[Two authors in search of persons. The relationship, the field, the history].

The story developing within the two-person field of the psychoanalytic situation is usually interpreted in terms of transference phenomena of body fantasies. Ferro, by contrast, elects a narratological approach. He traces the way in which micro-stories develop within the psychoanalytic field reflecting the unconscious psychic collaboration between the two participants. The point of analysis is creative narrative, not precise reconstruction. With reference to a number of actual cases, the author argues that in each psychoanalytic interview "persons" are created expressing the projective identifications taking place between the participants. The result is a story of the psychoanalytic couple in which sense impressions from early childhood assume a subsequent significance.

The impasse within a theory of the analytic field: possible vertices of observation.

The author attempts to place the problem of the impasse within field theory, showing that a prolonged period of countertransference metabolisation is sometimes necessary before an analytic process seemingly at a standstill can get under way again. Clinical examples are given to demonstrate the emotional difficulties with which the analyst must come to grips. The microprocess of the session and change in the analyst's mind are considered to be the significant loci of any possible resumption of therapeutic progress, provided that hope and patience do not fail, and subject also to the ability to adopt new and original viewpoints in the session.

In vivo genetic variability of the human immunodeficiency virus type 2 V3 region.

The principal neutralizing epitope of the human immunodeficiency virus type 1 (HIV-1) lies between two invariant cysteines in the third variable region (V3) of the viral envelope (gp120), and its amino acid sequence varies among different HIV-1 isolates. HIV-2 carries an analogous amino acid sequence between two cysteines of the V3 regions, but its functional similarity with the HIV-1 principal neutralizing epitope is uncertain. We studied the degree of genetic variation of the HIV-2 V3 region in fresh blood samples from 12 HIV-2-seropositive individuals from Guinea-Bissau. Polymerase chain reaction was used to amplify viral fragments of 465 bp containing the V3 region from cellular DNA. Nucleotide sequence analysis of the entire envelope fragment from each patient revealed that the degree of variation among field isolates of HIV-2 is comparable to that observed in the analogous region of HIV-1. Most of the HIV-2 isolates studied were highly related, suggesting the existence of a limited number of different viral strains in the cohort studied. Thus, the HIV-2 and HIV-1 V3 regions vary to a similar degree and may also have analogous functions.

[Familial dissecting aortic aneurysm].

A family is described in which two brothers (44 and 48 years old) had aortic dissecting aneurysms (the first one died, the other one underwent surgical treatment and is still living) in the absence of clinical features of Marfan syndrome and of systemic hypertension. Two of the six living siblings have aortic dilation on echocardiography. Histologic examination of the aortic wall at autopsy or surgery revealed a loss of elastic fibers, deposition of mucopolysaccaride-like material and medionecrosis. We can postulate a genetically-determined disease of connective tissue usually described as "Marfan's forme fruste".

Analysis of 16 patients with metastatic squamous cell carcinoma of the head and neck: implications for treatment.

The present paper reports a retrospective analysis of 16 patients with distant metastases from squamous cell carcinoma of the head and neck observed in our Institute in the last 10 years. Nine patients out of 16 underwent chemotherapy (7 patients), chemotherapy and radiotherapy (1 patient), or an alternating chemo-radiotherapy regimen (1 patient). The remaining 7 patients were not considered for a palliative treatment because of a poor performance status. Two complete responses, 2 partial responses, 2 stable diseases and 3 progressions were observed, with an overall response rate of 44%. Chemotherapy, when administrable, showed a temporary effectiveness, at least in patients with lung metastases. The combination of cisplatin and 5-fluorouracil seems to maintain the efficacy already shown in patients with locally relapsed disease.
