[Neurologic complications caused by use of cocaine, amphetamines and sympathomimetics].

Drug abuse has become a social and medical problem. Amphetamine and cocaine have a potent sympathicomimetic action, so they have important effects on the Central Nervous and Cardiovascular Systems. Their neurological complications are principally: psychic alterations, seizures and stroke (hemorrhagic and ischemic). The latter are the most important in the clinical practice. Their capacity to produce transit arterial hypertension and cerebral vascular constriction could be the physiopathological substrate of such alterations. Angiographic studies have shown lesions suggesting vasculopathy. In the last ten years cocaine abuse has become an authentic epidemic. We have reviewed its neurological complications, particularly the vascular ones--42 hemorrhagic and 24 ischemic--and the following conclusions were drawn: it should be considered as a risk factor in the younger age group; a short period of time between the last drug dose and the clinical picture is frequently seen; clinical features may appear with the first drug administration; no characteristic lesion in relation to the way of administration or consumption time was elicited.

Urinary neopterin excretion in pulmonary sarcoidosis: correlation to clinical course of the disease.

Urinary neopterin levels were determined by high pressure liquid chromatography in 36 patients with pulmonary sarcoidosis. Values were above normal range in 69.4% of all patients. Neopterin excretion was significantly higher in radiologic types II/III than in type I disease (p = 0.0265, Mann-Whitney U-test). Urinary neopterin showed a weak, but significant inverse correlation to disease duration (r = -0.36; p = 0.034, Spearman's rank correlation). No significant correlation could be found between neopterin and the presence of pulmonary or extrapulmonary symptoms, vital capacity, forced expiratory volume in the first second, steroid treatment, and serum angiotensin converting enzyme. Follow-up studies revealed that worsening or improvement of chest X-ray findings was invariably accompanied by rising or decreasing neopterin excretion, respectively. Patients with normal neopterin at initial evaluation had a higher tendency towards spontaneous healing, and none of them deteriorated during the follow-up period. We conclude that urinary neopterin excretion reflects one aspect of alveolitic "activity" in pulmonary sarcoidosis and that normal values probably indicate a favorable prognosis.

Studies on metal content in the brown seaweed, Fucus vesiculosus, from the Archipelago of Stockholm.

Concentrations of eleven metals (Al, Cd, Co, Cr, Cu, Fe, Mn, Ni, Pb, V and Zn) were determined in the brown seaweed Fucus vesiculosus collected from the Archipelago of Stockholm. Several factors which influence the metal content in the seaweed have been studied, including errors caused by epiphytes, sea exposure and differences depending on which part of the seaweed is analysed. It is concluded that, if all these factors are considered, Fucus vesiculosus plants are excellent bio-indicators of metal pollution. This is also demonstrated by a significant increase in metal content in transplanted Fucus vesiculosus near the city of Stockholm. The results from this investigation also indicate increasing metal concentrations, especially Cd, in samples from the northern parts of the Archipelago and the reason for this is discussed.

Absence status following metrizamide myelography: management of nonresponders.

Acute confusional state following metrizamide myelography has been reported to occur in up to 2% of patients. These patients have been diagnosed as having toxic encephalopathy. Recently, various cases have been reported to have a nonconvulsive status (absence status or complex partial status) who responded well to diazepam or clonazepam therapy. However, some authors have described cases of nonresponders to anticonvulsive therapy. Failure of therapy can be dose-related. For this reason, our patient was closely monitored by EEG to determine if dose adjustment was warranted, thereby achieving good results.

Pervasive assumptions, the points of view, and models of metapsychology: review and revision.

Metapsychology is the collection of higher level theories underlying and explaining the less abstract and experientially based theories of psychoanalysis. These theories are considered and formulated in interrelated contexts of force, excitation, time, organization, and adaptation. The principles are sometimes conceived of in the form of models, reifications which facilitate study and understanding. Further, certain psychoanalytic metatheoretical assumptions are so pervasive as to be treated within the viewpoints rather than specifically cited. They include provision of the basis for a psychological theory of mind in the biopsychological human entity, psychologically determined mental phenomena, and a lawful sequential logic. The laws and dimensions are inherently impersonal. Their ultimate origin is unknown and perhaps, unknowable. Freud referred to his theorizations as a kind of conceptual 'scaffolding', necessarily repeatedly revised as mandated by a continuous stream of novel observations and deductions. This contribution is intended as a current statement of those concepts.

Disease expression of ankylosing spondylitis in males and females.

The clinical features of 35 female patients with ankylosing spondylitis were compared with 70 male controls individually matched for disease duration. Mean age of onset and presenting symptoms were similar in both groups. No differences were observed as regards spinal symptoms, chest expansion, peripheral arthritis, extraarticular features or functional outcome. However, males had significantly greater spinal radiological change that was positively associated with hip arthritis. Our findings suggest that although axial disease is more severe in males the overall pattern of clinical disease is similar in both sexes.

The correlation of serum estrogens and androgens with bone density in the late postmenopause.

A pilot group of 16 women in the late postmenopause were evaluated for bone density by computerized axial tomography (CT) scanning and for hormonal milieu. A highly statistically significant positive correlation between lumbar-3 spongiosum density and both dehydroepiandrosterone-sulfate (DHEA-S), r = 0.67; P less than 0.005 and androstenedione (A), r = 0.56; P less than 0.03 was found. No such correlations were observed with estradiol (E2), estrone (E1) or an array of other hormones. The results of this preliminary report indicate a clear association of weak androgens with bone density in the late postmenopause.
