Court-mandated treatment: dilemmas for hospital psychiatry.

The authors present clinical material to illustrate the special treatment and management problems posed by different types of mentally ill offenders; they suggest that court-mandated hospital treatment is often destructive and unrelated to the needs of the patient, the community, and the mental institution. The failure to create new kinds of institutions, combining modalities derived from the hospital and correctional systems, is traced to poor communication among the disciplines involved. The "mentally ill offender" is caught in the interplay of these systems and is consequently both their victim and victimizer.

Superego factors in alcoholics.

This paper discusses superego fixations and regressions that predispose to and are caused by alcoholism. Two continua of superego functioning are explored, (1) the extent of internalization vs. externalization and (2) regulation of behavior vs. self-castigation. The superego constellations in guilty, binge, sociopathic, and deteriorated alcoholics are delineated to explain the interaction of a treatment program with these patients.

Increased specificity in measuring satisfaction.

The authors report a patient satisfaction study that addressed some of the methodological limitations of previous studies and attempted to increase the variance in satisfaction assessment by increasing the scope and specificity of inquiry. Same sex patient/therapist match, duration of therapy, individual therapy and treatment with staff social workers rather than psychiatric residents all were positively correlated with increased patient satisfaction. Satisfaction appeared to be a unitary dimension that could be tapped by a global score. Nonetheless, although overall satisfaction was high, one third of patients preferred an alternative treatment.

High-flow priapism as a complication of a veno-occlusive priapism: two case reports.

The presentation of two cases of veno-occlusive priapism, in patients of 36 and 58 years, in whom the different medical and surgical techniques employed, failed. The suspicion that high-flow priapism had been provoked by said surgical intervention was confirmed by bilateral arteriographs of the pudendal artery. The treatment, selective embolisation of the affected cavernosal artery with reabsorbable material, led to a rapid return of penile detumescence.

Immunocytochemical localization of transforming growth factor-alpha and epidermal growth factor receptor in human fallopian tubes and cumulus cells.

PROBLEM: Transforming growth factor-alpha (TGF-alpha) has been shown to be a potent stimulant of oocyte maturation and embryonic development. The role of maternal growth factors and their mechanism of action in early mammalian development is not well understood. METHOD: In this study, the presence of TGF-alpha and epidermal growth factor receptor (EGF-R) in human cumulus cells and fallopian tubes was investigated by immunocytochemical techniques. RESULTS: The fallopian tube showed intense staining for TGF-alpha in the apical region of the epithelial cells, and the cumulus cells showed intense staining for EGF-R on cell membranes. CONCLUSION: The presence of TGF-alpha in the fallopian tube epithelium and its receptor on cumulus cells suggest a paracrine mechanism between maternal growth factors and the developing embryo.
