[A study of the use of methotrexate, vincristine, cis-platinum, cyclophosphamide, adriamycin, bleomycin (MVP-CAB) in metastatic transitional cell carcinoma].

Eight patients with metastatic transitional cell carcinoma of the urinary tract were treated with the MVP-CAB regimen. All of them had bidimensionally measurable lesions. The MVP-CAB regimen consisted of cyclophosphamide 500 mg/m2, methotrexate 20 mg/m2, adriamycin 20 mg/m2, bleomycin 30 mg/body, and vincristine 1 mg/body on day 1, cis-platinum 50 mg/m2 on day 2, and prednisolone 20 mg/body on days 1-3, given every 3-4 weeks. A partial response was seen in five patients, minor response in one patient and no change in two patients. The response rate was 63% (5/8). The main toxic effect of the MVP-CAB regimen was leucopenia. In 75% of the patients there was a decrease in white blood cell count by not more than 2,000/mm3, but no severe complication was noted. In addition, mild nausea, vomiting, mild anorexia, alopecia and fever were found. However, these symptoms were transient. One patient died of pulmonary fibrosis induced by bleomycin after 3 cycles.

The stimulation of somatostatin release by hpGRF44 from rat hypothalamic cells and fragments in vitro.

Growth hormone releasing factor (GHRF) was examined to determine whether it affects somatostatin (SRIF) release from cultured rat hypothalamic cells and fragments in vitro. The hypothalami of rat fetuses were collected on the 17th day of pregnancy under a dissection microscope. Thirty hypothalami were placed in phosphate buffered saline, and the cells were dispersed with 0.1% collagenase. The dispersed cells were cultured in Dulbecco's modified Eagle medium (DMEM) containing 10% horse serum and 2.5% fetal calf serum at 37 degrees C under 5% CO2 in air. On the 12th day of culture, the cells were washed with Krebs Ringer bicarbonate buffer containing glucose (KRBG), and then incubated with KRBG for 1 hour. The medium was replaced with KRBG alone (control) or KRBG containing test substances, and incubated for another hour. SRIF released into the medium was quantitated by RIA. The mean basal release of SRIF was 14.7 +/- 0.9 pg/dish/hour. One-tenth, 1, and 10nM hpGRF44 stimulated SRIF release by 1.4, 1.5, and 1.8 fold respectively in a dose-related manner. Ten nM ovine corticotropin releasing factor (o-CRF) also stimulated SRIF release by 2.3 fold. One, 10, and 100 nM hpGRF44, 10nM o-CRF, 10nM thyrotropin releasing hormone (TRH) and 60 mM K+ also stimulated SRIF release from rat hypothalamic fragments. Removal of Ca++ from the medium resulted in a decrease of basal release of SRIF. In Ca++ free medium, 10nM hpGRF44 failed to release SRIF. One-tenth nM hpGRF44, 10nM GnRH, and 10nM VIP have no effect on SRIF release statistically. The results of this study demonstrate that a high concentration of GHRF stimulates SRIF release from the hypothalamus in vitro, suggesting a possibility that GHRF may increase the release of SRIF from the median eminence and the hypothalamus in vivo under certain conditions.

Ampicillin concentrations in human serum, gingiva, mandibular bone, dental follicle, and dental pulp following a single oral administration of bacampicillin.

Ninety-six patients who underwent the extraction of impacted mandibular third molars in a nonfasting state were given a single oral dose of bacampicillin preoperatively. Specimens of serum from venous blood (n = 107), gingiva (n = 57), mandibular bone (n = 68), dental follicle (n = 56), and dental pulp (n = 43) were obtained during the operation and assayed for ampicillin content. The mean peak concentrations in serum, gingiva, mandibular bone, dental follicle, and dental pulp all occurred approximately 90 minutes after administration. The concentration ratios of the tissues to the corresponding serum at the peak time were 0.50, 0.20, 0.34, and 0.61, respectively. Bacampicillin showed good absorption by the intestine, and sufficient concentrations of the resulting metabolite, ampicillin, were found in the oral tissues.

Ampicillin concentrations in human serum and periodontal membrane following a single oral administration of bacampicillin.

Ampicillin concentrations in human serum and periodontal membrane after a single oral administration of bacampicillin (500 mg) were assayed by the agar diffusion (paper disc) method. The peak times of serum and periodontal membrane were nearly identical, being approx. 90 min after administration. The peak concentrations of serum and periodontal membrane were 12.81 micrograms/ml and 6.97 micrograms/g, respectively. The concentration ratio of periodontal membrane to serum at the peak time was 0.56.

[Treatment of stage D prostate cancer patients with hormones in combination with cyclophosphamide].

