The effectiveness and safety of a very low molecular weight heparin fraction were evaluated in the prevention of deep-vein thrombosis in patients confined to bed due to hemiplegia consecutive to a recent cerebral infarction. CY 222 was administered within 48 hours of the stroke by one single daily subcutaneous injection of 0.6 ml (= 15,000 U AXa IC) during 14 days. This randomized pilot study involved 30 patients. The effects of CY 222 were assessed in a group of 15 patients compared with a control group of 15 untreated patients. No deep-vein thrombosis was detected by the labelled fibrinogen test in the treated group, as against 12 patients in the control group. Six patients (3 in each group) died during the study. One case of lethal pulmonary embolism was observed and confirmed at autopsy in the control group. In the remaining 5 patients, no systematic autopsy which would have asserted the absence of pulmonary embolism or drug-induced haemorrhage was performed. Numerous standard laboratory tests confirmed that CY 222 was well tolerated.

BACKGROUND: CA 125 and CEA are valuable serum tumor markers that can be used to monitor response to therapy in patients with various solid tumors. Systemic studies of CA125 and CEA have not been evaluated in lung cancer. In this study, we report the serum levels of CA 125 and compared it to CEA in newly diagnosed lung cancer and analyzed the serum levels of these markers pre- and post-therapy. MATERIALS AND METHODS: Two hundred and sixteen patients with newly diagnosed non-small lung cancer were evaluated. CA 125 and CEA levels were correlated with stage and histopathology. RESULTS: CA 125 levels and CEA levels were shown to be lower in patients with early stage disease as compared to patients with unresectable or metastatic disease. CEA levels were significantly higher among patients with adenocarcinoma, while there was no statistically significant relationship between histology and CA 125. There was a statistically significant difference in the CEA and CA 125 levels dependent on tumor size. Thirty-seven patients were analyzed for responses to chemotherapy and responders are more likely to have decreases in CA 125 or CEA. CONCLUSION: When abnormally elevated inpatients witlrlung cancer, CA 125 and CEA are useful indicators of disease extent, a useful clinical therapeutic marker, and may potentially have important prognostic value.
A new strain-gauge plethysmographic system, Phlebotest, was evaluated in 19 normal subjects, yielding 38 lower limbs for study. A capacitance mode was used which allowed both the left and right calves to reach their maximal volume expansion before releasing the cuff pressure (60 mmHg in this study). Parameters such as the maximum volume change (V sec), the outflow rate during the first second after deflation (F 1.0), the expelled volume in four seconds (EV 4.0) and the surface area over the curve during the first four seconds after deflation (IND) were automatically calculated and their side differences between the left and right legs were determined respectively. There was a good linear correlation between EV 4.0 and V sec (r = 0.9274, p less than 0.0005) and the EV 4.0/V sec ratio was calculated. In contrast to the previous reports, between three consecutive determinations there were no significant differences in the measurements of these parameters, and V sec consistently kept an identical mean value. These results suggest that using this new system only one determination of calf expansion is required. The reason might be that, in addition to some technical improvements, the computer-controlled capacitance function can secure the optimal venous filling in every determination.
Forty patients suspected clinically the first episode of deep vein thrombosis (DVT) were studied using a new computerized strain gauge plethysmographic (SGP) system, the Phlebotest. A capacitance mode was used to secure the obtaining of maximum venous filling before deflation in every determination. Following parameters were calculated automatically and instantly: the maximum volume change (V sec), the outflow rate during the first second after deflation (F 1.0), the expelled volume in four seconds (EV 4.0) and the surface area over the curve during the first four seconds after deflation (IND). The eighty lower limbs of the patients were divided into three groups according to the Duplex scanning results: proximal (n = 19), distal (n = 13) and no DVT (n = 48) groups. The diagnostic accuracy of the Phlebotest parameters was determined and compared with that of another widely used SGP system, the Perivein. In this case the maximum volume change and the maximum venous outflow (MVO) were calculated manually. The results of the two systems were compatible. For the detection of proximal DVT, the parameters of venous outflow were better than maximum volume change and the best single parameter was maximum venous outflow: F 1.0 for Phlebotest and MVO for Perivein. The two parameters had a similar sensitivity of 79% when 50 and 27 ml/100 ml/min were used as the lower limits of normal respectively. The best criterion for the diagnosis of proximal DVT was F 1.0 < 50 ml/100 ml/min plus the EV 4.0/V sec ratio < 60%, which had a sensitivity of 90%, specificity of 96% and overall accuracy of 94%.(ABSTRACT TRUNCATED AT 250 WORDS)

