Suppression of thyrotropin by 3,5-dimethyl-3'-isopropyl-L-thyronine in euthyroid and hypothyroid rats.

The effect of 3,5-dimethyl-3'-isopropyl-L-thyronine (DIMIT) a nonhalogenated thyroid hormone analog, on plasma and pituitary TSH levels was studied in euthyroid and 5-day-thyroidectomized (T-5d) rats. The minimal dose of DIMIT which suppressed basal plasma TSH for a 24-h period was 5.3 micrograms/100 g BW in the euthyroid rats and 5.2 micrograms/100 g BW in the T-5d group. The equipotent doses of T3 were 1.0 and 1.5 micrograms/100 g BW, respectively. Thus, the apparent DIMIT to T3 molar ratio ranged from 6.6 in the hypothyroid to 10 in the euthyroid group. The suppression of TRH-induced TSH release in the euthyroid rats required a high dose of DIMIT (9.2 micrograms/100 g BW), resulting in a DIMIT to T3 molar ratio of 17. Time-course studies comparing the effects of equipotent doses of DIMIT and T3 on TSH release rates in euthyroid and T-5d rats have shown a maximal suppression by 3--24 h after the administration of both T3 and DIMIT. However, the recovery from the effect was slower after T3. TSH synthesis in the T-5d group was similarly suppressed by DIMIT and T3. DIMIT has significant thyromimetic activity in euthyroid and hypothyroid rats, as evidenced by the suppresion of TSH synthesis and release.

Prognostic signs during continuous monitoring of the ventricular fluid pressure in patients with severe brain injury.

The presence of CSF in cases with intracranial hypertension is a favourable prognostic sign; its absence is indicative of a progressive and potentially lethal intracranial hypertension. 2. A series of characteristic changes in the absolute value of the VFP as well as in the amplitude and rate of the cerebral pulse can provide reliable evidence of the integrity of the cerebral circulation. 3. Short-lasting disappearance of the diastolic pressure towards the end of the ultimate plateau wave and subsequent significant lowering of both the systolic and diastolic pressures is an additional bad prognostic sign.

The effect of decerebrate rigidity on the intracranial pressure: an experimental study.

The mechanical effect of acute decerebrate rigidity upon the ICP and the mechanisms underlying the relationship between them have been investigated with experiments performed on 26 cats. It has been shown that: a) Extreme rigidity of the peripheral musculature with or without partial activation of the trunkal muscles produces no change in ICP, b) the simultaneous elevation of the intra-thoracic and intra-abdominal pressures is the factor primarily operative in raising and maintaining the elevated ICP, c) when cerebrovascular homeostasis is already defective a subsidiary but not unimportant role is played by the elevation of the systemic arterial pressure, d) under conditions of normal brain elastance mild and short-lasting spasms produce no effect on the ICP. In an animal, however, in which the brain elastance had been increased by inflating a small air-filled balloon, similar spasms produced a marked increase in ICP.

Applications of the Redy sorbent system to hemodialysis and peritoneal dialysis.

The application of sorbent technology to the treatment of uremia has been limited by the inability to adsorb urea adequately. Conversion of urea to ammonium carbonate and adsorption by zirconium phosphate provides a practical means of removing urea. This combination, together with hydrated zirconium oxide and carbon, removes uremic waste products from dialysate. Over 1500 patients are undergoing maintenance hemodialysis with this system. The dialysate composition can be modified for the treatment of patients with acute renal failure. This sorbent system is now being used for the regeneration of peritoneal dialysate. Twenty patients have undergone a total of 90 dialyses, each of 4 to 24 hours duration. One home patient has been solely on this system for two months. The application of sorbent technology to peritoneal dialysis may be the most promising approach to a wearable dialysis system.

TSH synthesis and release in the thyroidectomized rat: b) effect of T3.

The time and dose dependent effects of T3 on the anterior pituitary (AP) TSH release and synthesis were studied in rats 5 days after thyroidectomy (T-5d). T3, in various doses, was injected iv to animals which were kileed at various times later. Plasma and AP TSH were measured. T3 lowered plasma TSH to a minimum within 3 h. An inverse linear relationship was found between T3, up to 634 +/- 134 ng/100 g b.w. (mean +/- SD) and plasma TSH. Higher doses of T3 did not lower plasma TSH any further. At T3 of 634 +/- 134 ng/100 g b.w. the specific AP binder was calculated to be 68% to 90% saturated; thus suggesting a relationship between occupancy by T3 of the AP binder and the regulation of TSH release. The TSH release and synthesis rates were compared after 0.5 micrograms, 1 microgram and 25 micrograms T3. All three doses of T3 suppressed the release as well as the synthesis. The effect of T3 on the synthesis rate, however, lagged behind the effect on the release both in time and magnitude.

Hemodialysis in the treatment of psoriasis. A controlled trial.

We have treated seven patients with severe psoriasis with sham or true hemodialysis. Three patients received one 24-h course of true hemodialysis over 4 d followed by an identical 24-h course 4 weeks later. Four patients received one 24-h course of sham diaglysis over 4 d followed by a 24-h course of true dialysis 4 weeks later. Significant subjective improvement was noted by both groups after the first series of treatments but not after the second. No objective improvement in skin disease was found at any time. We were unable to confirm the many uncontrolled reports on the efficacy of dialysis in ameliorating severe psoriasis. Neither the heparin used for anticoagulation, the acetate present in the dialysis bath, nor the removal of a "psoriatic factor" seemed to affect the skin disease in these patients.

Development of a sorbent peritoneal dialysate regeneration system--a progress report.

A sorbent regenerative dialysate system for peritoneal dialysis has been developed (Pericycle). Clinical results and data demonstrate that the sorbent system provides a suitable dialysate. The sorbents effectively remove uraemic metabolites from the dialysate. Calcium and magnesium removed by the cartridge and glucose metabolised by the patient are replaced by the infusion system. The machine pumps regenerated dialysate into and spent dialysate out of the patients. A pressure sensor in the patient line prevents excessive inflow and outflow pressures by stopping the inflow or outflow pump respectively. The Pericycle provides a simple, safe, portable method for conducting peritoneal dialysis in the home or hospital.

Thinking with restricted language. A personal construct investigation of pre-lingually profoundly deaf apprentices.

By means of the deaf manual system of communication, Personal Construct grids were elicited from profoundly deaf apprentices. Contrary to expectation, the grids are not uni-dimensional, and they are as well elaborated as grids with the same supplied elements and constructs elicited from hearing subjects. Implications of the deaf subjects' grids are considered in the light of the relationship between thinking and language explored by Furth.
