[Abdominal abscess--diagnostic progress via sonography and computed tomography].

Remarkable progress in the detection of abdominal abscesses has been achieved in the course of the last few years by sonography and by computed tomography. Successful detection of abscesses depends to a major extent on sufficient knowledge about their formation and spread, especially under postoperative conditions. These problems are, therefore, dealt with first, Typical examples of sonographic and CT findings are then described and discussed.

[Roentgen morphology of intramural pseudodiverticulosis of the esophagus].

Two cases of oesophageal intramural pseudodiverticulosis are described. They presented with typical numerous pseudodiverticula, each one or several millimetres in size, within the mucosa. One patient had a fibrous stenosis. In both cases CT showed localised thickening of the wall of the oesophagus. The few reports in the literature concerning histology, clinical findings and aetiology are reviewed.

[Roentgeno-morphological pulmonary changes following bleomycin therapy].

100 patients with squamous cell carcinoma of the head and neck, who underwent a bleomycin therapy pre- and postoperative have been followed up retrospectively for radiological signs of bleomycin-typical side-effects of the lung. The frequency of such changes was 24% and there were 2 peaks after 1 week and 4-6 weeks from the start of therapy. An increased risk to develop an interstitial exsudation and fibrosis seems to exist only for patients with preexistent emphysema of the lung.

[Leeches in the respiratory system (author's transl)].

The leech as a foreign body and parasite in the human respiratory tract occurs principally in the Mediterranean countries, in Africa and Asia. It reaches the respiratory tract when water is drunk directly from rivers, lakes, etc. Ignorance of this fact may cause diagnostic difficulties leading to errors in treatment. Anaemia and respiratory obstruction due to leeches cause danger to health and life, especially in children. Fatalities have been known. Treatment consists of endoscopic removal of the parasite, which may be technically difficult, especially when the leech is in the region of the larynx and thus presents dramatic conditions for the procedure. A series of 120 cases treated in Algeria between 1962 and 1971 is presented.

Laser-assisted balloon angioplasty in complete femoropopliteal occlusions: preliminary results.

Laser-assisted balloon angioplasty (LA) using a Nd-YAG laser with a sapphire tip probe was performed in 40 selected patients with complete chronic femoropopliteal occlusions (SFA n = 30, PA n = 10), in whom the lesion was resistant to conventional guidewire/catheter traversal. Overall technical success rate was 77.5% (31/40). Technical failure occurred in nine cases due to calcifications (n = 2), imminent perforation (n = 5), or complete perforation (n = 4). In 1 case a Simpson atherectomy was done following LA. In addition, 3 cases of peripheral embolizations were managed successfully by selective fibrinolysis and thrombus aspiration. Follow-up studies up to 14 months demonstrated a clinical improvement in 87% (27/31). Early reocclusion rate was 4/31; after 2-14 months, reocclusion rate was 7/31. Our results demonstrate that LA may be recommended for chronic occlusions resistant to conventional guidewire or catheter traversal in spite of a relatively high rate of technical failure and complications, and recurrence.

[Full band impressions].

Complete impression with rings achieves the same definition than complete impressions alone and gives a final quality similar to unitary guided impressions. The required steps are the following: 1st clinical: adjustment of the unitary rings, laboratory: elaboration of metallic resin boxing and individual tray, 2nd clinical: complete impression with rings. This method is of a particular interest because of its facility and its possible adaptations to various impression pastes or clinical situations. No gingival conditioning is required to the impression, and increased indications of the method can thus be seen when esthetics or periodontal conditions are a priority.

[Castration anxiety and oral envy in sexual relations. Analytic studies with ethnological observations].

Ethno-psychoanalysis is the study of the unconscious in foreign cultures. It can however also be of use in understanding unconscious elements operative in our own culture. To illustrate the point, the author describes the "Koro" epidemic which occurs periodically on the island of Hainan off the south of coast of China. This epidemic largely affects young unmarried men and the author pinpoints the unconscious conflicts underlying this collective phenomenon. It transpires that the epidemic is in effect a species of rite of passage in which a group of young males make the communal attempt to overcome castration anxieties which are themselves the product of covert gender envy. In the second section, Gerlach reports on his psychoanalytic encounter with a young man in Germany displaying similar symptoms. The profounder dimensions of this condition were only comprehensible to the author on account of his knowledge of "Koro".
