[Residual stenosis following successful selective lysis of complete thrombotic coronary artery occlusion in acute myocardial infarct].

After successful lysis of a thrombotic coronary obstruction in acute myocardial infarction, both PTCA and bypass surgery can be useful in preventing reocclusion and providing long-term success in selected patients. One condition to perform such measures is a high degree of residual narrowing at the previous site of occlusion. Other investigations concerning the extent and further development of these lesions are methodically inhomogenous and different in their results. Following successful intracoronary lysis of a complete thrombotic occlusion, the remaining stenosis was measured in 106 patients using at least 2 angiographic projections both immediately after reperfusion, and 3 days later. The degree and development of the residual lesion were analysed with special regard to its anatomy and to the occlusion time. During the observation period, no mechanical intervention (PTCA) or bypass surgery took place. The 1st angiogram after thrombolysis revealed an average cross section stenosis of 90.5 +/- 6.2%, which decreased up to the control angiogram to 86.3 +/- 10.6% (p less than 0.05). In only 16 cases there was an improvement of 10% or more, in fact it was not relevant (less than 10%) in 66 patients, and in 24 a slight increase in residual narrowing could even be found. The decrease of eccentric (-5.5 +/- 9.2%) and concentric (-3.3 +/- 5.7%) lesions was not statistically different. Stenoses up to 5 mm of length (-5.7 +/- 7.2%), between 5 and 10 mm (-3.4 +/- 6.7%), and over 10 mm (-4.4 +/- 8.2%) again did not differ significantly. [Ultrasound diagnosis of malignant lymphomas].

The most effective treatment of malignant lymphomas requires an exact diagnosis of the spreading of the tumour in the organism (staging). In this respect the combination of various radiological methods, actually including both sonography and computed tomography stood the test. However, in order to put the ultrasound into the right place of the diagnostic-step programme one has to know the advantages as well as the limitations of this method: the sonography ought to be used at the beginning of the diagnosis and in controlling the success of the treatment of malignant lymphomas.

[Malignant immunoblastic lymphoma under a pneumolytic scar in pleural empyema].

It is reported on a case in which a 51-year-old man 28 years after the pneumolytic treatment of a pulmonary tuberculosis developed a thoracic empyema under the existing pneumolysis scar. On account of the considerable scar formation in the region of the right hemithorax the thoracotomy was carried out for the purpose of decortication. Histologically a malignant immunoblastic lymphoma was verified in the area of the old scar.

[Antithrombin V--a rare concomitant symptom in plasmacytoma and autoimmune diseases].

Antithrombin V as a pathological inhibitor of coagulation may occur in some rare cases as an accompanying symptom of various basic diseases. In the course of two years, antithrombin V could be identified in 8 patients, with plasmocytoma, lupus erythematodes visceralis, glomerulonephritis or colitis ulcerosa existing as basic diseases. Clinical findings in patients and those gained in analyzing coagulation, procedures of laboratory diagnostics for determining characteristic properties of antithrombin V as well as the therapeutic way of influencing the activity of antithrombin V are represented.

[Surgical infections in patients suffering from haematological diseases].

Due to their systemic character, haematological diseases may interfere with erythro-, granulo- and thrombopoiesis, thus causing the onset of infections especially in cases of neutropenia and thrombopenia. Not only the defence mechanism of the individual is impaired by the basic disease but by the different therapeutic measures as well. Close cooperation between the haematologist and surgeon is of fundamental importance here.

2-(Isoxazolylethenyl)phenoxypropanolamines: a new class of beta-receptor antagonists with antihypertensive activity.

The synthesis of a series of (E)-1-amino-3-[2-(2-isoxazolylethenyl)phenoxyl-2-propanols is described. These compounds were found to have beta- and alpha-adrenergic blocking properties, as well as hypotensive and antihypertensive properties. The beta-adrenoceptor antagonism of all these compounds was more pronounced than their alpha-sympatholytic and hypotensive activity. 3a was 16 times more potent than labetalol in beta-adrenergic receptor blockade and was effective in lowering blood pressure in acute trials on spontaneously hypertensive rats at a dosage of 15 mg/kg. Structure-activity considerations showed that antihypertensive potency was more sensitive to structural variations than beta-adrenoceptor antagonistic activity. However, in general, those compounds having the most potent beta-adrenoceptor blocking activity also lowered blood pressure most effectively.

[Epidemiology of thoracic injuries (author's transl)].

