Drug safety labeling for doctors.

New labeling regulations to become effective later this year will make it easier for the doctor to determine the safety of a prescription drug for the use intended. Rx labeling for physicians will be required to carry pregnancy precautions for protection of mother and fetus, and safety data on adverse reactions, interactions with other substances, patient precautions on use, drug abuse and dependency status, and overdosage information.

[The value of physical training as a preventive measure in chronic ischemic heart disease].

A critical valuation of the hitherto achieved results concerning the problems of the influence of a permanent training on the course of the chronic ischaemic heart disease with the following result is performed: -A perseverance training which was begun in adolescence and permanently continued allows to expect a genuine preventive effect concerning the chronic ischaemic heart disease, though not in every case. Moreover it certainly contributes to retard temporarily the beginning of the chronic ischaemic heart disease and to restrict the sequels of a myocardial infarction. -When a chronic ischaemic heart disease exists no effects on the arteriosclerosis in general and the sclerosis of the coronary arteries in particular is to be expected. These show a progredience which cannot be influenced by training. -Also the effects on the myocardium, particularly on the formation of collaterals and the evoking of adaptive effects in the myocardiac cell itself are not to be expected, when the chronic ischaemic heart disease has already developed. Nevertheless, under this condition a perseverance training seems to be not fully hopeless, since through peripheral effects and general effects secondary effects on the heart are possible.

[Felix Marchand--a retrospect in occasion of the 50th anniversary of his day of death (author's transl)].

In consideration of an autobiographical study of Felix Marchand and of ideas presented by him in various publications his importance for the pathological anatomy and medicine was discussed. His talent of deducing from analytical observations for the detection of generally valid principles was remarkable. His contributions to the theory of inflammation and diseases, to the significance of general and experimental pathology as a basis of pathology, and his understanding for the nature of pathological state as a dynamic event when he is speaking about pathological processes are to emphasize. In our view, Felix Marchand represented besides Rudolf Virchow the most outstanding personality of German-speaking pathology.

[Experimental studies on surgical therapy of acute infarct].

The authors examined the effects of reperfusion after temporary ischemia in 50 dogs. The morphologic alterations were documented by methods of electron microscopy, fluorescence microscopy, and histochemistry. Lactate and activity of glycogen phosphorylase were assessed. According to the results, the optimal ischemia interval is 30 to 60 minutes for rational application of reperfusion, while it is just possible after 120 minutes. After a 4-hours-period of ischemia reperfusion increase morphologic damage of myocardium and impairment of myocardial metabolism. Prolonged reperfusion of 7 days resulted in a reduced extent of infarction compared with controls. In non-ischemic myocardium the morphologic and metabolic alterations were less expressed. The effects of "no-reflow"-phenomenon and conclusions for clinical practice are discussed.

[Reperfusion of the myocardium after acute ischemic lesion].

In a complex experimental study on 52 dogs the myocardium was examined after ischemia and reperfusion of various duration. Morphologic, metabolic, and functional findings proved that a short-term reperfusion after coronary occlusion for more than 2 hours has disadvantageous effects on the regression of ischemic lesions. During the early period (up to 3 days) this is caused by the "no-reflow" phenomenon. Combined qualitative and quantitative findings are required for correct appraisal of the ultrastructural alterations after reperfusion. In acute coronary occlusion, early revascularization after passing the crucial initial phase is advantageous, because the extent of myocardia necrosis is decreased by the prolonged reperfusion, thus leaving a larger myocardiac reserve in case of re-infarction. Clinical conclusions are drawn.

[The qualification of different ditetrazolium salts as indicators in the oxido-reductase histochemistry (author's transl)].

15 ditetrazolium salts were examined to prove their qualities for histochemical techniques. The succinate dehydrogenase, the lactate dehydrogenase and the diaphorase I in hearts, muscles, livers, kidneys and brains of rats were demonstrated for it. The results show that NBT is the best allround tetrazolium salt for the histochemical demonstration of dehydrogenases. For the study of special questions it is suitable to use other tetrazolium salts, too.

Study of histochemical and biochemical changes in the liver of rats induced by high doses of isoprenaline.

The aim of the present work was to study by means of histochemical and chemical methods the 7-day course of changes in carbohydrate metabolism in the liver of male rats induced by a single dose of isoprenaline of 50 mg/kg administered subcutaneously. A statistically significant reduction was seen both in the level of free glycogen and lactate within 24 hours. The decrease of pyruvate level was not so marked. At the same time, there was increased, and within the hepatic lobules also extended activity of enzymes catalyzing glycogenolysis, i.e. alpha-glucan phosphorylase and particularly the branching Q-enzyme, glucose-6-phosphatase and LDH, whereas the level of malate and activity of SDH, which are constituents of the Krebs cycle, were found to be reduced. Cytochrome oxidase activity was changed after 24 hr compared to the controls. The obtained results indicate that an extensive glycogenolysis occurs in the liver of rats in the 24 hr following s.c. administration of isoprenaline, the major part of liver glycogen being degraded through glucose-6-phosphate to blood glucose and its metabolism via the Krebs cycle reduced. The observed metabolic changes are of reversible character and tend to normalize over the 2nd and 3rd day following isoprenaline administration.
