[Differed autologous blood transfusion in the "G" point hospital in Bamako. Prospective study apropos of 40 cases].

With a sample of 40 patients, the authors conducted a prospective case-control study on the feasibility of differed autologous blood transfusion at Point "G" National Hospital in Bamako, Mali. The method has been accepted by all the patients. No clinical nor biological trouble has been reported by the patients themselves. The authors recommend the use of autologous blood transfusion by other hospital units and the development of regulations governing its implementation.

[Effects of cholestatic bile acids on cytosolic calcium in isolated intrahepatic biliary cells].

In this study we measured biliary cytosolic calcium and examined the effect of cholestatic bile acids LCS and TLCS on intrahepatic isolated biliary cells cytosolic calcium. Cells have been isolated from bile duct ligated rats. Cytosolic calcium has been measured by using the Ca++ sensitive indicator Fura 2 and a cytofluorimetric method. LCS and TLCS (200 microM and 300 microM) increased the cytosolic Ca++ concentration of the cells. In contrast, the bile acids cholate and urso-desoxycholate which are choleretic had no effect. The number of cells which have increased their cytosolic calcium was directly correlated with the biliary acid toxicity. The increase induced by LCS and TLCS was abolished by removing external calcium. It is suggested that the calcium increase results from external calcium influx. This cytosolic calcium increase is known to be toxic for cells so it is concluded that this calcium increase is probably involved in the toxicity of LCS and TLCS.

Field diagnostic kits: a solution for developing countries?

An exact assessment of the animal health situation in a country is an essential element in formulating eradication and control programmes, and in regulating international trade in animals and animal products from that country. Due to a lack of human and technical resources, Veterinary Services in developing countries often lack precise knowledge on disease occurrence. Since the collection and transmission of reliable information on animal diseases in developing countries are major concerns of the Office International des Epizooties (OIE), a project aimed at improving this situation was implemented with international financial support. This project involved the development by the Centre for the Application of Methodology for the Diagnosis of Animal Diseases (CAMDA) of field kits for the diagnosis of the main diseases present in tropical Africa: rinderpest, peste des petits ruminants (PPR), contagious bovine pleuropneumonia (CBPP) and contagious caprine pleuropneumonia (CCPP). Several tests already exist, such as complement deoxyribonucleic acid (cDNA)-specific probes and polymerase chain reaction (PCR) for rinderpest and PPR, DNA probes and PCR for CBPP, capture enzyme-linked immunosorbent assay, the agglutination test and the immunobinding peroxidase test for CCPP, etc. With specific reference to these examples, the various problems faced by the OIE and CAMDA are reviewed.

A competitive ELISA using anti-N monoclonal antibodies for specific detection of rinderpest antibodies in cattle and small ruminants.

A competitive ELISA (C-ELISA) using monoclonal antibodies (mAbs) which bind to the nucleo-protein (NP) of rinderpest virus (RPV) for detection of RPV antibodies in cattle and small ruminant sera is described. Unlike virus neutralisation test (VNT), this test using mAb IVB2-4, can detect specific RPV antibodies without showing a cross-reaction with antibodies to peste-des-petits ruminants-virus (PPRV); by contrast, when mAb VE4-1 is used the test detects both RPV and PPRV antibodies, including low levels of antibodies that can be found in sera containing maternal antibodies. Although antibodies to the PPRV 75-1 strain are also detected with mAb 51-5-6, the test is suitable for assessing the immune status of cattle against the Rinderpest Old Kabete (RBOK) strain. The results from a panel of sera with a known status of vaccination provide evidence for a highly significant correlation between C-ELISA and VNT. This test may be a useful tool for a standardized and accurate determination of the immunity status of both cattle and small ruminants.

[Tumor-like focal nodular hyperplasia. Apropos of 3 cases observed at the Dakar Central Hospital from 1986 to 1989].

Authors report three observations of Focal Nodular Hyperplasia, Tumor-like on two: senegalese female patients, 16 years and 37 years old, and a young senegalese man, 24 Years old. The mean age is about 25.5 years and oestro progestative intake has bean noted once. Diagnosis of Focal Nodular Hyperplasia was difficult and has been done twice after surgical intervention for chronic cholecystitis and blood peritonitis. The only complication reported, has been a rupture of nodular, causing the death of a patient. This benign tumor that never degenerates, does not need any particular treatment unless a stopping of oral contraception.