UNLABELLED: The following details on thoracic traumata can be given as a survey: Occurrence: 6 to 10% of all injuries treated in hospitals (male: female - 3:1); about 8% open injuries (in Harlem N.Y: 86%); 80% occur as a polytrauma, 90% are combined with fractures. Mortality: 6 to 15% of all thoracic traumata and 20 to 30% of severe ones. ETIOLOGY: 60 to 70% traffic accidents, 15 to 20% during work, 15 to 20% at home, 2% sports accidents. Consecutive injuries of thoracic traumata are: Haemato - and/or Pneumothorax 35%, Lung contusion 20%, Heart concussion/contusion 15%, Rupture of diaphragm 4%, Aortic disruption 2%, Bronchus rupture 1%.

[Undifferentiated cell leukosis in pregnancy].

The appearance of an undifferentiated cell leucosis during pregnancy is relatively rare. However, it is increasingly more frequently to be reckoned with this fact, since the rate of leucosis is increasing. On account of the extraordinary situation in the individual case may be difficult diagnostic and above all therapeutic decisions which are discussed with the help of the represented case.

[New beta-sympatholytic agents. Synthesis and pharmacological activity of isomeric benzthiazole and benzoxazole derivates (author's transl)].

The synthesis and comparative pharmacological studies concerning structure-activity relationship of new benzothiazole, benzisothiazole, benzoxazole, and benisoxazole derivatives with beta-sympatholytic activity are reported. Most of the 4-benzisothiazole derivatives studied strongly act on cardiac beta 1-receptors in rats in situ (2.6-8.3 times propranolol). On the other hand derivatives of 4-benzisoxazole and 5-benzisothiazole are less potent. A strong decrease in activity was observed going from the benzisothiazoles and benzisoxazoles to the isomeric benzthiazoles and benzoxazoles. The active substances also block vascular beta 2-receptors. In the most cases they have little intrinsic adrenergic activity. The observed tendency concerning beta 1-selectivity in rats could not be confirmed in cats. The most potent 4-benzisothiazole derivatives in rats show also - beside some minor differences - very good beta-sympatholytic activity in cats after i.v. as well as after i.d. administration. Similar to the results in rats and cats the 4-(2-hydroxy-3-isopropylamino-propoxy)-1,2-benzisothiazole (LU 24329) was found to possess a high activity in conscious dogs by oral or i.v. application. Both in dogs (i.v.) and in isolated perfused guinea pig hearts LU 24329 is eleven times more active than propranolol in blocking beta 1-receptors. A negative inotropic effect as an expression of non-specific membrane-stabilizing action of LU 24329 is also demonstrated in guinea pig hearts. The effective concentration is 4650 times higher than that effective on beta 1-adrenoceptors. The active compounds have a moderate acute toxicity. The LD50 values in mice after i.p. administration are ranging from 55-174% of the propranolol toxicity.

[Staging of morbus Hodgkin by explorative laparotomy (author's transl)].

75 patients suffering from Hodgkin's disease underwent explorative laparotomy. This meant to correct the initial staging in a percentage of 34. There was an increase of postoperative complications in secondary laparotomies. In 4 our of 25 patients an unexpected involvement of the spleen could be demonstrated.

[Acute mature myelocytic leukemia as a form of the myeloproliferative syndrome].

On the basis of five observations of adult patients with the clinical feature of mature cellular leukaemia which proved to be therapy-refractory and which was characterized by a rapid course is referred to the necessity of the differentiation of such cases from the classical myeloic leukaemia. The cardinal symptoms of this type of disease, which probably is identified with the cases described in literature as atypical chronic myelosis, as paraneutrophil leukaemia or as acute myelofibrosis, and also shows common features with the juvenile chronic myelosis, are, apart from the mature cellular differential blood picture a short life expectancy (less than 1 year), an initial thrombocytopenia, a normal or increased activity of the alkaline granulocyte phosphatase, the lack of Ph1-chromosome as well as the bad therapeutic reaction to busulfan. The observation of the simultaneously existing fibroses of the bone marrow as well as of the final increase of immature blasts induced the classification of the clinical picture as a special form of the myeloproliferative syndrome.

[Experiences with primary and secondary laparotomy with splenectomy in Hodgkin's disease].

The explorative laparotomy with splenectomy belongs under adequately strong indication to the standard techniques of the dilated diagnostics and stage subdivision of lymphogranulomatosis. 61 patients of the Medical Academy Magdeburg were exploratively laparotomised from 1974--1978, of them 38 patients primarily before the beginning of the therapy and 23 patients secondarily 1--8 years after therapy. Changes of the stages before and after laparotomy, relations between affection of the spleen and histological type and primary histology, respectively, as well as weight of the spleen are discussed, postoperative complications are explained and adequate conclusions rendered.
