Developing practice plans in a public hospital.

As funding sources outside of tight state and local budgets are being explored by public institutions, there is increasing interest in practice plan arrangements at public hospitals. The efforts of the New York City Health and Hospitals Corporation (HHC) in developing and implementing practice plan policies and programs in a diverse system of healthcare centers are documented here. These efforts were initiated in conjunction with a review of HHC's contracts with major medical schools, and their success has proven to be largely due to the plan's built-in flexibility and strong administrative and medical leadership.

MR imaging of optic pathways in patients with neurofibromatosis.

Twenty-one patients with documented neurofibromatosis had MR examinations to evaluate possible intracranial disease. In five cases the indication was a known or suspected optic glioma. Two patients were examined because of a history of seizures; the rest were examined as part of a baseline evaluation. Eighteen patients showed evidence of signal hyperintensity on T2-weighted images. Lesions involved the optic nerves, optic chiasm, optic tracts, lateral geniculate body, optic radiations, basal ganglia, periventricular white matter, cerebellar white matter, and dentate nucleus of the cerebellum. Comparison between MR and concurrent CT scans showed MR to be superior in demonstrating the posterior extent of optic-pathway gliomas. In addition, MR showed focal areas of hyperintensity in the basal ganglia, internal capsule, cerebellum, and/or white matter that were not detected on CT. Although we found MR to be superior to CT in detecting intracranial tumors in patients with neurofibromatosis, and in evaluating the extensive involvement of known lesions, the full clinical implications of our findings remain to be determined.

Prognostic factors in cancer of the testis: an approach to identification.

The clinical experience at the University of Minnesota Hospitals between 1962 and 1977 was reviewed in the hope of defining the biologic features of testicular cancer, knowledge of which is essential to determine the therapeutic needs of each patient. This collaborative effort between clinicians and statisticians has attempted to define important variables in the assessment of risk factors that determine the choice of treatment for testicular cancer. A preliminary analysis of data on the first 78 patients tabulated was performed to test and plan an approach to the final analysis of a larger group of 282 patients. One prognostic factor, staging, is discussed in detail to illustrate the purpose of this exploratory testing. Retrospective analysis, according to stage, of patients who were referred to this cancer center revealed a poorer prognosis than should be anticipated. In reporting the clinical experience of an individual institution, problems of bias are introduced with respect to the nature of the types of tumors studied.