In order to determine the value and the role of real time B mode ultrasound imaging (USI) in the diagnosis of deep vein thrombosis (DVT) of the lower limbs, it was compared to bilateral contrast ascending venography used as a standard of reference, prospectively and systematically on 430 patients suspected of having DVT or pulmonary embolism. A total of 854 limbs were thus studied double blindly both by the two methods. The results corresponded in 95% of the legs with a sensitivity of 98% and a specificity of 95% for USI. Isolated thrombosis of the calf were detected in 91% of the legs and proximal thrombosis were in 100% in this series whatever their topography and extent should be and whatever be the degree of obstruction of the vein. The discrepancies between the two methods are related to: (a) Vein thrombosis especially located in the calf, in the soleal sinuses and the gastrocnemius with in most cases the direct image of the thrombus detected by U.S.I. more often than by venography, provided that the technique and the equipment are appropriate. (b) The absence of visualisation of venous segments with venography which is not specific of venous thrombosis. These veins when non affected by the thrombosis are not filled by the contrast medium if located above an occluded ilio-femoral or ilio-caval junction or when they are the site of extrinsic compression. The direct imaging of the vein and the surrounding structures obtained with USI enhances the diagnostic sensitivity and specificity and provides precision of the exact extension of the thrombosis. Venous study by USI is always coupled with the Doppler.(ABSTRACT TRUNCATED AT 250 WORDS)
In 6 type 1 (insulin-dependent) diabetics, treated with insulin since the age of 2 to 35 years (mean 12), coeliac disease was diagnosed between the 10 th and 73 th years of age (mean 26). Five were of North African origin. Digestive symptoms and severe malnutrition were present in all of them, associated, in the two younger, with a major growth retardation and, in one, multiple pathologic fractures. Biopsy of the small intestine demonstrated, in all, total or subtotal villous atrophy. The metabolic control of diabetes was poor, with frequent hypoglycaemic attacks, induced by minute insulin doses. Severe chronic complications of diabetes were detectable in all of them. Plasma anti-reticulin antibodies were present, at high titer before starting the gluten-free diet, declining slowly after starting this diet, and negative in the patients who followed this diet. Among the genetic markers (which were determined in 4), HLA A1 was present in 4, B8 and DR3 in 3 and DR4 in 3. The DR7 was not detected. The gluten-free diet, memorized by the patients by the use of simple rules, improved the digestive symptoms, and insulin doses could then be increased. The overall prognosis remained poor, due to diabetic complications and sociologic desinsertion. Coeliac disease occurs in 1 to 2% of type 1 diabetics and 4-6% of the coeliac patients are diabetics. Diabetic subjects from North Africa are at high risk of this association. Misdiagnosis of the coeliac disease compromises the metabolic control and nutritional state.(ABSTRACT TRUNCATED AT 250 WORDS)
To evaluate the contribution of echocardiography on the management of acute pulmonary embolism (A.P.E.) a study was carried out on 84 patients. For each of them echocardiography was performed: (a) the M mode shows an increase in RVDD/LVDD quotient correlated with the gravity of the A.P.E.; (b) the two dimensional mode allows 9 times the discovery of a thrombus which could be: static and broad based 6 cases or mobile, large sized 3 cases. So echocardiography can be systematically and initially be proposed for any patient suspected of having A.P.E.; (c) the absence of increase in RVDD/LVDD ratio 0.33 exclude the possibility of a serious A.P.E. and it is possible to postpone the pulmonary angiogram. On the contrary the increase of the RVDD/LVDD ratio must lead to the angiographic exploration due to the seriousness of A.P.E.: (d) the discovery of a mobile, large sized thrombus with hight embolic potential makes the catheterism of the right cavities dangerous and lead to an emergency embolectomy because of the high risks of a new A.P.E.
In a population-based cervical cancer screening program, the performances of five sample takers and their influence on the cellular composition of the smears and the numbers of epithelial abnormalities diagnosed were analyzed. As measured by the presence of endocervical columnar cells, there were remarkable variations in the quality of the smears between sample takers and between years of sampling. There was also a positive relationship between the presence of endocervical columnar cells in the smear and the number of epithelial abnormalities found. A lower quality of smear was reflected by a lower incidence of epithelial atypical changes found, both between sample takers and by a single sample taker over a period of time. These findings show that sample takers influence the cellular composition of smears and, therefore, the diagnostic outcome of smear screening. Their competence in executing their tasks must be frequently reviewed.
The significance of endocervical columnar cells as a high-quality parameter of cervical smears was studied. In a cohort of women with two successive screenings, the consistency of the cellular composition of the cervical smears and the relation between the cellular composition of the smears and the frequency of the diagnosis of abnormal epithelial changes was investigated. At the first screening, a significantly higher number of epithelial abnormalities was found in smears with endocervical columnar cells than in smears without endocervical columnar cells. A significantly higher number of abnormal epithelial changes was found on the second screening in smears from women whose smears from the first screening did not contain endocervical columnar cells than in smears from women whose smears from the first screening did contain endocervical columnar cells. The presence of endocervical cells should be considered a very important indicator of the quality of cervical smears. The chance of missing an abnormal epithelial change is increased in smears without endocervical columnar cells. When endocervical columnar cells are absent, the smear should be considered to be of unreliable quality and a repeat smear should be taken after a short interval, unless the absence of columnar cells can be satisfactorily explained.