Eleven patients with newly diagnosed stage D prostate cancer between June, 1984 and June, 1985, were administered 300 mg/day of Honvan in 3 divided portions, 4 capsules/day of Estracyt in 2 divided portions, or 100 mg/day of Prostal in 2 divided portions was administered orally, concomitantly with large-dose intermittent treatment of 1 g/m2/3 weeks of cyclophosphamide. According to the NPCP criteria, of the 5 cases given Honvan 1 case was PR and 4 cases were stable: all 3 cases given Estracyt were stable: and in 3 cases given Prostal, 1 case was stable, 1 case had progression and 1 case dropped out. The response duration of this combination therapy was 3 to 16 months (average: 9.9 months), and obvious improvements in the subjective symptoms and performance status were noted in the cases that responded to the treatment. As for side effects, gastrointestinal symptoms were observed in 9 cases, leucopenia in 3 cases, and thrombocytopenia in 1 case. All these cases, however, were slight and transient. In 1 case, jaundice and an elevation in GOT and GPT were detected, but these changes were also temporary.

Characteristics of opioid receptors in guinea-pig cerebellum and human placenta.

Characteristics of opioid receptors were examined in guinea-pig cerebellum and human placental membrane preparations. Two binding sites for [3H]-ethylketocyclazocine ([3H]-EKCZ) were found in guinea-pig cerebellum membranes whereas only one binding site was detected in human placental membranes. The potencies of kappa opioid agonists were decreased in competing with the binding of [3H]-naloxone to guinea-pig cerebellum membranes or human placental membranes by 5'-guanylylimidodiphosphate (Gpp(NH)P) and sodium. These results suggest that there may be true kappa receptors in human placenta and that kappa receptors in both preparations may couple with the adenylate cyclase system.

[Concomitant therapy with cefmenoxime and cefsulodin for refractory complicated urinary tract infection (especially caused by Pseudomonas aeruginosa)].

Cefmenoxime (2 g) and cefsulodin (1 g) were given twice daily for 5 days by concomitant intravenous drip infusion (mixed infusion) to 135 patients with complicated urinary tract infection (c-UTI) probably caused by Pseudomonas aeruginosa. The clinical efficacy was evaluated according to the criteria proposed by the UTI committee in Japan. Ninety one subjects met the criteria for c-UTI and were evaluable for drug efficacy. P. aeruginosa was detected in 44 cases (including mixed infection with other organisms). The overall efficacy rate was 73% of the 91 cases; 75% of the 44 cases with P. aeruginosa and 70% in the 47 cases without P. aeruginosa infection. As to bacteriological response, the eradication rate was 91% (105/116) for all cases. By organism, the eradication rate for P. aeruginosa, Serratia spp. and Citrobacter spp. were 82 (36/44), 100 (12/12) and 100% (10/10), respectively. The eradication rate for gram-negative rods was 93% (99/107). Twenty-three strains appeared after treatment, and the majority of them (13) were yeast-like organisms. There was only one strain of P. aeruginosa. As for side effects, eruption was found in 2 cases. Cefmenoxime and cefsulodin were administered concomitantly to patients with c-UTI which was suspected to be caused by P. aeruginosa. The high overall efficacy rate of about 70% on the average was obtained regardless of the causative organism and disease state. The eradication rate of as high as about 90% was obtained excluding Enterococcus faecalis. Neither severe side effects nor abnormal laboratory values were found. It appeared, therefore, that this dosage regimen was useful for the treatment of refractory complicated urinary tract infection.

[Treatment of early gastric cancer by endoscopic double snare polypectomy (EDSP)].

The indication for EDSP was studied in 75 patients with early gastric cancer diagnosed endoscopically which had been obtained over a period of 11 years (1979 to 1985) at the Cancer Institute Hospital, Tokyo and the following results were obtained. EDSP consists of two procedures using double channel fiberscope; a sessile or depressed lesion pulled upward by one snare cautery (standard EDSP) is transformed into a subpedunculated one, which allows another snare cautery of pseudostalk, and it is excised by using coagulation current. EDSP was performed in 53 cases of IIa type and 22 cases of IIc type. Excision by one snare cautery was possible in 90.9% (40/44) of IIa lesion measuring less than 2 cm and in 100% (14/14) of IIc lesion measuring less than 1 cm, the total being 85.3% (64/75). There was no residual cancer in 41 cases of IIa lesion (93.2%) and 17 cases (85.0%) of IIc lesion with two recurrent cancers confirmed by follow-up endoscopy. Residual cancer was found in 6 cases (9.1%) which were operated on. The results mentioned above show the usefulness of EDSP and selection of case without lymph node metastasis permitting complete excision by one snare cautery is of much importance. EDSP is indicated for IIa lesion of differentiated type measuring less than 2 cm, IIc lesion of differentiated type without ulceration measuring less than 1 cm and IIc lesion of undifferentiated type without ulceration measuring less than 0.5 cm that is not located in the fundic gland area.
