cutaneous lesions. The surface marker of leukemic cells from six cases was ordinary inducer/helper phenotype (OKT4+ and 8-), but in one case leukemic cells showed OKT4+ and 8+. In another case, a significant amount of leukemic cell infiltration was found in the thymic cortex. Calcium content in the bone of ATL cases was lower than that of the patients without ATL (control group), and six cases with ATL (50%) were complicated by severe hypercalcemia. Neither adenoma nor hyperplasia of the parathyroid glands was found in any case. In most severely hypercalcemic patients, bone trabeculae were actively absorbed by numerous osteoclasts and partly replaced by fibrous tissues. In two normocalcemic patients, skeletal calcium content was also markedly reduced by osteoporosis, but the activation of osteoclasts was inconspicuous. It was speculated that the manner of bone resorption in ATL cases was diverse and there were some clinicopathological subtypes in ATL from the viewpoints of cutaneous lesions, hypercalcemia, and bone lesions.

Gastric leiomyosarcoma with massive myxoid degeneration. A histochemical and ultrastructural study.

Histochemical and ultrastructural features of a gastric myxoid leiomyosarcoma from a 55-year-old man were examined. At autopsy, the tumor was located mainly in the greater omentum and was directly connected to a coinsized gastric tumor. Multiple hepatic metastases and peritoneal disseminations were noted. Light-microscopically, the tumor was composed of a prominent myxoid stroma and ovoid or rounded tumor cells. The myxoid stroma was stained weakly basophilic with hematoxylin and eosin, and was mainly composed of hyaluronic acid. Tumor cells in the stomach were spindle-shaped and apparently myogenic. Ultrastructurally, the gastric tumor cells showed a loose cohesion with junctional apparatuses, pinocytotic vesicles, basal laminae, and cytoplasmic filaments with focal densities. Tumor cells in the omentum and the liver, however, were poorly differentiated and showed an epithelioid nature in part. This unique leiomyosarcoma is reported with some differential diagnoses from other myxoid sarcomas.

Minute hepatoma with excessive copper accumulation. Report of two cases with resection.

Minute hepatomas with prominent copper accumulations were resected in two women, aged 60 and 62 years, who had never suffered from jaundice. Mild elevation of serum alpha-fetoprotein level was found in both patients. One tumor was diagnosed by celiac angiography, and the other was determined by an ultrasonic echogram. Microscopically, these two tumors were relatively well-differentiated hepatocellular carcinoma, though having less-differentiated foci. Many cancer cells contained numerous copper granules stained by orcein, Victoria blue, and p-dimethylaminobenzylidene rhodanine. Ultrastructurally, cancer cells contained many secondary lysosomes with an electron-dense material. We concluded that the excessive copper in the cancer cells was aggregated lysosomal copper metallothionein, and that it might not be carcinogenic but stored by an altered metabolism of copper and copper-binding proteins with the neoplastic transformation.

[Diagnostic variations among eight pathologists in the histologic classification of lung cancer].

In order to estimate the magnitude of variations among pathologists in histological diagnoses of lung cancer, eight doctors were asked to independently diagnose 73 preparations of the lung (16 biopsied cancers, 27 resected specimens, 21 autopsied specimens and nine benign autopsied cases). The rates of correct diagnoses by the pathologists ranged from 81.7% to 100%, averaging 91.8%. There was no significant difference between the lowest rate and mean or median of the rates. The variation of the diagnosis tended to be more prominent in large cell carcinoma than in adenocarcinoma or small cell and squamous cell carcinoma. It was suggested that the diagnostic criteria for large cell carcinoma varied to some extent among pathologists.

Simultaneous occurrence of a calcifying odontogenic cyst and its malignant transformation.

The authors report a 48-year-old woman with a lesion in the maxilla, where a typical calcifying odontogenic cyst and a malignant tumor occurred simultaneously. She was diagnosed as having a malignant calcifying odontogenic cyst (tumor). The patient died of intracranial extension 20 months after diagnosis. Metastases were not detected.

Breast cancer in women of Japanese and Caucasian ancestry in Hawaii.