HIV-2 and HIV-1 AIDS cases in Senegal: clinical patterns and immunological perturbations.

The serological and immunological parameters, disease patterns, and social characteristics of 39 human immunodeficiency virus type 2 (HIV-2) seropositive CDCIV cases seen in Dakar, Senegal were studied. These data were compared with those obtained from 48 HIV-1 seropositive CDC stage IV patients. Social characteristics of populations infected with HIV-1 or HIV-2 were clearly different. A patient sex ratio of three men to one woman was found for both viruses. In addition, the immune status of nonsymptomatic HIV-1 and HIV-2 seropositive people was evaluated. The correlation between abnormalities of the immune system and clinical status was similar for the two infections. Clinical symptoms of both diseases were the same, but this cross-sectional study could not address the questions of differences between the two infections in latency and development of disease or specific manifestations of HIV-2 infection. This study suggests that HIV-2 infection may contribute to the present AIDS epidemic in West Africa.

Morbillivirus group: genome organisation and proteins.

The Paramyxoviridae family is divided into three genera: Paramyxovirus, Pneumovirus and Morbillivirus. In the last group, there are four closely related viruses which are seriously pathogenic for man and animals, and usually cause acute diseases. At least two of them (measles and canine distemper viruses) can cause a persistent infection which leads to a chronic disease of the nervous system that, in the end, is fatal. For a long time, the biochemical analysis of morbilliviruses was hampered by the high susceptibility of some of their proteins to proteolysis. With cloning and sequencing technology, more data on the biology of those viruses are now available.

Peste des petits ruminants.

The peste des petits ruminants (PPR) is proving to be a disease which has an increasingly significant economic impact on a number of countries in Africa and the Middle East, and possibly also on the Indian sub-continent. The antigenic relationships which exist between the PPR and rinderpest viruses pose problems for diagnosis which complicates rinderpest control and eradication programmes. Progress has recently been made in regard to diagnosis (specific nucleic probes and monoclonal antibodies), as well as control (homologous vaccine). International legislation remains to be established and epidemiological surveys should be conducted in order to determine the exact geographical distribution of the disease.

[Comparison of adrenaline and clonidine to extend the analgesic effect of bupivacaine without glucose in spinal anesthesia].

So as to determine the effects of adrenaline and clonidine on the duration of isobaric bupivacaine spinal anaesthesia, a prospective controlled study was carried out on 20 ASA I or II patients. They were randomly allocated to two groups. The patients of group I were given 4 ml of a mixture including 3 ml of 0.5% bupivacaine, 0.2 mg adrenaline. The patients of group II were given 4 ml of a mixture including 3 ml of 0.5% bupivacaine, 0.15 mg clonidine. The segmental level of sensory loss was tested using forceps. The time course required for maximal spread on the sensory blockade did not differ in the two groups: 4.00 +/- 1.01 min. for group I and 3.60 +/- 0.90 min. for group II. No difference was observed between median highest levels of sensory analgesia. Regression times were 520.70 +/- 79.52 min for group II and 360.40 +/- 51.47 min. for group I. Significant prolongation of motor block was also associated with the addition of vasoconstrictors. It is concluded that addition of 0.15 mg clonidine may be useful to increase duration of isobaric bupivacaine spinal anaesthesia.

[Crohn's disease. Apropos of 2 recent cases collected at the Dakar Central Hospital].

This report covers two cases of Crohn's Disease, one in a 27 year old Senegalese woman, the other in a 38 year old Senegalese man, both presenting abdominal pain, diarrhoea and loss of weight over several months. The ulcerated lesions were staged on the colon, respecting the rectum, and reached the ileum in one patient and the esophagus in the other. Following X-rays, endoscopy and consideration of the history, Crohn's Disease was diagnosed. Treatment with corticoids produced rapid improvement with the ulcers healing after one month. The unusualness of Crohn's Disease in Black Africans adds interest to these two observations.

[Epidemiology of digestive tube cancers in Senegal. Apropos of 18,000 endoscopies carried out at the Hospital Principal of Dakar].

