[18 months long-term analysis of effectiveness, safety and tolerance of sumatriptan s.c. in acute therapy of migraine attacks].

The aim of the open prospective study was to investigate the efficacy, safety and tolerability of subcutaneous sumatriptan in acute migraine treatment. Patient self-treatment was monitored over a time period of 6-18 months with 6 mg of sumatriptan administered subcutaneously by an autoinjector. In total, 2,263 patients participated in the study. Headache intensity was documented by the patient in a headache diary before and 1 and 2 h after application. During the study period from October 1991 to June 1993, 43,691 migraine attacks were investigated. Treatment with sumatriptan was efficient in 89.5% of the attacks. Headache relief was achieved in 71%. Headache reoccurred in 22.7% of the attacks, and therefore a second injection was administered. An intraindividual treatment efficacy of 80-100% was achieved in 82.9% of the patients. During long-term treatment the ratio of effective treatment and headache frequency was constant; 4.9% of the patients withdrew from the study because of adverse events and because they felt the treatment was not effective. A total of 44.5% of the patients reported adverse events that were serious in 1.7%. Subcutaneous self-treatment with sumatriptan in the acute treatment of migraine is effective and well tolerated by the patients.

[Gangliocytic paraganglioma of the duodenum].

A duodenal gangliocytic paraganglioma was found to be the cause of nonspecific abdominal complaints in a 68-year-old man with a known carcinoma of the rectum and the prostate. The duodenal gangliocytic paraganglioma is generally regarded as being benign. In this case it is shown to be an additional tumor in a patient with two pre-existent malignancies.

The interaction of laser energy with ureter tissues in a long-term investigation.

This study investigates tissue responses after laser irradiation of the rabbit ureter, which serves as an experimental model for rectourogenital fistulae of children. Twenty-five rabbit ureters were irradiated intraluminally by a Nd:YAG laser 1320 nm (2 Watt, 20 seconds and 3 Watt, 8 seconds) via an applicator with radialsymmetrical light distribution. Immediately, 2 weeks, 4 weeks, 8 weeks, and 16 weeks after irradiation, the ureters were X-rayed with contrast solution and prepared for light and transmission electron microscopy. For the parameters employed, no apparent morphological differences could be observed. Immediately, the central laser zone showed a transmural thermonecrosis prevailed by cellular destruction, condensed ground substance and occlusion of most vascular lumina. Peripheral laser zones displayed urothelial vacuolations. Between 2 and 16 weeks, urothelial regeneration and ingrowth of granulation tissue caused a luminal stenosis or occlusion followed by transformation into scar tissue. In some peripheral laser zones, a hydroureter with marked luminal dilatation developed. We conclude that the ureter is occluded if the expanding force of the growing scar tissue exceeds the hydrostatic pressure of the obstructed urine. A laser occlusion of rectourogenital fistulae will be easier to achieve since fistula occlusion does not entail an obstruction of the urine flow.

Catheter-directed laser coagulation of atrial myocardium in dogs.

The feasibility of transcatheter laser coagulation of atrial myocardium was tested in a canine model by using a combined electrode-laser catheter. In 17 anaesthetized beagles a total of 264 lesions, 12-19 per dog, and 3-5 in each area were produced in: (1) the lateral walls, (2) the posterior right atrial walls, (3) the inter-atrial septum, and (4) the atrial appendage. The power source was a continuous wave Nd:YAG laser. Irradiance was 1 kW.cm-2, at a wave length of 1064 nm, the irradiated spot diameter was 2.0-2.5 mm, and the pulse duration 5-60 s. Local intra-cardiac atrial potentials recorded from the targeted areas during laser irradiation dwindled after onset of the laser pulse, and their amplitude was reduced persistently by pulses of 15 s or longer. Histopathologically, the acute lesions showed intramural haemorrhage and coagulation necrosis, but there was no tissue vaporization or crater formation. Chronic lesions showed clear-cut oval-shaped areas of transmural fibrosis. Diameters of lesions, dependent on the amount of laser energy applied, measured from 4.6 +/- 0.5 mm (450 J) to 7.8 +/- 1.4 mm (1800 J). Follow-up lasted 6-25 months (average 10.5). All the animals survived the procedure without complications. Transcatheter laser coagulation of atrial myocardium is safe and can be performed in a controllable manner by using the catheter system presented.

Laser catheter ablation of atrial flutter and of atrioventricular nodal reentrant tachycardia in a single session.

This report describes an adolescent with refractory atrial flutter (AF) and atrioventricular (AV) nodal reentrant tachycardia who was cured by Nd: YAG, 1064 nm, laser application. Two impacts of 30 s each (power 30 W and irradiated spot diameter 2.0-2.5 mm), applied via a novel laser catheter system, abolished both AF and AV nodal reentrant tachycardia. The procedure was without complications or recurrent arrhythmias.

Light- and electron-microscopic study of the rat esophagus following intraluminal argon laser irradiation.

36 rat esophagi were irradiated by argon laser via an applicator with circumferential light distribution. They were perfused with glutaraldehyde and studied by light and transmission electron microscopy immediately, 2 days and 14 days after irradiation. Immediately after irradiation the laser center showed destruction of the keratinized stratified squamous epithelium. The collagenous fibers of the connective tissue were altered; fibrocytes and fibroblasts were severely damaged, and the microvascular lumina were occluded. The smooth muscle tissue and skeletal muscle tissue showed myofilament defects and initial karyonecrosis. There was decreasing damage of both fiber types up to 4 mm from the laser center. After 2 days the morphology of the laser center was not different from that seen immediately after irradiation. At a distance of 2 mm a partly differentiated new epithelium emerged below the necrotic epithelium. An inflammatory reaction was found in the connective tissue. After 14 days the esophageal wall was replaced and the lumen was occluded by young granulation tissue in the former laser center. Peripherally the esophageal wall appeared almost normal. As the rat esophagus serves as a model for esophagotracheal fistulae in newborn children, our findings indicate that the argon laser should be capable of occluding these fistulae likewise.

[Improved model of a fecal collection device for the prevention of coprophagia in the rat].

Previous fecal collection cups with tape fixation frequently led to serious necroses of the rat tail due to strangulations, and did not allow for growth of the tail. A recently developed new model gives space for rapidly growing tails and is securely fixed by a cannula perforating both fecal collection cup and tail. It is transparent, cheap to be made, and easy to empty and was tested in 118 animals for two and four weeks. In no case the cup did loosen or had to be removed due to infections, strangulations or any local problems.