A pathology review of breast cancers in Japanese and Caucasian women indicates more numerous in situ carcinomas in the Japanese. Carcinomas with uniform nuclei were also more numerous among Japanese. Japanese women showed more extensive lymphocytic infiltrates adjacent to their tumors than did Caucasian women, and also showed more conspicuous sinus histiocytosis in tumor-free lymph nodes. Fewer Japanese women had lymph node metastases and those with metastases were less likely to have three or more nodes involved. Of these differences only those relating to local invasion, nuclear grade, lymphocytic infiltration, and sinus histiocytosis were statistically significant, but the demonstrated differences are internally consistent with differences in breast cancer incidence and mortality in the two races. Since the two races share the same medical care system and similar environments, the basis of these differences is probably a genetic modulation of hormonal balance and/or immunologic response.

[A case of spindle cell carcinoma of the breast].

A forty-year-old woman noticed a mass in her right breast. In April, 1984, a radical mastectomy was performed and the specimen was subjected to histopathological and ultrastructural study. The well-localized tumor revealed atypical spindle cell nests in the desmoplastic stroma. Based upon a squamous differentiation of the cancer cell nests, the tumor was diagnosed as spindle cell carcinoma. The histogenesis and prognosis of this rare tumor are discussed and the myoepithelial origin is suggested.

[A case of precancerous hair follicle tumor on the large pudendal lip].

A case of precancerous hair follicle tumor of a 60-year-old woman is reported. The tumor was detected in the large pudendal lip of an ATL-suspected patient. Microscopic findings showed that the tumor was a hair follicle tumor of low malignancy, such as a "malignant trichilemmoma", or "follicular Bowen's disease". Histogenesis and pathogenesis of this tumor are discussed including the relationships to ATL and "Trichilemmal horn".

[An autopsy case of well differentiated leiomyosarcoma of the jejunum].

A 54-year-old man had a leiomyosarcoma of the jejunum, 8 X 6 X 5 cm in size resected, on December 17, 1982 and died of liver metastases with hemorrhagic ascites (4,000 ml) on July 5, 1984. The malignant potential of a smooth muscle tumor based on the mitotic index of tumor cells has been stressed for many years. However, our biologically malignant autopsy case showed not only an exceptionally low mitotic index in tumor cells of the surgical specimen but also in those of the autopsy specimen. In this report, the fact that the degree of mitotic figures sometimes fail to indicate biological behavior is discussed.

Familial intrahepatic cholestatic cirrhosis in young adults.

Two siblings with intrahepatic cholestatic cirrhosis and their brother, who had a potentially related disease at the time of accidental death, are presented. The onset of disease occurred during adolescence in all 3 cases. The initial sign was mild jaundice or portal hypertension. There was no abnormality in the countenance, cardiovascular system, or vertebral column. Except for the brother who died from an accident, jaundice gradually increased. Death followed due to cirrhosis. Liver biopsy specimens of these 2 patients showed diminution of interlobular bile ducts with no significant cholangitis. At autopsy, the livers of the 2 patients showed biliary cirrhosis without extrahepatic biliary obstruction. In both cases there was an accessory lobe on the right hepatic lobe. Histologically, septal bile ducts showed pronounced papillary proliferations of the epithelium; there was also a decrease in the number of small interlobular bile ducts. Excess copper accumulation in the liver was ascertained. It is suggested that the disease in the 2 autopsied cases is intrahepatic cholestatic cirrhosis due to hypoplasia of the intrahepatic biliary trees.

Occurrence of breast cancer in relation to diet and reproductive history: a case-control study in Fukuoka, Japan.

An epidemiologic study on breast cancer in relation to diet and reproductive history was conducted with 3 populations at different levels of risk for the disease: Japanese in Fukuoka, Japan (low risk), Japanese in Hawaii (intermediate risk), and Caucasians in Hawaii (high risk). This report describes the Fukuoka portion of the study. We interviewed 212 patients with histologically confirmed breast cancer and an equal number each of hospital and neighborhood controls to determine risk factors for the disease. The results indicated that "overnutrition" (a high-fat and high-animal protein diet) was not statistically associated with the disease. As noted in previous studies, age at the first birth was positively related to the disease; those women who gave birth to their first child when they were 35 or more years of age showed a relative risk of 5.0 compared with those giving birth at less than 20 years of age. A past history of both natural abortion and of benign breast disease were significantly associated with the disease. On the whole, the results obtained from this Fukuoka portion of study were in good agreement with those from the Hawaii study during which Japanese and Caucasians in Hawaii were investigated.
