Keratinocyte grafting: a new means of transplantation for full-thickness wounds.

Twenty adult individuals with chronic leg ulcers caused by venous insufficiency, and 5 patients with full-thickness burns were treated. Twenty of the patients (15 with leg ulcers and 5 with burns) were grafted with separated autologous keratinocytes. In these cases the cells were fixed to the wound bed by a fibrin net. Five other patients (with leg ulcers) were treated with fibrin without keratinocytes. In 16 of the 20 patients grafted with keratinocytes in a fibrin net, the defect healed completely in 14 to 21 days. On the other hand, the fibrin net without keratinocytes failed to significantly accelerate the process of reepithelialization. Our experience suggests that a rapid healing of full-thickness skin defects can be achieved through keratinocyte grafting.

Enhancement of Candida albicans killing activity of separated human epidermal cells by ultraviolet radiation.

Ultraviolet irradiation enhanced the Candida albicans killing activity of freshly separated human epidermal cells in vitro. The stimulation was dose-dependent and was not due to soluble extracellular factors acting on non-irradiated epidermal cells. The enhancement of the killing activity remained unchanged when epidermal cells were depleted of Langerhans cells. Protein synthesis inhibitors and prostaglandin antagonists abrogated the ultraviolet-induced augmentation of killing activity.

The functions of polymorphonuclear leukocytes emigrating into the skin.

The functions of polymorphonuclear leukocytes (PMNs) emigrating into the skin and of others separated simultaneously from the circulation were determined and compared in healthy individuals. The PMNs emigrating into the skin were separated with a skin chamber technique. The C3 rosette forming capacities of the skin migrating and the circulating PMNs were similar. The chemotactic responsiveness was evaluated using three different chemoattractants: zymosan-activated serum, casein and lymphocyte-derived chemotactic factor. The skin PMNs gave practically no chemotactic response in the Boyden chamber, irrespective of the chemoattractant used. On the other hand, the skin PMNs exhibited higher plastic surface adherence, nitroblue tetrazolium reduction and Candida albicans killing activities. The data indicate that some functional activities of PMNs undergo alteration during in vivo emigration into the skin. The alterations, overall, may be of importance in the physiological protective function of the skin.

Drug fever caused by PUVA treatment.

Here we report the case of a patient who developed drug fever during PUVA treatment. This was the only manifestation of hypersensitivity. Neither UV-A irradiation nor 8-methoxypsoralene caused a febrile reaction.

The effect of mouse erythrocyte rosette forming lymphocytes on lymphokine production in T-cell cultures.

An enhancement effect of mouse erythrocyte rosette forming (MERF) cells on the production of migration inhibitory factor, chemotactic factor for neutrophils and skin reactive factor in T-lymphocyte cultures stimulated with the purified protein derivative of tuberculin was observed. We consider it likely that the MERF cells, possessing the appropriate cell surface constituents to construct an immunogenic moiety, present antigen on their surfaces to elicit lymphokine production.

Inhibiting effect of foetal renal antigen on leukocyte migration in human glomerulonephritis.

In 141 cases of glomerulonephritis confirmed by renal biopsy it was demonstrated that foetal glomerular basement membrane antigen caused a migration inhibition most frequently in minimal change glomerulonephritis. Cellular hypersensitivity was less common in membranous nephropathy, membranoproliferative (I-III) glomerulonephritis, IgA nephropathy and lupus nephritis. The correlation between LMT positivity and the occurrence of renal tissue IF activity was a linear one, but in one type, minimal change glomerulonephritis, there was no such correlation. The occurrence of LMT positivity does not show any considerable difference in glomerulonephritis with and without nephrosis.

Increased IgG Fc and C3 rosette-forming capacity of granulocytes from patients with psoriasis.

In the present work the proportions of IgG Fc and C3 rosette-forming granulocytes were studied in patients with severe psoriasis. The percentages of erythrocyte-antibody and C3 rosette-forming granulocytes from psoriasis patients were significantly higher than those of healthy individuals. Sera from patients with psoriasis did not influence the rosette formation by granulocytes from healthy persons. The increased IgG Fc and C3 receptor activities of granulocytes in psoriasis may account for the hyperactivity of these cells enhancing the susceptibility of the polymorphonuclears for at least some naturally occurring stimuli.

[Nitroblue tetrazolium reduction activity of circulating and skin-migrating leukocytes in psoriasis].

The nitroblue tetrazolium (NBT) reductive activity of polymorphonuclear leucocytes separated from the circulation as well as that of leucocytes migrating into the skin collected by skin chamber technique was studied with a quantitative method in patients with psoriasis vulgaris and in healthy persons. The NBT reductive activity of the leucocytes migrated into the skin was significantly higher in the healthy persons and slightly but not significantly elevated in psoriasis when compared with the cells separated from the blood. The dye reduction by the circulating cells was enhanced in psoriasis. On the other hand, in psoriasis the NBT reduction by the migrating cells did not differ from that of the leucocytes in healthy persons. The most likely explanation for the lack of the enhancement of activation during migration into the skin in psoriasis is that the polymorphonuclear leucocytes are already present in the circulation in hyperactive state.

Transfusion therapy for erythropoietic protoporphyria.

Five patients with erythropoietic protoporphyria, in whom beta-carotene treatment had proved unsuccessful, were treated on 5 or 6 occasions with washed, packed red blood cell transfusions. Following the transfusions, the photosensitivity and the protoporphyrin levels in the red blood cells decreased considerably. This treatment may be useful in reducing the high protoporphyrin level in patients with a rapidly deteriorating hepatic function.