314 bowel cancers were discovered in 219 men and 95 women of negroid race in the course of 18,000 upper and lower endoscopies taken over the past 5 years at Dakar's Main Hospital. The average age of the sick persons is 50 years. Gastric cancer (170 cases) is the most frequent, followed by recto-colic cancer (71 cases) and oesophagian cancer (61 cases). The use of endoscopy and perhaps dietetic changes in the urban area, explain the large number of bowel cancers discovered in Dakar.

[Diagnostic suspicion. Clinical, epidemiological, radiological, and endoscopic aspects of bronchopulmonary cancer in Senegal. Apropos of 700 endoscopies performed at the Hôpital Principal de Dakar from 1987 to 1990].

700 bronchial fiber endoscopies were performed at Dakar Main Hospital over a period of 3 consecutive years. 40 broncho-pulmonary cancers were identified (34 epidermoids, 4 adeno-cancers, 2 small cell cancers) among the 80 suspect cases. Comparison of clinical, radiological and endoscopic data revealed the following information: 1. most frequently radiology showed retractile, pulmonary condensation syndrome (40 cases); 2. a large predominance of close, bulging tumours (45 times) in relation to endoscopic bronchial stenosis (22 times); 3. almost exclusive identification of bronchial epidermoid cancer in this series (34 times); 4. the masculine sex and use of tobacco remain 2 unavoidable elements in this pathology. Bronchial endoscopy, a simple, indispensible examination, allows the lesion to be visualized, and a biopsy, on its own or associated with endoscopic cysto-brushing and alveolus washing, can then furnish conclusive histological proof.

[Epidemiology of cancer of the digestive tract in Senegal. Review of 18,000 endoscopies performed at the Principal Hospital of Dakar].

314 alimentary tract cancers were discovered in 219 black males and 95 black females at the occasion of 18.000 endoscopies (high or low) performed during the 5 past years at the Principal Hospital (Dakar-Senegal). Mean age of the patients was 50, gastric cancer (170 cases) was the most frequent localization, then rectocolonic one (71 cases) and oesophageal one (61 cases). The more frequent utilization of endoscopy and may be a modification of diet in urban milieu might explain such a high frequency of discovery of alimentary tract cancers in Dakar.

[Evaluation of the mean annual risk of tuberculosis infection in the town of Conakry].

A study of tuberculous disease was carried out in children aged 5 to 13 years old who were first year pupils in primary schools in the town of Conakry (Republic of Guinae) with a view to determining the annual risk of tuberculous infection. In total 4,198 children distributed throughout 15 schools were tested after first looking for BCG vaccination scars. In the sample tested 1,444 children (34.4%) had vaccination scars and 2,754 (65.6%) did not. Amongst the 1,444 children with BCG scars, 1,367 (94.7%) were reviewed 72 hours after one unit of tuberculin RT 23 to have the skin reaction RDI read. Amongst these 210 (15.4%) had an area of induration greater than 6 m.m. diameter. Amongst 2,754 children who did not have BCG scars 2,655 (96.4%) were reviewed for the reading of the IDR: 330 children (12.8%) had an area of induration greater than 6 m.m. diameter. The percentage of children with an IDR greater than 6 m.m. as well as the mean diameter of induration was significantly greater in the group with a vaccination scar. The age of the children influenced the size of the induration. A factorial analysis revealed at the same time an age factor and a significant scar factor. Calculations from the prevalence of areas of induration with diameter greater than 6 m.m. in non-vaccinated children revealed an annual risk of infection of 1.52. A number of cases of pulmonary tuberculosis with positive microscopy in Conakry town is estimated at 90 per 100,000 inhabitants.

Chorionic villus sampling (CVS). Randomized study of efficacy of two transcervical biopsy methods: aspiration canulas and small forceps.

A randomized study of two transcervical biopsy methods was performed. One method involved the use of an aspiration catheter and the other a tissue biopsy forceps. 120 chorionic villus samplings were performed in 30 patients. The percentage of success in taking biopsies and the resulting number of karyotypes were identical in both groups. The amount of removed trophoblastic tissue was greater when the forceps was employed. When the aspiration catheter was used, a sufficient amount of trophoblastic tissue was obtained for karyotyping purposes. It was easier to insert the aspiration catheter than the forceps. The echogenicity of the catheter and forceps were identical. The aspiration catheter has two advantages. Firstly, it is malleable and fits to the anatomy of the cervical canal and the site from which the biopsy is to be taken. Secondly, it is disposable.
