Vinblastine, bleomycin and cis-diamminedichloroplatinum in the treatment of advanced testicular carcinoma. Possible importance of longer induction and shorter maintenance schedules.

Twenty-eight patients with stage III or recurrent stage I or II testicular cancer were treated with four to six cycles of vinblastine, bleomycin and cis-diamminedichloroplatinum, and then with vinblastine maintenance for one year. With a median follow-up of more than 20.5 months for all patients, complete remission has been achieved with chemotherapy and operation in 23 (82 per cent). One of these 23 patients has had a relapse, and only three are still receiving maintenance therapy. The toxicity incurred by the extra cycles of chemotherapy in patients with extensive disease was no greater than that experienced by patients receiving smaller doses of drugs. These results confirm the high response rate reported for this combination of drugs and strongly suggest that some relapses after complete remission can be prevented by longer remission induction schedules. The value of maintenance therapy is questionable and needs further study.

Osteopathia striata syndrome. Clinical, genetic and radiologic considerations.

Osteopathia striata, an autosomal dominant disorder, has been diagnosed in a 19-year-old mildly retarded woman. In addition, she has macrocephaly, a leonine facies, disfigurement of the lower jaw, a cleft palate and mixed hearing loss. Roentgenograms of the skull and long bones show thickening of the calvarium, particularly at the base, mandibular hyperplasia, and striations in the long bones and pelvis. Except for the cleft palate, which has not been previously reported, and the retardation, which appears to be quite uncommon in this condition, these findings are characteristic of osteopathia striata. Because the disorder may resemble several other conditions, the differential diagnosis should include osteopoikilosis, the autosomal dominant form of osteopetrosis, and hyperostosis corticalis generalisata.

Group counseling in adult patients with advanced cancer.

The effect of a structured, interdisciplinary group counseling program was studied in 30 newly diagnosed adult patients with advanced cancer. These were compared to 30 patients who did not undergo group counseling. Group counseling resulted in a significant improvement in patient perception and self-concept. This structured educational and psychological support program provided a mutal support experience for newly diagnosed patients with advanced cancer.

Intralesional immunotherapy of malignant melanoma with mycobacterium smegmatis cell wall skeleton combined with trehalose dimycolate (P3).

The clinical efficacy of intralesional immunotherapy utilizing Mycobacterium smegmatis cell wall skeleton (CWS) and trehalose dimycolate attached to oil droplets was investigated in 15 patients with advanced malignant melanoma. Patients received 300 microgram to 1050 microgram of the CWS combined with one-half that amount of trehalose dimycolate every 1 to 2 weeks for a total of 8 treatments. Therapy was continued if regression of injected lesions only occurred. Therapy was discontinued if regression of noninjected disease also occurred. Six of the 15 patients had regression of at least one injected lesion. Four of these 6 patients also had regression of noninjected disease lasting 4+, 6, 16 and 18+ months. Response was highly related to immune status. Six (83%) of 7 patients who reacted to one of a battery of skin tests responded. All 8 patients who did not react to skin tests failed to respond to therapy. There was no correlation of response with sex, prior therapy, disease-free interval or presence of visceral disease. Mycobacterial CWS and trehalose dimycolate is an effective immunotherapeutic agent. Additional studies of purified immunoadjuvants are warranted.

Myocardial positron tomography with manganese-52m.

The biodistribution of radiomanganese (54Mn) was studied in mice, rats, and dogs. Disappearance of radioactivity from the blood was extremely rapid, with a half-time of approximately 0.8 minutes. This resulted in very favorable myocardium-to-blood ratios, even at early times after administration. The myocardial uptake in dogs was greater than 3% at three and 15 minutes, with myocardium-to-blood ratios of about 40:1 at 15 minutes. Positron tomograms obtained with 52mMn clearly demonstrated regional myocardial perfusion. There was good correlation (r = 0.89) of microsphere-to-radiomanganese distribution in the infarcted dog heart.

Serum parathormone in the identification and surgical management of hyperparathyroidism.

A prospective series of 200 patients with persistent hypercalcemia had an abbreviated diagnostic work-up consisting of parathormone radioimmunoassay, chest roentgenogram, intravenous pyelography, and serum protein electrophoresis. All patients with hypercalcemia and hyperparathormonism had neck exploration if roentgenograms failed to reveal evidence of ectopic hyperparathyroidism. Serum iPTH proved to be at least 96% accurate in predicting parathyroid disease while at the same time resulting in considerable diagnostic economy. An elevated iPTH was particularly helpful in distinguishing between hypercalcemia due to destruction of bone by malignancy and primary hyperparathyroidism with a coexisting malignancy. Further, measurement of parathormone was useful in evaluation of postoperative hypercalcemia.

Clinical application of the functional matrix: mandibular reconstruction and monobloc functional therapy.

A 13-year-old black girl with an extensive ameloblastoma of the mandible required subtotal mandibular resection and immediate reconstruction with use of an autogenous, ipsilateral, corticocancellous graft from the iliac crest and monobic functional therapy. Twenty-seven months later, clinical evaluation showed normal mandibular function and no facial asymmetry. Bony architectural changes after 27 months included reformation of the coronoid process, angular process, and external oblique ridge; preservation of mandibular length and thickness; and maintenance of the alveolar ridge height. The reformation of the coronoid process suggests the importance of the functional matrix (temporalis muscle) in skeletal tissue growth. The monobloc appliance in conjunction with bone grafting is an effective means for transmission of biophysical stimuli and for maintenance of appropriate stress for remodeling of bone in the mandible.

The pineal gland does not control rod outer segment shedding and phagocytosis in the rat retina and pigment epithelium.

Diurnal patterns of retinal outer segment shedding and phagocytosis by the pigment epithelium were examined in pinealectomized, superior-cervical-ganglionectomized, and sham-operated rats. Phagocytosis was quantitatively similar in all groups. Sharp increases in the number of large phagosomes were observed soon after lights were turned on in the tree sets of animals. Pinealectomized animals kept in constant darkness over a 24 hr period also exhibited normal shedding patterns. Our results suggest that the pineal does not exert a major influence on the daily rhythms of shedding and phagocytosis observed in the retina-pigment epithelium unit.

Role of nonverbal communication in the perception of leadership.

A 2 X 3 design was used to assess effects of nonverbal communication and sensory modality of presentation of stimuli on 107 undergraduates' perception of leadership. Nonverbal communication and modality of presentation had a significant effect on perception of leadership. These results suggest that nonverbal cues should be seen as essential in impression-formation.
