Plasma concentrations of 6-keto-prostaglandin F1 alpha in patients with hypertension, cerebrovascular disease or Takayasu's arteritis.

The peripheral venous plasma levels of 6-keto-prostaglandin F1 alpha (6-keto-PGF1 alpha) were determined by radioimmunoassay in 23 control subjects, 14 patients with essential hypertension, 19 patients with cerebrovascular disease (CVD) not taking aspirin, 12 patients with DVD taking aspirin, and 12 patients with Takayasu's arteritis. There was no significant difference in 6-keto-PGF1 alpha levels between the control subjects and hypertensive patients. In CVD patients and patients with Takayasu's arteritis, the plasma 6-keto-PGF1 alpha levels were significantly lower than those in control subjects. The internal jugular venous and femoral arterial plasma levels of 6-keto-PGF1 alpha and thromboxane B2 (TxB2) were measured in 10 CVD patients not taking aspirin. The patients with occlusive lesions of major arteries exhibited higher TxB2/6-keto-PGF1 alpha ratios in the internal jugular venous plasma.

Twenty-three cases of primary cornea guttata.

PURPOSE: To evaluate the clinical aspects of patients with primary cornea guttata and the specular microscopical findings/morphology in their corneal endothelial cells. METHODS: Twenty-three patients consulting Kanazawa Medical University Hospital or related hospitals in Ishikawa or Fukui prefectures and diagnosed with primary cornea guttata by slit-lamp examination underwent an analysis of their clinical features and corneal endothelial cells. RESULTS: Most cases were middle-aged to elderly women. None of them complained of visual impairment due to cornea guttata. The 46 eyes of these cases were included in this study. Specular microscopical findings revealed that the size and density of dark areas varied, with asymmetry noted in some cases. The parameters of the endothelial cells comprising mean cell area, the proportion of hexagonal cells, and the coefficient of variation, were almost within normal limits except for 1 eye of a 79-year-old man who was speculated to be at an early stage of Fuchs endothelial corneal dystrophy. Three cases had undergone cataract surgery, but did not show significant changes in endothelial morphology before and after surgery. In a statistical study on endothelial cell morphology, cell parameters except for minimal cell area correlated significantly with age. Although the density and size of dark areas correlated with each other, neither correlated with any of the cell parameters. CONCLUSION: Cornea guttata does not lead to visual impairment or abnormalities in corneal endothelial cell parameters except for a small number of cases that must be considered to be at an early stage of Fuchs dystrophy.

Long-term in vivo left ventricular assist device study with a titanium centrifugal pump.

A totally implantable centrifugal artificial heart has been developed. The plastic prototype, Gyro PI 601, passed 2 day hemodynamic tests as a functional total artificial heart, 2 week screening tests for antithrombogenicity, and 1 month system feasibility. Based on these results, a metallic prototype, Gyro PI 702, was subjected to in vivo left ventricular assist device (LVAD) studies. The pump system employed the Gyro PI 702, which has the same inner dimensions and the same characteristics as the Gyro PI 601, including an eccentric inlet port, a double pivot bearing system, and a magnet coupling system. The PI 702 is driven with the Vienna DC brushless motor actuator. For the in vivo LVAD study, the pump actuator package was implanted in the preperitoneal space in two calves, from the left ventricular apex to the descending aorta. Case 1 achieved greater than 9 month survival without any complications, at an average flow rate of 6.6 L/min with 10.2 W input power. Case 2 was killed early due to the excessive growth of the calf, which caused functional obstruction of the inlet port. There was no blood clot inside the pump. During these periods, neither case exhibited any physiologic abnormalities. The PI 702 pump gives excellent results as a long-term implantable LVAD.

Development of an antithrombogenic and antitraumatic blood pump: the Gyro C1E3.

The Gyro C1E3 is a centrifugal blood pump. Its antithrombogenic and antitraumatic blood features were demonstrated by prior studies. Based upon these studies, a mass production model of the C1E3 is becoming commercially available. Therefore, this feasibility study was conducted using the mass production models of the Gyro C1E3 for long-term cardiac assist in ex vivo animal experiments. Five healthy calves were used and 15 pump heads were applied for different time periods (Group 1, 30 days; Group 2, 14 days; Group 3, 10 and 7 days; Group 4, 4 days; and Group 5, 2 days). Activated clotting time (ACT) was kept at 200-250 sec. All five calves demonstrated neither abnormal signs nor abnormal blood examination data throughout the experiment. During necropsy, no thromboembolism was found in any downstream organs. Groups 1-4 showed thrombi inside the pump heads while two pumps in Group 5 had no thrombi formations. Bearing deformation or possible wear did not increase after 2 days of pumping. The C1E3 is capable of long-term assist circulation. However, after 2 days of pumping, careful observation is necessary since thrombi may occur inside the pump when ACT is controlled under 250 sec. During the weaning stage or low flow (under 2 L/min), over 250 sec of ACT is recommended to assure the safety of the patient.

The effect of the calcium antagonist nimodipine on the gerbil model of experimental cerebral ischemia.

The gerbil model was used to assess the therapeutic effects of the calcium antagonist nimodipine on cerebral ischemia. Transient cerebral ischemia was produced in each gerbil by bilateral common carotid occlusion of 10-, 15- or 20-min duration. Nimodipine (0.01 or 0.1 mg/kg) was administered intraperitoneally just before the carotid occlusion or 10-30 min after the removal of the arterial clips. Morbidity of each animal was evaluated using the stroke index, and the sum of stroke indices was calculated for evaluating the overall morbidity during a particular period of reperfusion. Mortality was observed for 24 hours after clip removal. Although, depending on the timing of the drug administration, the low-dose (0.01 mg/kg) nimodipine worsened the morbidity in the gerbils with 10-min ischemia, the high-dose (0.1 mg/kg) of the drug had a clear beneficial effect on the mortality associated with cerebral ischemia. These results are considered worthwhile for further trials to assess the usefulness of nimodipine as a therapeutic agent in the management of the acute ischemic stroke.

Effect of perindopril on 24-hour blood pressure levels and hemodynamic responses to physical and mental stress in elderly hypertensive patients.

This study assessed the effects of an angiotensin-converting enzyme (ACE) inhibitor, perindopril, on the 24-hour blood pressure (BP) profile and hemodynamic responses to isometric exercise and mental stress in elderly hypertensive patients. We performed ambulatory BP monitorings and echocardiographic studies during hand-grip exercises and a mental stress test before and after the 10-week administration of perindopril in 11 elderly patients (mean age, 71.8 years). Office BP was significantly decreased by perindopril treatment. Both daytime and nighttime systolic BP decreased significantly without any change in circadian pattern (P < 0.05). Daytime diastolic BP was also significantly decreased by perindopril (P < 0.05); nighttime diastolic pressure was decreased, but this result was not statistically significant. The reduction in 24-hour BP was associated with a decreased left ventricular end-systolic dimension (P < 0.01) and an increased fractional shortening (P < 0.01), suggesting reduced afterload due to arterial dilatation. The left ventricular mass index was not changed by perindopril therapy, and neither were systolic and diastolic BP responses to the handgrip exercises. The increases in systolic BP during mental stress was augmented after perindopril therapy, although systolic BP during mental stress after treatment was significantly lower than before treatment (P < 0.05). These results indicate that perindopril is effective in reducing ambulatory 24-hour BP levels and is associated with improved systolic function caused by dilatation of resistance vessels.
